Today, July 3, 2026, my partner was admitted for short physical rehab. The people are attentive but the facility needs urgent updates. Today is one of hottest days (95F) in 2026 and AC is not working properly. The central atrium /office area are cold but the actual residents/patient rooms and even nursing stations and hallways are not cool at all. I don't know how the place operates like that for senior people. I brought two fans from home to circulate air in my partner's room. I see some long term residents installed their own window AC units?. I do not know how tomorrow will be. Hotter! Just let people know since it seems CareOne Lexington team reads the post. Updated: AC worked better the next day. Nursing, physical therapy folks and medical team are very attentive so far.

The aids and nurses provided great care for my dad while he was first at the rehab Revere unit and then the Monroe long term care unit. The compassionate care and the kindness of the staff including the enthusiastic activity director was a comfort to my family, especially to my mom who was there every single day for almost 3+ months.

Our mother was in the Munroe Unit for 14 months (2025 to 2026). The staff there was absolutely amazing. We highly recommend CareOne in Lexington Ma. She had the same caregivers during that entire time who treated her with dignity, compassion and kindness. We visited regularly, and mom was always clean, dressed and in the activity room with other residents. Mom always said (I love it here, they take good care of me). The staff at CareOne Lexington Ma work very hard, work as a team and they take pride in their work. They love their patients like family. We witnessed the staff go above and beyond for our mother and others many times in the Munroe Unit. The facility is always clean and the staff always has a smile. At the end of our Mothers life the staff was extraordinary and supportive. Our family will be forever grateful for the comfort, respect, compassion and kindness the staff showed our Mother. From the day my Mom Patty arrived in December 2024 until her passing in May of 2026, the focus on care and comfort never wavered. I always appreciated the interaction she had with Joyce and her staff, especially DJ who spent time with Mom and made an effort to visit with her on her walks about. The business office worked with me for care options and Melissa, the unit manager, notified me about her status and her hospice options. I am forever grateful that Mom had this care team in the last part of her life.. thank you all so much

I came here for a visit. The stench of urine was so overwhelming that I could barely stay for an hour. I was nauseated by the smell like I’ve never been in my entire life. It was everywhere. Many patients appeared to be in soiled clothing. It was incredibly sad. I would never send a loved one here, I’m sorry to say.

Highly recommended. If they were 10 stars available, I would choose that score. Our Mom really enjoyed her time at CareOne Lexington. Most people don’t understand what a tough job it is to be a nurse or a nursing assistant. The staff are very kind and provided excellent care. After a year, Mom went on hospice care. When she passed, the staff were so wonderfully supportive of our family to help see us through a difficult time.

My sister Angeliki was a patient there twice these past 6 months with a 2 month admission each time. I have nothing but the best to say about Care One. From the cleaning people to the nurses to the administrators to the secretaries and everyone else that I may have not added . Many thanks to Jen Dyer which without her help my sister would not have been able to stay longer for rehab care due to insurance conflict. I felt safe and secure at Care One and overall was impressed by the attentiveness of the staff with the exception of perhaps a couple of occasions where I had lengthy waits for pain meds, etc.,particularly in the middle of the night. I was not particularly thrilled with the quality of the food, in general, but I realize it's an institutional setting and that goes with the territory. Maybe it's me not having done more legwork but I thought mental health could have been more readily available. Something that stands out as exemplary was the team switching me into a new insurance plan which enabled me to stay longer, etc.. I guess the fact that I would not have minded staying longer at Care One is testament to the overall quality of the facility.

I was a patient at the CareOne facility in Lexington for a little more than three months while recuperating from a broken ankle in one leg and a serious infection in the other leg. My overall opinion of the treatment I received was excellent. The nursing staff provided outstanding care seven days a week. Experts from other local hospitals would visit when a second opinion or another level of expertise was required. The therapy department at CareOne in Lexington has a well-deserved reputation for effective treatment and results. Both the physical and occupational therapists excelled at setting goals for me and working diligently to help me achieve them. The food was surprisingly good. The kitchen staff records your preferences and needs when you first arrive and three times a day, they provide nutritious and enjoyable meals. There are always several choices to choose from. Special note has to be made here for Mark, the recreational director, who is amazing. He knows everyone’s name in this large facility and always has a smile while he engages patients with endless games, movies, special holiday events, and even live music once or twice a month. The last performer I heard before I left was a well-known Elvis Impersonator and he put on a fantastic show! Mark’s loving and upbeat personality is a powerful medicine you can’t dismiss. I also want to single out the executive director of this complex facility, Ryan, who has skillfully hired some of the best nurses and aids in the area. While I am certainly glad to be home and walking on my own two feet again, I miss some of the wonderful friendships I enjoyed at CareOne in Lexington. Also, getting a warm breakfast served in bed every morning at 8 a.m. was nice, too!

