Elevate of South Holland is great!! My mom have been here for 2 weeks everyone has been so nice extremely nice it haven't been one time I have been there and haven't been treated with kindness my mom says she have nothing negative to say and she is hard to please. The staff answers when my mom put on the nursing light she is not waiting they come very quickly!! DJ takes really good care of my mom she is an exceptional Cna when my mom have her I rest easy she is everything she dont missa beat and always smiling!! Danita in Concierge is phenomenal so nice easy to talk to always respond to my needs when I call and get things done in a timely manner. Ms Carol another Cna and Teri Hamilton the nurse I love her spirit she is also very nice!! The wound team big thanks to them especially Tiffany she is so nice and patient a pleasure to speak with. Its a few more I just dont remember thank you to the rehabilitation team as well and thank you Elevate of South Holland for taking care of my mom being so kind caring and friendly and doing all that you all do on a daily basis I know it's not easy task but you all have a way of making it look easy. From the time I entered the facility thru the time I exited it was pleasant. I was greeted when I walked in and was given instructions to the room of the person I was visiting. The place was cleaned and smelled fresh and the staff overall was kind. I visited my friend’s mom and she to stated they were very kind. The bathrooms were clean as well as the rooms. There was someone cleaning when I came in and when I left. Keep up the great work, it’s a joy to see

The Staff is so friendly and nice, whenever I call about my Sister they were always available. I really know what I would not have done without , Vicki, Danita , Timica expertise and Patience, and there kind words of encouragement. My close relative stayed at Elevate Care on two separate occasions, and both experiences were very positive. The CNAs were attentive, kind, and always willing to help with even the smallest needs. Their patience and compassion never went unnoticed. The physical therapy team was also outstanding. They were knowledgeable, encouraging, and really worked hard to help my relative regain strength and confidence. The social workers were equally supportive. They were helpful in communicating with our family, answering questions, and making sure everything was in place for a smooth transition. Overall, the staff at Elevate Care showed genuine care and compassion. We are grateful for the dedication of the entire team and the role they played in my relative’s recovery.

My brother Joseph was here at Elevate Care South Holland. When you enter, you can sense it was well organized. No offensive odors, patient room and common areas clean. The staff is very patient and family friendly. Social worker Kia, the Director of Nursing, the Doctor, the Physical therapist, Occupational therapist. Speech therapist and last but not least Travis and Donita. If there were any concerns, they were addressed in a very timely manner. Thank you Elevate. The start of 2026 was difficult, after four back to back hospitalizations my mother was left with declined mobility and unable to care for herself. The insurance denied her inpatient acute rehab referral even with appeals we were left to figure out her care on our own. With less than 48 hours, I found Elevate South Holland via personal search. I placed a call and spoke to Travis Crockett . From the time I walked in the facility, Leah, the receptionist - was very sweet the smell was pleasant , the staff greeted me, and the tour was very thorough. Travis asked the most important questions to anticipate my mothers arrival and the appropriate level of care potentially needed. Travis set a very high expectation and it was met throughout the duration of her admission by the Nurses, Diaylsis Team, Providers, Dietary Staff , CNA’s, and Physical Therapists. We were supported by the Social Worker Kia and Business Office, Constance & Timica. Denita with Guest Relations made sure we had all the support available regarding offerings available to her while admitted. Deonna, the unit manager made herself available to ensure infection control was managed timely for the quality in safety of other patients. Isaac, the director of nursing - Ebrihim ( very efficient) Jazmine, Toyin, Ashley , all of the Nurses were attentive to the residents. Kevin, Tiesha and Rae were exceptional in care as aides. I witnessed their shift in care of others hygiene & personal care (bathing). Our experience was very special and I appreciate Elevate to the utmost. I feel terrible as there were more people than I can count that was absolutely outstanding, from the activities coordinator, to John the administrator. This was a very great experience. In the event we can no longer provide home care for mom. Elevate South Holland will be our 1st choice. My mother was unable to stand. The following is her with family while out on a weekend pass. With The Help of Kelly and her team - Anthony & Tammy from PT the assisted her back to standing and walking 30ft as of today.

CNA’s appear to be overwhelmed and overworked. For me The wait for patient care was entirely too long. There were three nurses assistance in particular and showed me great personal care and I want To give them commemoration. Daphne Grace Evelyn James All the great service on the second floor They were all very genuine and authentic And show great personal care. I appreciate them very much. Tim was a great nurse on the second floor He showed great patience and personal care, and he was very authentic and genuine. The physical therapy staff was OK also and the scheduler for my ride

My mom was a resident of Elevate Care for almost 4 years (as of March, 2026). She made her transition last week and I would be remiss if I didn’t thank the ENTIRE 2nd floor Nursing and Care team for the love, support and friendship they provided to my mom, Mrs. Harding. Please forgive me if I don’t recall all the names, but Kia, Danita, Raven, Rachel, Brittany, and Val you were part of the family, and that does not go un-noticed. To anyone that interacted with Mom, thank you. God Bless You. I know that others may have different experiences when having to make difficult decisions about the care of their loved one. Stop in for a tour after a long and stressful day. The building seemed Super Busy but one of the best tours I've had looking for placement for my love one. Admissions director Travis was very knowledgeable and helpful. Everyone was nice with smiles on their face. Great experience.

