Pay attention to detail there are signs and red flags as soon as you enter. They just want your money and the care is not in this healthcare facility. The office managers will give empty promises of change and improvement but over all the care team is somewhat neglectful yet polite… they will greet you while your mother is soaking wet and disappear before you can ask for help. Prepare for the run away and to be tired

My mom spent some time at Lakes of Monclova for rehab in early 2026. The rehab facility was great. PT and OT helped my mom regain mobility that we weren't sure would gain back. We are eternally grateful for Lakes of Monclova and the staff. What a blessing. I will recommend this facility continually.

I had the honor of being one of their santas for the residents christmas party. The staff was friendly and the environment was beautiful made it easy to help spread cheer. Event was well planned and all the residents had a great time. Looking forward to next years event.

I truly loved so many aspects of this facility”the environment was warm and welcoming, the people were kind, the food was delicious, the rooms were cozy, and appreciated the happy hours and social atmosphere. These are all things that made her stay pleasant and, in many ways, gave us peace of mind. Unfortunately, I cannot give a full 5 stars due to a serious issue of negligence that put my mom at risk. After repeated swallow studies, it was clearly documented in her chart that she requires thickened liquids only, as she aspirates on thin liquids. Despite this, on at least six different occasions during her stay, my mom was given regular thin liquids by staff. This placed her at significant risk of aspiration”something that could have had devastating consequences considering her age and the lack of immediate equipment for such an event. I raised this issue repeatedly to staff - nurses, aides, the social worker and speech therapist. Each time I was reassured that it would be addressed, but the problem was never corrected. I even placed a sign, visible right when walking in, Thicken Liquids but it was still missed by staff. Even the day before her discharge during her 20 day stay, I caught her being given thin liquids again. Thankfully, I spent a lot of time with her and was able to intervene when given thin liquids, but had I not been present, the outcome could have been serious. If not for this ongoing negligence regarding her thickened liquid requirement, I would have gladly given the facility 5 stars. Every other aspect of care and community here was wonderful. However, consistent disregard of such a critical need is something that cannot be overlooked, and families should be aware of this risk when considering the facility.

My father in law James Reno has been here for the last 6 months or so. He loves it there all the people there are amazing with him and all of us that come out for a visit. The only thing I would say is I wish we would of taken him there sooner.

Lakes is absolutely awesome!! They treat their residents with caring and respect,and do the same with family members.They have daily activities, keeping the residents involved and active,the favorite being Friday happy hour! My brother has been there multiple times and has always been treated great.Anytime he has an issue,it's the first and only place he wants to go! I would highly recommend Lakes of Monclova to everyone!!

Our mother decided in August, 2024 that she was ready for assisted living/long-term care. The move to the Lakes of Monclova has been life changing for the entire family. The staff are very attentive from facility care personnel to facility and department directors . The number of activities (exercise, crafts, bingo, trivia, etc.,) is exactly the socialization mom needs. Additionally, there are so many unique themed events Winter Ball, Hollywood, Kentucky Derby, to name just a few. Not only does our mom enjoy the events but the family looks forward to joining in. Lastly, the nurses and aids are so attentive making each resident feel seen. They make an effort to familiarize themselves with mom’s likes and dislikes, her family and ways to make her feel special.

I wanted to take a few minutes to highly recommend The Lakes Of Monclova! Our mother was a resident there for over 3 years and the staff were so kind & supportive of our mom and entire family. Watching over our elderly parents is a really difficult phase in life but the management and staff provided such peace of mind. LOM has the most amazing activities and celebrations as well! We have so many wonderful memories of these special occasions with our mom. Sending another HUGE Thank you to each and every single staff member that served and loved our family! ™™

My dad has dementia and is a current resident. He is well taken care of and the staff and aides are very accommodating. They have a happy hour every Friday and monthly brunches to keep family members engaged.

Stopped in during a happy hour to visit a family member of a friend. They had food available while socializing and it was excellent. The facility was calm, inviting, and clean. Staff were pleasant and engaged. I wish this place would have been on my radar when my Dad was looking for an assisted living facility.

We recieved a call regarding my Husband s Aunt .. regarding blood pressure and they were going to do an EKG and they would call us back in an hour. An hour later no call and we attempted to call them several times later with no answer . My husband finally got in touch following day to say everything is fine

Grandmother was hesitate to go into nursing facility for rehab, once she arrived at the LOM she was instantly settled by the caring staff! Grandma doesn’t want to go anywhere but LOM for rehab going forward! Thank you!

