My husband was sent here for his rehabilitation from a serious infection. He was there for 4 weeks in hall 5. He was given wonderful care. OT and PT are wonderful. He enjoyed the food too. I highly recommend it for care.

Husband was sent here from Kaiser for rehabilitation and pain management due to a knee replacement. I was so disappointed in the room they placed him in. I was appalled..I hated leaving him there. Looked like a dingy unkept room filthy sink, toilet bowl hadn’t been cleaned or sanitized in who knows how long. Noticed the bed had been slammed up against the wall that had paint scrapings all against the floor and wall. The fluorescent light above his head was cracked & broken. Thank goodness maintenance fixed it by his insistence. I had asked for housekeeping to please come in to clean the floors etc. I wasn’t there my son in law witnessed someone came in with a damp mop sloped it over floor without sweeping then left. The sink, or toilet hadn’t been cleaned. Locks to the restroom( no shower) are on the outside of door not inside to lock so no one could walk in on you which happened to him. Hallway floor hadn’t been vacuumed it didn’t look clean, food trays were set everywhere outside the rooms. The second day he was there the toilet broke got him completely drenched had to run him over some more clean clothes. In pain dealing with that. His pain management was so out of control one RN stated order was for every 3 hours another stated every 4 hours. The staff are all amazing hard working doing the best they could helping him through this difficult time. The facility does need some upkeep maintenance a coat of paint etc. Doesn’t look at all like the photos or a hospital cleanliness setting. Appreciate all the staff who cared for him they were very kind and professional. Respectfully CB

After the initial couple days working the bugs out, I was placed in the care of long-time RCM Emily Kennedy and I thrived with this team and my Kaiser-Permanente team under guidance of Nurse Nichole. This SNF represents the best of us, leading with hearts and let me be a strong participant and advocate in my care. They would be five star were in not for them dining services communications structures; fortunately my friends and family helped me survive this detail.

The facility is little misleading at first sight, it could use a coat of fresh paint and some wall patching on the inside. That said, I would still rate as five stars because of the medical professionals and staff that took care of me. You are not going to find a more thoughtful, empathic, responsive team as the Hillsboro group. I could name lots of them, but my primary caregivers I like to acknowledge are PTA Randy and Karen, my OT Stacia and my caregivers Rowena and Ayramus. I would have no reservations recommendng Hillsboro Avamere to my friends and family.

The people and the food was awesome. For as busy as they were call light response was still within reason. The OT and PT were great going home after less than 3 weeks.I think everyone here is a godsend they were my heroes

Took 45 mins. Or more to get any help after ringing the bell for emergencies. Covid was rampant. We had to get tested almost every day. The facilities were not clean, especially the bathroom. They were clearly shortstaffed. The food was unbelievably huge and inedible for me. It was all in all just not a good place for me.

I m so grateful for every person at this facility! My father in law was there for almost 2 weeks after surgery and the service he got and the people who are there are truly a blessing. I had read some old reviews and was apprehensive about it..I have to say that everyone who works there is amazing and they do their work whole heartedly. thank you avamere rehab.

It is ok the main complaints I have is that they’re under staffed especially over night only 2 people and they don’t pay attention to the patients and the food is overcooked and usually cold Ive worked in commercial kitchens before and they need to cook in batches and not keep the in the warmers so long maybe some other help part time

I just spent 2 weeks at this facility after having surgery at Kaiser Westside to repair a broken femur. I have only good things to say about the staff, nurses, rehab people and Kaiser liaisons who took care of me during my stay. Thanks to all of you. I have a way to go to reach full recovery, but am on my way.

We were visiting Portland for an event and unfortunately my wife fell and fractured her femur bone. After surgery at OHSU they sent her to this facility. I read some of the reviews and had some concerns about the level of care she would receive. We stayed for 18 days and received care that exceeded our expectations. The staff was extremely helpful and caring. The only reason I didn't give them a 5 star rating is because the facility could use a refresh and some of the equipment is in need of replacement.

