The care my husband received was very good, which was my main objective. The care manager and head nurse were extremely helpful and available to an anxious wife when needed. The staff treated my husband with dignity and respect and did everything within their power to tempt his appetite, keep him comfortable and make him feel at home. The activities with other residents were a source of stimulation and I was pleased to have him participating in that in so far as he was able. He was in memory care and that is more complicated than assisted living. The staff treated all the residents with great patience and preserved their dignity and I found that very comforting.

The place was really nice and the caretakers (staff) really were nice and made sure that everyone was eating lunch. That was when I toured the place. Very clean and the cater to what and when the person eats.

Greg and the rest of the staff were absolutely wonderful. Greg worked with the family in answering all our questions. We had a very short turn around time and there was no problem in getting everything done. Everyone was warm and made us feel very welcome. Beautiful place.

My dad has moved into Sunrise on 9/11/2023. He loved being there after a few days of adjustment! Unfortunately, he fell on 12/2/2023, and never got back out of bed. The staff there was all so kind to him. He was well loved and cared for, as was I! Near the end of dad's life at age 95, the staff cried with me and gave me big hugs, which I needed! I would highly recommend Sunrise Five Forks for your loved one! It feels like going to your grandma's house! Thank you, Sunrise, for loving my dad so well!

We toured the memory unit recently. I found the facility to be exceptional. Surroundings & atmosphere calming/comfortable. Staff attentive and appropriate with residents in various stages of dementia. (Very positive engagement w/ all residents.)

I have worked with Greg Lawrence, who has been so pleasant and accommodating, to place our mother at Sunrise. Placing a loved one , a mother, can be very stressful but Greg calmed a lot of fears. Sunrise at Five Forks is beautiful, clean with a warm atmosphere. My niece worked at Sunrise in another location in Atlanta. She was so impressed with Sunrise that we pursued this one at Five Forks and have been very pleased. Mom moved in! We moved our mother into Sunrise about a week ago. She is 97 and cognizant with some dementia. I stayed with her two nights and 4 days. The care is exceptional. Each caregiver is conscientious, patient and understanding. professional. The atmosphere is so welcoming and the residents are happy, enjoying companionship and fun .

Meeting Millicent was the best interview I ever had in my entire life. She made me very comfortable and I felt like I was home with my family that was the feeling I had been there at this facility. Even though my time will not fit with their hours but I still felt like I have met a family and friend, I appreciate their time and effort meeting with me and I will always cherish it for the rest of my life. I never had an interview with someone that is very caring towards her job and love every one in the facility and treat them like a family. Someone like Millicent is very hard to find, she goes above and beyond to find me a position to work with her but still did not work because of the time, this showed me she cares a lot for everyone. She cared so much for the client, they love her and seeing them the smile in their face when they see Millicent that was the best thing ever it really put tears into my eyes believe me is tears of joy.

I have been working at this facility for a couple of months now, and my coworkers and supervisor are absolutely amazing. They genuinely care for the residents and do everything they can for them to receive the best care. My coworkers are super encouraging, friendly and they always keep it real My supervisor Ms. Millie has been the best supervisor I have ever had, and she is always there to listen to our concerns and encourages us to do our best I’m so glad God has blessed me with a job here in this facility because it is helping to grow in so many ways. I always look forward to coming to work here 🙂

My Mother-in-law moved into SunRise at Five Forks about five years ago and the team that cares for her have been wonderful. There have been changes to the personnel but each time they have gotten excellent new team members. Veronica, the Executive Director, and her team do a fabulous job of caring for all the residents. Millie and Linda are so compassionate and help out where ever they are needed, in addition to their assigned jobs and treat everyone like they are part of their family. I highly recommend SunRise at Five Forks to everyone.

I have been working with this facility for over 4 years and the care and compassion and leadership at this facility I would be happy to have my mom at this facility. Or dad lol. Covid has been hard but they have done a wonderful job a wonderful job with caring for the patients and their families! I look forward to a continued relationship with this facility.

I would like to thank the staff at Sunrise/Five Forks for their hard work. They work hard to make the residents comfortable. My father has been there almost a year. He has had health issues that seem one after another. The nursing staff has worked hard on the various issues to get the medications correct. The Events Coordinator has worked to get participation from my Dad. Dining room maintains a special diabetic diet that he enjoys. Not to mention the receptionist, cleaning staff and hall workers that keep him straight. Also a plus that the coordinator checks in with me periodically just to make sure all is good. It's nice to rest assured he is enjoying life to the fullest, in a caring and safe environment.

My Mom has been a resident here for nearly a month (May 2022). So far, so good. Mom enjoys being around folks her age: she already has a group of friends as if they have known each other for years! There are daily activities; restaurant-style dining for 3 meals a day with a broad menu; a Sunporch with rocking chairs; and on and on.

My mother is 91. She was living independently at her own home until disaster struck and she tripped and broke her collar bone at her mailbox. That happened and so much more... topping it all off with the COVID VIRUS.Sunrise at Five Forks is a lot like its name- this place is bright and cheerful. When you enter it feels like a happy holiday. All of the staff are kind, loving, and tend to my mom and the other people who live there with utmost care, compassion, and empathy. My mom is comfortable and safe. I love Sunrise and the employees who by the grace of God have the capability and desire to help others. They are diamonds in the rough.

My Mom has lived at Sunrise at Five Forks since 2004. She loves it there. The staff is wonderful with evaluating what residents need and meeting those needs. The fellowship between residents and between residents and staff has been especially meaningful to my Mom. This place is her home and these people are her 2nd family. I cannot recommend highly enough!

My father has been a resident at Sunrise at Five Forks for 2 years. He enjoys the social activities, especially bingo and playing cards with friends. He also loves the meals that are served. Covid restrictions were difficult for residents, families and staff, but everyone tried to make the best of the situation, while keeping the residents safe. Communication with families is excellent. The staff is also very friendly.

My mother lived here in the Memory Care section (Terrace Level) from Aug 2019 until she passed in June 2020. My father could not have chosen a better place and based on our experience, I dont hesitate to recommend it. The staff and the quality of care was exceptional both for my mothers needs and ours as a family. She was treated lovingly as an individual and always with dignity. She was carefully monitored and the staff communicated well with each other and with us. The slightest change in her condition was noted and treated appropriately. We visited every day, always welcomed warmly, until COVID-19 rules restricted visitation. Even then, the staff were in touch with us almost daily. After my mother passed, we were very touched that the section director and one of my mothers nurses came to her memorial service. Several staff members called in the weeks following to check on my father. This sincere concern for us just underscored our belief that my father chose the best place for my mother when the hard decision was made. If you are looking for a safe place for your loved one, especially in need of memory care, take a look.
