We were pleased with the appearance and cleanliness of the assisted living rooms and dining room; however, after our Mother lived there a few months things became an almost daily battle. The housekeeping and laundry services were non-existent; there was no consistent person to communicate with; the staff was often absent from their posts/work and had no intention of caring for Mom as she required. After our mother was moved to skilled nursing, the care she received there was better, due to the fact that we got Hospice involved. The rooms on that side of the building are very rundown and dated. At least they do have some qualified nursing staff for that area, which is the facility's only saving grace.

I've only been twice to visit a patient. I used to take care of him at his home. His daughter said, he misses me. So I went to visit & the common areas are nice & clean. But his room was dirty. I couldn't take it & i cleaned the kitchen, bathroom & made the bed. That took almost 2hours. I told the daughter about it & she called & complained.

Wendy Rimel went above and beyond to keep our family member taken care of during her care there. Wendy kept constant communication with the family and gave us peace that she would love and care for our family all the way until the end. It made an impact on us during a hard time. This facility is truly blessed to have her!

Clean facility, great staff, good food! My mom's been there for a month now and I couldn't ask for better people to help her. Update: 4 months have gone by n my opinion of this facility continues to grow. Genuinely nice people! Really nice facility! I highly recommend oak tree!

My father was admitted for physical and occupational therapy a couple weeks ago and I just want to let everyone know that I'm so grateful for the care that he's gotten here.

Why is it you raise their rent and the quality of your staff working have gone so down hill house keeping doesn't hardly clean the rooms anymore the food has gone down hill and the dietary don't wear hair net they have long weads and the whole staff always on their cell phones every one nursing house keeping dietary I think it should be a monthly meeting on your clients conplains.a very concerned person of the clients rights. These issues have been brought to the front office no response it's sad because this Home was at its highest caring for the clients rights but not anymore its really sad to see this home going down hill. God bless someone to fix this problem. My prayers are with you'll. Why paint a pretty pictures just go look at the carpets and the patios how dirty they are. Please understand these places need to be clean and take good care of our families and good clean dietary workers and better food. Looks like it slides really down hill on the weekends it's bad and the CMTs seem so confused when giving the clients meds not all CMTs you do have a very few good ones. Maybe it's because their always short staffed.i want to thank the good staff that took good care of my dearest friend. She has passed but hope the place gets alot of improvement to the place and the lazy staff that is there don't get me wrong their is great ones there but they get over looked and the ones that take advantage of our elderly by not doing their job shame on you they could be your mother or father and even your grandparents, what happened to respect your elders.

Oak Tree took care of my Grandmother and my aunt and uncle, so I have multiple experiences with them. I have always been continually impressed with the level of care and attention to detail by their staff. They genuinely care for the residents as opposed to just giving care. Rest easy knowing that your loved ones are in good hands.