My mom went here post op. Stayed a week. I personally liked the facility. It was clean (no smell), staff was very nice and accommodating to my mom. From the receptionist, RN, CNA's, Social worker, everyone I dealt with were kind, caring and compassionate. I can't speak to the food quality, as my mom had a feeding tube. I gave a 4 and not a 5, only because my mom was unhappy there. She didn't like that is was an older facility and did not like that her bathroom, was shared by 4 people. She said it took forever for someone to come with she pushed her call button. Full disclosure: my mom is an impatient person. Also, I know there is a staff shortage. Most nurses were from out of state. They were all so nice and helpful. I have been a patient myself, at other skilled nursing facilities and if needed I would chose Avamere to rehabilitate.

The physical therapy seems to be great here, and mom liked the people she worked with, which is huge. However, it's of great concern to me that I have a really hard time getting in touch with staff and getting called back in any sort of reasonable time frame: I've left multiple messages, multiple days without getting responses, and unless I'm persistent enough to keep calling until I get someone, and then to call back if I'm transferred to voice mail and insist on being transferred to someone who's actually there and willing to answer the phone, I just get dead air - no response. It was also really upsetting to have talked to 3-4 people and left multiple messages about the fact that mom only had access to her reading glasses though I dropped her regular glasses off for her - it took me going there on Sunday after having dropped them off Wednesday and insisting on talking to the nurse (no mean feat in the time of COVID) that they even looked - all three of the bags I'd dropped off for her with clean clothes, her glasses, and her shoes (the ambulance took her with two right shoes, which she then wore all week without anyone noticing) had been stashed somewhere and not given to her.

Father was transported here after being in ecmo treatment at legacy Emmanuel and then a small stint at Vibra rehab facility. This is the final step before he heads home(hopefully). The staff have been nice and friendly, however this facility is not equipped for the families of those admitted in the facility. My father is in a shared room, with now another person. Their Is a lack of privacy, and space. Hence the mention of the facility not being friendly to those who want to visit thier loved ones, as we do. This is a big blow to the moral of my father who went through a traumatic experience, and having somone with him is helping him recover from the serious illness he experienced. I wish avamere and Kaiser would take this into consideration when they recomend someone to certain rehab facilities. I am hoping my dad has a short stint here because this a big blow to his moral. Also one rest room to be shared by 4 people? Doesn't seem right. If we would have known this, I would have asked to keep him in Portland. Atleast we had privacy there.

Just came from a visit with our neighbor who had a fall at home. This place looks great from the outside, but inside is another story. No receptionist at the front door. The room our neighbor is in is old with popcorn ceilings with that ugly yellow hospital colored paint. It is quiet and we are not sure about the care, but one nurse came in when our neighbor had soiled her Depends, ripped the sheet off and told her she was disgusting!!! Anyway, that nurse is not allowed in our neighbors room. Also the diet they have her on is gross. Pureed corn and food that looks like dog food. Keep requesting swallow studies, but not sure if the person knows what they doing as far as the swallow study goes. Our neighbor was eating regular food in the ICU!!!! Anyway, this is a typical old rehab place that looks wonderful in the photos, but not a place I would ever have one of my family in. A 3 star rating is way above would I really wanted to put give this place. I feel so bad that our neighbor was sent to this place by Kaiser. She doesn't deserve to be treated the way she is. Oh and she has a mark across her belly from the gait belt that one of the therapist used on her. Abuse I tell you!!!

My my wife Michelle got transferred here from the ER room at Kaiser’s Westside Medical Facitity in Hillsboro’s after suffering 3 broken bones in a fall in Beaverton during a trip from Livermore in California. The care and food here has been excellent. I especially want to thank Nichole Ferree RN, the excellent care coordinator, who went above and beyond the call of duty in coordinating between the Kaiser North West and Northern California regions and in arranging with Medjet Assist, who we have a contract with, a transfer for Michelle to a rehabilitation facility in Pleasanton CA near our home in Livermore. The whole Avamere and Kaiser staff made Michelle’s stay and initial rehabilitation here as pleasant as possible. We definitely recommend Avamere Rehabilitation of Hillsboro CO to anyone who might need their services.
