My mom stayed at Autumn Lake’s rehab facility for about 2 weeks. I cannot say enough about the staff. When my mom first arrived she could barely take a couple steps. Their physical and occupational therapists, with their expertise and kind inspiring words, gave my mom the motivation she needed to recover. The nurses and CNAs were so compassionate. I am so thankful for the care my mom received at this facility. Special thanks to Latika, Rose, Terese, Tabitha, Gina, Heidi, Alex, and Kari. You all certainly made a difference. We cannot thank you enough!

My aunt was in the facility for rehab recently discharged home. Phenomenal care was given. You’re greeted with a smile & feel wanted the moment you walk in. The nurses care is exceptional. The communication is great. I can’t say enough good things about Autumn lake at Greenfield. We’re happy we decided to go with this rehab.

My loved one is staying in the newer part of Autumn Lakes for over a month of rehab and IV antibiotics. This place is amazing. It has a wonderful atmosphere, it is very clean, quiet and the rooms are large with a cozy feel. I visit every day at different times mostly in the evening and every staff member I have met has been nothing but extremely kind and so caring to my aunt and to me. The level of care she is getting is top notch, from her room care, IV’s, physical therapy, food, and other daily needs, it all has been outstanding. My aunt herself has said how happy she is staying there. A special shout out to the wonderful evening nurses and staff who I see daily and make it such a welcome warm environment. Thank you Autumn Lakes for all you do!

It was very difficult to select a place for our loved one for hospice. Autumn Lake staff did not disappoint. The Director of nursing, The nursing staff, The Housekeeping, the receptionist and the business office were wonderful to work with. The admissions coordinator was not as helpful as she could’ve been, but as far as the facility and the staffing they were wonderful. No complaints.

This facility has a rock star clinical team, who are dedicated to delivering the best quality of care to their residents. The facility is clean and free of odor. The facility has staff longevity, which is a hard find in some facilities. It happens in every facility, when an employee is asked to leave a facility, for one reason or another, they never take accountability for their own actions and start writing negative reviews under alias names. It's so sad!

This place is beautiful and comfortable. First impression of the facilities and surroundings was a WOW. I walked in and there was no one around for at least 10 minutes, There was no one to greet me. Later I visited a relative every day and I can say I was not greeted, mostly ignored as I continued to walk to the room. Not even acknowledged, no smiles. Sad to say it totally disappointed me.

I am very happy that my father in law in was able to go to autumn lake for rehab. There were a few bumps in the road, but the nurses and CNA’s were very kind. I was able to bring in food to my dads room each night and store snacks in the mini fridge. At times the staffing was short, but that is the reality with COVID these days. My father in law was able to have a recliner which really helped him sleep at night since he doesn’t sleep in a bed normally. I would recommend this facility again if he needed care.

Autumn lake is truly a great health care facility. When looking for the proper care from my family member I wanted the best care and services I could possibly provide. Autumn lake health care was the perfect fit. The staff was so pleasant and respectful to my family. The rehab experience was great and my family member is doing amazing with all the help that he was provided.
