This home was very good until the nurse resident manager left for health issues. History of understaffing causes stress to aides. Many good aides don’t last. Requires a lot of follow-up to make sure your loved one is taken care of. Lack of communication to family members about their loved ones even when they are not doing well. Terrible record keeping of your loved one and they don’t pass on information to aides during shift changes. During sundowning, they have tied patients to their chairs with sheets. I bought a couch for them and they allowed patients to sleep on it during the night who urinated in it. After my mom died, I reported them to the NJ Ombudsman and they were cited for violations.

This marks one month since my husband passed away. I took care of him for six and one half years as he suffered from Dementia. Finally, I placed him in Fox Trails as I was no longer able to take care of him. What I was able to do for so many years - Fox Trials was unable to do. In three months, their lack of diligence in caring for him led to pneumonia, bed sores, UTI's and finally his death. My husband is no longer here, but I pray this review prevents other families from going through what I have had to.

Families beware before placing your loved one in this facility. From the beginning we were lied to and told that there was only one room left when, in fact, there were ten rooms available. My father was at Fox Trail for three months and five days. He died in the hospital after his second trip to the ER with viral and bacterial pneumonia, failing kidneys due to three UTI's that were never eradicated and an open wound on his bottom. Communication between the center and home is next to non-existent and the final blow was that they never even sent a condolence card to the family after he died. I truly believe that their incompetence and lack of staff led to my father's death. If I could give zero stars, I would and that would be being generous.

I was extremely disappointed with the level of nursing care to my mom. My mom fell 6x's in 3 months and finally broke her hip after multiple other falls with even being supervised in the bathroom. My mom was pretty strong walking before breaking her hip esp. with a walker but was never identified to other new staff members that she had fallen and needs supervision and reminding. Poor shift to shift reporting. After her broken hip my mom steadily deteriorated with urinary tract infections and dehydration since my mom was not being changed and cleansed adequately (after her hip broke and was more work to move) nor offered adequate fluids with the medications (diuretics) she was taking. During the night shift calls were not answered. Supervision was very minimal when supervisory staff was out sick or out on medical leave. I feel after Covid without supervision from family members, care was compromised. With the types of residence they have with memory care issues, it was difficult to get information first hand. I would give a recommendation of less than 1 if that is possible and would recommend thinking twice with putting someone with memory issues in a facility who supposedly specializes in these types of patients. There was no such special care at least after Covid isolation.

It has been a long year for our family in caring for our Mom. When our Dad died two years ago, our Mom(his wife of 54 years) began to decline She fell last August and suffered a brain injury, which increased her challenges. Since then, she has had several medical facility, nursing home, and assisted living stays in Ocean County and Morris County. Our family is life-long residents of Morris County, and very close, with many family traditions. Mom owned her own business for 35 years and has always been an independent woman who could take care of herself. Since Mom could no longer live on her own and needed 24 hour care, we needed to find a COMMUNITY(not facility) for her. THANK GOD WE FOUND FOX TRAIL! All of the facilities my brother, sister and I looked at, or tried for Mom(CareOne-Morristown, Paragon Village-Mount Olive to name a couple) were large and over-crowded, VERY regimented and residents had to be on THEIR schedules, expensive, had a medical feel rather than a home feel, and gave us false information. We learned alot through our experiences. By the time my sister and I went to look at Fox Trail, we were skeptical and not very hopeful that it would work for our Mom. We met with Andrew who gave us a tour, answered our questions, welcomed our information shared about our Mom and LISTENED to us. He encouraged us to give Fox Trail a try. We then met Stephanie, who is the Director of the community and she said she would go and meet our Mom first to determine if she would fit in there. Mom at that time was at a facility 2 hours away, but Stephanie went anyway. NONE of the other places we tried truly went and got to know our Mom, BEFORE we signed up! Mom has been at Fox Trail, Chester since November 1, 2019. There have been some problems along the way with regards to staff turn-over, lack of communication between medical staff and aides, and activities in the beginning. However, with Stephanie the Director taking charge, we have been SO HAPPY with Moms COMMUNITY! Before COVID, we could visit Mom whenever we wanted. As her daughter, it was nice for me to go and get to know her care-givers, check on her room, do activities with her, see how things were being run, etc. Mom has 7 grandchildren, many Morris County friends, and a loving family who are always welcome to visit. Of course these visits were not allowed once COVID broke out. However, even without being allowed to visit mom, Fox Trail enabled us to call Mom and facilitate FaceTime with her whenever we wanted and kept up communication with us throughout. We often said, Thank God we chose Fox Trail for Mom because it is a SMALLER place with more personalized care! Unfortunately, there were positive COVID cases at Fox Trail but none of the residents had to be hospitalized and well-managed care kept the other residents, including our Mom, safe. Watching the news with Mom at a care center made us so anxious during the pandemic, but Stephanie and the staff at Fox Trail keep us informed and in touch with Mom-we are forever thankful for this! Now we are able to do porch visits with Mom- wonderful! There is a large porch, and a lovely patio for residents out back. The addition of a new activities director has brought a variety of calendar events, if Mom CHOOSES to join-remember she is independent and the staff recognizes this! There is important medical attention provided daily by nursing staff and monthly by a physician(consistent and informative).I could go on and on! Having Mom to have to be in any care center has been beyond difficult, but having her at Fox Trail makes it a little easier on all of us. In summarizing, this is what is most important there: SMALLER AND PERSONALIZED, ENCOURAGE INDEPENDENCE, WILLING TO HELP WITH FINANCING, WELCOME VISITORS and PLAN ACTIVITIES FOR ALL OF US TO JOIN WITH MOM, LOCATION, PRIVATE ROOM TO HAVE SOME OF MOMS BELONGINGS WITH HER, OPEN COMMUNICATION AND AVAILABLE MEDICAL ATTENTION, FRIENDLY CARING STAFF. I-my family encourage you to go visit and see if it is right for you!