My mother was there for rehab after a shoulder surgery. They really made us feel like home from the amazing food, happy hours and brunches, to the great therapy team that really made her transition to home to smooth. When my grandmother needed skilled rehab and then end of life care; I did not hesitate to ask for her referral to be sent to The Lakes of Monclova. My grandma received the very best care and as a family we felt very supported. The facility was always clean, the staff was caring, compassionate and welcoming and the rehab department was top notch. I would not hesitate to refer a family member or loved one once again to The Lakes of Monclova.

I don't even know where to begin when it comes to The Lakes of Monclova and their amazing staff! When my mom had an unexpected hospitalization and was not able to go home, I knew right where I wanted to send her. The staff went above and beyond to make sure she felt right at home. I knew no matter if I was there or not she was in amazing hands. I would come in daily and the staff would tell me over and over again how great my mom is and she was a breath of fresh air to them! Mom would talk on how the young staff was so gentle and kind to her even in her most vulnerable moments. You can walk in at any time during the day and you are greeted by a smiling face, asking how can I assist you. The facility appearance is above and beyond what you would expect, clean from top to bottom! I Could go on and on! Thank you to all the amazing staff at the Lakes for taking such amazing care of my Mom!

My husband has been a resident at The Lakes for almost two years. The entire staff has always been so kind and helpful through the difficult process of no longer being able to care for your loved one by yourself My entire extended family always comments on how the facility exceeded their expectations with the family events, car show, Camp Monclova, happy hours and brunches The facility is well maintained and any issues that have come up are addressed immediately Leaving my husband in the hands of others has been very difficult and knowing that he is well cared for is really the only comfort that makes it bearable I appreciate everyone at The Lakes and all they do to assist more than words can say

My mother was at the memory care facility for approximately 3 months. The staff was patient, caring and compationate. We received excellent guidance from Tessa, the director. She provided family classes to help us understand the progression of dementia. I would highly recommend the Lakes of Monclova for your family member that needs memory care.

My husband lives here and it is always so clean. The administrative staff is very nice and helpful. The chefs and service is very good. I have gone on more outings with my husband since he moved to The Lakes of Monclova then when were living at home. I know he is in the best place he could be. I thank God everyday!

My father had to stay at The Lakes of Monclova for rehab and skilled nursing.. I cannot express how wonderful the facility and staff are. The nurses, nursing assistants and PT staff are so kind and compassionate. I highly recommend this facility.

My husband and I had the fortune of him being able to rehab at Lakes of Monclova following serious back surgery. He was convalescing there for 5 weeks. During that time, we got to know the staff really well. We were both extremely impressed by their sincere dedication and caring for their patients.....the kind of care you only read about but seldom have the pleasure to experience. Moreover, the nurses, nursing assistants, and management most impressed us by their teamwork displayed in the dining room, in helping fill in where needed on the floor, and in making the environment more family like overall. I definitely would rate this establishment a 10 out of 10. When you are in agony and extreme pain, this is the kind of staff you want to serve you. They really make you feel welcomed and they go out of their way to make you as comfortable as they can. Thx Lakes of Monclova for making my husband's rehab such a pleasant experience.

My family and I brought my father to this place for a a temporary stay for medical reasons. The place was very clean, the staff was nice and very attentive. The rooms are spacious, clean and beautiful. A great place for patients with Dementia or other medical issues. I would highly recommend this place to anyone who is a caregiver of a family and need to recuperate.

Staff are very nice. For the amount paid each month,for a private facility, amenities really subpar. Very difficult to get in touch with a resident. We were told to bring in our own phone, which we did, but my friend cannot call out, nor can they transfer any calls to her room.you have to call the main phone then they have to connect to their cordless, and walk into her room? Is this the 80’s? TV reception unreliable also. Facility is clean and prepared food looks decent.

I have a friend there. She has mediocre care at the most. I would not recommend this place - food is prepared by staff who follows a recipe - when they are doing this they cannot take care of patients. I realize no one can get help these days - but if I wasn't there, many times her food is cold or barely edible - she has never been offered another choice even when she doesn't eat. Two people take care of 10 - the ratio is not working. She is loosing weight.

Very new and Clean, but not a lot of staff-client interaction. No organized activities mid week afternoon. No music or TV, just newspaper and magazines to read. All beds were made, all clients dressed and rooms orderly.

This place is relatively new, very clean, pretty decent atmosphere, the staff seem like they have a good handle on things, I have yet to see anything that jumps out at me that says hey I would not want my family here. All in all its a very decent place, its in a very good part of town, and the country type atmosphere is very refreshing for a nursing home.