Can’t give a zero so I will go with 1, the smell is overwhelming, bed separating curtains have what looks like blood on them, toilet seat has rust and not sure if is feces on it. Bed would not work when I first got there. Walked in to have husbands roommate with his [language removed] hanging out of his diaper. I am working right now to get him the [language removed] out of there. Notified nursing station of medical issue and response was negative at the end. Do not go here. I literally am calling a cab right now to go him.

I was recently admitted to your facility for PT. I want to write to express my gratitude for SOS in the Hancock Unit. SOS treated me with nothing but respect and dignity. He went WAY out of his way to make my stay comfortable and stress free. Not only did he do that for me, I saw him do it for every patient he encountered. We need more SOS’s in the world. I spent a month here and it was pretty good. There were four people that were very nice then there were a few that wasn’t but I have to say that can belive that at any business. Ryan is the main guy that runs the show there very nice guy easy to talk to and if you have a problem he’ll try and solve it. Then Cheryl, who is right under Ryan made me feel very welcome when I got here. then there’s four nurses who are very nice and very helpful the other ones are nice also but the four I mentioned go above and beyond to be helpful to the patients needs.

After posting a review that severely criticized the facility for not warning me that covid was in the facility when i visited a patient, i was contacted immediately by the administrator and the corporate office. Through my conversation with the administrator, that took place on a sunday no less, we determined that my exposure happened just one day before covid was first discovered on the ward. At that time, warnings were posted in the facility and visitors were encouraged to wear masks. Unfortunately,it was just a case of very bad timing.

Care One is an excellent facility. My grandmother had a broken bone and was sent here for rehab/physical therapy. The staff were so good to her. They really listened to her and were so kind even when she got frustrated. She would call me all the time to rave about the all the staff and how kind they were. The whole team is superb and we are so lucky that my grandmother got to go here. I highly recommend this place to anyone who has a loved one that needs some rehab support after a significant injury.

For a post-hospital stay, my family member (80 yo) stayed at CareOne Lexington for about 1 week. He was very dehydrated (part of the reason he had been hospitalized), but when a friend visited the first day, no one had provided water or anything to drink in his room. When the nurse was queried, she said that he'd had juice at lunch. This was NOT enough fluids. Also, it took 25 minutes for anyone to respond to the call button. When he was transferred out of the facility, no one thought to send along his walker, which he needs. I live out of state so couldn't monitor everything in person. Also, although the room was clean, there were also a couple of roach traps (with roaches) in his room. Not a pleasant sight. The administrator and social worker were pleasant and responsive, but obviously someone, somewhere was falling down on the job. I definitely would not want to send anyone to this facility unless I was assured that its deficiencies were addressed.

The staff was wonderful! It started with Erica in admission’s and having the intake process be streamlined and easy to understand. Colleen and Erin with OT, Cherilyn and Lalitha with PT, the nurses -especially Thamar and Christele were exceptional! Caitlyn with social services and aides, Maricelena and Clementine were helpful, kind and encouraging. So many other staff members we did not get their names but- meal delivery, cleaners etc. also did a wonderful job. My mother had delirium after her surgery the care team at Care One was instrumental in helping her come out of it and heal. She is back in her home and doing so well. We would highly recommend Care One to family and friends.

My 96 year old father was at Care One Lexington for 7 weeks (Jan 2023) before he was transferred to the VA Hospital in Bedford for hospice care. Care One was exceptional with him. The staff took such good care of him. I was there daily and he was clean, groomed, bed was changed daily, room cleaned daily. They made sure he had his favorite hot cocoa. I thank them for their compassion and hard work.

If I had reviewed this business a few years ago, there would not have been enough negative numbers. Back then, it was a mess, award nurses station not manned, inadequately trained staff, sloppy housekeeping, etc. Now I am very happy to report that it has made a complete turnaround. The building, while still needs more room around the outside of the building for vehicles, has a very clean appearance inside. Bright lighting, clean floors, all staff -badged. With resident medical staff either on property or on-call, CNA'S everywhere. And just because someone is a Physical Therapist , does not mean that they do their job with blinders on. If they notice some about a patient, they notify the medical staff. The food is more than adequate. Very tasty and varied. Don't get me wrong. As with any care facility there are problems. This one, the CNA's tend to get lax in their duties at times. Not knowing each individuals needs and requirements. A regular training course each month would be helpful. Language is sometimes a problem. Again with the CNA's. . English only !

I was extremely happy with the care that my elderly mother received while at CareOne Lexington, MA. My mother was in a private room for 21 days that I found to be impeccably clean. She was given exceptional care from the nursing staff. My mother suffers from Parkinson's disease and is unable to feed herself. The CNAs took their time to feed her each meal, which is a 45 minute process. I found the whole Staff at CareOne to be extremely professional, kind and caring.
