My mother had a 5 month stay at the Fountains. When she initially moved in, she was the lowest level of assisted living, thus she was primarily independent and did not require much in the way of services. This period of time was very good, and we were VERY happy with the experience at the Fountains. However, after three months her health took a turn for the worse and her needed level of care went up significantly. Unfortunately, she did not receive the level of care that we were paying for. On many occasions, we found her in her apartment and her portable commode had not been emptied or cleaned, thus there was a bad odor in the apartment. When an aid did come, they would often walk out without emptying it and we would have to chase them down to get it emptied. When we complained to the office staff, we were told this would be addressed in their meetings. In many cases things did improve, but unfortunately, one of the aid's verbally reprimanded my mother for reporting this. She intimidated her to such a degree that my mom was afraid to report any other instances. When this was reported to the office staff, they again said they would talk with her. In the end, things did not improve, and we chose to remove my mother from this facility and provide care for her on our own until she passed away. The night before she moved out, the aid told my mom she was glad she was leaving and a few other nasty things. When I shared this with the office staff, they accused my mother of making up the entire incident. Needless to say, this was VERY disturbing. Lastly, we had a number of billing issues while we were there. Nearly every month there would be errors in my mom's bill. Funny, all of them benefited the Fountains. This required my time to dispute the errors and in every case the charges were dropped. My advice would be to check your bill very carefully as their process is sloppy at best, and crooked at worst. The Fountains is a beautiful facility and billing issues excepted, it seemed to be a great place for someone that was more independent and did not require much in the way of care. However, I would be very wary of utilizing their services for a loved one that was not independent and relied on their staff for their well-being.

Our family highly recommends The Fountains at Hosanna. We toured 9 facilities and knew when entering The Fountains that this would be our mom's new home. From greetings at the Front Desk, good food in the dining room, friendly staff, variety of daily fun activities to choose from, nurses and RA's that treat the residents with dignity and respect. Our mom said I really like it here because they listen and care about me.

I moved into a one-bedroom spacious apartment a year ago after two bad falls living alone at the age of 82. Living independently here has been above and beyond my expectations. Artfully designed facility with caring staff who treat me like family. A plethora of creative weekly activities appeal to my sense of wellness and artistic stimulation. I choose underground, heated parking, and daily evening meals in the Minnehaha culinary dining room, even though three meals a day are in the offing. After being widowed twice, I have found a friend in every resident here. The Fountains fulfills my life-long dream of living in a sorority. Oh yes, we have fun guys, also. Guest apartment, Club Room, Bistro, Happy Hour, upscale Hair Salon, and superb live music weekly at the minimum. Grand piano, studio size, natural lit art classroom, cozy library, plus large Theatre with cushy seats. The exercise gym offers three distinct classes by top-shelf technicians who customize movement to meet personal needs. In today's world, I feel safe here in a very friendly spiritual fortress. Let the games begin! Shuttle to grocery stores every other week and special seasonal trips. Residents grow a spacious garden in the summer with flowers, organic vegetables, and herbs. Learning new things never stops: crafts, knitting, choir, movies, and lectures, to name a few. A grant from McPhail brings in highly trained and talented music/art history folks who gear their interesting, interactive presentations to appeal to folks of our bygone generation. Special recognition and honor here are highlighted for Vets. On-site health care, OT, PT, and other modalities pique my interest. For some, it is a stairway to heaven. For me, I plan to live graciously well into old age with the assurance that my every little need will be addressed with sincere professionality and love. My aunt Mary had lived in the Westwood Senior Living in W. St. Paul for three months. She fell while living there and I was not impressed with the level of care or attention my aunt received. They even put rodent sticky pads down all over her apartment to catch mice. My aunt had an elderly cat that could of gotten stuck on this, I was appalled. We decided to move our aunt to Lakeville to be closer to us. After visiting every senior loving facility in Lakeville, we decided on The Fountains. The level of actual care and attention to detail at The Fountains exceeded all of my expectations. Many of the staff members have been working at the facility since it opened. From the daily care takers that helped our aunt with her medications to assisting her in getting down to meals, all of the care was exceptional. There is a genuine love for all the residents and actual care taken to insure their daily needs are tended to. In the end our aunt was on hospice and The Fountains staff worked hand in hand with hospice to make sure that our aunt was treated with dignity and love all the way to the end. Our aunt passed at The Fountains and the staff again, operated with skill and excellence. Not all of these Senior Facilities are the same. Not even close. I would give The Fountains and A+! My Dad has been at the Fountains now for almost 1.5 years. Either one of my sisters or I are visiting almost every day so we have a pretty good sense of the Staff at The Fountains and the facility. We find the staff just wonderful. They are not only providing a service they care about the residents and go out of their way generally to do whatever needs to be done. The management listens to the residents and the families and makes adjustments when needed. The facility is beautiful and there are many activities for the residents to take part in. One thing that distinguished the Fountains from other places we looked at was that they can care for people with special needs and difficult medical needs. We found that they were very flexible in terms of Services.

