My husband was there about a month the staff was very attentive and the place was always kept clean .He needed intense physical therapy to walk again after being bed ridden snd recovering from surgery on his but the most disappointing is that my husband is badly in need of a mobility wheelchair and discussed this with both social workers of warren barr .He was told by 1 social worker that you get sent home by Saturday because he was getting discharged. On the day of discharged he was told by his social worker that they will have someone come to your home and measure him for the wheelchair after day one .I called the Jeanette she told that she will passed it to another social worker to call because now sll of a sudden she no longer his social worker after she introduced her self as assistant social worker .I have been calling Jeanette and spoke with her she ensure that the main societal worker will call us back never received a call back .I call the assistant social worker and left several messages now that he home she doesn't bother to answer my calls or return any messages after we discussed how badly he needed a mobility wheelchair. After a survey call we praise the facility but the lack of no communication or return turn the praise to a very sour notes and misleading because this only way to get the 380 lbs left completely gone from a stroke to his doctor appointment with my bad back I am unable to push him so I would never recommend Warren Barr to anyone.

The facility is mediocre. Some staff are helpful and others not; food was mediocre; communication among staff was inconsistent; rehab staff are very good but must be asked about progress. The medical doctors were very knowledgeable and supportive. However, after discharged, we learned the doctors were from Community Home Physicians and we received separate billing for their services. This was not explained and resulted in additional charges. The social worker was dependable and supportive.

I was a patient at Warren Barr for Rehab. I had top notch excellent care. Nurses, CNAs and Staff were all so very helpful with everything I needed. Drs listened and gave great advice. I would recommend if anyone needs rehab to check out Warren Barr in Orland Park.

Overall this is a great facility that excels in patient service. Staff comes by daily to determine if my husband required anything to make his stay better. Staff overall were very friendly and accommodating. There are extraordinary staff I would like to personally commend: Odessa and Jeff are exceptional therapists. They patiently directed my husband through his various activities improving his ability to function better with his disability. Meena of Guest Relations came by daily insuring all needs were met. She is an amazing front line ambassador for this facility that improved my husband's stay at this facility tremendously. His nurses, Margie and Tracey, were very thorough and caring performing their duties thoughtfully exercising both sensitivity and proficiency. Also, we were blessed to have exceptional CNAs, Angie, Danielle and Ray. These women provided services to my husband that was beyond exceptional. Whenever he needed their help they were flawless in meeting his needs especially when transporting him from bed to wheelchair to bathroom in a timely and professional manner. I will never be able to thank them enough for the care each of them provided my husband.

I have returned here every time the word rehab is said. The food sucks however my therapy dream team is here.Vanessa is the p.t. half, Kelly the o.t. half. My first time here I enyerred needing a wheelchair to get around left approx.3wks later using a walker!

My Mom has only been here for 2 days - Thanks to a very alert Nurse, she saw she was having difficulty breathing. Got her to Palos Hospital immediately. Unfortunately, Mom is now going into Hospice care - My choice was easy Warren Barr. I know that they'll make her final days better.

Definitely not the best option available to care for your loved ones. Our family experience was quite negative. Overall, guest services was courteous, but the listing of contact numbers to reach various depts were all unworkable numbers; the social worker was hostile in nature and lacked empathy & wasn't forthcoming when concerns were raised; the nursing staff failed to follow proper protocol in ref. to the shower schedule & changing daily attire. Immediate action on the administration side is needed to prompt change.

Our family’s experience at this long-term care and rehabilitation facility was, unfortunately, more negative than positive. My father was admitted for rehabilitation within the dementia unit, and while there were some dedicated and compassionate staff members, the overall experience did not meet our expectations. One of the most concerning issues was the quality and timeliness of meals. The food was frequently unappetizing and, on multiple occasions, served hours later than scheduled. For residents in a structured care environment”particularly those with cognitive impairments”consistency and proper nutrition are critically important. The repeated delays were troubling. The overall ambience within the dementia ward was also difficult. Bed and chair alarms were constantly sounding, often accompanied by staff loudly instructing residents to sit down. While we understand safety protocols are necessary, the environment often felt chaotic and stressful rather than calm and therapeutic. For individuals with dementia, a peaceful and reassuring atmosphere is essential, and this was lacking. Physical therapy services were adequate in quality, and the therapy staff themselves were kind and professional. However, there were several days when my father did not receive his required physical therapy sessions. Ultimately, I felt compelled to visit the facility daily to ensure he was receiving the care and rehabilitation outlined in his plan, and at times to assist personally. Families should not feel they must be present every day to guarantee basic care standards are met. It is important to note that there are good nurses and staff members who clearly care about their patients. Unfortunately, there were also some staff members who appeared disengaged, frequently on personal devices and seemingly resistant to being approached with questions or concerns. This inconsistency in professionalism contributed to an overall lack of confidence in the level of care being provided. While we appreciate the efforts of the dedicated individuals on staff, significant improvements are needed in communication, consistency of therapy services, meal service standards, and overall patient-centered care within the dementia unit.

