July 2026
I’ve stayed here several times for therapy and I keep coming back because I’m treated so well here. I rate it highly, not for the food, which has room for improvement, but for the staff. Everyone I’ve come in contact with really has a heart for the work. As a disabled person, I really appreciate the helpful and caring attitude that everyone displays. The therapy staff is experienced, the facility is organized, the rooms are kept very clean. I have recommended this place too may own family. If you find yourself here, you WILL new cared for.

My Grandmother is a current resident here. I HAD to make a comment on two staff members that made her and our transitional experience here amazing. To Veronica and Amanda: TYSVM!!!! Your incredible attention, devotion, AUTHENTIC love and care- made a huge difference. I can not TY you enough. Such wonderful people. Treated us as family and made my very complicated grandmother comfortable in this stage of life. Ty, Ty, Ty I will never forget the two of you.

My 95 year old mom spent 20 days here and left yesterday. We were very pleased with the food, attention, care she received. PT and OT, as well as speech therapist were all excellent! ( Mac, Scott and Holly) Only one aide over the 20 days we felt should not be in the line of service. Dana was rough/impatient with my mom and we voiced our complaint once while there. Ones we would like to mention for their caring and loving care given are, Sissy, Jordan, Tonsie and Lonnie!!! One night mom crawled out of bed and injured herself. A complaint was filed and Lee ( Director) promptly called us. Mom has been in 3 skilled nursing facilities and this is definitely the best! Thank you!

It’s not a rehab. It’s an old folks home.
January 2026
I cannot say enough good things about my experience at this short-term skilled rehab facility. From the moment I arrived, I felt supported, encouraged, and truly cared for as a patient”not just a number. The therapy team was absolutely outstanding. Physical and occupational therapy pushed me just the right amount every day, helping me regain strength, balance, and confidence I honestly wasn’t sure I’d get back. When I first arrived, I struggled with basic movements, but by the time I discharged, I had made huge physical improvements that changed my daily life for the better. The staff as a whole was compassionate, patient, and attentive. Nurses and aides were always responsive, kind, and willing to help, no matter how small the request. They consistently checked in on me and made sure I was comfortable and progressing safely. What stood out most was how personalized my care felt. My goals were listened to, my progress was celebrated, and I was constantly motivated to keep improving. The positive attitude of the staff made even the hardest therapy days feel achievable. Thanks to this rehab stay, I’m leaving stronger, more independent, and far more confident than when I arrived. I am incredibly grateful for the care I received and would highly recommend this facility to anyone needing short-term skilled rehab.

If you needed a sign that maybe you should run away, this is probably it. See attached pic. Seriously, who writes this stuff?!?! Hopefully not the nursing staff. The top line says they have an outbreak but are apparently not sure how if it's covid or influenza. Probably time to buy better tests. The next line encourages you to "Please Come In!" even though you shouldn't until the covid and/or flu outbreak is over. The poster says "If you're sick, stay away from others". Might be better to not allow these sick folks into the building at all. And "Wear a mask to protect yourself". lol. If you need protection, then for the love of all that is good and holy, don't go in here and hang out with the covid and/or flu people that got invited in above. The poster says to avoid touching surfaces, but you're going to tough handles going in and around the building, so might be better to glove up before you head inside.

Haley is an awesome nurse. She's a keeper!!
November 2025
Care of patients is not good at all. Staff sits around in chairs and laugh and giggle. This happens every time I go there which is quite often.

August 2025
My family member was treated with the utmost care”like the most important patient on earth. From the moment admission was needed, to her first meal, through rehab, and finally at discharge, the experience was outstanding. Every member of the staff was compassionate, supportive, and always present when needed.

