My mother had a recent visit, an illness had made it so no food tasted good to her, but the good prepared by Park View Post Acute was some of the first that she said tasted good in over a month. This skilled care facility is in a nice setting and is a nice part of Santa Rosa. A good place for a short term stay to care for a loved one.

My mum was in rehabilitation at Park View Post Acute again for the second time , with the staffs, nurses, caregivers, social workers, and doctors being very helpful with my mom care and her stay there. They update me with everything going on with her, and the most part love the place where we can go in and out anytime we want and bring her favorite food from home or restaurants.

My father in law spent two weeks at Park View and I have nothing but praise for the wonderful care he received from the nursing staff to the OT and AMAZING PT services. Jasmine, the social worker, was always responsive to my phone calls and questions. My father in law gained weight while there because he really enjoyed the food 🙂 The staff were always helpful and attentive to us when we visited and to my father in law. Keep up the amazing work and the great care you give to the vulnerable population you serve.

The staff is wonderful, skilled and compassionate. The food is much better than normal hospital food. There is always fresh vegetables and fresh fruit. It’s very impressive that they can deliver real fried eggs for breakfast , still warm to that many patients. Most facilities have switched to liquid eggs so hats off to Parkview . The whole staff overall is outstanding, but extra standouts in nursing were Malia, Ryan, Ray and Suzette. In Physical Therapy try to get Kate or Sisley. In occupational therapy, Maya is terrific. Best CNA,s were Philoberto and Maria. Mattie and Savi were really helpful in social services. I believe Parkview is the very best facility in Sonoma County and highly recommend it. Be sure to ask for the front of the buildings where there are only two beds in the room. The back rooms have three beds to the room

My husband needed extra physical therapy help after hospitalization. Park View Post Acute was one of his options as they work with Kaiser patients. A few years ago he had a friend that was a patient at Park View and Eddie would visit him. In addition he had worked over 20 plus years for a local florist and had delivered flowers frequently to patients who were at Park View. He always said the facility was bright, clean and very friendly. When Kaiser informed him of his options for post acute facilities he immediately said Park View. He loved all the staff. They all joked with him and were helpful and intent on helping him and all the patients. He/We highly recommend Park View Post Acute if you ever need a little extra help.

Park View is amazing! Skilled nursing and nursing homes don’t have a very good reputation. Some are downright scary, so I was frightened when my nephew was sent there after a terrible fall. He will be there for 3-3 1/2 months. After experiencing the care & relationship building there, I was sold!!! There are lots of staff there, and everyone smiles & says hi to you as you go by whether they know you or not. The people who work directly with your patient establish relationships with themand their family!!! I don’t know how I got so lucky to have my nephew at Park View!! Feeling blessed!!

I found Parkview to be an excellent and accommodating facility. They went above and beyond to treat Joyce with respect. Joyce is a very demanding and critical individual. However I found that the care providers, on every level went out of their way to provide her with the comfort that she demanded. Especially when she was not understanding of her treatment. I was also very grateful and impressed by the services that didn't directly have to do with her personal health care. The Social Worker and placement counselor went above and beyond my expectations. I was able to facilitate her placement after her discharge with the help Parkview provided. I would also like to say that as a family member, and power of attorney, I was given all the information, and tools to create a seamless transition from post acute care to board and care. They were kind and helpful to me dealing with the many working parts that necessitated her healthcare journey. I can't imagine what it would have been like without the wonderful people at Parkview. I Thank them all from the bottom of my heart! Thank you very much! At the risk of to much information I must point out that with my need to care for her special needs daughter, and imminent loss of her independence as well as the need to move her to another county, they were patient and willing to see us through. For me I must mention that her care and progress at Parkview gave her daughter the comfort she needed to understand that her mother was recovering! Once again let me thank Parkview and everyone there! I am Teri Lopez and I would definitely recommend Parkview to anyone finding themselves needing the Services provided.

My 92-year-old mother tripped and broke her hip. After surgery was completed she was sent to Parkview acute care. Over the years, I have been to several nursing home type facilities, so I was a little concerned that she would be sitting there doing nothing most of the time. To my great surprise and happiness, she was well attended by the staff with short wait times for help. Physical therapy and occupational therapy were both very informative and cared about her improving so she could go home. Of all the nursing facilities, I have to say this was the cleanest and best staff of all I have been in. If this type of care is needed, I highly recommend Park View Acute Care

I got nothing to compare it to, but people were nice enough. It was a struggle to get my Hickman lines flushed and the bandage changed. I had to be re-hospitalized because I am immunocompromised and caught a cold from someone. Basically it's sitting around surrounded by seniors who have mental issues. The sundowners scream at night. Food is terrible, I basically ordered Door Dash everyday. I heard it's one of the better Skilled Nursing Homes but pray you don't find yourself in one. It sucks.

