GREAT STAFF! They are all wonderful with my dad. He was admitted in early March, and the lady who does the new patients was just amazing. She made everything so much easier on our family! They are also really good at communicating what is going on with your loved one as well. My dad has dementia, and he has good days & bad.

My mother was here for rehab for a short period and graduated from therapy. The therapy department is wonderful and we loved her nurse that was consistently present, attentive and so sweet and caring, Lexy. My mother adored her! She wasn’t keen on the food but overall a great place for your loved one to get strong enough to go home.

I had a very good experience being here at this rehabilitation center. Rebecca is a very hard worker and she treats everyone with kindness and love. This facility has alot of good and hard workers. I enjoyed my stay here and would recommend this rehabilitation center to anyone

I really like the nurses, i really am thankful for Rebecca the concierge and Camille the UM and Lexi a nurse, for all thier help every day. Justin the social worker is a great guy, helpful as well. I can't forget Eric in the kitchen for great meals and of course Riley the D.O.N. and i can't forget the NP Jen for keeping me healthy.

Optalis. I find this place to be an outstanding facility. The staff members are outstanding most of whom all go above and beyond to help you with your personal needs. Rebecca, the concierge is always there answering questions and doing the best of her ability to help you get things done that make things easier for you. The women up front always there answering the phones listening to problems taking messages relaying messages and making sure that public transportation is taken care of for individuals outstanding job ladies I believe that's Heather and Holly. All the CNAs / nurse aides. Going above and beyond doing the best they can to help make sure that the individuals that are less fortunate making sure that they're comfortable and their needs are taken care of. Outstanding job. The doctors, Dr Jennifer outstanding young lady great personality great bedside manner. Always listen to the problems the issues and does the best of her ability to make sure things are taken care of. The nurses outstanding. Kristy I love her dearly she is outstanding has an awesome fabulous personality always going above and beyond. Tracy is an outstanding nurse a little more of tight sometimes, but all in all an outstanding young lady as well. On a scale of 1 to 10 I give Optalis a 10 +

I called the admission director Heather in a panick and she was great at helping me navigate getting my grandmother into their rehab. From arriving to leaving everyone was great. Rebecca helped guide us even after the admissions part. Dawn and Andrea were her favorite nurses and The therapy department was amazing. Sam, Amanda and MiMi were so nice and caring with her in therapy. We are so grateful for everyone at Optalis. I would and have recommended this facility for both skilled nursing and rehab services.

Second stay for my mother in law. She has been there almost 2 months. I feel the staff is good with patients, but understaffed, which delays a patients wants or needs. Food portions are a bit on the small side, but food tastes good. Lots of activities to be involved in. Overall, it's a good facility and close by so we can visit almost daily.

I was a patient at Optalis Health and Rehabilitation Center in Three Rivers Mi. I was there for 2 years and 3 months the staff took care of me everyday. I would recommend Optalis Health and Rehabilitation Center to anyone of my family members or friends. The staff are very pleasant and ready to help when you need them to. Optalis has some great Activities to do every. I enjoyed my time at Optalis Health and Rehabilitation Center, but I am enjoying my time with my family at home even more now every day. If I ever have to go back I will choose to go back to Optalis Health and Rehabilitation Center in Three Rivers Mi.

We had the unfortunate decision to place our mom into a nursing facility early this year. Optalis was recommended by a number of friends who had chosen Optalis for the care of theirs. From our first meeting with Miss Heather, admissions director,we felt so comfortable our mom was in good care. The compassion and professionalism of the staff was exceptional.We can't thank Julie enough for all her work and knowledge in transitioning mom to the Medicaid system. You took all the worry and work load away from us so we could make this as pleasurable as possible for mom and us. A special thanks to the entire nursing staff, what wonderful care you have provided. You all make such a difference in your patient's lives. Mom has adjusted very well to her new home. It's nice to hear her talk about meeting new ,and some old friends, playing bingo and the other activities. Thank you all. You have been our angels Sincerely, Richard and Stacey Boris

I come to Optalis frequently with my mom, who is a nurse there. I always see how happy her patients are and how much the staff cares for them! I have fun when I get the chance to go to work with my mom and learn about the nurses’ jobs.

Wonderful atmosphere with upbeat, helpful staff. They welcomed my young children and I with open arms to play Bingo and share snacks while visiting together. It was great for my own family as well as the residents who were clearly overjoyed to have such spontaneous visitors. Optalis Health workers went above and beyond to focus on where they maybe need to apply more resources and seemed ambitious to solve any minor inconveniences that arose, and each time with a smile or gleeful joke. My son has already been asking about when we get to return for more games and fun!! Can’t wait to be back!

Optilas is such a warm and comfortable place to be, for rehab or skilled nursing. The nurses and aids are so nice. Such as, Angel, bridgette and Tory. There's so many more great ppl in this building. I love it here.

This facility has always been close to my family as it’s the one we’ve always used for family members. Since changing to Optalis, the atmosphere has been a lot calmer. Staff members care about the residents and always go above and beyond for them. My aunt has been here for just short of 3 years and I wouldn’t want any other facility taking care of her.

The meals from their kitchen look so good! Great portions & great options for residents. Don't even get me started about how amazing the nursing staff is, talk about caring and friendly. I am so relieved to find Optalis. They are unlike other facilities. The head of each department is a joy to deal with. They take excellent care of my husband and his complex needs. They are caring and well trained to take care of the needs of each resident. This place just keeps getting better.
