The sisters of Phi Alpha Omega visited us on Saturday and treated us to stations of Spa, Craft, and Music! The spa ladies gave us manicures and facials; even sons glue-on nails! The craft station featured decorating cookies and donuts, which they provided. The goodies were dated with coffee which they also brought with them. The entertainment station featured a Karaoke machine. For the safety of all, I did not sing but others had a wonderful time. It was delightful to have young faces and young ideas. I think both groups had fun and learned a lot about the others. I hope that this sort of experience will be more frequent as it is enriching to all.. Hay Day! Visit Sali’s farm!

This facility treated my father with dignity. My father’s face lit up when he saw certain employees. The therapist are very caring and looked for ways to help my Dad thrive. Lilac at Bayview in St Augustine's, it’s skilled nursing facility came through for me when I needed to take care of my mom in Atlanta. They offered me a room for my spouse to stay while I was away instead of having to worry about him going with me. My husband fell 7 months ago & has become wheelchair bound. This facility did a good job of taking care of him & letting me know his status. They let me know he wasn’t eating very well & a nurse friend of ours came & got him to eat. After he returned home the liaison, Mary Buckner, for Lila’s Bayview, kept In touch to make sure he was ok & we were satisfied with the service he received. Truly a good experience. I recommend this facility & will use again when necessary. Rich’s wife.

I recently had care for rehab following at least 3 heart attacks. I had surgery at Flagler and Dr put a pacemaker. I must say that Lilac Bayview is the most comprehensive treatment I've ever experienced. The entire staff work together in a magical flow of kindness and compassion. The food is home made and so delicious. They treatment of residents is "over the top", exceptional, always smiling. I am almost blown away with the patience, gentle touch, and eager to help th e residents. Everything is very clean, quiet, and well done. Physical Therapy helped tremendously wit my body language, pain, and so much compassion. I will go ther in a flash if I need more rehabilitation. Thank you , Bayview for all that you give and all that you do for myself and and all residents. You are so kind. May many Blessings fall upon you

My grandfather went for physical therapy after a hip replacement and I want to make it clear all three of these stars are for Anton and I would give him 5 stars personally and the facility and care only 2. Care was kind of slow but I get it as the nurses are dealing with a ton of patients. My actual gripe is that my grandfather's staples were never removed and a week later had the be removed by a home health nurse and he is now in sepsis with CDiff at the hospital. Also they have called me and used a robot to try and get me to review them.

This review is long overdue! I would like to tell you about Re Shane Lewis. This nursing supervisor works every weekend at around 16 hours each day! Sometimes she even picks up shifts and works during the week as well! She is someone that no matter what you go to her with or what you ask her for or tell her what you need she does everything in her power to take care of it! 99% of the time it is taken care of! She comes to work with the biggest smile on her face for everyone she sees! I think it’s very important for people in a skilled nursing facility to be greater with a smile and spoken to as much as possible, because I believe it makes them feel better and feel like they are somebody! That’s Re Shane!! Also, she is one of the hardest workers in this facility! She is constantly doing something that needs to be done! There are so many wonderful things I could say about her, but I would fill up the whole review section for this facility! What I will say is, she is a wonderful human being, and she truly cares for everyone here! I truly don’t know what I would do without her! She is a tremendous asset to Lilac At Bayview!! I think the world of her as she deserves it !!!!!

The CNA staff were very helpful with my mother’s visit there. I felt as though the physical therapy department and the occupational therapy department were very lax in there getting my mother back into shape. The head nurses did a good job, but we’re not able to control the proper care of my mother it seemed as though they had too many patients and not enough staff, or the staff was not willing to participate in the Care as frequently as needed. The food was absolutely terrible in the kitchen had no idea what was going on. Medications were delivered on the wrong schedule and I had to constantly stay on them about that. my mother was very happy with the nurses when they were in the room with her, but the delay time was very long between visits. they promised me that she would get a bath twice a week in the shower and she never received one in shower bath the whole time she was there for three weeks. To me this is unacceptable. They did do in the bed, wipe down on a regular basis.

She truly cares about her patience and does everything she can in her power to make sure that we’re very well taken care of! I was a patient at Bayview for about 10 weeks. I cannot thank the nursing staff enough for the care they provided. Nurse Neenee, Nurse Q, Nurse Michelle, and Nurse Betty were the best. The CNA's , Tiffany (T), Betty, Jazz, Jasmine, Precious and Kim went beyond to take care of me. I could not even lift my toes, could not walk or get out of bed. So they were there for me 24 hours a day. Julia and Jess were amazing in PT. They got me walking again and with a lot of patience. I am now on a cane now , thanks to them. And of course I cannot forget Anton. He was always there for me when I needed someone and everyone else was busy. Anton, you were my hero. The only bad thing, was the Kitchen.. Food cold, frozen, and never hot or right. Too Bad, most of us had food brought in.

My Son who has been a resident at Bayview for a few months always feels comfortable with the staff. He says they treat him with respect . I go often and see the staff interact and am very pleased my Son is well cared for. He needs a lot of care physically and mentally and staff is always there for him. Just make sure you label all clothing and blankets or they get misplaced. Other than that feel comfortable leaving your loved one in their care. Always keep in mind it's not home and job the staff has is very difficult kindness and patience for them is so appreciated., they're always doing the very best they can. Staff was helpful. Do not trust Dr. [Name Removed]. He never signed off on death certificate for my mother. Irresponsible and negligent after a week of her passing. Hospice Dr. responded immediately. It took a week before she was cremated because of his lack of caring for his patients.

My father has been a patient in your facility for a few weeks now. It was brought to my attention that his request for a copy of his medical record upon discharge tomorrow was declined by someone named ‘Robin’ from medical records, who came to speak with him. My father communicated to me that ‘Robin’ said his records “belong to Bayview” and it would “take weeks to get a copy” to his healthcare providers. However, numerous regulations, including HIPPA, provide my father with the legal right to a copy of his record upon request. I called tto speak with anyone in administration. The woman who called me back identified herself only as “Shelley” and sounded painfully aware that she was stepping into a patient’s family-member complaint. I asked how she was, and her response was a deeply sarcastic “fabulous”. I was immediately confused by her complete lack of professionalism and flippant response. The call went downhill from there. As a medical professional, licensed in two states, I am familiar with hostility from employees who are on-call and have to make actual calls, even though it is part of their job. Anyway, I was in no way prepared for a nasty tone coming from someone who is literally responsible for the welfare of my elderly father. I explained to Ms. Nutter the issue with my father’s request for a copy of his medical record and added that I had already contacted The Joint Commission regarding numerous other violations I’d been made aware of since my father’s stay began. Ms. Nutter responded by laughing, “I don’t answer to The Joint Commission”. I think you get the gist with Shelley Nutter, who’s last name I had to look up on your website because she is THAT professional.. I have no doubt her passive aggressive unprofessional behavior isn’t new on this day. Is that really the way you want your business represented? I will obviously file complaints with any of your accreditors in St. John’s County, the state of Florida and anyone else who will listen. I have no doubt that there are wonderful people who work for you. My dad apparently met a few. However, your facility is only as strong as your weakest link.
