Two years ago my sister told us that she had cancer and only a few months to live. My sister was caregiver for my then 102 year old mother who was mostly in a wheelchair because she was so afraid of falling. Immediately, my husband and I started to look for an assisted living facility close to our home. We were surprised how many there are just within a 10 mile radius. For three months we searched for the perfect place for Mom. Narrowing it down to three places, I started to visit each one several times, talking to residents, and caregivers, and watching interactions between them. Whenever I went to Park Creek Place there was always an uplifting atmosphere. The caregivers were very attentive to the residents, and you could tell that they genuinely loved what they were doing. Even the residents seemed to care for one another and looked happy. On about my 5th unexpected visit, while walking to the front door, I came upon a maintenance man I had seen several times before. This time he was sweeping off the porch and said "Good morning" using a sincere smile that I had seen many times before. I asked him "Why do you always seem so happy?". He told me, "Because this is a very happy place to be". That sealed it for me. This was definitely the place for my precious mother. My sister passed away on August 3rd 2019, and my husband and I went to Florida to bring Mom, now 103 yrs old, to Park Creek Place. Of course Mom was very apprehensive and my heart was breaking, leaving her in a unfamiliar place with people she did not know. What a total surprise. When we walked in, several staff members greeted us with huge welcoming smiles, fussing over us and helping with luggage. Many residents also greeted her saying "Welcome Irene". Staff and residents were expecting her and were genuinely excited to meet her. No more than five minutes in her apartment, an aide came with a plate of fruit and asked what she could do to help with settling in. I knew, we had picked the perfect place for Mom. Although Mom was very shy in the beginning, staff was able to coax her into joining in on some of the many activities. She also started physical therapy, and within two months her wheelchair was put aside as she gained strength and confidence in walking with a walker. Her grandson thought it was a miracle seeing her out of the wheelchair. She was never a good eater and weighed about 96 lbs. One of the kitchen staff told me, "Your Mama is too thin. I'm going to fatten her up." Instead of her usual one egg and a quarter piece of toast in the morning, he was able to get her to eat cereal with fruit, juice, egg, toast and coffee every morning. The kitchen staff were probably the best anywhere, and the residents just loved them. April 20, 2000 was Mom's 104th birthday. Despite Covid virus, there was a surprise birthday party outside on the patio with about 20 staff all distanced and wearing masks. They let my husband, son and I join the festivities keeping a safe distance behind the fence. A memorable video was taken preserving this precious memory and sent to me. This is something I will always treasure. Things have changed so much since this terrible Covid Virus, and although entertainment from outside resources can not happen until this passes, the activitie director manages to keep the residents busy and entertained with crafts, exercise class, games, happy hour, movies and much more. When the hairdresser was restricted from entering the building a staff member took over and kept the residents looking just fine. No matter how old you are, looking good helps you to feel good, and my mom at 104 is still the diva. Staff paints her nails almost every week and she still puts on makeup every day. She always tells me how nice everyone treats her and that she feels safe and well cared for. Any time i have a concern, it is taken care of immediately. Everyone from the aides to nursing to directors are approachable and really care. This means so much to me and all my family. I am thankful every day that I found this wonderful Park Creek Place.

My father was home with us with advanced dementia and with small children and working I was broken hearted. My heart became full when I met Park Creek Place Memory Care. They cared for my father as their own. Jason and his team showed me what true nursing is about. I am forever grateful for Jason and his professional compassionate team. Five stars isnt high enough of a grade for them! Thank you so much.

Park Creek Place has been AMAZING. The aids, nurses, activity staff and administrative staff have all been so caring and compassionate. They were so accommodating when we were in a difficult situation with my parents health and needed to find a safe and loving place for them after ill health. Both my parents are living at Park Creek Place and I am out of state; they have been so helpful and kind and I have confidence they are well cared for not only physically but also their emotional and mental needs as well. Everyone at Park Creek has been so quick to respond and help in any way they can.

