Grand Brook Carrollton is a wonderful community! They helped us transition mom from assisted living elsewhere to their memory care. I not only have the privilege of having my mother in law here but also am employed here so I have a "birds eye view". We moved mom from saddlebrook in frisco after several medication mix ups and falls. I worked there also as a Life Engagement Coordinator. My family is so much more relaxed about her care here. Well worth the bucks!!!

I've had my father at AL in Carrollton for about 2 months now. The staff from the Director on down are caring and really take care of the residents. Special shoutout to the staff nurse, Keesha, who is fantastic. The facility is relatively small (this is a good thing) and a bit older but well cared for. I had my father is a very large, nice and new facility prior to this. It was a nightmare, constant staff turnover, uncaring staff, bad leadership, atrocious housekeeping, etc., etc. The staff is what makes a memory care facility run and Autumn Leaves has a top notch one. Thank you to the entire staff there!

I have had a very difficult journey with my mom and was at the end of my rope trying to figure out how to keep her safe and well-cared for. We were referred to AL from the hospital and it had been nothing but a wonder experience. Even though Mom just moved in today I've been doing things behind the scenes for several days. They had a red carpet welcome for her with a sign and balloons! And then their is Keisha, the dementia whisperer. I've never seen anyone like her. She got my very agitated mom calmed down and her hair brushed. She has a VERY clear process to acclimate the patients that makes perfect sense. This is the first time in a long time I don't have a knot in my stomach when I talk about my mother. Look no further!

I cant praise this place enough! The staff is the most caring and wonderful group anyone could ask for to take care of their loved one. It is always clean. They do lots of activities. And they respond to any concerns or problems very quickly! Alma, Maribel, Jasmine and many many others there have made my husband feel like family— not to mention how kind and caring they all are to me and my son! Thank you so much! Love all of you!

My wife of almost 53 years has advanced dementia. When her dementia got so severe that even with help coming in, I could no longer care for her at home I searched the area and visited many care facilities before deciding on Autumn Leaves. The first 2 weeks she was there I regretted every second of the decision as I came to grips with the situation. Since then I have not regretted the decision. Its more like family than a nursing home (and it doesnt have that nursing home smell). She has now been in Autumn leaves Carrolton going on 3 years. Having been in the medical profession for over 40 years I have learned to appreciate high quality care when I see it and I give Autumn Leaves five stars. If I even think I have a problem, they are easy to talk to and prompt to respond. Usually they find the problem and take care of it and they always notify me if it is of any significance. The facility is well maintained but truthfully it is not the prettiest facility I visited but Sharon really isnt aware of that. Pretty is for the family, care is for the resident. And this care of my bride has been truly fantastic.

I am amazed that there arent more reviews of this incredible facility. We researched more facilities than I care to count with a great amount of disappointment, we had actually chosen another facility when we got a call from Karen, I will always consider that call to be the turning point for my 96 year old mom. Karen very quickly convinced us that we needed to look at one more facility before we made our final decision. That was the best decision ever. After our tour and many conversations with Scott, Brett and the staff we immediately signed up. So it has Been 4 months and mom is thriving, she absolutely loves this place. She is getting care far beyond any expectations that we could have imagined. I know this because they allowed me to put web cams in her room (every other facility said no to this request which was an immediate red flag). We watch the care she receives every day. Thank you Autumn Leaves from the bottom of my heart!!!

Exceptional in every way. The staff at this community is 5-star everything. Scott the administrator is a tough and loving taskmaster with an inimitable and palpable commitment to residents, family members and the staff. Kim the activities director is over-the-top creative, loving, and committed to improving the everydays of her residents (does this lady ever do anything but smile, love, laugh and encourage?); Brett the nurse: what a guy! Not just a clinical expert on dementia but truly engaged and involved with the residents! Such a positive presence with an all-knowing eye on every residents status and health. Melissa the admin who is one-part admin and ten parts friend and confidant for the residents and families. Karen the community outreach person who is equally cheerleader, ambassador and caregiver in her own right and to the delight of residents and her families. Bonnie the lead med tech and resident representative knows everything about every resident and ensures that every resident is living their best life every day. Daniel the maintenance guy approaches ever request with kindness and understanding - even those outside of his normal responsibilities, like Dan, can you help us assemble this chest of drawers? And can you adjust the A-C in our apartment? And can you, can you, can you ... And the presence and pride of Cree the chef who completes the family oriented atmosphere with exceptional culinary skills, meal planning, and love, which encompasses a true family connection in following in her moms footsteps as an incredible, everything-from-scratch chef at Autumn Leaves. Plus a whole host of caregivers who are eager and extremely selfless in caring for our vulnerable loved ones. This is a once-in-a-generation team who epitomizes love, service and excellence in every way. If you have a loved one with Alzheimers, this place takes care of them AND you! Thank you Autumn of Carrollton. We are eternally grateful for your love of our beloved Father (and us kids and grandkids, too.)