My wife is here in this facility pretty familiar with Elevate Care because I have family in another building as well I'm very fond of Tish the Activity Director she's very personable and encouraging. She also does amazing with the activities she does . At first I wanted to take her out of this facility but thanks to Tish I feel comfortable with her being here .

My mom is here my first time here today everybody was nice and they introduced themselves as well her room was clean a little dark and outdated but other than that it didn’t smell at all so at this moment I don’t have any issues or concerns so far. Well today is my moms last day at Elevate these last 2months while she was here they took good care of my mom from the staff,CNA, nurses, nurse practitioner and also Kia the social worker. I want to say thank you guys for all the help while she was here I would recommend this facility if you need a place for your loved ones

I'm so grateful for all of the staff at Elevate Care South Holland. My dad's 1st night and day was rough as a new admission but they handled him professionally and kept me informed. Now that he's gotten used to the staff he is calmer and doing much better. The facility is clean and the patients seem well attended to. I've been very impressed with this facility so far.

My mom was admitted to Elevate South Holland about 2 weeks ago and "so far so good". The Administrative, Nursing and CNA staff have been professional, supportive, and responsive, making this unexpected health crisis a little easier to navigate. Sending a special thank you out to Danita and Kia for their professionalism and regularly checking in with me to ensure that all of my mom's needs are being met. My mom's regular CNA, Erica has also been responsive to all requests without hesitation, although I know her job isn't easy, as well as all of the CNAs who have helped my mom. Kudos to the Nurses who have also been great. And, PT and OT have not disappointed and are helping my mom with her strength and mobility. A minor downside, the place is a little outdated and could stand some cosmetic work, but the care is why my mom is there, so as I stated in the beginning, so far so good.

My husband is at Elevate Care Nursing Home on a temporary basis for rehab. He has been there for 1 month. During this time I have been updated by the nurse, the nurse’s supervisor, the physical therapist’s supervisor and the wound care specialist. They are all very professional and thorough with their updates, leaving me with calm/relax feelings. The receptionist, doctor appointment scheduler, the physical therapist, the CNNs and actually the entire staff have all been very nice, professional and helpful. I am scheduled to have a conference call with them in a few days. This is very comforting. My husband is enjoying the food and feels safe in his private room. The place is clean and updated. I feel comfortable knowing my husband is getting the great care he needs and deserves. I have no doubt that he will come home stronger than before and will be ready to get back to his normal daily activities. So far, so good! Thank you and bless you all for your individual services!

Elevate was amazing! The receptionist was pleasant, the checkin process was efficient & the building smelled fresh and clean! Often nursing home facilities smell of dried urine, but Elevate smelled amazing. The maintenance staff & nursing team are doing a great job. The CNA who assisted my aunt was funny, personable & loving. It was apparent that she takes pride in her work because her bedside manner was amazing. My aunt’s nails were done, her hair was freshly braided & she smelled fresh. The room was inviting & warm. Great experience! Highly recommend.

This place has horrible customer service the workers. They are unhappy, and they complain to the patients letting them know that they are overworked and underpaid when you go to the director and try to talk to somebody to handle the problems in a appropriate way they dismiss your issues say they’re constantly in a meeting or you might get two days before somebody reaches you regarding your loved ones care and it could be something a serious mis management of medication my mom developed bed sores there and they asked her did she have them before she came trying to pass blame . Also, the employees there they are stealing items from their Patients we are doing a police report so please if you have people that go there take their identification cars, their debit cards and anything personal that they can get because they are being victimized at their location

This facility has the best therapy group (PT, OT, speech) and wound care team I have come across dealing with long term care facilities as an RN and now as a family member of a resident. My family members strength, mobility, and mental acuity was brought back to how he was previously before hospitalization. His wounds (admitted) were properly treated and healed in no time at all. I admit my visits are not always perfectly pleasant dealing with some of the nursing staff but overall the good staffers outweigh the not so good ones. My complaints are heard and dealt with quickly and professionally. It is not easy to feel comfortable leaving a family member in these facilities but there is a level of peace knowing that our concerns are heard when we voice them out, and that there are still people who actually care about their clients. Shout out to the staffers who make this experience bearable: Admissions Coordinators Travis and Danita who made sure we are being taken cared of. Miss BC - ADON who had been so receptive of complaints and actually works to correct them. Nurses Diana, Shay, Jennifer, Debra whose caring hearts are a blessing CNAs Ericka and Faith - they do their best even during their bussiest days Therapists Anthony, Mael, Tammy, and Kelly - you are all awesome. Kezie from the business office who patiently explained to me the business aspect of my family member’s stay The wound care team whose names I missed to ask you are likewise awesome. I may have missed some but I am truly thankful for you all! God bless!
