While visiting Forest Creek Village, I noticed a staff member named Robert who truly stood out. He was clearly busy, yet still took the time to notice a resident sitting alone. He warmly engaged her and included her with other residents, never realizing anyone was watching. That small moment spoke volumes about the culture of this facility. Robert was a natural caregiver, kind, attentive, and genuinely present. From that moment on, I knew this facility had a friendly, local neighborhood feel where residents are truly cared for.

From the moment I entered Forest Creek Village, I could feel the warmth of their staff and residents. I have worked in healthcare for 28 years and the leadership team at this facility is top notch. You can tell they truly love their job and it shows. I was most impressed with seeing the residents with a smile on their face and waving their hands saying hello to me. The building did have some renovations happening but it looks great and smelled good. I was pleased with my visit.

I have a brother in Forest Creek Village. I could not be more happy with the choice we made. From the time you walk in to visit, you are greeted by a very pleasant and with a smile receptionist. The office staff, nursing staff, nursing aides, cooks, and cleaning people are all nice and most know me by my name. Most of all, the staff was as supportive to me, as much as they were to my brother. I’m convinced the negative reviews are fabricated. Forrest Creek is amazing! The facility is always spotless and the staff is always so kind. Everyone our family has encountered here is incredibly helpful and friendly. Other facilities we’ve experienced were very dirty and the staff made it even more miserable. But here at Forrest Creek, we love it knowing grandma is safe, happy, and properly cared for. She loves her bingo days!

The way ALL the staff cares here is special!!! It brings me joy when I come in and see the nurses and aides having so much compassion for the residents. Danielle the DON goes above and beyond for her residents. She has a great ADON who is the same way when it comes to her residents and their care. You can always find a resident the office of Ashley, Social Work. She visits with residents all day long. Matt, Administrator is great. He is easy to talk to about anything and he always comes through for his team and residents. Housekeeping does a great job keeping the facility clean.

The staff here has been very friendly and accommodating for me and my students. The care the staff provides the resident's is above reproach. They really go above and beyond for each and every resident they care for. I would like to recognize the DON Danielle and the ADON Jennifer for the amount of help they have been at making this site a great experience for my students. Hello my name is James Mitchell I was in Forest Creek Village few years ago and met my best friend Mary Lee Miller we became very close while we were in there we watched movies and drink soda and ate Candy and had a blast she was a older woman and I was younger but our friendship and our bond grew very tight We Were released a year later I decided to move in with her as she was all alone in her house sad to say recently she has passed away we always used to talk about our time at Forest Creek Village and laugh about the stupid things we had done and everything as I was going through some of her belongings I found a note I will quote her exact words the best care in the world is here at Forest Creek Village remember you're the very best Mary Lee Miller just wanted to share this with everyone if you need their help look them up and let them explain everything to you and get the help you need I couldn't walk when I got into Forest Creek Village and I left walking and now I am walking and getting on with my life I wish my friend was still here with me but she's not God Rest her soul remember Forest Creek Village is number one and Miss Patrice will take care of you make sure you're okay and that you're getting what you need and so will the staff. I had the pleasure of touring this facility late one evening recently. The front entrance feels warm and welcoming. We were instantly greeted by the receptionist and a few residents when we entered. The residents were smiling and talkative and were just as welcoming as the staff! It was evident from the time we walked in that the residents are well cared for here. We toured the entire building and were greeted with a smile by every staff member we saw, and multiple residents as well! It was so comforting to see so many happy residents and how caring and interactive the staff were with the residents as we walked the building. Parts of the building are older and not as updated as others, but the entire building looked and smelled clean, there wasn’t a single bad odor or untidy area throughout the entire tour! The most impressive part of the entire 7pm evening tour was meeting the ED, Neha, the DNS, Danielle, the medical director, Dr. Sweeny and his NP, Amanda, at the catered meet and greet for residents and families they were hosting that evening!

I was there 6 years ago for 6 weeks rehab , following a serious crohns disease flare up. The staff was Absolutely Phenomenal and Patrice is an Angel. My aunt has been at Forest Creek Village for five years. The current staff - Jennifer Hinton, in particular have been just wonderful to her. Amiyah and San have been terrific as well. My aunt is progressing into dementia and has never complained about any of the staff. She would say something if she was being mistreated as she has complained to me about certain roommates over the years. She says she is well fed, in fact she eats too well! I've only been able to visit her twice this past year due to Covid but I've felt comfortable knowing she is being cared for by Jennifer, Precious, the nurses, the certified nursing assistants, the kitchen staff, therapists and activities personnel.

My Mom is here for end of life care. The staff has been wonderful. They are keeping her clean and comfortable. They are very sweet to her and even filed her nails for me! My Dad has been here for 3 months, I don't know if that is time enough for a fair appraisal. The outside facade is beautiful, inside is old and sad looking, no updating since it was built many years ago. It is maintained and fairly clean. It is small and cramped, halls are narrow for people in wheel chairs to negotiate, there is no place where you can meet in private with a resident. My friends told me at first not to judge a "book by its cover". The care was what mattered, not the appearance. I've found so far, they do not follow up on what they say they will do--medically or in social activities. To inform patients of changes in dining room schedule they announce at meals in a very noisy, echoie dining room the change. My Dad is next to deaf he never hears these announcements and has gone for meals at the wrong time. He has trouble with time and date concepts, there are no calendars or clocks in the building which might help a little. RX drugs appear to be given properly, so far with my Dad but other things such as drops applied to ears for wax removal, patches for nerve pain are applied slip shod, the pain patches fall off nearly as fast they put them and they can't seem to figure out to tape them around the limb to keep them on. The business administrator is really good on taking care of the big things (such as Medicaid apps) but lackadaisical on things like getting payment wirelessly transferred from the payor to their account. The Therapy department is the best aspect of the facility. They began work with Dad the minute he came in, they are very personable and communicate with me frequently and professionally. The admissions person is fantastic, of course, but absent quite often. There is very little personal interaction between the staff and residents (except for giving basic care). Staff can pass a patient and never acknowledge their presence let alone greet them by name. I was told doctors would communicate with us after visits, I have yet to get anything from a single doctor, getting information from the staff is as painful as pulling teeth without Novocain. I've tried for weeks to get a form to have my dad seen by a doctor for a specific problem, still waiting and asking. I'm not complaining without taking action. I am there 3 or 4 days a week, 2 hours each time usually. After 3 months of asking who do I talk to about... and getting a different answer each time I was told they have a "customer relations rep. Surprise! There is a sign on the back of every room door with that information--who goes in and looks on the back of doors? They do not want the room doors shut. I don't think I've been unfair, this is all from experience and I've addressed every one of these concerns to them at least once. Dad is on a waiting list elsewhere waiting, waiting, waiting. I need to at least find out if this is is normal nursing home behavior and procedure.

My mom has been in three different homes. They all have good ,and they all have bad. So far Forest Creek Village staff has been good. Just as good as the other two homes. Putting your relatives in a care facility is not easy. I believe that is the whole problem. It will never be as good as there home and independence. It's not the same life to them or to the family. It's hard to put loved ones in hands that are unknown.we have to work with whatever place they are in. We can work through problems. The staff at any facility will work with you.
