I have been a resident at this facility since 1/16/26-7/2/26 The building is of old converted construction and occasionally, there are problems with the heat and AC. The building does need a complete plumbing and AC update. The maintenance staff does work very hard to keeps things operating properly. There are a few young ladies who work here making appointments and doing other paperwork for the patients. I will not mention any names but we all know who they are and I have to say Thank You to to them for their help and pleasant attitude. Sometimes I drive them crazy but they are very understanding and accommodating. The facilities social worker is very pleasant and helpful. There are minor (and sometimes) major operational problems with the facilities three Hoya Lifts. My room, the hallways and common areas are cleaned everyday of the week (not that they need It). In other reviews, some stated, upon entering the building, there was a strong odor of urine. I have been here for some time and I have never experienced that. There are everyday activities to keep the residents occupied and stimulated. The food is not 5 stars but it is fairly good for nursing home standards with a few outstanding dishes. There is a problem with some of the kitchen staff not being able to read and understand what you ordered due to the language barrier (or they just don't care). The kitchen director is constantly trying to correct the problem. However, as of this writing, the problem continues almost every day, three meals a day. I hope that he fixes it because at times, it can be very, very frustrating. Going on six months and NOTHING has been corrected. You would think that after six months, the problems would be corrected but not this place. Some will think that I'm a racist and I don't really care because everybody that knows me, knows differently. Everybody is entitled to work, however you should be able to communicate in the language of the country where you reside and are employed, no exceptions. Dock them an hour's wage for each mistake and in a few days, they'll be speaking perfect English:) As far as the nurses and nurses aides are concerned, its the same no matter where you go. Some are pleasant and like doing their job and others are very lazy and just plain miserable and unfriendly. There are some that would expend more energy trying to get out of doing their job properly than doing it right in the first place. I am not going to mention any names but I think that those not so nice and unfriendly people know who they are. However, to the pleasant and helpful staff (which there are quite a few and I won't mention their names), I have to say Thank You for making my stay a little more bearable, you were very kind to me. What drives me crazy is how some of the nurses are not fully prepared when it comes to providing such items as wound dressings, colostomy supplies, etc..... When one nurse gathers supplies for a patient, the others act like squirrels stealing the stored supplies for their patient's use. Example: I needed a wound to be dressed which should normally take no more than ten minutes. An hour later, the nurse returned because she had to search two other floors for the proper dressing which is extremely inefficient, time consuming and not very helpful for the patient. As I leave, I am giving this facility a 2.5 star rating and I hope that the owners and staff read it and make the necessary corrections and improvements because when you're recovering from an illness, the patient does not want to tolerate all of these petty, easily fixed problems. I would like to mention a few names of some of the people that were so kind to me but I won't. They know who they are:)

My mom received respite care from This facility and the staff went over and beyond our expectations. Charles was phenomenal with his caring and attentiveness to mom and Janet treated mom as if she was her mom. She also was phenomenal in her care. I highly recommend this facility if you are a caregiver and need some form of respite care.

Over the past 8 years, and a series of extended stays for Mom, our family can without question recommend CareOne at Madison Ave as a first-rate nursing home and rehab facility, led by a capable dedicated staff across all departments. Starting at the front desk, every single person there makes it a welcoming and comfortable environment for residents and visitors alike. It’s always easy to communicate and coordinate vital details with Admissions, Billing, and Social Workers. Outstanding Nurses on every floor, attending Doctors, endlessly helpful Health Aides, a topnotch range of physical and cognitive Therapists, all of whom do a wonderful job. An equally caring Activities Team who keep residents involved with a fun monthly calendar ensuring daily personal interaction with others. Constant maintenance of clean rooms and surrounding areas. And just as important to a happy stay, they offer menu choices based on diet, and VERY GOOD Chefs' homemade meals. Having gone through this process, I realize that finding/deciding on the best place for any individual situation can be a challenge. So I’m sharing our experience to hopefully make your search easier, and if you’re considering options throughout the tri-state area, I’d say it’s well worth a visit to explore CareOne at Madison Ave. It’s housed in a well-maintained sprawling old mansion set back in a quiet location away from the bustle of town, yet right down the street from the excellent Morristown Medical Center, nearby access to route 287, plenty of onsite parking. Everybody we’ve encountered on staff through the years takes care of the residents as if they were their own family. That’s why we truly see this as a home away from home. There’s no greater compliment than trusting others with the care of a loved one. Thank you, CareOne!

