This facility (WAS!!) Great 2 1/2 yrs ago when I first started ad there Maintenance Director. At the time they had an administrator that ran a very tight ship . However this facility definitely went down hill quickly . From lawsuits for stealing employees PTO from the Department of Labor and Insdustry , to not pay vendors or contractors, ceilings collapse on a monthly basis because the owner refuse to have the entire sprinkler system fixed and would rather fix small sections at a time . The local police see this facility as a nuisance. The fire department and fire chief are fed up with having to come this facility on a regular basis because things don't get repaired as they should . They are always understaffed in every department. The landscaping is terrible if not next to non-existentent , overgrown hillsides and broken Fencing . The housekeeping- what house keeping ? It smells like urine and feces throught the entire building . All of the equipment is outdated or doesn't work properly. The kitchen staff has to work with limited resources and rarely gets new equipment when desperately needed . I've never worked for a worse company , I was on call 24/7 and came every time I was called upon not only as maintenance but also central supply ordering, stocking , housekeeping and cna . The amount of stress from the current administration was unbelievable. The administrator is incompetent and has no clue how to run a facility. Out of 8 administrators in 2 1/2 years the current one is the worst by a landslide. Residents wait with the call light on for hours , during covid dying Residents would ask why there family wasn't there and family members would ask to please let there loved ones know they can't come in because of covid but yet the dying Residents where left to assume there family didn't care about them . The conditions these Residents live in is terrible and sad most of all . They also stole Resident stimulus checks they didn't know any better or had no one on the out side to stand up for them . DO NOT PLACE YOUR FAMILY MEMBERS OR LOVED ONES IN THOS FACILITY IF YOU TRULY CARE FOR THEM .

As a current resident and have been since January so I have a good ten months experience, there are a few things I dislike but you cant like everything, no matter where you go, theres bound to be some things you just dont care for but compared to the good they do, the few bad things are nothing, I mean, I hit the call button, either a nurse or an aid answers pretty fast, they take care of my needs pretty timely, I try to keep my room organized but being paralyzed, theres things I just cant do and they come in and organizes it for me, they have edible food(this sounds laughable because who wouldnt? I heard from other patients about food in other facilities and they dont, I dont know myself if true or not but not taking the chance if I dont have to), on site laundry, nurses and aides majority of times are happy go lucky and greet me with a smile on their face, if those few times they arent feeling up to it, I put a smile on their face, the entire from office greets you with a smile and whats amazing is they all have an open door policy, not one time have I ever went to one of them with an issue that I was turned away, they handled it. If you are looking to have your loved one placed somewhere for care, please look into this place and I will be here for a few more months so please, stop in and ask me questions, my name is John Davis, I am in room 110.

Ive been here twice now and Ive enjoyed both stays and the staff, they are very good and caring Social services has helped me with many things, as well as the case manger janice and nursing staff, I would return again!

The staff is extremely efficient and nice. The building is well maintained.The food was better than I could have ever imagined, going into a nursing home. Being that it is a small nursing home, the opportunity for staff to really get to know the residents is incredible. I am discharging from the facility today, and I am very pleased with the overall care that each individual showed to me during my stay. I would definitely recommend this facility to others in need.

My 95 year old aunt has been in 3 nursing homes in the past 6 years and has received the best care by far from North Strabane Rehab and Wellness Center(NSRWC). The staff is very friendly and responsive. The facility is very clean with the hard surfaced floors being scrubbed just about daily and the dining room table cloths changed daily and as needed. Her room is always clean and tidy. My Aunt's cloths are always clean even though she has difficulty feeding herself which reflects the personal assistance provided by the staff. She also seems to really enjoy her meals. Her appetite has never been better. The management and staff at NSRWC are commended for their efforts in providing excellent resident care.

The staff at this facility will bend over backwards to make sure the residents are happy and comfortable. I have been a resident for approximately two months now and I will be sad when I leave. The staff cares for you as if you were one of their own family members. Thank you for your hard work and dedication!

Putting a loved one into a nursing home is literally entrusting a "stranger" to take care of your loved one. At North Strabane Rehab & Wellness, this is not a "stranger". Rather, this is "family". The team there is always available to answer any questions that arise, and always answer with expertise and clarity. Placing my mother into their care was not an easy decision, but having the trust and knowledge that they are taking top-notch care of her is simply amazing.