My dad lived here for about 5 months. The beginning was a struggle because he was going through dementia but once he got settled in he seemed to really enjoy the staff. Nancy Juarez, the social worker, was really helpful during his stay and helped us in any area that we needed. My main complaint is there were a ton of beeping noises but those are meant to keep the residents safe. I asked to remove my dad's security beepers on his wheelchair and bed which made his stay a lot less stressful. The food was great according to my dad.

My sister is in your facility at this time. She loves all of the people working there. The food is delicious, but has only one do complaint. The food arrives cold. She was told that the procedure to deliver the meals was changed during Covid. That was almost 6 years ago. I think it's time to get your residents hot meals. Thank you!

My stay at Warren Barr Orland Park was in general an overall pleasant experience. Who Five Starred my stay was Chef Darius who went above and beyond satisfying my vegetarian diet as well as the overall visual presentation and culinary gourmet level taste that was an amazing addition to each meal daily. As the cherry on top, Chef Darius made regular check ins with patients/ residents and their families to ensure we all happily satisfied with his presentation and overall quality of the meals including superb desserts he and his culinary staff provided.

My brother was a resident of Warren Barr, very impressed with the care given by therapist, he suffered a stroke and they worked very hard with him. We had Jeanette Davis as a social worker she made sure to call and provide updates answered all my questions and concerns, she even responds to weekend calls. When it was time to discharge transportation scheduled transported with ease, support in place we are thankful to her, because of this we will definitely refer Warren Barr Thank you all for caring our family

Kudos to the amazing staff on the memory floor at Warren Barr. Staff is always attentive to my mother's-in-law needs. She's been a resident here for almost two years. When approaching the building to visit her, I am greeted by a smiling and helpful valet. As you enter the front doors of the building the reception staff always acknowledges your presence and welcomes you to the community. Arriving off the elevator onto the third floor, you will more than likely encounter a happy resident making their way to their room or an activity being held in the dining room. I believe the care and concern given by the nurses, CNAs, housekeeping, and maintenance staff have a positive affect on the residents' lives that live here. I believe the reason this is happening at Warren Barr is because the management team is hiring the right people for the job. They also are continuing to encourage, teach, and build up the staff on this floor to continue to do what's right for each and every resident. It is my belief that this can only happen when the management team overseeing this floor believes in their mission. This team must be excited, positive, and driven to enhance one another's job experience and the lives of the residents they are caring for daily. Those of us that have loved ones with this disease know how difficult it can be on caregivers. To know your loved one is going to get the best possible care in a community, makes all the difference in the world to the family member's peace of mind for their loved one's safety and care. Wish I could have given a 6 plus but i couldn’t because they seemed short staffed!! Hire more employees! Clean, good meals and snacks activities and family oriented!! I couldn’t not leave a review without giving Big ups to Jeanette my mom’s caseworker! On top of everything!!! Connie daytime CNA and Demetrie and Deshay her evening CNA’s’• fabulous!!!! The nurses ? My grandmother was a patient at Warren Barr Orland Park on two different occasions within a 5 month period. The first fall resulted in a fractured pelvis and the second fall resulted in a broken hip requiring a hip replacement. Both times, my grandma transferred there for sub-acute rehab. Our experience would not be the same without Shanita. She remembered my grandmother after caring for her the first time and she took incredible care of her both times. I happened to be 3 days post-partum after my grandmother's second admission. Given that I was unable to come visit her, Shanita took the time to call me almost everyday with updates regarding my grandmother's needs, progress, and any issues that arose. She even offered to face-time me so I could see my grandma since I physically couldn't visit her. When my grandma felt sad, Shanita cheered her up. When my grandma refused physical therapy due to pain, Shanita made sure she was appropriately medicated for pain-relief AHEAD of her scheduled physical therapy session so she was able to better tolerate the pain and participate in physical therapy. Shanita motivated her and encouraged her on a daily basis. My grandma was delighted to return to warren barre the 2nd time knowing that she would be in good hands with Shanita AGAIN. I am in the medical world myself and I can say that there are few people who genuinely care for their patients (and their patient's families) to the extent that Shanita does. She has a heart of GOLD and she was made for this profession. I will be forever grateful for Shanita! Our transfer process back to home couldn't have gone more smoothly than it did. Shanita and Ayisat (a wonderful social worker) worked together to ensure that all transport, dialysis, follow-up appointments, home health care, and home physical therapy were all coordinated so we didn't have to stress about it. Hence, I owe a big thank you to Ayisat as well! Lastly, to Mena, the physical therapist, I also want to say thank you for your patience and daily encouragement in working with my grandma. He never gave up on her, even on her bad days. Thank you Shanita, Ayisat, and Mena! You are a dream team!