To my friend Rebecca who is Recreation Director at CareOne in Morristown. I am writing a review for some of your events and activities. I have been in a nursing home for 4 years, 3 and a half here. I am an 82-year-old male. The events here include; arts & crafts, religious, baking, fitness and athletics, concerts, movies, nursing home meetings, holiday decorations and luncheons, birthday parties, snacks, talks/chats, games, singing, dancing, pet visits, ice cream socials, picnics & BBQs, days for dogs to visit, planting and flowering. We are located on a corner property with a lot of trees and lawn on the old millionaire's mile, i.e. the street of 100 millionaires. I am aware of a lot of the events. I also spend time with other departments like physical therapy, cognitive therapy, psych-local therapy, so I do go to all activities. They are all a listed on our monthly calendars which are posted in everyone's rooms. The events I have attended I enjoyed thoroughly. Becca is an amazing events manager and resident council management head! Bill C.

As with any rehabilitation facility, there are both positives and areas that could use improvement. The Good: One of the first things I noticed when I arrived was how clean and welcoming the facility was. Having been in six other rehabilitation facilities before, I was pleasantly surprised by how responsive the staff here has been to patients’ needs. By far, this facility has had the best response times compared to my previous experiences. The physical therapy team works hard to engage patients and is both compassionate and professional. The nurses are highly skilled as well. Having been a patient in healthcare settings for many years, I can honestly say the nursing staff here demonstrates a level of skill that, in some cases, exceeds what I have experienced in hospitals and other facilities. The administrative staff I’ve worked with has also been responsive and helpful. The Bad: Unfortunately, some of the nurse aides could benefit from stronger management and supervision. While response times are generally better than other facilities, call bell times could still improve. The meals also need significant improvement. Some are acceptable, but many could use much more care and attention. Although there is a menu and substitutions can be requested, the food quality remains inconsistent. Overall, I am hopeful to continue making positive progress so I can safely return home and ambulate independently again. Since I am just beginning my therapy journey here, it is still too early to fully judge its effectiveness. However, if I had to choose a rehabilitation facility again, I would come back here. I can honestly say that CareOne at Madison Avenue has been, by far, the best facility I have experienced compared to the six others I have been in. May God bless you in your recovery.

My experience overall was good. The staff made my stay comfortable and made me think about how people really care! The physical therapy department was the best! They got me to walk more and taught me how to take one step at a time; as with life. Terry, my unit nurse, Monique and Camille were very nice ladies, nurses and my cna. Jonathan, was very good with me, in making me laugh and know when times are serious with my situation. It was a great place to come and get better.

I have been here at CareOne Madison since February 2023. I always liked Morristown and I was at my college 50th reunion in Madison and my girlfriend loved Madison. CareOne in Morristown is a beautiful old mansion with a very nice piece of property. I am currently getting psychological, physical and cognitive therapy. They have plenty of events to go to and I always find ones I like. Sometimes I get my favorite foods, and they try very hard to please. They do have a nice group of people to socialize with, be it males and females. The view from my window is plenty of nature and so far, I have been on three floors in three different rooms. Excellent staff and visitors. CareOne in Madison Ave. is definitely a winner! Sincerly, William E. Connelly

I recently had my aunt as a patient at Care One in Morristown for the past year and eight months, until her passing on Friday, April 10, 2026, at the age of 91. During her stay, I was truly fortunate to work with such wonderful staff. A heartfelt thank you to Caprice Wiley and Maribel Diaz on the administrative side, as well as a HUGE thank you to the nursing staff, Twania Smith Burke, Frederic Debra, and the entire nursing team and aides who cared for her daily with such compassion and dedication. I would also like to recognize the Suncrest Hospice staff, who visited her weekly and consistently went above and beyond in providing comfort and support. I could not have made a better choice for her convalescent care. The kindness, professionalism, and genuine compassion shown to her gave me peace of mind every single day. I am deeply grateful to everyone who played a role in her care.

I'm currently at Care One recovering from a case of orthostatic hypotension that left me unable to walk. I'm doing better and have requested a discharge to continue my convalescence at home. There are good things and not-so-good things about this facility. Good: the physical therapists are hard-working, compassionate, and professional. The nurses are, for the most part, very good. The building, while aged, is clean, although it definitely could use a renovation. The administrative staff I have dealt with is responsive. Bad: sorry to say, a number of the nurses aides are lazy. The other day, it took over 30 minutes after using my call button for someone to come. The food absolutely sucks. Some meals are OK, but many are barely edible. And although they give you a menu, the selections are limited and even then you don't always get what you requested. Overall, I'm happy to be going home. I accomplished what I needed. Would I come here again? I don't think so.