I cannot say enough good things about Park Creek Place. My sister and I could no longer take care of our father, who has dementia, at home. So we began to look for a memory care facility for him.The setup of Park Creek Place is very warm and inviting. The large, open courtyard-like room in the front serves as a gathering place for activities, and as the reception area for visitors. There are two identical wings off the main room with dining areas, enclosed gardens, activity rooms and the bedrooms. What we liked is that the residents can move freely among all the different areas safely according to what they feel like doing. The bedrooms are set up nicely and have plenty of room. Our experience in visiting many assisted living facilities is that the people who work there make all the difference in the world as to the feel of a facility. And the people that work at Park Creek Place could not be nicer. The community is small, and feels like someones home rather than an impersonal institution. Every staff member knows every residents name. They have been so helpful in transitioning our father to the community, and in making the family feel comfortable that he will be well cared for in his new home. They have been excellent at keeping the family informed about the important and the routine events in his new life.Another impressive thing we noticed about the staff is that everyone is willing to do whatever is needed at a particular moment, even if it is not within their usual job description. For example, one day Beverly, the director of memory care, was leading a group activity and another day she was helping a family move in by finding a hand cart for them to use. Johnna, the community relations director, helped us set up a FaceTime conversation between our mother and father when they could not be together. The number of daily activities and special monthly activities planned for the residents each day is impressive. Phylicia and Nancy are terrific at including every resident in a particular activity. Because some residents speak softly, they make the effort to lean in when asking the resident a question so they can hear the residents answer, and then repeat it for the group. They cheerfully help the residents move to various areas for each activity, sometimes putting their arm around the residents shoulder to gently guide them, or holding anothers hand or pushing them in their wheelchairs. What impressed us is that every resident is included, and no one is ignored. Our father has received excellent medical care from the Charissa and the Care Management staff. When he needed to go to the hospital they called us immediately and kept us informed as to his diagnosis and when he would be returned to the facility. We appreciate that they have regular care plan meetings scheduled with the family, and that they are available to discuss concerns and issues at any time.Update- My parents both moved into another Enlivant facility called Wyncote Place.

I cannot rate thus facility highly enough. I had to place both my parents in a very quick, crisis situation, from out of state. One has serious memory care needs, the other was in need of long term rehab. The facility is sparkling clean. Residents can walk freely, even going outside to a large enclosed patio. With only 40 residents, it was a personal, friendly community, not a warehouse. The staff is incredible, top to bottom. The executive director made a 7 hour drive on a weekend to do the evaluation required for placement. The director knows not only every resident, but all of their family. She was greeting our extended family members by name immediately. Everyone has been trained so well for dementia care and is so respectful and gentle with residents. The rooms were large and well designed so my parents were easily transitioned from their home. The medical support we have seen is terrific- responsive, professional and very high quality. Do not miss this as an option. It has been a level of care that changed our family. UPDATE My dad passed recently. The end of life care he received at park creek was exceptional. They arranged for a terrific hospice organization and could care for him when other facilities would have forced him to go to a rehab site. It meant he could be with my mom at the end of his life, cared for and in a familiar place. The care my mom has had in transitioning, after 67 years together with my dad, has been wonderful. Very grateful. UPDATE OF 12.2018 New management after enormous turnover and we are struggling to make this work. Very serious issues not being addressed. We will try a bit more but after 2 months of inattention my hopes are low. UPDATE OF 2.2.2019 We moved to the personal care facility and are thrilled with the level of care, the sense of community and the physical building itself. Happy conclusion for my mom but still concerned for the status of the memory care unit. Visit at unexpected times.

We just moved my grandparents into Park Creek yesterday in response to their increasing struggles with memory loss and age-related illness. The facility is gorgeous, spacious, and well-maintained. The room was perfectly turned down when we visited on the first day, and today (the second day), I saw a maintenance worker replacing a door handle that I hadn't even noticed was awry. The staff is excellent and genuine -- everyone is compassionate, well-organized, and impressively chipper. Johnna in particular has been a dream throughout this difficult transition: ever-attentive, hardworking, and dedicated towards accommodating every aspect of my grandparent's move. She remembered my name after our first meeting, and volunteered her phone number to my mother for contact at any hour of the day. Overall I've been very impressed. I am extremely grateful to Johnna and all of the staff there for making such a scary time for my family so much easier.] Honestly, I'm only withholding the fifth star because it's only been two days and I don't want my review to seem less credible. I would be surprised if I didn't go back and revise it in the next few weeks.

We recently moved my husband's mom into Brookdale and have been very pleased with the difference between this and where she was before this new home for her. The staff treats the residents with respect and dignity, learning about their favorite songs, their family and their personal preferences. They pay close attention to their medications, only medicating when necessary in order to allow for them to be the most "with it" as possible. Familiar songs, stories and trivia allow all the residents to remember the people, times and memories (lots of Oh What a Beautiful Morning, Nat King Cole, Dean Martin, Frank Sinatra, etc) they can still tap into, which allows them to feel comfortable and at home in the charming "town square" where everyone gathers daily. I actually look forward to visiting her there every week!
