I have been in and out of Azalea Gardens for the past couple of years and have always had a great experience. I have always witnessed the staff going above and beyond for their residents. They keep the residents clean and well presentable. I'm unsure of the new director but the girl who was running it for a couple months always went above and beyond. I can't seem to remember her name but she worked as a med tech and worked her way up to sitting in the director position. She was the highlight of Azalea Gardens her and the current nurse Michelle made an amazing leader group they worked well together always. Everytime I was there I witnessed her interacting with the residents and staff and making sure they are all in a GREAT mood. I seen her working every position in the facility when it was needed, I wish she would come back and help brighten the atmosphere again. I just wish the positive atmosphere that the previous director created would return, just doesn't seem as positive when I come in anymore. Michelle the nurse is AMAZING.

Same problems here. Still waiting on refund from over a month ago. Always blame it on corporate. Food served is hardly what's on menu. Disappointed in billing problems. This place may need to be investigated.

Financial - bookkeeping - accounting for charges a disaster. Always paid on time or early - consistently received credit memos or money due. When I addressed this issue personally with the office at Brookdale was told dont worry about it - it takes a while to get every thing straight. Two months after taking Mother out of Brookdale received a check for $4,000 from Brookdale. Two days later received an invoice with $4,000 due. Therefore not only did family have to be concerned and check on welfare of Mother, always had to deal with concern and worry regarding payment being processed correctly.

My grandmother is a resident at the Brookdale location in Muskogee. Unfortunately, she suffers from vascular dementia and is no longer able to live at home alone. This was a perfect fit for her because it is a memory care assisted living that is focused on the needs for individuals suffering from dementia and Alzheimers. Everytime we visit her she is out of her room doing activities or visiting with the other residents. I am so thankful for her to have the opportunity to live in such a wonderful environment with such kind and caring staff.
