Management and staff are very nice and pretend they care about you. When you actually ask for help or need assistance they promise you this that and the other but never follow up or follow through. They intimidate, harass and bully those who stand up for themselves. The ratings they have online are mostly from staff and residents they forced into writing so they could have a higher rating. Management are aware of drugs, parties, homeless people living in the facility and choose to look the other way. Out of the 10-12 members of management are family. Sisters, cousins and spouses. The assistant director usually has her young children with her, leaving little time for her to follow up with her job. But plenty of time to carry on private resident meetings or concerns in from of her small children who see and hear things they shouldn’t. PHI and HiPAA always being discussed. When you complain to anyone about an issue, you are bullied and harassed until you shut up. And don’t ever complain again. Only then are you once again treated as human. Management that are not located on site, corporate offices and such, are related to the ones here and prove that it does no good to complain or expect a decent level of care. You are expected to shut up. Take care of yourself and be a good girl or boy.

My comment is regarding Alec at Avista. He handled a big issue in my brother's room today with professionalism and compassion with as little effect on my brother as possible. Quite an asset to this facility. Thank you Alec. Job well done.

Update 8/19/2025. My husband passed Tuesday evening. The staff was so caring and I knew his passing was a lost for them has well. Please know I knew they cared for him like he was their family member. Update 6/4/2025. Thank you to CJ and Ami for looking out for my husband and thinking of us has family. We're so blessed to have Avista. What can I say so far this place has been great! I feel like they really care about my husband. I'm kind of glad he's somewhere safe since I can't take care of him anymore. It's close to home the staff is wonderful. There's always residents sitting outside or in the lobby and no one really has ever said anything bad about the place I like it cuz it is close to my house. Also I'd like to say Ryan that covers the front desk to screen us He's a real nice young man

I really think this place is a home for me since they really go out of their reach to help you get settled and get you to feel like it's your home. It really has fit my needs beyond what folks think I need. I have a tank of 3 hermit crabs and wanting to get a bigger tank along with 2-3 more crabs as a emotional support animal. We went to visit one of our friends on the second floor in the memory care unit, and we noticed how friendly and thoughtful all of the staff members were to the residents. Especially Pam Davis, who arranged a special ice cream sundae event for them, which they all loved! Everyone seemed happy to be there. Keep up the good work!

We put my mom Jolene here about 6 months ago. Her previous last three experiences were very bad. Then we came here! Mom and I love this place! From the staff Ryan, Rob, Janet, Shannon, all the med techs, shower personnel, cafeteria help we get nothing but kindness and assistance. The apartment is fantastic! The food very good! Having a med tech on every floor gives me piece of mind! I would highly recommend this facility where kindness is their motto and it shows every day!

I never really had a real family growing up, but being here at Avista has felt like the closest thing to one. I've been through a lot in my past, and finding a place where I feel safe, cared for, and understood means everything to me. I trust the staff here with my care, and that trust is not given lightly. The team does a great job making sure my appointments are scheduled and that I get to them on time. It’s one less thing I have to worry about, and I truly appreciate that. The breakfast food is always something to look forward to, and I especially enjoy the little social events like popcorn socials and root beer floats”they bring joy and a sense of community. I live in a studio room, and while you might think it would feel small, it doesn’t at all. It comfortably fits my bed set, recliner with tv stand and a dining table set. It’s cozy but still gives me space to feel at home. If I had one wish, it would be to have more chances to play the Mexican version of bingo”it’s something I really enjoy and would love to see more of. Overall, Avista has become a place where I feel valued and cared for. It’s more than just a senior living facility”it’s home.

It’s a tough reality when you realize you can’t provide the level of care needed to a loved one. My mom was diagnosed with Alzheimer’s and it changed everything. It changed her and it changed me. She was unable to manage basic every day living tasks required to live on her own. I was frightened she would burn her house down or wander out to the street and get lost or hurt. She didn’t know I was her daughter and worse often she didn’t like me. Trying to help her only made her angry. I was ashamed of myself for the years of wondering how anyone could put their parent or parents in an assisted living or memory care facility. I had to admit to myself I could neither provide my mom the care she needed, nor could I keep her safe. It made my heart hurt thinking of her future and knowing nobody with Alzheimer’s ever gets better (but hopefully someday). I toured several facilities and many had nice assisted living options but when I got to the behind the scenes locked door of memory care they were dismal and depressing. When I came to Avista it was a different story. You can feel the positive energy when you first enter the lobby. I didn’t allow myself to get my hopes up as I went to the 2nd floor. What I found as I exited the elevator was a memory care unit that was bright and inviting, the residents where being attended to in a caring manner. I was quite impressed. Despite the guilt and sadness I felt I knew I had to do what was necessary. There was no hesitation about moving my mom into the memory care unit at Avista. Yes, it was difficult initially knowing my mom and others required various levels of care 24/7. However, I witnessed the miracle of kindness, compassion and strength shown by C.J. Thomas the director of the memory care unit. She manages a wonderful staff that performs every kind of task you can imagine (and those you don’t even want to think about). C.J. is right there leading by example helping residents, dispensing medications, showering and dressing residents as needed and doing whatever needs doing. She’s smart and she shares her insight with the staff and explains how and why things need to be done. My mom was in memory care for 18 months, unfortunately her health declined and she passed just recently; less than 2 weeks ago. Knowing my mom was safe and treated with compassion, respect and dignity means everything to me. I’m thankful to Avista for their wonderful facility and forever grateful to C.J. for what I call a mini world where everyone comes together and you know it’s a safe place. She helped me through this journey in ways I could never have imagined, just as she’s done for so many other families.

