I moved my mom into her new home at Long Creek on 7/22/2024. I highly recommend Long Creek…tell them I sent you! I'm so happy I found Long Creek after looking for over 6 months. I used well-renowned agencies, local specialists, and friend referrals. I looked at other AL facilities, memory care facilities, and group homes. I found myself saying "this is the place" as soon as I walked in the front door. The atmosphere is immediately welcoming…clean, bright, cheery, cozy and warm. From the Executive Director and other community leaders, to the caregivers and administrative staff…I met several people with longevity at Long Creek, they were knowledgeable, pleasant, friendly, seemed happy to be there, they respect each other, and they are a cohesive team. I stayed for a meal service and it was quite impressive! They have daily chef specials as well as regular daily items that can be ordered if something different is desired. The selections are presented menu style, the residents get to 'place their order', and the food is delivered quickly. It's a restaurant style experience. The facility is clean, the people care about my mom and the other residents, and the communication has been better than any other facility I've experienced. Mention me as your referral! The stars are for the residents because the care is horrible and I'm not saying it because I'm not longer there I complained about it for a very long time to management…BUT THEY DONT CARE ONCE THEY GET THAT MOVE IN

Finding a place for your precious parents to live and be well taken care of is a big deal. Our sweet momma has Alzheimer's and our sweet daddy has been her full-time care taker until he had a stroke. After daddy's hospitalization and rehab stays, we tried living at home with 24 hour care, but after several weeks, we realized that was not a sustainable long term solution and the search was on! God led us to Long Creek and we are so thankful He did! Not only is the facility physically beautiful and clean, but the staff are so very kind and helpful! They have been a delight to work with and we are excited to have our sweet parents as part of this loving community!

Mom has been at LongCreek for almost 3 years. The caregivers are so loving to her! They all hug on her, and tell her they love her. I love that the staff prays together every morning and at family events!

When my 80 year old father was being release from a rehabilitation center and we were told he could not go home because he could not care for himself the hunt was on… we frantically set out to find a facility that he would like and fit both his needs and ours! We looked at several assisted living community and found Long Creek! Once we walked in… we knew this was the place for him! It is absolutely beautiful! We were treated by amazing staff that welcomed us with open arms! The establishment is spotless! Fresh water and cookies are offered daily at the entrance. They offer so much here! The rooms are very spacious! My dad had a 1 bedroom with living room kitchen and for handicap assessable bathroom and walk in closet as well as entry way closet! As for care… excellent! The patients have life alert necklace that call for a staff member any time. They also check on each patient every two hours around the clock unless patient notes otherwise. They offer PT, activities, transportation to and from doctors, salon care, a podiatrist comes every 3 months, a nurse practitioner comes 2 week to check on patients as well as medications can be delivered directly to the facility for medication help! The food is well balanced and excellent! The portion size is perfect and the dinning is restaurant style! Not every patient likes the same thing; therefore, they can pick and choose what is best for them! Family is welcome anytime to join a family member for only $10 a meal. Patients are welcome to come and go from the facility as needed and just check in and out with the kiosk! One day my dad left the building and he forgot to sign out and they were calling me to make sure he was ok to leave and was safe! They have wonderful staff here with little to no change over! Beautiful amenities and courtyard! My father was able to recover greatly due to the excellent care he received here: doctor, food, med management, PT and daily assistance. He has since been able to return home! I highly recommend Long creek for anyone looking for care for an elder! If my father should need permanent care… we will definitely be back!

It's obvious that it's run by people who have no concern other than keeping the beds filled to collect their prize. Short staffed and over inflated

Recently, when I arrived at Longcreek to visit my mother I decided to take note of what I was seeing and feeling because it made my heart full and happy. It was a pretty afternoon, lovely weather, you know perfect weather after winter. This is what I observed. 18 people happily playing Bingo, 9 residents on the patio enjoying a lovely afternoon. Caregivers, sitters, family members, carpenters, the director, the wellness director actively collaborating, friendly receptionists, and most of all happy and engaged residents! Longcreek is a caring, lovely, clean, active and well appointed home for the people who reside there. I am very thankful to them for the outstanding love and care shown to my precious mother.

