The Heritage is an amazing place and they have become my family. My mother became very ill and they took great care of her. Moms dementia became worse and they helped me know how to transition mom into the Legacy Memory Care. Chloe Korecky and Robin Coffman made the transition go really well for me and helped me deal with it all emotionally. They continue to help me with my mother. The Legacy is an amazing place for my mom. They have so many activities for her to be involved in. Soon after mom was ill my sister became very ill from uncontrolled diabetes. My sister has been in the skilled nursing facility. No one has ever cared this much about my sister with all her mental issues. All the nurses and staff and Chloe have taken great care of her. They also take her across the street to visit our mom. Jessica Ward was very helpful as well in giving me advice on how to do what is best for my sister. Jessica and Chloe helped me realize I needed to be my sister guardian to do what is best for her. My sister goes on day trips and seems happier now with the right meds and being cared for. Taylor Kimmel is also very kind and makes my sister smile. The other week they brought in a kitten and my sister loves cats but she is not able to have one anymore. They all go above and beyond. My sister and mother are getting the best care I could ever have imagined. They all have helped me through this very difficult time and knowing my loved ones are getting great care is a blessing. Chloe, Taylor, Robin and Jessica are amazing people who truly care about the people there. Its more than a job to them. My sister has been in other nursing homes and none even come close to what the Heritage has to offer. Chloe has made it happen that my sister is now going to be in a beautiful assisted living apartment. I never thought this was possible. They all continue to amaze me.

After my father passed in March of 2025 (who was a resident in the health center for 7 months) mom was in their house alone and was not getting out as much as we would have liked, nor eating consistently. After a tour of The Estates facility and apartment in December, we talked that evening and easily agreed this was an excellent choice for her to continue living independently but with the worry and stress of living alone removed. We signed the papers the following day! Since that time, she has made many friends, is involved in the daily activities and enjoys the meals that are served in the beautiful dining room. The staff is very friendly and attentive and makes her feel welcome. We couldn't be more pleased! Knowing that she is being cared for 24/7, is very comfortable in her new home and has access to friends and activities, allows us both to sleep better at night.

I spent 71 days at the Heritage. The therapy staff were very encouraging. I am most thankful for Val and Madison. But the majority of the therapy staff were great. The Nurses were also encouraging and supportive. Thank you to Cidy, Jamie, Tracy and the Stephanie's, there were others that I can not remember your names. The activity staff, they go beyond helping to pass the time that is left after therapy and treatments. The trips outside the building were so enjoyable and I loved learning new ways to play Bingo. The dietary staff had their work cut ou. Over all the food was good. Breakfast was my favorite meal of the day. And it was great that there were choices available it something did not suit my taste. Housekeeping also did a nice job keeping the building clean. There were times that it was noisey at night due to loud tvs. After midnight tvs should be turned down or maybe earbuds need to be provide to the night owls. Over all it was a goid experience. I might by back after a joint replacement.

It's so sad that my mother had to wait 1 hour to get her lunch delivered to he room in the nursing care unit. It came in cold and it was warmed up in the microwave. She didn't even get anything to drink just a plate with food. They say that they get fresh hot meals.... Not true!!

I’m currently a resident. Everyone here has been so kind and knowledgeable. The PT/OT Department is top-notch. The nurses are incredible. And the nursing aides have been kind and helpful. The food has been nice and they care about our nutritional needs. I can’t say enough about the activities, all the fun we’ve had takes your mind off your troubles a little while and allows you to meet others. Being here allows my troubles to be a bit less

The lengths that Taylor and Chloe went too to help me with getting my Dad into The Heritage and helping me with learning the paperwork was incredible and a lifesaver. They went above and beyond to help me and ease the process.

My wife is a resident and is well taken care of the staff. The Rn LPN STNA are on mark she is well taken care of when I leave at night I have no worries that everything is going to be fine. Even the kitchen staff is amazing they make sure everyone has their needs met in detail And the residence always have something to do with the amazing activities provided. Five stars all the way I am thankful for their service I can't not post a second review so I am going to update this one . This place it's amazing I can't imagine going anywhere else and getting better care or treatment than what you can hear. Everything from the care your loved one gets two activities to the incredible kitchen staff in administration it doesn't get any better. The nurses and stna are above and beyond average.

I’m a single man that was having a knee replacement in Toledo but lived in Findlay. It was recommended that I go to an assisted living skilled nursing facility. I would highly rate this place 10 out of 10 for cleanliness, friendliness, and people wanting to go out of their way to help me With my daily activities. I was greeted at the door by a smiling lady named Chloe. After telling her that I had spoke to Taylor and was looking for she said she was busy and would be glad to help me and give me a tour. That smile on Chloes face went a long way throughout the whole tour. Everybody was friendly was greeting me and making me feel comfortable. When I was finished with the tour, I finally met Taylor who greeted me the same way as Chloe. I highly recommend The Heritage to anybody for their goals and needs. You definitely cannot go wrong with this place and their staff. If I ever needed to go to a place like this, I would highly recommend The Heritage! Thank you so much to everybody that made me feel at home and we’re concerned with making me feel better. I enjoyed to stay so much there that I have decided to go back and finish out my physical therapy with their outstanding staff.

ED Ben Perkins demonstrates outstanding professionalism and leadership, and he runs a very smooth, well-managed building. My mother-in-law is always exceptionally well cared for. She requires assistance with feeding, and I never have to question whether her needs are being met. She can be a picky eater, but the staff consistently make sure she receives delicious, well-prepared meals”and she never complains. Hats off to Ben and the entire staff at The Heritage for their dedication and excellent care.

My father in law is here and I can't say anything bad. We know he is safe and he is treated well, with dignity and respect. Their therapy department has done so much for him and he has made great improvement from when he went in there. He gets 3 good meals a day. His apartment is well suited for older adults. The staff is friendly, caring and helpful.