This place looks nice and homey most staff are friendly and nice but some of the nurses was hateful and rude to my mother the first evening she was there., they made what could of been a good experience in to a bad experience for her. She is all of 49 and was at the time very weak and frail, she could not walk at the time and could not stand and was told she could just sit there until she could do it herself. Which she could not do so she sat there for a while this nurse stood there. Finally a male nurse came in and she told him and he told her he would take care of her and he did a great job and made her feel taken care of. I will do thing differently if this ever happens agian

The time we had at the villages has been amazing from the very start. The admissions team”Alex, Emily, and Charlisa” were absolutely amazing. They made a difficult time so much easier by walking us through every step with professionalism, compassion, and clarity. Their kindness and support made the transition smooth and stress-free. Once admitted, the nursing staff provided excellent, attentive care, and the therapy team was instrumental in helping my loved one regain strength and independence. The facility is clean, welcoming, and truly centered on patient care. We are deeply grateful and highly recommend southern hills to anyone in need of skilled nursing or rehabilitation.
June 2025
Our experience at VASH has been nothing short of outstanding. From the very beginning, the staff made us feel supported, respected, and genuinely cared for. The nursing team is incredible”consistently professional, compassionate, and responsive. They took the time to understand my loved one’s needs and always treated them with patience, kindness, and dignity. Equally impressive is the therapy department. The physical, occupational, and speech therapists are not only highly skilled but also deeply committed to their patients’ recovery. They created a personalized plan that helped my loved one make real progress, both physically and emotionally. We could see improvements week by week, and they kept us involved every step of the way. The facility is clean, comfortable, and well-run. The entire team works together seamlessly to create a warm and healing environment. VASH gave us the peace of mind we needed and exceeded our expectations in every area of care. I highly recommend this skilled nursing facility to anyone looking for top-quality nursing and rehabilitation services.

My physical and occupational therapist were amazing!! I was usually with Madison. She kept me up and going!! Therapy 6 days a week and I loved it! Also accolades to Victor, a 7-7 CNA. He was so nice, caring and kind. We would talk a lot just about anything. When Victor was working I knew it was going to be a good night!! Thank you Victor for being you!!

Discharge was horrible the lady Kim was very rude
April 2025
I was there for 2 1/2 months. I will go back if needed. For the majority of people who work there are great, and personified . There are several mainly night techs are just collecting a check. I will mention one of the most worthless techs he’s called Tee he hides and he is truly there to collect a check. I will say there are a lot that takes 30 minutes to answer when you ring. If you need help on toilet you’ll be there for a while. The food is decent, kitchen staff were wonderful to me. I had to go to dialysis very early and Melissa always made sure I had my breakfast before I had to go. My night techTee was supposed to go get my breakfast, he would always hide and I had to go get it myself if Melissa wasn’t available. I made friends there patients and staff I’ll keep in touch with. Overall experience was very positive.
March 2025
My wife suffered at The villages at Southern Hills.. She would call for pain meds and would often have to wait two or three hours. Sometimes they wouldn't even answer and she would have to lay in a wet diaper. As I write this I can hear another patient moaning in pain. This is highly unacceptable and unprofessional. I don't know how this is allowed by any place. If this review is taken down won't make a difference as I am copying it.