If you or a loved one are looking for an assistant living and or memory care facility look no further than Long Creek! The staff is very welcoming and knowledgeable about senior living. My grandmother has been a resident for three years now and she absolutely loves it. Recently I went to visit her during a "Spring Fling" event and I can see why she loves it so much. We had a full evening of activities and I must admit the food was absolutely delicious! I am so very grateful for the the staff at Long Creek for all of their love and kindness.

They have AMAZING Directors who are very helpful, and their passion, love and caring needs for their residents and loved ones/family members truly shows! I have personally seen their dedication, exceptional care and services as well ensuring that their residents are well looked after and treated with the upmost dignity and respect!! If there were any issues or concerns with your loved one, any of the Directors or staff members will immediately respond to help assist you! I just want to say thank you and that I sincerely value & cherish the opportunity to serve seniors together along side Long Creek and my other Civitas Communities!! I have worked side by side with Long Creek Assisted Living & Memory Care in Sunnyvale and other Civitas Communities to help provide any additional care and comfort that their residents might need. Doing both Home Health & Hospice Care services with one company & now just Hospice Care services with a different company, to their wonderful residents!

We had such a great time at the Long Creek Assisted Living Center "Senior" Prom tonight! May 19, 2022 It was THE BEST party EVER there! The staff did an excellent job! They all worked so hard on this event. Fun and food were outstanding! Thank you to everyone that made this event so special for residents and family!

Hello my name is Sharon i work with Care Plus, i have been in the building a few times and i have to say i love the way you guys are running the building and how easy you guys make it for me and other members of my teams to come in and help,i was going to write a review about the company but i thought a email would be better i agree with everything that your reviews says especially about ms Jasmine she is outstanding and a very hard worker she made sure i had everything i needed when i came into the building she even helped me assist with the resident.Jasmine is not only a good manager but she cares about her staff was helping them on the floor serving and also very hands on.I have been to plenty of places and i was amazed with your managers because im so use to managers just staying in the office.So from Care Plus to Long Creek THANK YOU GUYS SO MUCH FOR MAKING OUR STAFF FEEL WELCOME INTO THE BUILDING.

First i would like to state this is one of the best places that i have visit ,and that im so thrilled that these guys are keeping up with the covid 19 rules ,even though my grandmother is not here anymore I would like to thank Jasmine for everything from passing meds and making it to the front office YOU GO GIRL ,HOPE AND LAVETTE YOU GUYS ARE AWSOME AS WELL,i do want to state friday oct 30 i heard a family member threaten the staff which is so not cool and shouldnt be allowed into the building anymore she put everyone in harms way.SHE ALSO SAID SHE WANT THEM 3 Black u know what gone meaning the ladies in the front .somethings to be done about her,STAFF KEEP UP THE GOOD WORK AND REMEMBER YOU CANT PLEASE EVERYONE .if it was up to me my grandmother would be TEAM LONG CREEK .

Had interview with company ,seems like a nice place to work,both managers i talked to seem real cool and care about the people that works for them ,only problem i would say being a caregiver is not a easy job u got to love what you to,the pay is horrible 10.00hrs for a caregiver i understand its an assisted living but you have places that starting pay is 11-11.50hr ,i find thats the only problem,wish the pay was just a little more since its only 7mins from my house.

my father in law loves this place,he loves the staff but its always one person he speaks highly of when my husband and i talk to he says if u need anything done call and ask for Jasmine she gets it done.Truthfully it gets done i spoken with Jasmine a few times and every time i have spoken with her she has taken care of it right away,i think they made an amazing turn around since she have been the new awd she loves the residents and always have a smile on her face,YOU HAVE FAMILY that dont understand that in a nursing home its more then just their love one needs that need to be taken care of so they complain about everything but my family think that LONG CREEK is doing the best that they can do and im so happy this is home for my father in law i wouldnt change anything.

Walked through this community in the fall of 2019, and was welcomed by a friendly and caring staff. I felt like I was walking through a resort rather than an assisted living facility! The place was beautiful, with lots of bright, beautiful art and rooms. Very inviting and relaxing court yard, several rooms to sit and read or watch TV outside of your own room. Beautiful open dining room with great food. The rooms are nice & spacious, everything looked new and inviting.