Our family was cautious because of other's reviews. Warren Barr is the place to go if you want your loved one cared for properly! All of the staff were responsive to my Grammy's needs and the needs of our family. PT & OT made sure to ask he goals for home. The nurses and CNAs are kind and always make sure she's clean and dry. They made sure to adjust her in bed so she could get relief from the bedsore she got at Elevate Care in Palos Heights. She will be discharged on Friday and Jeanette, her social worker, has lined up everything we need to care for her through the holiday weekend. Our family is grateful for Warren Barr.

I was there for about a month total (2 different stays) for stroke recovery after 4 strokes. Staff was great. Some excellent nurses aides and wound care. Physical therapists and occupational therapists were excellent as well. Were attentive and kind. Place was always clean and quiet. Ryan and Jeanette were great to help with any questions or concerns. Mena was my pt and totally prepared me to go home. I had a great experience and would highly recommend.

My mom has been here for several months now. The staff members on the 3rd floor (memory care)are just wonderful. My mom is interacting with the other residents, she gets exercise and therapy. She enjoys the different activities that they offer. It really takes a special person to work with dementia patients. The nurses, cnas and staff members are so patient and caring. When I have questions , they will address them. Everyone knows my mom, and they honestly care. Thank you memory care!

My family and I was skeptical about coming to Warren bar because of the negative reviews we were given however we had the pleasure of working with social worker Jeanette Davis who has went above and beyond from traveling to the kitchen to taking on other responsibilities to ensure our mom was comfortable she is what Warren Barr needs, she has called to updating on our mother’s condition she is very welcoming THANK YOU JEANETTE for all you help.

Warren Barr Rehabilitation in Orland Park is the best place I could have helped my close friend to get admitted to. They really helped him with his wound care from the recent amputation which he had excellent care, physical therapy, the RN's are so attentive, precise and sharp with the patients care, but the Social Worker Jeanette Davis is Excellent at all she did for my friend and constantly keeping me informed of every step of his process of care, I never seen anything like it, and I have worked in the medical field myself. Ms. Davis enjoys the work she do and it shows, even within the facility all the workers communicate the plan of care together. And I really appreciate Ms Davis for all she has done for my friend and I will never forget her and the experience and caring attitude that her and the staff showed, I'm so blessed that my friend was able to be at Warren Barr Rehabilitation and im so thankful God guided me to this facility and I will recommend this place with the beautiful staff you have to others that are in need. The staff and whole team at warren barr in orland park has made my 94yo mom stay wonderful. Case manager JEANETTE DAVIS has made the experience pleasant and hands on from day 1 to present. From letting us know how they will care for our mother down to after care which has been a truly blessing. With out her help, 3 sons would be helpless in setting up after care. Keep up the Great Work !! May 27, 2025 Update of Mom’s care. We’re Highly Disappointed. Mom was transferred to Long Term Care today. This is from my sister’s visit. Hello family, it is very important that everyone participate and visit mom on a regular basis. She was moved to the second floor between 11:30 and 12:00 today. When my Sister made it there at approximately 5:30pm, mom was not in her bed, and the bed was freshly made. She was in a wheelchair in the dining room slumped in the chair, her head back sleep with her gown up and no blanket on her. She had been there the entire afternoon. The afternoon CNA was not aware that mom was her patient, her nurse was there and did not know mom was his patient until my sister pointed mom out and gave him her room number. Mom's neck and legs were hurting her due to her being in that position in the wheelchair. She did not have lunch, and My sister had to request a tray from the 1st floor for her dinner. The staff saw her there in the dining room and did nothing. My sister spoke with Chris, the head nurse, to voice our complaint and let her know that the entire situation is unacceptable. I will contact Jeanette, her social worker, in the morning to further complain. Someone dropped the ball, and I'm going to find out who took her upstairs and parked her in the dining room, and did not notify anyone that she was there. We requested pain meds for her. Chris said that she would find out how something like this could happen. There is no excuse, we are all very upset and disappointed. After such great care and reviews; how and why did something like this happen???

My 90 mother recently stayed for 3 weeks at the Orland Park facility. Her stay was wonderful and the staff is caring, friendly and very hard working. She was happy,safe and well cared for. Alisha was so helpful with all details of her stay. I am very grateful for the pleasant experience we had here.