This the most WONDERFUL PLACE EVER!!! Walking through the doors you Feel The Warmth and Caring throughout. The Staff is 5 Star and more. My mother became a resident the end of November 2024 and the staff is so attentive to her and caring to her. You can feel the Love that is within the walls of this establishment from the front desk clerk , nursing staff and all of the other staff. My oldest sister found this location and myself and my other sister are so very happy! If anyone is looking for a place for there family to call home look no further because the Villages of Southern Hills is Home! God Bless Everyone for the First Class Quality Care they give to each and everyone ! Thank you all !
September 2024
My mother was recently diagnosed with dementia and has had issues for at least the past year which caused many frustrating moments with different healthcare and social services trying to get my mothers issues addressed. Everyone here has been fantastic however I would like to single out 2 individuals for their amazing support and help in getting my mother the help she needs. Daisha went above and beyond helping me get my mothers clothing needs addressed and Tee Fletcher was a true godsend in helping my mother get into a long term care facility, I cannot express the relief and how grateful I am with my experience here. I highly recommend this facility if you’re trying to get your family members needs truly addressed. Thank you everyone!
August 2024
Great place and friendly staff. Projected to have a family member there for 60 days and they were out in 10!
June 2024
The Regional Director, Alicia, and her team managed to place BOTH my parents in a week and a half. They navigated financials, medical records, placement, and other requirements I know nothing about. I did have to do my diligence on providing documents and information, but we'll worth the efforts. Thank you guys so much! I recommend without reservation.
June 2024
Villages at southern hills did a great job caring for my grandmother. Thanks to Valerie -PT, nurse Waylon.
June 2024
I just placed my mom in long term care here. The staff is amazing! They worked hard and very fast to get my mother placed. My mom said when she visited she felt safe and free. The facility is beautiful and clean. Thank you Alicia Todd for all your hard work helping to get my mom placed. Everyone has been amazing from the administration to all the staff. Thank you again for working so hard to place my mom.
May 2024
I can’t say enough about the kind and respectful care my mother was given during her stay for rehab. Everyone that we encountered seemed to really enjoy their job which was shown in how they cared for mom. Thank you so much VASH and I would recommend your facility to anyone in need of rehab.
April 2024
This review is for a rehabilitation stay. The 3 stars are given for the quality of the physical therapy and the therapists - the ones that worked with my parent were excellent. Also, there are some employees that seemed to care and were doing their very best. Otherwise, this is a truly scary place that seems to be poorly managed. I don't know when it was last evaluated by Medicare, and given a "5 star" rating, but that shouldn't be. I understand there is a general shortage of health care workers, but staffing at Villages seems particularly chaotic. Some aids were outwardly angry and rough. Clothes were sometimes left in the floor, or with a wet adult diaper in them. New patients came in that required PPE outside their door and a warning sign, but staff left their door open all time. Nurses that dispense medication sit by at their computers watching while helpless patients sit for hours in the eating area waiting for the hall aid to take them back to their room. Dining area staff do not cut up food. The doctor assigned to my parent never appeared (staff said he never comes in person), but instead his PA came usually during lunch and just talked to my parent, no physical exam. Other doctors visited with patients in detail during lunch so all of us at the table heard it all. Hall aids sometimes said they had more than 20 people to take care of. Medical supplies with names other than my parents name were left in the room. Hours went by when no one checked on my parent. Medications were not given at regular times - the schedule seemed to depend on the nurse, whether they stuck to the regular time (2 hour window) each day. Staff conducts a patient evaluation with family after 1 week to make recommendations on follow-up care, and that's it. They never do another evaluation, so when the patient is discharged, you may be poorly prepared to care for them if things have changed. Social workers admit they have too many patients to keep detailed information on. If you or family are fairly independent and can toilet, shower and get yourself to the eating area by yourself, then you might be OK here. If you are helpless or family members are helpless, family needs to be with you daily to monitor what is going on.

Well I know everyone has their own opinion but I have to give Villages at Southern Hills and big 1000 because my father was in there for rehab and the whole staff was really good taking care of him when my mother and I couldn't be there he really enjoyed the food that was prepared for him so thank you Villages for taking such good care of my dad but there's one person who stick out over the rest her name was Sissy she's such an amazing person and she truly enjoys her job and she stepped up everyday to check in on my dad if he wanted a shower and just to say hey we will miss seeing her smile everyday but my dad is now back at home but I just wanted to say a big thank you for the care everyone gave my dad we love you all may God bless this facility and everyone working in it. Thank you again

After being hospitalized, for a devastating injury, my loved one was transferred to a step down facility for physical and occupational therapy. We knew, quickly, that he couldn't stay there because the care they rendered was horrific So we began the search for somewhere better. After a trusted source recommended The Villages at Southern Hills we made the call. To our great fortune, Alicia Conner-Todd RN, took our call. She listened intently to our concerns and wasted no time in starting the transfer process. She kept us informed every step of the way and even went above and beyond by driving to the 1st facility to pick up the required paperwork after they appeared to be dragging their feet. Alicia continues to check on us, provide support to us, and assures us she is still available for assistance if we need her. She is professional and proficient yet kind and caring. Alicia Conner-Todd RN is a great asset to the already, very fine facility, of The Villages at Southern Hills.

