My parents will have been in "Independent Living" four weeks as of tomorrow and honestly, it's been a pretty big mess. There is a new Executive Director, new Community Relations Coordinator, new Activity Director and a lot of turnover of other staff in these 4 weeks. I hope all the new people can pull this together but it's really been stressful in ways it should not have been. Communication is non-existent. Mealtimes are disorganized, scattershot and mishandled. Getting answer to questions takes multiple follow ups and there are way too many unanswered issues. I feel for anyone stepping into this without a good support system because, at least in Independent Living, residents are left out of the loop and not made to feel very welcome. I am giving this two stars because after some big initial problems, my parents now do like their villa and they are making friends with other residents. But wow, we expected and were promised so much more. I do not blame staff on-site who all seem very nice. I very much blame Civitas for what seems like a great deal of mismanagement, lack of care or adequate training and incredible staff shortages. I hope I can come back to this review soon to upgrade it but I am not holding my breath. Edit/update: This coming week will be 8 weeks since my parents moved in. After writing my review 4 weeks ago, I heard from several Civitas execs, all who seemed genuinely interested and concerned (unlike the operations person I met on-site right after the Executive Director left or previous staff I had emailed.) To their credit, personnel were brought in to oversee systems/practices and make changes which happened almost immediately thereafter. The new ED and CRD are now fully in charge, seem genuinely caring and highly capable. Dinner service and activities calendars have been revamped and are improved. Communications are better and accessibility to facilities for the IL group improved. I am pleased to report progress in many areas of previous concern. I hope Civitas will continue to provide enough staff, appropriate training and compensation/rewards to maintain their talented and dependable people. Time will tell. …I will boost my star rating from 2 to 4 because of the changes.

It was very hard for our 93 yr. old mother to leave her condo, where she lived independently and was happy, however she was experiencing memory problems. The Stone Creek staff have been very welcoming and supportive. As Mom was able to decorate her apt. to her taste with familiar furniture and paintings, she felt right at home. She loves the food, and she is a foodie! She always says, "Everyone is so nice." She also comments on how clean and well-run it is, and she is a 'neat freak.' The staff are very supportive of family and their feedback. They are excellent re. containing the minimal Covid outbreaks that have occurred. We recommend Stone Creek highly.

Choosing an Assisted Living Residence is a very important decision, but I whole heartedly feel your search is over. I'd pick StoneCreek every time because they effortlessly try to make your every moment count. For example, I'd like to thank Crystal Creyer, the nurse. I am a companion for one of their residents, and I had a concern related to her medical needs. At other places my voice would have been ignored. But, not here. Crystal, worked very hard to understand, address and resolve the issues. Things got complicated, but she was able to bring smiles back to our faces. Crystal gave us that comfort we needed to feel. She's a no none sense kind of nurse that you want handling you medical care and she is a strong patient advocate. StoneCreek has passion for their residents. There's a harmony of hope & helping each other at SoneCreek. I could give you positive examples of things I see all day long, but go see for yourself. It doesn't matter who you visit with, Ann the Director, Emily the Asst Director, Fernando the Chef or Archie in Maintenance because they have the same goal... YOU! Best wishes on your search. My dear friend recently died here. The administrator should be jailed for the lack of interest in the care and the treatment of the staff. The staff is shorthanded and very underpaid. The food was repulsive until the last few weeks of her life. If I could leave a negative review, it would be warrented. State board complaints are incoming. This place should be ashamed of themselves and anyone considering coming here should stay away. It's awful and tragic.

This place is amazing, not only is it a beautiful facility but the Wellness Director Crystal Cryer is top notch. She is so passionate about the care of the residents and making sure that they receive excellent care and have everything they need. She is reachable 24/7 and will always go above and beyond even if it is not her department. I would not hesitate sending my loved ones to this facility.

Frankly, it depends upon the day or week. The stars didn't allow me that option. I would really rank it between 3&4 most days. Just when I'm happy with things, & management decisions seem to be working well , something happens that changes that perception. We went 3 years without any Covid, the only facility in the area to be able to say so. Now suddenly we have 5 residents with active cases (one was sent to hospital. Perhaps it's the omicron variant. But it changes perception of management. We have far more turn-over of employees, including management. Many shifts are shout-staffed. We grow attached to certain staff members, & they leave for a job with better pay. I wonder if this is policy from this facility, or if it comes from corporate. It is easy to blame things on corporate. The caregivers are by & large wonderful! They follow their training & many times go beyond. If they can find a way to go above and beyond, they can do so. I would guess this counts for 85-90 % of the staff. So, while management may be lacking in many areas, the immediate caregiving staff makes up for it. I'll use another review to talk about food.

This place has really gone downhill. Promised all kinds of excursions and activates when moving in but nothing since. Resident phone lines go down quite often and very slow to get them back up (Days). Phone receptionists have a real attitude if you call and attempt to ask questions. Especially on Saturdays. Even have been given incorrect medications that landed Step Mother in hospital. Did not get a call until after I received call from Social worker at hospital and called them. Look close if thinking about moving someone in here.

Everyone at Stonecreek is so nice! From the sweet, helpful front desk person, Alyssa, to the kind nurses, Roe, Ella, and so many others. The medical staff is always willing to listen to concerns. It is not an easy time to run or work at an assisted living facility and they do so much to make it great. Shout outs to Carrie, Aquanette, and Barbie, who work so hard!!

My mother has Alzheimers disease. When my dad passed, we moved her into memory care. She lived at another memory care facility for 18 months. After multiple changes in management & other upsetting factors, we moved her to Stone Creek when they first opened. There are only 16 residents in memory care. Mom gets lots of attention and its much more peaceful. The care givers are attentive. We are so thankful that we moved her to Stone Creek before COVID hit. I would highly recommend Stone Creek for myself or any loved one.

My dad spent the last 6 months in memory care at this facility and I have absolutely nothing but GREAT things to say about the facility and the staff. My dad had been in and out of hospitals, rehab, and nursing homes the past 2 years and every other place he had been was a dump and the staffs were cold and indifferent. Stone Creek was the exact opposite. Stone Creek is new so everything still looks immaculate but the thing that impressed me the most was the staff. EVERY SINGLE PERSON we had met and interacted with was caring and patient, not only with my dad but everyone else in the facility. We installed a granny cam that recorded everything, so we even got to see and hear them when we were not there. I hope this place is still open when I need it. Love you guys.
