My 97 year old dad was there 8+ weeks for physical and occupational rehabilitation following a bout with COVID 19 and 2 week hospitalization. While the CNAs and nurses were generally pleasant and professional, staff turnover was extremely high, staff morale was horrible and it appeared to be under staffed. I was there with my dad every day spending hours and only saw his physician once. The shower was never cleaned and my dad was never showered the entire time he was there. He got C-Diff while there and was discharged to come home by the facility’s physician even though they (RN and doctor) knew he was very sick. Within hours of being home he was transported by ambulance to the ER, was admitted and died 10 hours later. He was a vibrant, mentally sharp person despite his age even after recovering from COVID. I do not recommend sending your loved ones here.

I give this place five stars of 10 out of 10 the place was very clean. It smelled nice. The staff was very respectful. The food was great. I really enjoyed my roommate that I shared a room with my experience was great. I really don’t have anything bad to say about the place. I recommend this for anyone that needs to be in a rehab center or needs to get help in the future, but my best overall experience was the intake manager. Brittany runs the first day. I met her when she did my intake. She was so nice respectful understanding there’s nothing that I needed that she did not help me with her smile brighten my day she come and checked on me every day to see if I needed anything to help me heal and recover, as well as on my journey to go back home. She was the best part of me being there thank you Brittany for everything that you did and for making me feel better and like I was at home.

My great aunt has a private room, which has been great for her comfort and privacy. I’ve met some really awesome people during our visits. We did have a small issue, but it was taken care of quickly, which I really appreciated. A special shoutout to Nurse Beck, who was very helpful and kind. The woman in admissions was also attentive, welcoming, and made the process much easier for our family. The food could be a little better, but overall it’s a nice facility with friendly people who genuinely seem to care. I’m very satisfied with our experience and would recommend it to others.

The facility is exceptionally clean, well-maintained, and has a warm, welcoming atmosphere. During my visit, Britney was incredibly kind, professional, and helpful. She took the time to answer my questions, provide information, and made me feel comfortable throughout the process. Her compassion and dedication truly stood out. What I appreciated most was how personal and attentive the staff are with residents and visitors alike. I’ve learned over the years of being a caregiver of a loved one and having to place them in rehab, that anything managed by Optalis, run the other way. I’ve had my live one at Fountain Blue in Livonia, Woodward Hills in W. Bloomfield, Manor of Novi & Optalis of Canton. Horrible experiences at each one. You almost have to stay all day with your loved ones just to make sure they are not neglected, mistreated or singled out. The CNAs hide their name badges, mean, sneaky, socialize too much. Leadership hides from responsibilities and do not call you back. To the Canton centers credit, they hired a Patient Advocate who fought for the patients. One time I was there to visit, they were so short staffed, they had 20 patients to one CNA & the head nurse did not oversee the responsibility of the CNA properly. Please RUN THE OTHER WAY, Canton needs to be shut down. Police came twice while I visited on a couple visits.

It was at Optalis of Canton that I found care, consideration, personal attention, safety, security, growth, challenges, and an all around very positive experience. One highlight was my PT time. Henry, Jamie, and Michelle took particularly good care of me, ensuring I was safe, secure, challenged, supported, and improving. Jamie went the extra mile to kindly and calmly care for me when medically mysterious episodes took me from PT two times during my stay. He was able to communicate my needs when I couldn’t and seemed to understand them intuitively. I felt secure, seen, heard, and understood during scary times, and for this I’ll be ever grateful. On the daily, Jamie’s humor and creativity met me right where I needed meeting. He ensured I engaged in fun and personally challenging exercises through which I was able to grow and progress. I knew I was safe. I knew I was celebrated. I knew I wasn’t alone. Each of these components remain invaluable as I continue to heal. Thank you, Jamie! And thank you, rehab team!!