I had the great pleasure of being in this facility for rehabilitation for several weeks last year. This was my first time in a skilled nursing center so I did not know what to expect but it was an incredible experience. My rehab team - George, Jennifer, and Valerie were very dedicated and made sure that I got the physical therapy I needed to make a good recovery. And I appreciated so much Sheila's help in getting me to and from therapy. The nursing staff....in particular Jessica and Charlene....were top-notch. The facility was experiencing some staffing challenges, but everyone went above and beyond the call of duty to ensure the patients received more than adequate care. I don't know that I have ever seen a harder working group of people. A very unexpected pleasure was the activities arranged for the guests.... from weekly bingo games to movies complete with popcorn and snacks. I actually participated in a finger painting class....something I was at first reluctant to do, but thoroughly enjoyed. And she published a daily calendar of all activities and hand-delivered it to every resident's room first thing each morning. The food was good.....much better than expected. If you didn't like what was on the menu, there was an alternate menu that could be ordered from anytime. The grilled cheese sandwich and tomato soup were very good and the Cobb salad was excellent. I was lucky to be there during Christmas when the entire staff went all out to decorate the facility beautifully. The administrative team was very professional and available to answer any questions that arose. And the cleaning ladies were SO nice and friendly and did a bang-up job with my room every single day. The Executive Director, Lee, plays a very active and visible role in managing the property. He was readily available to anyone who had any concerns or needs. (He actually personally delivered a new mattress to my room!!!) I will definitely be paying a return visit after some upcoming surgery.
January 2024
Lee the adminstrator of this facility has been here for years! That simply does not happen in long term care. The staff are amazing and ready to help. I recently did a survey to become a vendor for this company and let me tell you how amazing the facility s: 1. It is clean 2. Seasoned staff! 3. The residents seemed happy! 4. Top notch rehab facilities! 5. Excellent DON and Admin! The nursing team where happy and welcoming to the facilities!

December 2023
Meds are usually late. The food is supposed to be cut in bite size pieces. It's not. Her soiled diaper is sometimes left on her frail skin for HOURS. Her bed & gown get wet and they make her lay in it for hours sometimes. They have been told to leave her call light within reach because she has partial paralysis but they don't do it. Same with her phone. The CNA's (most of them) are cordial and proficient. My experience with the RN's , not so much. One exception and we thank God for him. I spoke with Administrator Lee. No results.
September 2023
My mother was here for a week. Overall experience was a B. Day shift was great. Evening shift needs some more people. They where shorthanded.
September 2023
My great grandma is currently at this facility for rehab to build up her strength so she can carry out her daily routine easier and continue to enhance her mobility. First, I want to say the Occupational Therapist Robbie is a godsend. She was extremely helpful and very patient and compassionate. The night nurses aides, however, need to be fired. They left my granny (who has to wear a diaper due to incontinence) in the same diaper from 6pm (when my grandma changed her) to 8:30am when I got there and changed her in the same diaper. She was soaked and stunk and was really embarrassed. She said nobody ever comes and checks on her and she needs constant supervision. I was appalled by the lack of care provided by the nurses aides here.
May 2023
I unreservedly recommend The Villages at Southern Hills to care for your loved ones. My aunt was a resident and the staff was loving and patient and kind. My aunt had many challenges and a lot of decisions had to be made at various times regarding her care. Lee, the Executive Director, kept me informed and was always available to answer questions. He also checked on my aunt daily. Thank you to everyone at The Villages!
March 2023
While the general services was basically satisfactory. I would NOT give the food served to my dog! Many patients simply left it on their plates. In order to have something to eat I asked my family to bring several cans of soup. They also brought sandwiches and salads. The food served by the skilled nurse establishment is garbage! It's a shame that I had to provide my own food. I even brought a small refrigerator in order to have fresh food. I will never recommend this skilled nursing facility!!!!!

January 2023
My stay at this facility was very helpful and pleasant. I was there for skilled nursing because I had undergone open-heart surgery. The staff was attentive the nurses were kind, friendly and knowledgeable, and the meals were good!
July 2020
My father was transferred to this facility for short term rehabilitation. He left the hospital in good spirits and his health was very stable. Upon arrival we were treated as if we had pissed someone off. The nurse aid (nice neck tattoo) was extremely rude and was only interested in sitting in a chair in the hallway and texting on her phone. The nurse call button goes unanswered. It took over two hours on one occasion. Medicine is not given on time and sometimes not at all. My father was on a special diet and he was given regular food three times in a weeks period. Within the week he was infected and had pneumonia. When I told them I was moving him to another facility they told me I couldn’t and it would take a doctor’s order and several days. Needless to say I told them to go to hell and I moved him to another facility that same day. Within two weeks at the new facility he was happy and healthy. Within a month he went home and is still home. I am positive he would have died in this place. PLEASE don’t send a loved one there! They only care about the Medicare checks.
