I came across the Azura T.I.M.E. program at a Memory Cafe they sponsored. It was a great introduction to the activities done at the TIME program. This program is exactly what my Mom needed...socialization, challenging puzzlers, games, songs, crafts and just plain fun! The compassion and joyfulness of the leader creates a positive and inviting experience for everyone. Mom looks forward to attending TIME and that is most important. She really enjoys it! I know she is in a loving and accepting place. That gives me peace of mind. Thank you, Azura!

I cannot promote a better place for your loved ones. My brother and I had looked at so many facilities and thought we found the perfect place until I Stumbled onto Azura in Brookfield. My mom has never been happier. The staff is the best, and the activities are engaging and fun. The residents are made to feel that this is their home, and not a facility. If interested you can sign up for an app to keep you updated on daily activities. Everyone here is top notch and I feel that my mom is happy, safe, well taken care of and loved. I give Azura at Brookfield an Attttt Ebony the house manager is the best, when ever i need help assistance or support I can count on Ebony to help me or put my anxiety at peace. if i n need to just talk or have questions from Ebony she is always available and in a good mood she never makes me feel as though i am bothering her with anything. Ebony always has a smile on her face and joy in her heart which quickly puts me at ease. I feel as though the staff truly engage with the residents and treat them like family which always puts me at peace knowing my mom is loved. Having to put my mother in memory care has been a vary painful decision for our family to make, but realized very soon we picked the right place for her. I have been most satisfied with her care and help since our new house manager has arrived i feel as though my communication since Ebony as arrived has truly been the best. thank you everyone for everything that you do you are all angels that walk on this earth bless you all lots of love Robin daughter of Jean Creel

Eboney, the house manager at Elmbrook where my aunt resides, goes above and beyond. I am my aunt's guardian and POA but live out of state. Eboney, has done a wonderful job keeping me updated and informed as well as keeping my engaged. Eboney and the staff exhibit an abundance of patience and understanding as my aunt's dementia increases, causing paranoia and difficult mood shifts.

My father John has been residing at Azura Brookfield for almost 2 years. I feel he is cared for very well, and staff genuinely care for him! Eboney is more than phenomenal! Any question or concerns I have ever had was discussed immediately. She provides wonderful reports and is always smiling. The communication is wonderful and very much appreciated, as I have never asked for reports, yet she is very happy to provide and give dad hugs and love when I'm not there physically with him. Our mother has been at Azura of Brookfield for 3+ years. She initially moved in during Covid and Azura misrepresented everything from the "Executive Chef" to activities offered (which were essentially non-existent for the first 18 months) and remain incredibly basic. The first 2 years, I would rate them at 0 stars. There were 5-6 executive directors, they had no handicapped access buttons, and the staff was unable and uninterested in performing the baseline duties of their jobs. Multiple complaints were filed with the WI DHS and families moved loved ones out. We only refrained from moving our mother out of concern that her memory may decline from the change in environment and we wanted to believe Azura would improve as we actively worked to help make changes. I have raised them to a 3 star rating primarily because they now offer outings, made a great suggestion for supportive services, and most of all because of Tyeisha, the Broadway House Manager, who is a 5 star employee. No matter how frustrated we are, Tyeisha always listens and responds with grace and professionalism. Any request she follows through on immediately and staff have improved under her hiring. One of my biggest complaints is that phones are rarely answered and all calls are forwarded to the House Manager 24 hours a day if no one answers; which again, she is always so gracious about and happily responds to. However, why Azura cannot hire a receptionist or service that can answer the phone is perplexing. The phone rings to the kitchen or lobby area and not to staff phones. It has been a complaint since Day 1. If we had to do it all over, we would choose a facility with a larger number of residents. My mom moved from a place where 60-80 residents attended dinner to 5-8 attending dinner at Azura because of the "home" concept. Without question, Tyeisha and the outside support service are the best parts of our experience with Azura.

My mom was fortunate to call Brookfield Azura "home" for her last 2 1/2 years. Years filled with new friendships, lively dinners and, her favorite, sitting on the front porch bench visiting with residents and family. Thanks to Azura staff, Bluestone Physicians Group and St. Croix Hospice she was able to remain in the comfort of her cozy, familiar room even as her health failed. Our family is forever grateful.

Azura has been an amazing new chapter in my dad's life. Since moving him in April, he is truly "living his best life". The rooms are large and warmly inviting to help make it feel more like home. The staff have been attentive and friendly to him and to our family when visiting. The activity director (Jessica) is a true gem at Azura. Her connection with my dad and other residents is unmatched. He loves participating in games and music, working in the garden and going on outings, all of which keep him active, occupied and very happy. It has given our family comfort knowing he is so cared for. Thank you to the entire staff for everything you do.

I chose Azura because of its newness and proximity to my home, Being new , beautiful facility and location, but that newness comes with problems, inexperienced staff, empty rooms you provide everything except bed one night table , minimal professional staff, one daytime overworked nurse, inexperienced nursing aids, mean well poorly supervised and back up. Pricing starts low till they get you in, then goes up monthly, currently paying $10300 per mo. Started at $7600, 8 mo. ago, second and third shift 3 people to take care of 27 residents. Had my wife fall out of bed, crack her forehead bled all over floor wasn't found till 7am I took her to hospital, not checked for 16 hrs, no records kept

Our mom came to Brookfield Azura with some minor memory issues but worsened over relatively short time, as did her heart health. The staff has been great to work with in understanding her condition and its worsening, getting us connected to explore and then sign up for hospice service, and now to transfer her to the building next door for Memory Care. It is not an easy journey; it's been helpful to have their approachable staff with us.

