SHE CALLS IT HOME My mom chose to move from the family farm to Oak Pointe of Carthage 6 1/2 years ago. We have never once heard her wish she was somewhere else. She calls it home and will brag about how good her life is. We watch the staff smile and do their work with kindness and professionalism even during stressful times. Being retired from a career in healthcare, I understand the nature of the job, from the stresses to the rewards. We have seen many staff changes throughout the years, as my mother-in-law was a resident even before my mom. They are far from perfect and would tell you so. However, we’ve never seen them give up or misdirect any frustrations they may be experiencing toward their residents whom they serve. My mother’s dementia now takes her to some very dark places at times. They love and guide her through those. They are her safe place, and my mother loves them for it. Thank you Oak Pointe. Dana Cook

This facility is great, amazing people, there were a few that fell short of that but no longer work there. The ones that do work hard and are apriciated. Everyone is friendly and works together to give the absolute best care each amd every resident deserves.

They do not take care of the patients in memory care very well. These patients need more help because they cannot take care of themselves. They don't know how to critically think about medications. They don't question obvious issues, and just follow tasks on a paper. Maybe the assisted living is better, but memory care is awful.

This is a wonderful facility if your family member does not have prescribed medications. The staff is mostly kind and patient. They try to coordinate activities frequently with the community and within the facility. Their facility is clean and the food is good. On a surface level, the care plans and ideas are great as well. But when it comes to making changes based on family concerns, they have continued to fail. They have made multiple medication errors that have sent my family member to the hospital. In these emergency situations, obtaining accurate, timely information is crucial. They are often unable to provide this. When they do provide documentation, it is sloppy and often inaccurate. The staff sometimes cannot interpret the documentation because it is so confusing. Safety is by far the most important part of caring for others. When staff and their direct leader are making conscious decisions to go against a physicians order for a medication, all trust is lost. If you are unable to critically think about the effect of these types of decisions, you are in the wrong field. These decisions are too high stakes to not take them seriously. I understand we are all human, but making multiple mistakes and never changing processes or standard work is a sign that safety is not as important to the facility as it should be.

Very kind and helpfull director. Was able to get a lot of questions answered but after I left, of course, I had thought of some questions which I will reach back out to the director again. The property was very nice and clean and had a pleasant atmosphere overall.

We cannot say enough about how important Oak Pointe of Carthage has been to our family. My mother-in-law came to live in memory-care the summer of 2017. A year and a half later, they helped guide us through her passing at her bedside. Little did we know that only one year later our mother would need to move from our huge family farm into assisted living. She chose Oak Pointe and immediately felt welcome. We have never once felt her needs have not been met or her voice unheard. Every time we call or visit it is positively received. Even through the toughest year they've ever faced. My mother broke her hip the day we were headed to my step-father's funeral. This was a very stressful time for me especially when her physician stated she needed to be in assisted living. I had no idea how to start that process but by chance saw a new building in Carthage and stopped by to ask questions. From the very beginning, Oak Pointe helped me through my difficult journey that began initially telling her she couldn't go home but that better yet there was a place that would allow her to keep her independence AND would provide her help with her medications, laundry, food and be on call should she need help. (and so close to my home.) This was hard for me to tell her but I couldn't help notice the sense of relief that took over her face. This new place provided both of us some peace of mind. I will always be thankful for all the help Oak Pointe has provided thru her various stages of need: getting her to doctor's appointments when I couldn't, and keeping a watchful eye on her throughout the day while I was at work. Her apartment is so beautiful and spacious. She was able to put many of her favorite pieces of furniture, paintings and treasures inside making it a mini version of her home. Maintenance has been helpful in getting pictures hung right where she wanted and helping it feel like home. My mother has always said she feels like a queen with help getting dressed to bathing and never having to worry about laundry or food. She loves the staff and says they are all so caring, sweet and always willing to give her a helping hand. It has been 4 1/2 years now and mom is in hospice nearing her end. Oak Pointe has allowed me to stay with her every step of the way. I can't thank Oak Pointe enough for being there for her and my family. I highly recommend this place. Oak Pointe has created a place that has allowed my mother to live her most dignified self. (Food is good too! The cooks all know what she likes helping her feel special and part of the Oak Pointe Family) Would I do it the same if given that chance-you bet!

Facility was meeting all federal covid-19 guidelines. All staff wore PPE gear that matched their uniforms. The facility maintained a strict 2 factor security access protocol and has 0 covid related deaths or illnesses.

I was concerned about my grandpa going into a home and really wanted to find the right fit. I found that at Oak Point. It’s a friendly and caring place that has a home like feeling. Every time I visit him, I am reassured with my choice with the stories he tells me of the fun activities they do. Could not be happier with the service and care he receives. If you are wondering where to go, Oak Point is the right fit without a doubt.

My mother has been at Oak Pointe in Carthage since it opened and I have nothing but good things to say about the facility and staff. My sister and I rotate in here every 3-4 months. We always eat a couple of meals in-house to check the quality. My last visit was this week (mid Sept 2018). The building and grounds were emaculate. It’s looks just as it did when they opened. The food was yummy at both breakfast and lunch. I did not eat dinner this trip but I have no reason to think it would have been any different. If Mom has any problems the staff calls right away.