The memory care department at Avista is amazing. The care staff are loving, caring and Energetic! The second and last Tuesday of the month they have a couple come in and perform the drum circle where the residents have their own Tambourines As well as bangles it is a fun time. If you’re ever in the neighborhood, please come and do a tour. You will love what you see.

My mother is a resident here. She's lived here a few years. She is in her 80s and has severe cognitive issues. The dementia requires her to live in the memory care unit, which is password protected to keep our loved ones safe. Her memory issues and cognitive decline were too much for my brother or I to handle safely, so this was the best place for her to be. When you arrive to the facility you go to the front desk and sign in via tablet that takes your picture and prints out a name badge. The last event I went to was the Thanksgiving meal. They had a buffet style setup where families could sit and eat together.

My mom lives here and we are so happy with her care here. She has lived in a couple of other facilities in town and she really disliked them for whatever reasons, but she is nice and chill here and loves the staff. Yes it looks a little funky in places, it’s a typical older Phoenix building and to be expected with the price point. They have a super beautiful gazebo outdoors for residents to enjoy the weather. Def recommend.

when the people you care about the most reach the place in their lives where they’re not able to do things on their own, it can be an extremely sensitive & nerve-racking thing choosing a place that can help to take care of them. I have had zero complaints with any of the staff or care that we have experienced. One member that stands out to me is Rob. He’s always been incredibly kind and gracious to me and my family since day one. Glad to know him, and glad to know my family is in good hands!

We were new to this experience and needed to find a quality facility with good people and we needed it quickly. Our search ended at Avista and it was largely due to meeting Rob Bailey. It’s a very nice facility and the staff is great but Rob went above and beyond to help us get through numerous challenges that our situation presented and always with grace. I was also impressed with his interaction with everyone we encountered on several visits there. It’s obvious that he takes a personal interest in and cares deeply for those that call Avista home. I could not recommend more highly.

After losing my dad in March and struggling harder with her depression, anxiety and mobility, living in her house became much more than she could cope with. So we found Avista. She's been here since October and making some improvements. I love the staff!! They're friendly, courteous and willing to help. The place is clean. I've been to a few places over the years when my dad went in for respite care and the first thing you notice is the smell especially near the residents rooms. Not here, it always smells like they just cleaned. The Assisted Living Apartments are nice, just the right size for someone with mobility issues. Mom doesn't feel so overwhelmed in it. The food is good also. Mom says she enjoys eating there and lets the cook know. Some of the staff knows her and gets her coffee just the way she likes it before she sits down. Rob is super friendly and quick to help you. Joanna also is super friendly and quick to get things done on her end. My mom is pleased with living here and is glad we found Avista. In turn that makes my brother and I feel good about Avista. They've made the transition from living on her own in a house to a smaller apartment much more bearable. For that I am grateful!! THANK YOU TO EVERYONE AT AVISTA!!

Two years ago I was introduced to North Mountain when friend of mine became a resident there. I was very impressed with the Facility, Staff and other Residents when I visited my friend Kevin. Yesterday, I viewed the facilities with one of my agents to show her the options available to one of her senior tenants. Robert was very gracious to talk to us, and very knowledgeable about the systems in place to help the people who need a facility like this. Once again I found the Staff friendly, and Group living facilities Clean, Cheerful and generally Awesome.

This is a low income property that takes ALTCS through ACCCHS. My father has lived there for almost 3 years. Since he has lived there we've had so many issues I couldn't even begin to capture them all. The big ones are mismanagement of his medication, not having enough staff to follow through on things they committ to. Ie: "I'll be right back with your XYZ" and they never return. They provide residents with a call pendent for when they need assistance and more often than not, no one comes when you push the button. My father fell in his room, he pushed the button and laid on the floor for over an hour with no assistance. The rooms are the best part of this facility because every resident gets their own room and restroom. Also, Becky is the best caregiver at this site - she actually cares about my dad . Outside of that, this place is equivalent with a low quality motel. We've even had to file a complaint against a caregiver who was too rough with my father and if you told him he was being rough he would fight with you. Keep looking for a place for your parent, this isn't where you want them.