I looked at several rehab facilities for my brother (Rob)and Optalis Canton was the only one that I saw people actually doing therapy. During my visit, Henry (PT) took the time to explain how therapy would work. My brother’s team Jaime (PT), Reenal (OT) and Sherry (Speech) took the time to understand what my brother needed to get better and the results were amazing. Then there is Gladys (Wound care) who did a wonderful job treating his wounds. Each day, the nurses and aides had encouraging words. We are so thankful that we chose Optalis of Canton.

I have been to many rehabs, but this is the best one I’ve ever been to because death is nice and caring and giving they care the patience they give to the patient and leather patient. I love how do recommend yes we have limitation number one number one if you need to rehab come here please you won’t regret it.™™ BTW, is always one CNA taking care of 29 patients by themselves that’s not right they need help understand I’m outpatient. I feel bad for people that’s helping us need more help. Please have to help that’s needed. It’s always one scene a thing of all of us like all of us repeat all of us by themselves only one CNA has heard on them out of them. I appreciate them and do good work with me more help. SMH makes no sense.

My husband Charles Hollis was staying at this rehab due to a knee replacement. First, of all I would like to thank all of the nurses and cents there for the amazing job t heh did with my husband. Brandi, was q of the sweetest and most helpful person. Every since he hot there she has been on it. She definitely k m own her job well. Secondly, we personally wanna give a HUGE shout out to the dietary workers Kesha and Dontrell. They showed up and showed out! Very friendly and fun to be around. Definitely gonna miss them. Thirldltthe PT's Barb did am excellent job staying on my husband. He's tr u ly g gonna apply all that healing g knowledge that Barb gave him. T he OT Reenal was very funny and caring. This place is very unprofessional. My mom did not get the best care here. She was here for 2 weeks and had to go back to the hospital. Her experience while she was there was unacceptable. We do not think this place is filled with kind and caring staff. There was only one or two people that were actually helpful and cared, one of them being Chasity. Thank you Chasity for being the only one who cared about our situation with our mom. Oh and they also lost her laundry while she was there and couldn’t find it, along with some other important items. Glad she is out of there and never going back.

The administrator here is the absolute best. She shows, empathy, she’s concerned about the patients, she’s visible on the floor, she’s helpful and the list goes on. The staff there are not the best! They are lazy, and lack empathy. Not all staff because there were some there that did an excellent job with my mom. The administrator is on top of things and she gets to the bottom of things when there is an issue. I’m confident with her leadership skills that I saw while my mom was there will get this place cleaned up, and make it top notch. Physical therapists Overall great experience at OPTALIS HEALTH & REHABILITATION of Canton. They have a friendly and welcoming physical and occupational staff. Including Stacy she was one of the first person I met when I arrived and was very friendly, loving and encouraging. Nancy my physical therapist is very knowledgeable and professional and also answered all my questions. She makes me feel safe, secure and confident. I hope this makes her feel good about the quality of care she gives to her patients and the great attitude she has. I really enjoy working with her. Mary is a hoot she has me looking forward to my appointment everyday! She is also a very knowledgeable and professional occupational therapist. I just love her to pieces. They all worked really well together and have me believing I am going to be one of their success stories, they can help me up and moving around and able to go home and live my normal life. Their positive attitudes make the difference in my care and confidence. Your rehab facility is amazing. The nurses are attentive and so helpful. The physical therapist and occupational therapists that work with me are so amazing, awesome and understanding given the amount of pain and stress I am in. CNAs What made it worth the stay was meeting the CNA aides and the Physical/Occupational therapists Precious, Princess, Rachel, Caroline, Jordan, Darrion, Karen and Danielle all are an exceptional CNA aides. Their extensive knowledge, professionalism, and welcoming demeanor make my stay at OPTALIS experience outstanding. These ladies are all one of my favorite staff members. (CNAs & therapists) Notably, Precious’s attentiveness to my needs and her unhurried approach truly set her apart. She treats me like family. She takes the time to listen, share insights, and ensure that I have a great rehab stay. Rachel, Precious and Darrion are hands down some of the most experienced and professional medical staff that I have ever come across. When they are on the schedule, I often refer to them as "the wizard". They has a tremendous ability to make their patients feel like family and offer the best personal care and attention to all their patients. Darrion & Jordan also take great care of me like family! These two CNA’s keep me smiling and laughing all the time. They make the rehab stay a positive experience. I would recommend this facility to my friends and family. I tell them you did a great job taking care of me, I’ll be lucky whenever I get you taking care of me. The warm accommodations of other staff members like the nurses, nursing administrators , physical and occupational therapist, housekeeping staff and the kitchen staff, all add to my overall exceptional service. If you're looking for a seamless and enjoyable rehab facility to come to or to bring a family member, I highly recommend OPTALIS and their fantastic team. I couldn’t have had a better healthcare experience than I did at OPTALIS HEALTH & REHABILITATION! The staff is wonderful and I cannot say enough nice things about my stay at OPTALIS. I was treated so well and the experience with the doctors and nurses was amazing.