I don't do reviews, however in this case I must. I want to extoll the mission of Fountains at Hosanna (FaH) in their care for my Mom. FaH staff from the Front Office to the Care-Givers are a class above in their humanity and excellence in skilled care for my Mom and her friends. I have been pleasantly surprised at the pleasant and friendly atmosphere where staff move with a purpose and a smile. Thank You Fountains!

I cannot say enough about the wonderful care my parents received at The Fountains. The staff was amazing caring for their needs and really connecting with them and loving them (and our whole family). I moved my Mom from out of state in June to the Fountains so that she could be close to family. It was hard for her at first to leave all her friends and her home. In the last 5 months, she is thrilled to have found many new friends and activities! The Fountains is very welcoming and the staff is exceptional. The staff and residents at The Fountains at Hosanna are welcoming, friendly, and caring. My parents recently moved to The Fountains because they were no longer able live independently. Through the transition, the staff at The Fountains has come alongside them and me to help smooth the way. I especially appreciate their regular and helpful communication. I went there for a tour with my mother while we were considering assisted living. Well, I was considering it; my mom never really was interested. It seems like a well-run, clean and happy place. Our point of contact was Beverly Kossack, who reached out to me after apartments finally opened up months after I first toured the grounds. And she continued to be helpful: we wanted to see what a typical dinner was like, and she gave us one free of charge. Total professional! My mom declined because she can't imagine herself living in an apartment with helpful people around her, But I think this is a place anyone with a loved one who needs assisted living should check out.

My mom lived at The Fountains from 2016 until she passed in 2021. She started as an independent resident- to assisted living - to hospice care during COVID. Through all those transitions she was able to remain in her own apartment. The staff and administration were careful to listen and quick to resolve any concerns on our part. They were flexible and creative to help us during the final phase of our mom's life. We were very blessed to have the people at The Fountains be part of our care team. I highly recommend their services.

My mom lived here since 2016 (she passed away recently). The staff at the Fountains is truly the best! They loved my mom and treated her like family. It is such a beautiful and well cared for facility. I love how my mom could walk over to Hosanna for services as well. She enjoyed her time there. I would like to call out a special thank you to Pastor Mari (the chaplain). She is truly the best and the most kind person. My mom loved her. I would also like to thank Jasmine (nursing staff) for her impeccable care of my mom. Jasmine knew my mom so well and showed her patience and kindness and love every day. What more could you ask for from a care facility! Truly the best! You would be lucky to have someone you love live here. We moved my mother to the memory care here about a year ago and it has been a wonderful place for her. The facility we were at previously was really dropping the ball with timely medication administration, and responding to our family's concerns on other care issues. The staff here communicate very well, are easy to find and are extremely attentive to the residents. There's always someone popping in to invite my mom to activities or administer care. The meals are amazing, the care is great and we couldn't be happier. Highly recommended!

My father-in-law was a resident here for a short time before going into memory care. The facility is beautiful clean and the staff friendly and extremely helpful. My father-in-law Was very happy here. If they had had room for him to transition into memory care we would have kept him here

When the time comes where you are unable to maintain your own home or need somewhere to go then living at the Fountains is about as good as it can get. It is very clean and well maintained. When I came I was in a wheelchair and had to be under Assisted Living. After a while I was able to walk with a walker and finally by myself once again. The best part was that I was able to stay in my same apartment and did not have to move when I went under being an independent resident. It is a faith based complex and know of no other that has the benefit of having a pastor on hand at all times and we have programs that work for all faiths. All are friendly and have a bus to take us shopping once a week for necessities and other outings a couple times a month. They have all kinds of activities and you can be as active or as quiet as you wish. Lorna Claywell

Our whole family loves the care and surroundings my mom has at The Fountians, We moved mom and dad in there about two years ago, in the independent care status, when my dad left for his Heavenly home six months later, mom was able to stay in their same apt. and we upgraded to assisted care. She is 93 yrs old and is so content and thankful for the love and care she gets, (and so are we) . She loves the staff and knows them all by name, and participates in the many activities and options she has. The food is great,( my fist hand experience) and when mom can not get to her church down the road, she has a wonderful church practically down the hall. I totally know The Fountians at Hossana was our answer to prayer.

I have only lived here since June but so far I have nothing but good things to say. I love all the activities they have and that you can do as many or as few of them as you like. I have COPD and my breathing has improved since moving in. The place is very clean and well maintained. I would highly recommend this place to anyone looking for a forever home.

I live at the Fountains and I LOVE it. I love my room and the view from my window. The staff here is so helpful and nice. They all know my name and treat me like family. My Father has been here for over a year now, in that time I have to say I have never heard him say anything negative about his care, nor have I observed anything. I visit him often and the staff is nothing but kind and outgoing, from the kitchen staff to the front desk everyone is amazing. Huge bonus that it is connected to Hosanna so we can take him to church each Sunday, he loves it!