My husband has been at Care One Madison following frequent hospital stays. The very competent Administrator sets the tone for a warm , caring staff and facility that is clean and well run. The nurse has identified, on multiple occasions, the beginnings of critical health emergencies and has responded promptly. The staff has been wonderful - the nurse - the nurse aides - the doctors - speech therapist- the physical and occupational therapist- the social services support staff, have all helped him regain strength and movement. I’ve had assistance in scheduling doctor visits and transportation. The facility has provided daily recreation activities- and has helped to keep my loved one safe, medically healthy and well cared for. Barbara

In 2018 I was here for 3 months, now I have been here for the past two years. And I’m glad I came back, the therapy helped me tremendously! Marisol was my therapist, she is the sharpest tool in the shed. She’s a good person and belongs in the medical field, she helped me gain the ability to walk again back in 2018, and she’s still here helping others. When I had my first had my stroke I could not eat or speak but Courtney helped me regain those abilities and still now helps others. In therapy now Dhynni is amazing, we get along very well and I can now start moving my left hand, and ability I lost after my second stroke in May of 2033. The receptionist are super efficient, and kind. I also want to thank the entire therapy department for all their help with restoring my abilities. As well as all the aides and nurses that I was fortunate enough to come in contact with. I wish to thank the entire management team of this facility, most of all the Director Deb and all of the department heads that meet everyday at the long table to review every single patient and their needs. A thank you goes to the kitchens staff to all the work they do. And finally a shoutout to Ebenezer and his whole group of handy repairman for keeping the building going and helping me with all my needs. Overall I have enjoyed my stay and I’m happy I have experienced this facility.

Terrible experience. Do not come here at night. The night staff were incredibly rude. A family member of mine needed assistance with changing, and I asked the two midnight nurses to help something they are paid to do. Instead, we had to wait 45 minutes while one of them sat on her phone laughing and doing nothing. Completely unacceptable behavior. On the other hand, the morning staff were so kind and caring they treated my family member with respect and compassion, which we truly appreciated

I have been here 7 times now and I mostly come back for the therapy. I am independent and I don't really need the nursing attention but i think Aleesha, April and Sam are pretty good nurses on the second floor , but some others can be better . The therapy is great, they take care of you and consistently follow up. I know when I come back here the therapy is great compared to exploring and going to another facility and not knowing the care that I'm gonna get. The therapy staff all know me and they know how to treat me good. But I like the atmosphere and the day to day routine. I have been joining in the activities more now, I go to Bingo more often now and it's fun. Overall the therapy and atmosphere here is truly amazing that is the main reason why I keep coming back!

I’m 74 years old and I congenital medical problems therefore I have been in a few facilities over the past 10 years, I was in a facility in Somerville and the difference between this facility and the previous is like day and night. This facility is the closest thing to living at home. The staff here ranks 12/10, unbelievably caring and helpful people. I have been through a number of personal situations and everyone in the facility reaches out as if they were truly family related. I wouldn’t leave this facility to another one for any reason. My son lives in Brooklyn and I could move closer to him but I choose to sacrifice the distance for this care. This is my home.

I had a close loved one in this facility for two months for rehab and he genuinely enjoyed his experience, and is now able to walk again after an unexpected surgery.

There is nothing more important than my mother’s care, and I could not be happier when we moved her to CareOne at Madison Avenue. From the maintenance staff always making sure her room and the facility are running well, to the nurses and aids who are so caring and make the time for her care, to the dietician and chef who are always on top of things and strive to make good food options, to the back office staying on top of the details on the business and insurance side of things, to the cleaning staff that keeps this mansion beautiful and spotless, and the laundry care is second to none. I am very happy my mother is here. This is how it should be and gives me hope for my mom.

Maribel has been assisting me with the Medicaid process. She has been amazing! Where other areas of the staff leave a bit to be desired not Maribel. She is knowledgeable, organized, and follows through. There is never A problem seeing her she’s always available. All the employees should have the professionalism and accessibility that Maribel has . She truly is an asset to the facility.