I am so happy I found Azura and was able to place my mom here! My mom is a very active person and Azura makes sure to plan lots of activities for all the residents. I'd love to see even more outings, as I think that really helps elevate spirits for all. Recently, they fulfilled my mom's wish to play tennis again through their MOSAIC Dreams program, and the day and outing were fabulous - heartfelt thanks to everyone who helped make that a reality! (Now she wants to go golfing - any takers?) My only wish for all residential care facilities would be a more stable staffing situation, but these are the times we live in and everyone is doing the best they can. Thank you Azura - my mom now considers you "home".

Azura is a great facility. The staff and Amy were nothing but friendly and helpful from the beginning. My family and I were very grateful they accepted my mom there as a resident. She did need a high level of care and they were able to help her out in so many ways! The only reason I'm giving a 4 star is during the intake process it was missed that my mom was taking chemo pills once a month. The facility didn't allow this due to the safety of the staff. I understand all of this however it meant my mom's stay was short lived. Azura was understanding and did work with us on the medication miss.

I visited this facility after watching it be built from the ground up and am so impressed with the beautiful welcoming physical building-warm colors, decor, and era collections on the walls that would resonate with residents. My interactions with staff were especially impressive. They seemed to be both professional and caring toward the residents, making them feel at home. I observed several seasonal activities going on as well as support for families who had someone in the last stages of life. I would not hesitate to refer anyone to check out Azura of Brookfield!

Azura of Brookfield is a beautiful and well-planned community. The individual "houses" are welcoming and have layouts that feel comfortable and more like home. A great deal of thought went into the design with a nice entry, lots of natural light, two secure outdoor spaces that are open to residents, good room sizes (including bathroom and shower) and large indoor resident spaces (living room, dining area, sun room, den). At all levels, directors, managers, nurses etc. were very responsive to questions and concerns. They paid attention, followed through and offered suggestions. I felt that my loved one received good care. As with so many businesses, staffing is a challenge. Azura managers are working to assemble caring and stable teams, but it is a struggle to find and keep the right people. While I felt that sometimes room cleaning and laundry, for example, could have been better, overall, Azura is a great place. If you are searching for a place for a family member who needs assisted or memory care, please consider Azura!

When our mother needed to transition from independent living to a memory care facility, we researched many of the top communities in the Metro Milwaukee area. The primary factors in making this choice included finding an environment in which she would be safe, engaged, happy and cared for by loving and professional caregivers. We have satisfied these criteria and more - for our mom at Azura in Brookfield. The day of the move-in, our mom was greeted warmly by staff at the door; staff from nursing, facility management, even the head chef came to say hello. While COVID precautions were in place during her initial stay, staff worked to make her transition as comfortable as possible. We have found staff to be available, responsive, professional and caring; communication with the care team has been excellent. The facility itself incorporates state of the art safety considerations into the environment, while the atmosphere is homey and comfortable. Transitioning a loved one to a care facility is an emotional and stressful experience. The entire team at Azura has been wonderful to work with and has minimized the stress of the move for both our mother and her family. Thus far, we could not be happier with our choice.

Justine, Amy, Brenda and Dan were so helpful getting everything all set up. The staff is very friendly, kind and attentive to our Mom. They do all they can to make it home sweet home. This review is long overdue! My Dad was one of the first residents to move into Azura Assisted Living and Memory Care Brookfield, and in the 5 months since moving in, my brothers and I can't be more pleased with the quality of care our dad has received! Justine made this difficult transition for our dad go smoothly. She has been there every step of the way, and is always available to answer our questions, give us advice, or give us words of encouragement. The entire staff (Justine, Amy, Suzette, Oscar, Brenda, Dan, Jeff, and everyone else who works here) has made our dad feel welcome. Our dad feels at home in this wonderful community! He loves participating in the various activities Azura offers everyday. Our dad loves singing, so his favorite weekly activity is participating in the Songbirds Choral Group, as well as Karaoke. He also loves to mingle with the staff and residents, and looks forward to Happy Hour every week! My brothers and I are so glad that we found Azura! We highly recommend this place for anyone who is going through the difficult process of finding a good place in which to move to move their loved one. Thank you, Azura Brookfield staff, for making this transition stress-free, and making a nice home for our dad!

I would recommend Azura Brookfield to anyone looking for quality care for a loved one. My mom lived at Azura Memory Care in Brookfield and she received excellent care there. I feel the caregivers truly cared for the well being of my mom and were very kind to her and us as family. Even during a pandemic, where we visited through a window, or in a room 6' apart, I got to know the staff and they were always kind and attentive to my moms need's. My mom had daily behavioral sun-downing and the caregivers handled this very well with efforts of comforting and redirecting her in a kind manner. I often worked directly with Justine at Azura. Justine's opinion, knowledge, and kindness in making us as a family aware of what my mom needed in each step of my moms journey was priceless. Often giving us news that was difficult to hear, but needed to be said. Please consider Azura in Brookfield when looking for care for a loved one. Jennif
