I’ve been here for almost 14 months and my experience has been exceptional. The nursing staff and aides are courteous and supportive. The dining services are well planned and the staff aim to serve a meal and experience that is memorable. The activity department seeks to program events and activities that cultivate relationships and foster community. Our housekeeping and maintenance team is responsive and thorough when it comes to attention to cleanliness and whether the rooms and property is in good repair.

The campus is nice, clean, and welcoming! The staff was very helpful in assisting us with finding a place for our loved one to receive rehab. We really appreciated their guidance and support throughout the process.

Such an excellent therapy team! If it wasn't for them I wouldn't be walking now. The whole building has such wonderful nurses and staff and I am so grateful I've gotten to meet them all. If it wasn't for them I wouldn't be able to get around or get by. I appreciate everyone here. I love everybody and they all love me. Everyone looks out for me and makes me feel awesome every time I come here. I can't explain how great everybody is here, and any time I'm sick I'm coming back here. I want to express my thanks and gratitude to the team at Clearvista lake health! We were in quite the quandary as to the next steps we should take with my brother n law. Kate, director of admissions, sat down with me and my family and discussed our best options for his care. The transition couldn't have gone more smoothly - the staff was so sweet to him and he decided one of the nurses was his bridge partner. It was so comforting knowing he was getting well taken care of and making friends. If you are at a crossroads and need assistance and a place for your love one to stay - I highly recommend Clearvista.

Not a good place to go for Rehab after you are sent from the hospital. My husband had his heart medicine lowered because of Pneumonia problem. His heart medication got lowered because of this treatment. He was sent to this Rehab center -- for 3 weeks -- w/o checking his heart medication?? As for rehab, he only got simple exercise for 45 minutes during Mon thru Fri, which he could have done better at home AND could have seen his heart doctor to update his heart meds.!! -- During his 3 week stay at this Rehab, he only saw doctors once a week who did not suggest his lowered meds. while in hospital. -- Do NOT go to Rehab after getting discharged out of hospital. Go home do see your doctors and do outside Rehab facilities after being discharged from hospital. -- While in Rehab, you do not see doctors about your condition and the Rehab is a JOKE. -- My husband almost died because his heart med (Water pills) were lowered for the 3 weeks in Rehab. and the Rehab ?? did not do anything but get him closer to death. -- A day after he was released from Rehab, he almost died. We admitted him to IU North that fixed the heart problem he had gone thru from the Pneumonia problem to Rehab -- but that messed up his heart problem while in Rehab. -- Do not do Rehab because they cannot fix you medicine problems and they not help with REHAB problems -- very low, few exercise problems!

My mother lived here for several months and was very well taken care of by the excellent staff in the assisted living area of Clearvista Lake health campus. We’ve worked closely with the Rehabilitation Department, Megan and her team are truly a cut above anything we’ve encountered throughout our journey. The Assisted Living services are just as impressive. Every department’s staff has been incredibly supportive and kind. While I could highlight a few standout individuals, I also want to recognize the many behind-the-scenes team members who have been equally vital to our experience. The facility itself is immaculate”you can tell from the moment you step into the lobby. You’re warmly welcomed by Cheri, and it genuinely feels like you’re chatting with a longtime friend. The resident programs, led by Destiny, the Activity Director, and her team, are engaging and create a strong sense of community and belonging. This facility is AWESOME...in many ways!!. I have experienced a truly gracious, compassionate, professional group of people in every area of service, which is very unusual. Not once have I felt that what I asked, needed or felt concerning the care of my husband was unimportant. From phone conversations, normal visits, and critical decision making meetings, they have made me feel that my presence and concerns were important and were effectively addressed! My husband also expressed to other family members how comfortable and happy he is to be a resident at Clearvista Lake!! I wish I could leave a 10 Star rating if that were available, but I guess 5 will have to do. Much thanks, love, prayers and appreciation to this AWESOME group of people!!! My wife Pamela received care and kindness from both staff and residents of Clearvista Lake Health Campus for almost two and a half years. During this time, she enjoyed visits led by staff to the Indianapolis Zoo, a Cat Cafe, a Christmas Lights show, and more. Pam loved to walk with friends and family to greet and talk to the residents and staff in the rest of the facility. When weather allowed, Pam walked outside with friends, family, and staff. Pam enjoyed the many wonderful holiday activities planned by staff; she loved to paint and work on simple jigsaw puzzles. As Pam’s health declined more rapidly, in the last half of 2024, her Clearvista caregivers worked especially hard to keep her actively engaged and they carefully monitored her medical needs. Pam’s Clearvista family not only took good care of her, but they took good care of me as well. My experience with the wonderful people of Clearvista Lake HC was much more positive, even amazing, than I ever imagined. I will forever be grateful.

Today, 8/26/23, we were visiting our mother. Mom wasn't feeling well. We mentioned to the staff and we were told the nurse would come in to check on mom. Mom was going to sleep, so went to the lobby to sit a while. It seemed like minutes later, Nurse Dani found us to report to us mom's condition. We very much appreciated the promptness of Nurse Dani's response. Thank you Nurse Dani.

My sister was in the facility for a month prior to her passing. Based on experience all I can say was that I am so glad we choose this facility. It was clean, they treated their residents with respect, kindness snd patience. My sister was always clean her room was clean, to me the facility was ran like a hospital. I selected Clearvista Lake Health Campus for my mother to come for rehab after experiencing a traumatic fall this year. From my first meeting with the staff members giving me a tour, and now moving my mother to the assisted living section, I have experienced nothing but professionalism beyond measure. You are greeted with smiles and friendly hello's from staff members as soon as you enter the facility. My mother's care has been absolutely wonderful throughout her stay here for the last several months. The staff have worked to help her regain much of her independence, offered support and compassion when she felt frustrated, and supported my family throughout this ordeal. If you are searching for a state of art facility that also offers professionalism, compassion, complete medical care, and friendly faces this is simply the best choice. I am a caregiver for a patient there. The staff is wonderful! The place is very clean and the building always smells nice. It’s not like any other rehabilitation/nursing home I’ve ever been to. Management was proactive in assisting with the best next steps in her care plan and the executive director, Justin’s main focus was that she received the best quality of care. Clearvista Lake Health Campus is a gorgeous building inside and out! They have several perks. They are known for their semi-private large double rooms. The therapy gym is huge, even with a workout station outside. The activity program has lots of opportunities for your love one to be involved. I even heard they're going to have outdoor entertainment, walking clubs, picnics by the water and movies on the lawn. Sign me up! You are right across the street from Community Hospital.This does not sound or look like a typical Health Campus! Don't take my word for it, check it out and see for yourself!

Clearvista Lake Health Campus is a very beautiful campus, with a very loving staff. The nursing staff treat all residents as if they are all family. They are very attentive to the needs of the residents and the families. The therapy department work very diligent at getting residents up and mobile in the shortest amount of time. The management team go above and beyond for the families, residents, and staff. Everyone at this building from management to housekeeping go above and beyond for all residents, and truly have a servants heart.

My father has been in the rehab portion of Clearvista Lake Health for 3 months, after another stroke and the care has been exceptional. We are moving him to the assisted living portion of this facility and can't express how helpful, professional and caring everyone has been that has helped my father. My husband has been a patient at Clearvista for close to 4 weeks. He has been well cared for by the nursing staff, aides, activity staff and administrators. The occupational and physical therapists are top notch! The cleaning staff are so friendly and provide the same great care to his room that the medical staff provide to him. The social worker has been extremely helpful and knowlegable. The front desk staff person makes everyone's needs her priority. All in all a great experience for us!