Being a patient here for about 2 and half years now, I’ve had a good time. Everyone has been nice and sincere. they go out of their way to help you. I appreciate how clean the building always is, they clean my room everyday. And for the most part the food is good. I mainly go to bingo, poker and Pacino in terms of activities, the activities people are very very helpful and try to make the activities as pleasant as they can. If you ask for help the staff always comply and see what’s wrong and that makes me feel extremely comfortable here

My husband (the patient) and I had a great experience with this facility. We were treated with a lot of attention and respect which we always appreciated. The staff helped us feel as comfortable as possible, especially Carlos, an aid on the third floor and Amanda, one of the rehab therapists. Both genuinely went above and beyond for my husband, and we truly appreciate their work and compassion. I just wanted to say a big thank you to everyone who helped my husband in his road to recovery.

The staff is extremely helpful and friendly, When I came here I couldn't walk, but now, I am able to walk with a walker, thanks to the rehab that I had.I wish the food was better. The nursing staff is very helpful, and some I consider family. I consider alot of the staff here at Care One part of my family.

My father stayed at Care One twice within the past year. I was anxious about his recovery, especially since everything that happened to him was so sudden and unexpected. During his time at the facility, he not only regained the ability to walk with the help of a walker but also improved his speech significantly thanks to the support of the speech therapist. The staff was incredibly kind and attentive, the nurses always made sure we had everything we needed. We were especially grateful for the therapy staff. They showed so much compassion, not just toward my father, the patient, but also toward my mother, who stayed by his side throughout his recovery. Their care and dedication truly went above and beyond, making a difficult situation feel more manageable. When my father had to return a second time, it was bittersweet. While we were saddened by the setback, we were also comforted knowing he would be in a place we trusted for his recovery. Once again, thanks to the incredible rehab team, he was back on his feet within three weeks. I would recommend Care One to anyone in need of rehabilitation services, especially for the excellent Rehab Department. The activities were also nice part of the facility, my dad really enjoyed the live music concerts and the monthly birthday parties.

I like it here, I think it's nice. I very much like the nursing staff and I had rehab here, they helped push me to my best. I recommend this place. I used to be at another facility that was good but I think this place is exceptional. Some of the aids are better than others. I try to go to all the activities even if I'm not interested, like the card games.

CareOne at Madison Avenue has been exactly what my mother needed after spending a year at another place that was not very good. From the staff, the food, the cleanliness and the overall feeling you are in a home versus a facility, I sleep better at night now and so does she. She is well cared for and the staff does not have big turnover like many other places, so my mother gets to know who is taking care of her long-term, and the staff gets to know her. I am very happy and so is my mother.

My uncle was placed at CareOne Madison after a horrible experience at overlook medical center. At overlook they lost his EKG. How could this happen????? Overlook and all of AHS is a complete disaster. Even more of a disaster is Summit Medical Group. SMG is all about money. They continually want a credit card. AHS must have a financial gain from Morristown Post Acute. They pushed hard for him to go to a horrible center with horrible results from the NJ Department of Health. Thank goodness that I did my own research and found CareOne at Madison Avenue. I had to fight to get him the care that he deserved but we did and CareOne surpassed our expectations.

Care One in Madison is where my family decided to bring my grandmother for therapy following a heart procedure. Not just because I work here but because I knew she would get great care. The therapy team is top notch. They treated my grandma as if she was their own. My grandma never exercised prior to her condition, but seeing how therapy helped her she learned the value of it and continued getting healthier even once home. Nursing listened to concerns and was able to help my grandma in all the ways she needed in order to heal.

What a wonderful nursing home/rehabilitation facility. Its so clean and well maintained. It has numerous amenities to help keep their people more than satisfied. Warm and inviting. Administrator, Deborah Hall is the best part of the facility. She is so caring and empathic. Always makes sure everyone is well taken care of and all faculty is attentive to their residents. She is a breath of fresh air. She always has a smile on her face so to keep all (residents /visitors/faculty) upbeat, happy and full filled during their stay at CareOne at Madison Avenue. If you ever need a nursing/rehab facility I highly recommend CareOne at Madison Ave in Morristown

My mother recently stayed at CareOne Madison Avenue in Morristown. The level of care and compassion was outstanding throughout. The entire staff from front desk, therapy and nurses were all phenomenal. The level of support, compassion and kindness was evident by all. Recovery is not rushed, and is truly conducted with the patients well being as the priority. We have experienced other rehabilitation facilities in Morristown and nothing compares to CareOne. Highly, highly recommended.

I came to Careone Madison reluctantly one year ago because of the past Careone reputation. I was pleasantly surprised that this building and these people truly gave me a new outlook and made me feel at home. I saw people who truly cared for the residents and a place that accepted me for me. They truly made me feel like a superstar! Everyone has contributed. It's a team effort here.
