This facility exceeded all my expectations. The therapy team was knowledgeable and attentive, creating a personalized plan that helped my dad progress quickly. Nursing and support staff were consistently kind, efficient, and respectful. The entire operation is well-organized, clean, and focused on patient success. I would confidently recommend this rehab center to anyone needing short-term recovery.

My brother recently had a recovery stay here. We thought it would be a month but ended up only being 2 weeks. They were very proactive in his recovery. Visitations were simple. Facility is clean with great staff!

My aunt was admitted to Brickyard Portage for rehab after a hospital stay. During her time there, the Therapy team was amazing. They encouraged her to regain her strength for a safe discharge home and supported her during the process; they even followed up with her inquiring about her progress since discharge. Nursing leadership were very knowledgeable, the DNS answered any all questions respectfully and gracefully. I am very grateful for all the staff who cared for her, and the compassion given to my family during this tough time.

This facility went above and beyond for a recent family member. Making sure he was comfortable and well taken care of. They went out of their way to accomodate any of his extra requests even though we all knew they were busy over the holidays. Of course nursing homes get bad reps but myself and family being in the medical field understood that they truly are selfless here trying to accomodate their patients.

Mediocre at best. Didn't follow thru on post op required procedure. Didn't provide important information on patient discharge.

Do not I repeat do not leave your loved ones clothes at this facility with them. They will disappear. Been dealing with them trying to find my husband's clothes for over a month. Bought him brand new clothes over the last few months . Can't keep affording to huy clothes. I've put grievances in I don't know how many times yet nothing gets done. I mistakingly left my work jacket in his room and now that's gone too. Make sure you take pics of all items brought in this place plus the front desk is supposed to have u fill out an inventory sheet which at the beginning I didn't know that. All they would say is we will have them put his name on his clothes Didn't realize they were ironed on so when the clothes get washed they will eventually fall off. Missing 8 shirts at this point and a couple pairs of sweats. I get really upset when they only have my husband in a hospital gown because of this issue. His nurses and aides are all nice. Just not happy with laundry. Had bought a laundry basket for my husband's clothes and that I will be washing them myself. Had to buy him more clothes. Someone apparently doesn't read and took his clothes out of his basket to laundry which they are not to be done here nor or they to be taken out of here. But they did and some of his clothes are not tagged with his name on them so again he will be missing more clothes. This is unacceptable. Sorry I'm posting this but people need to be aware of what's going on Don't know what else I can do. Putting a note above his bed using black marker doesn't matter. Some people don't read. I guess I have to come up here at 11:30 at night to remove his clothing from that day so nobody takes them. I don't know what else to do. Grievances don't seem to do any good.

Remarkable Radiant Regina ( The Social Worker) was absolutely amazing. She truly cares about our family member. Her care and compassion deserves to be commended. She deserves a trophy. This facility has truly been a blessing. We are thankfully thankful for all that you do. God bless you all.

Wonderful Wednesday evening March 12, 2025 I came to visit my Granny after work. I must commend Nurse Sharon for her super supportive spectacular care for my Granny. I also want to say thank you for your staff Ollie and Tee because these young ladies were so kind and patient with my Granny. I am so thankful for all the workers here. They are doing a wonderful job. God bless you all. You are appreciated.

I am so happy my sister has found a good home for her to be taken care of. The staff is very caring and very accommodating to the things she asks for and needs. I highly recommend this nursing home as it is also very clean in the food is very good.

I been patient at brickyard health care center is a great place I was a resident there for 3 months and the staff is incredible very compassionate hard-working down to earth caring The food is very good and physical therapy is the best My physical therapist name was rod and a director who wants a place Latoya she is the best very cured and compassionate very kind and understanding The patients is a little bit of everything there I was there physical therapy mostly everybody was but they had other things going on with them but I focused on myself I was able to see mental help with a psychiatrist physical therapy share for my my health care that I'm going through the nursing staff or Nita is very good My best nurse was Jesse and a sea wing but I could honestly say this place was very caring kind understanding welcoming if I made any spelling areas please forgive me I was using Google speech to text ‘

My sister spent two months at Brickyard. While it is far from perfect, we believe she received good care here. The nurses and aides were kind and attentive in meeting her needs! They all communicated with me regularly. Her room was cleaned every day, and she was kept clean and comfortable. I would like to give a shout out to all the therapists! You did a great job helping her get strong enough to return to her assisted living residence! Thank you!!

I'm a resident here and on Wednesday the eggs we're bad they we're squared and toast was burnt breakfast was very nasty lunch was very delicious smoked sausage sauerkraut was absolutely delicious that's the best smoked sausage sauerkraut I've ever had

My boyfriend was recently here for rehab. The social worker, Regina, and the discharge nurse, Mia, were very kind, professional and a pleasure to work with. Being in a rehabilitation facility is sometimes not a pleasant experience so having caring, trustworthy and skillful staff really makes a difference. I really appreciate you ladies and everyone else at Brickyard in Portage.