I had to choose a rehab after I got out of the hospital and I was told by many people who had stayed here before that they had a really good experience so I decided to come here. I am very happy that I did. They were very nice and the food was ok. The therapy that I got was so good. I am able to walk again after my hip surgery which is something I didn’t think I was going to be able to do. Thank you so much to those girls that helped me get stronger and especially Alicia and Kendall who got me to get home thank you for everything

My father recently received care at the Warren Barr facility in Orland for approximately ten days. Initially, I had reservations about his transfer, but I am delighted to share that the staff exceeded all expectations. The care team consistently kept me well-informed about his progress. In particular, Jeannette Davis, an outstanding social worker, ensured seamless communication between my father’s care team and his assisted living facility, even working weekends when necessary. Kendall White was exceptionally supportive during his admission, ensuring he was settled in comfortably. Additionally, Nurse Tameka, despite a heavy workload, patiently addressed all my questions and made certain I had the opportunity to speak with my father daily. As he is now being discharged back to his assisted living facility, I am truly grateful for the exceptional care and dedication demonstrated by the entire team.

Mom had care and love from their staff. So much so that they became more than just a doctor, nurse and CNA. All her needs were met but seeing staff personalize her care was heartwarming. I would personally like to thank the team at Warren Barr Orland Park on the first floor that take care of my mom . You all are the bomb! We were really concerned whether or not my mom would be taking care of, but they have been doing a superb job. My family and I want to give a special shout out to Janet Davis the Social Worker on the first floor. She goes over and beyond to make sure my mother gets all the services she needs. She’s very thorough, polite, kind-hearted and attentive. I know she has a hectic schedule but she takes out the time to explain everything and assists the family when we have any questions or concerns. Place is so so. Takes a long time for staff to come to room when call light is on. Man at the front door is quite rude! Very unfriendly! Several things stolen from my mom’s room including $5.00. Not a lot of money but it’s the point of it.

After a simple surgical procedure in October, I had to seek out Rehabilitation facilities for my IV therapy and Muscle strengthening therapy. I was very leary about making the correct choice. Warren Barr has always had a very good reputation in the Chicago area so I figured let's give it a try. To my surprise I found this facility to be Welcoming and the staff is extremely helpful. Of course there are those who jist want a check and truly don't care, but the majority of the Nurses (Bri, Erin and a couple of the Male Nurses are very knowledgeable and friendly). There have been so many CNA staff which I've encountered here that are very pleasant and helpful. I understand with their workloads and staffing issues, they can not be attentive 100% of the time, but 95% is better that 50% you may find at other facilities. I would just like to thank the entire Administration staff and PT/OT Staff for making my stay as pleasant as possible. Lastly a big hats off to the Social Work staff and Mrs. JESSICA DAVIS, for keeping on top of my appointments and Transportation.

My husband has been in Warren Barr and we are so very pleased with his care. The personnel can't be nicer. From the receptionists to the doctors! Each cna and nurse has been extremely compassionate, professional and patient! No matter what shift we can only say great things about each employee! The nurse managers and social workers have been so knowledgeable and accommodating. We came here because the physical therapy department was highly recommended which is why we chose Warren Barr. It has been so wonderful we can't recommend them highly enough. I would especially like to thank Javin, Mo, Alicia our social worker, Martina, Kierra, Dante, Lamia who has truly done miracles with his pt! Betty the receptionist greets everyone with happiness! I will miss each and everyone who has touched our lives at a very difficult time with such compassion no words for our gratitude!!

I've been here five months and my stay has been beautiful. The staff here is kind and come to your side when you need help. From therapy to nursing to housekeeping, they are all great. The food is an acquired taste. Shoutout to Carmen, Chrissy, Jonnita, and Antoine. Also shoutout to Angie, the best CNA out there. All the staff here makes me feel right at home and only wants to see me get better and stronger.

South side nurse bri (red head) first floor is the best nurse my number 1! She is fantastic my room was 122b very helpful and prompt! Angie and malanie the CNA's where both very helpful and very delicate to their work! Thank you

They were the best.They helped me get back on my feet and allowed me to get to acute rehab.Thank you for everything you guys done for me. I wanna shout out to the head director so sweet. I stayed at this facility for 2 plus weeks and had an extraordinary experience. You hear so many negative things regarding rehabilitation centers, but I would not hesitate to return to this facility if such a service was required. I would like to give a mention to the following individuals who provided exemplar professional service: Martina-RN, Jeanette-Social Worker, Syreeta-RN, Dianne, LaShunda-CNA, Danta-CNA, Nisreen-OTR, Quinnisna-RN, Brianna-RN and Cynthia-RN. Order of names is no reference to level of service provided. I would like to thank you from the bottom of my heart for making my experience enjoyable. Continue the good work and I wish you the best in your careers.

Just got here and not real sure about this place. Nurse already lost her paperwork. Floors are disgustingly sticky. I will try to be patient and pray for the best. Will update later.

Since the day I admitted my mother into this facility all of the staff has been very helpful and hands-on every single time I have been there to visit my mother.

I have nothing but great things to say about this facility. I had a loved one stay there for rehab. The care was exceptional! The Administrator is knowledgeable and makes her presence known daily.