I visited a friend at the facility about a week ago he is here for rehabilitation and was very nervous about coming to a facility. But I must say the staff was very attentive and made him feel very comfortable and at home. The Director of Nursing was very knowledgeable with answering all of our nursing questions which helped us with gaining an understanding. Catherine the administrator welcomed us with a big smile and positive attitude also providing us with direct contact for any other concerns we may have when she is not in the building. When entering the facility it was very clean and you can tell it was being well kept. They also have a very sweet concierge which is a bonus and she went over everything with us very thoroughly. I must say we were very sketchy about this choice but everyone made it very pleasant for us I would definitely recommend this place.

The nurses here are very kind and it feels very much like home with a nice place to lounge in. They are excellent nurses with each patient. The doctors will set up appointments for families to see where they are as far as their goals are long term. My mom has been there for 2 days so far for rehab, and we're not getting a great impression. She's 90, has dementia and uses a walker, but the place would not provide her with a walker or let us bring one from home until she saw the PT in a few days. That meant she would be at risk of falling if she tried to get up to use the bathroom by herself. Only after insisting on being provided a walker for my moms safety, did we finally get one. Very short staffed, no one is ever around. When I tried to call to ask about my moms condition the first night, I got a voicemail saying no one could be reached, 5 times in a row.

The nurses here are very kind and there's many things to do for daily activities including bingo a mini salon living areas that feel very much like home with nice couches to lounge in and look at the scenery. They work with each patient with their specific needs. Example my mother is doing physical therapy that helps strengthen her legs and learn how to use her walker and wheelchair. The doctor will set up appointments for family meetings to see where they are as far as their goals are long term. They serve breakfast lunch and dinner delivered to your room which you have access to a TV in your living space, to watch when you are laying in the bed with safeguards on each side so you have no worries of them falling from a bed. They also have access to an emergency button at their side for any reason they need assistance. And a personal bedside phone to call family or friends I highly recommend this place. It's a great feeling to see my mom doing so well. Lastly word of advice if your family member has favorite snacks I would recommend bringing them something to snack on between meals or after dinner.

They won’t get him up, they won’t give him a shower, I don’t believe they give him any kind of attention. If anything is said , we are simply ignored. I guess the only thing we can do is file a complaint with the state and try to get him out of here.

Place looks clean. Seems very understaffed. Most phone calls were not returned. No way to find out how your family member is doing. Took two days for my mother to see a doctor who sent her to the emergency room. I flew into state and they lost her belongings. While waiting another family was in lobby complaining....not great.

I was there in November until December 14th when I was discharged. This place is horrible. I went a week without a bath finally my caregiver complained and the head nurse gave me a shower. The nurse at night did not give me my medication but signed off that he did. I had to wait 5 hours at one point for pain medication. Some of the CNAs there are rude and disrespectful to the patients when they have to be changed at times. I was there because I broke my ankle and had to learn to walk with out putting weight on my leg and it was awful. I will never go back there and will tell everyone I know not to go there.