I’ve been here five months, this is the best place if you ever need care. I would recommend to anyone who needs help, please try to come here before anywhere else. I am 82 years old and could not walk before coming here and now I’m walking up and down the halls. They have saved my life. They don’t let you lay in dirty beds and they all feel like family. The nicest people work here and they all treat me so well. If you come here to see somebody, I would like to have you meet, Ben, Taylor, and Chloe. Ask for them, they are amazing and will give you a tour of the building including the beautiful dining area with the good food!

The admission coordinator Taylor made my family’s admission process so smooth. We can’t thank her enough. The aides, and nurses are amazing as well. The food is amazing as well. We love the fact that Chef Kyle makes some of the most amazing soups to. I would recommend The Heritage for your family needs as well!

I was there recovering from cervical fusion surgery. And the staff was amazing. Food was very good. And they offered lots of things to do. Plus my therapy people were there best. The nurses and staff were friendly ands caring and did their job well. Thank you for such good care.

I've been here at the heritage for several months and they made each and everyday feel like the heritage is my second home I'm here for care and therapy until I'm strong enough.To return home and take care of myself to which they are so good at helping me achieve my goals and help keep me from being depressed from what i'm going through the kind wonderful staff, therapist, kitchen workers, house keeping , maintenance, and then we have the CNA'S (1st,2nd and 3rd shift)not only took care of me but also talked with me. laughed with me and kept my happiness from dwindling away I'm really glad to be in a place that has so many good people with good hearts. Everyone has been very helpful and seem to care for the residents. The biggest thing I see which is common to nursing facilities since COVID is the retention of staff. The ones who do remain are all great. The nurses are all helpful in care and really give top efforts.

I went to the Heritage after getting out of the hospital to gain my strength back so I could be able to be stronger to go home. All the physical therapy workers, aides ,nurses, cafeteria workers, housekeeping, and maintenance were very kind and compassionate. Even the office people. I would recommend the Heritage it was clean and it felt like home to me. Very good people!

My granny has received excellent care and I cannot express enough gratitude to those who care for her. She had some of the best staff taking care of her and more importantly, treating her like family. I truly found everyone, from management to dining staff, to be kind, and patient, and hard working. The quality of service was excellent. I had a 3 month stay and the consistent empathy and care from the staff was unbelievable. I truly left there feeling like family. I also love that their therapy department offers outpatient services that have been extremely beneficial for me and my progress. I would recommend this facility and therapy staff to anyone who will listen.

I can't Thank the staff and all involved in my care at The Heritage! I broke 2 ribs and "hyper extended" my back. I was there for 8 days. Everyone there was OUTSTANDING! My meds, food ( really good! ) PT, OT... 5+ stars! I'll be going through a surgery and will go to The Heritage, as I KNOW I'll get the Best Of the BEST Care! Thank you all! Nursing homes can be scary. NOT at this facility!

It's been a little over a year since my mom came to the Heritage. Starting out I talked with Taylor and she was able to get my mom a room there. Taylor has been such a God send! She did all the admission stuff. She even checks in on mom still periodically. This facility has really made my mom comfortable. The staff is friendly and always greet me with a hello and smile. My mother in law has recently had to come for rehab after fracturing her arm. When at the hospital they said she would need to go to a nursing facility we told them the Heritage. When I went to visit mom I stopped in at Taylor's office to ask if she could find a way to take my Mother in law. Taylor was already on the phone working on admission. My mother in law loves the place and is even in the process of moving into assisted living here. Thanks Taylor for making both admissions go so smooth!!!

As a resident of The Heritage and I do consider the residence to be my home. First and most important is the medical. The Heritage has a staff of doctors, nurses, and aides to ensure that you receive the medical attention you need. The Heritage also offers 3 meals for you every day. Breakfast, you build your own breakfast. Lunch and dinner have menus, so you choose a very nourishing meal. The meals are served at 7AM, Noon and 5PM. Whether you are on a diet or other plans, they will cater to you and your meal. Now for the activities, The Heritage has activities for every need. From exercise class, craft corner, color your world and of course bingo! And they also have Bible study. Heritage also has housekeeping and maintenance for your needs to ensure your home is what you want. You deserve the best, and the best is a 5-star home which is The Heritage. My husband has been in the short-term nursing care rehab unit for the last 28 days and Thanks to the dedicated caring staff and management he has made tremendous progress! He has especially enjoyed the special dinner for Father's Day, activities such as Bingo and musical entertainment and the challenges that the Therapist set up for him so he can return home soon! I can't speak to what it's like living here. I have delivered things here many times. They always seem to be doing activities and the residents seem happy. Been to some not so great assisted living places in the area. Employees are always very nice too. Very clean. If you are looking for a place for yourself or loved one it's worth checking out

I am quite pleased with this facility. A relative had an unexpected illness that led to what will possibly be a lengthy stay for rehab in a nursing facility. He has only been here three days, but so far my extended family and I have been treated with respect, the patient's needs are handled in a timely manner, the staff have interacted with both him and us in a proper, but friendly way, and the food has been top-notch. I like that my relative can obtain the therapy he needs within the same facility, and that his caregivers allow us to participate, instead of treating us like we're in their way. I was able to walk down and encourage the patient during pt, was encouraged to feed him in the room when he chose not to go to the dining room, and was asked my preference on whether he was showered, etc. The staff also allows him to make the decisions about whether he'll be placed in his chair or his bed. The rooms are up to date, and have individual hvac units, so the patient's comfort can be maintained. I don't know what I could complain about at this point. After the experience with another relative at another medicaid facility for the past 6 years, I'm so grateful that this relative is at this facility!!