I can't say enough good things about Park View Post Acute. From the moment Mom arrived, she was treated with kindness, professionalism, and top-notch care. The nursing staff is attentive and compassionate, always making sure she was comfortable and had everything she needed. The physical and occupational therapy teams are truly outstanding. They designed a personalized rehabilitation plan that helped her regain her strength and independence faster than I expected. Their encouragement and expertise made all the difference in her recovery. The facility itself is clean, well-maintained, and welcoming. The meals were nutritious and well-prepared, and there were plenty of activities to keep residents engaged and social. What really stood out to me was the staff’s dedication to their patients. They took the time to listen, support, and motivate her throughout her stay. Thanks to their excellent care, she was able to return home feeling confident in her recovery. I highly recommend Park View Post Acute to anyone in need of skilled nursing or rehabilitation services. They truly go above and beyond!

Hello, my name is Shawn Manica and I'm receiving care at Park View Post Acute facility. I want to take a moment to give HIGH PRAISES to my exceptional CNA, Araceli D. I've been here since February 4, 2025 and I have the pleasure of witnessing, Aracelis work ethic. It's absolutely fanominal especially with me. She's very very attentive, kind, supportive to other residents, staff and especially me! I really hope she gets the recognition she so greatly deserves. I absolutely love her! She is one very hard worker and I express this to her everyday she's assigned to me. I can not say one discouraging word about her! I simply think she is AMAZING!! PLEASE, show her the recognition she deserves! Thank you so much for the opportunity to say something about, Araceli D. She's a wonderful person and a CNA to me and others. My mother had a medical event towards the end of 2024 and after a hospital stay, we had her go to Park View. They were great with her rehabilitation. The medical staff really cares about the work and my mom felt supported. During her roughly three week stay, she got stronger, ate healthier, and regained capacity across the board. I recommend Park View if you have options and they have availability for a SNF. Challenges: It was not always easy to get to folks by calling in. If you are local and can be there in person, this was not an issue. Parking in the lot is also a challenge even though it seems like there is ample parking, the tennis club and the other commercial storefronts are clearly fighting to keep enough parking for their needs too, so leave time for that especially if you are visiting mid-day. You can end-up circling and waiting for a while.

It’s hard for people to be in the position of needing post acute care. Our medical system does not provide much, and in fact we had to advocate really hard for this. I think Park View is reasonable and has a lot of good staff. The food is good and the place is clean. My friend is getting what she needs: a safe place to learn how to safely walk and handle herself. Her team communicated her progress and plans to her and her loved ones very well.

All staff are very kind and easy to get along with. They're taking very good care of my mother. Night CNA's do not respond to call lights promptly. My mother has been told to be quiet and then just walk out of moms room. Therapists are extremely sensitive and helpful giving mom help walking. I've called several times to speak to nurses station and no one answers the front desk. I let it ring 20-30 times several times a night. Someone should be at the front desk to answer the phone. My mother has had bad virus and I couldn't even check on her.

My husband spent several months at Park View Post Acute. The facility is light and bright with a nice outdoor patio where my husband and I could walk and talk. My husband specifically requested hard boiled eggs with every meal which they provided. He was satisfied with the food overall. The nurses, caregivers, and therapists were all very nice. My husband and I had arranged to get married while he was staying at the facility. The Marketing and Admissions Director Ricci Erfemejia went above and beyond to help make sure it happened in spite of my husband's tenuous physical condition. And when we returned from the Church, Ricci presented us with a beautiful card that had been signed by many of the staff who had been working with my husband during his stay there. He even took a few photos of us together while we were still basking in the joy of the moment. Those photos turned out to be particularly special because my husband passed very unexpectedly only one month later. I will always be grateful to Ricci for making our special day even more special than I expected. I pray that God will reward Ricci and all the staff for the good work that they do on behalf of their clients.

I speak my truth

My step father is going here tomorrow. He suffered a stroke and needs rehab. Unfortunately he’s not strong enough for acute rehab. I am praying to God this place is better than Apple Valley where he has gone after a previous stroke. I will now discuss why Apple Valley is not an option but it was horrible for both my parents - I won’t set foot in that place as I’m sure it would cause a ptsd episode of sorts. I decided to write this review prior to his stay and then update as we go through his care at the Park View facility. I will try anything to help him get better care. I am just completely saddened that these facilities all seem to be at such a lower level compared to Sutter hospital, which is where they are referred from. This is the care designated for our elderly human beings It’s a complete travesty that this is how people are treated at their most vulnerable stage in life. What is wrong with US? Please Park View, take care of my family member. He is a Vietnam Vet and a kind soul and deserves good care and treatment. I’ll be coming in frequently to see him and will update this review as I see the care he actually receives. Thank you! This rehab was much better than apple valley in sebastopol. Even if you live in sebastopol, I recommend making the drive. Apple valley is literally a nightmare and at least this facility shows compassion and kindness to their patients. I only gave four stars because I feel all these facilities need vast improvement. Unfortunately our societies doesn’t care for the elderly the way they should and it’s so sad that the low standards are simply excepted.
