My father had his journey at Grace Point and our family will never forget how we each felt valued, listened to, and understood. The name of the facility is just what it means, the residents are treated with respect, dignity, patience…Grace. It is often so difficult to find such an organized, clean, beautiful place on the outside, as well as finding the beauty and compassion within the hearts of the entire staff of a healthcare facility. I have multiple stories I would love to share of the staff being kind and going the extra mile for my dad and his visiting loved-ones; from the CNAs to the amazing compassionate housekeepers, to the nursing personel, to the top administrative team members, to the activity staff, to the friendly and helpful receptionists. True beauty both inside and out. Our family couldn't be more grateful that Grace Point Place in Oak Lawn exists. Thank you for all you provided for my dad, the respect you give to families, and for being the BEST in grasping the concept and reality of Memory Care. I am just a family friend of a former resident (may he rest in peace). I helped move him in, set up his room, etc. What I witnessed was nothing short of spectacular. The facility was top quality, clean, and up to date. The staff was incredibly friendly and helpful, and even I, a friend to the family, felt comfortable and at ease and welcome. What I did witness was the family. In a time where they were scared, stressed, and unsure of everything, Grace Point's policies and staff created such a great atmosphere, that the family and resident were immediately relaxed and at ease about everything. I have seen and been to a few facilities like Grace Point in my life, and I have to say, they are at the top of list! I wish I could give more than 5 stars! Thank you Grace Point for doing it RIGHT!

Grace Point Place staff are very attentive to their residents and everyone is great at communicating the needs and activities of each resident. They prepare a wide menu of food which is very much enjoyed by their residents. Grace Point staff provide comfort and compassion beyond measure in memory care, activities of daily living, joyful social activities, and provide exceptional live entertainment for all to enjoy. I'm a vendor who goes to hundreds of communities per year. I only write reviews if things are really wonderful or really bad. Grace Point Place is consistently clean, and I've consistently witnessed compassionate and loving care on my visits. I've worked with the activity director, and I've worked with the administrator at another community, and I can emphasize that they do their job with love and with joy. My mom lives in memory care nearby, and if there was an Anthem Community in my area and my mom was there, I would rest easy.

Thank you to all the Healthcare Heroes at Grace Point Place your courage , sacrifice and dedication hasn't gone unnoticed. GPP is a clean, efficient environment, staffed by caring , competent staff, and a highly- qualified leadership team . The food is amazing. The residents are engaged in daily activities, they enjoy bowling on Thursday's and chair Zumba . I will recommend this community to anyone looking for assistant living/memory care for their loved one's to call home.

After residing in an Assisted Living Facility for over 6 years my parents moved into Grace Point. As my parents dementia has progressed, so has their need for a higher level of care. We were originally told by the Assisted Living Facility that my parents would be able to live out their lives there. The Assisted Living Facility was not able to give my parents the level of attention and care they deserved. I am relieved to say that Mom and Dad are adjusting to their "new home" and their level of care has immensely improved. All we want is for our parents to feel safe and treated with the dignity they deserve.

A couple weeks ago I came with my neighbor to visit her friend whose family just moved her to Grace Point. Upon my arrival I was very pleased to see how the staff interact with the community. They had multiple activities going on, and the community seen to be happy a such a great time. My neighbors friend have adjusted to Grace Point and appears to be vey happy. I give Grace Point 5 stars, Service Beyond Repair!!

I put my husband in Grace Point Place back in January . It was the best decision I ever made . Everyone is wonderful they take excellent care of him . The place is so clean and the food is great . When ever I have question or concern they are always there for me . Shannan Dahlman was there from the beginning made the transition as easy as possible . I'm so thankful that I found this place

Grace Point Place My brother in law is at Grace Point Place. After being in Health Care for over 27 years i am very familiar how facilities are run. Along with my sister we are very happy at the care our family member is receiving here. They have being very supportive to my brother in law and sister during this transition. Its not a easy decision to make. The staff is very kind and engaging. This facility always has activities or social events going on. My sister couldn't take care of her husband anymore however she is very pleased at the care he is receiving at Grace Point Place.

My cousin was a resident at this godforsaken facility. I attended a care plan meeting on her behalf with the Executive Director, Director of Nursing and Regional Nurse. One of our main concerns was engagement, safety and supervision of residents. Clearly Grace Point did nothing to fix the problem as my cousin is now in a hospital being moved via hoyer lift and totally unable to walk. This place is bad, bad, bad news. They lie and cover things up. They don’t do anything right including giving residents the medicine they are prescribed. You have to stay on top of them all day long or your loved one will truly suffer. One nurse even read confidential information from another residents chart to my cousin’s daughter! Please, find a better place for your loved one.

My mother entered this facility happy and fully mobile. After six weeks she was miserable and unable to walk. Her medications were mismanaged and I had to practically beg them to give her the proper dosages. It took a full month for them to finally get her two pills administered correctly. It must not be a good place to work because they have a different nurse for practically every shift from an agency which leads to inconsistency in care. The care staff is usually seen sitting in the back of the room sleeping or staring at their phones. I had to change my moms bedding every day or she would have been sleeping in urine soaked sheets. Every few days I would have to take the soiled linens home and wash them myself or Grace Point would just let them pile up smelling and creating an insanitary environment (picture attached.) The residents are not properly supervised and are rarely engaged in an actual activity. The wheel chair residents stare at a TV all day in the common room and the mobile ones walk around aimlessly and without purpose. There are no breakout sessions or programs specifically designed for higher functioning residents. The Executive Director Dashira is just the absolute worst and doesn’t care about the residents or their happiness. Since I removed my mother from this facility for all of the previously mentioned reasons, the management has failed to return all of my calls relating to documentation I’ve requested and other business matters. There is something seriously wrong with this place. I just hope the Anthem corporation intervenes because the residents and their families deserve more than this.

GRACE POINT PLACE has been my saving grace. My 94 year old dad had an accident in June 2022 that excellerated his dementia. After trying almost everything, my father's caregiver suggested Grace Point. I am an only child and my dad is a large part of my world but he needed more then my home could offer. My dad has been a resident for a bit over a week. He is so calm and content. He loves all the staff, Tiffany, Brianna, Michelle, the very nice CNA that shaves him ( sorry I forgot his name) and Linda to name a few. They are very welcoming. I was concerned because my dad lost most of his english after his accident. The staff have been great with my dad's needs. They have figured out translations when needed. Shannon has been great with my many many questions. She was also very supportive when trying to make very difficult decisions. I am able to breath knowing my dad is safe and in a place that he is comfortable. The only suggestion I would have is to give us an easy way to get a quick update when we are unable to stop in for a visit. I don't want to interupt the nurse or categivers from the residence care to update me with a phone call. If there was a way to possibly text an update. I know some daycares are able to update parents this way about their children. Thank you to all the staff at Grace Point. You are truly some very special people. Dementia is horrible it is the worse thing that has ever happened to our family but Grace Point is helping us keep my dad happy and calm.

Use to be a nice place now its a circus Lori is unattentive to the residents,staff and the building. She's rude and unprofessional. The building no longer has in order and staff just do whatever they want thats why they can't keep anyone there. They not properly feeding the residents a well balance meal The building is dirty they keep trash piled in the halls, floors are sticky and everything the new maintenance guy spend more time in Lori office than the up keep of the building and Shannon is just there to be there. I will never recommend no one to move they love one there under they conditions and I wouldn't even work there through agency ever again. U can also tell corporate don't care just long as they have money coming in cause there's no way they should be allowing some of the things thats going on.

My mom has been a resident at Grace Point Place since June of 2019. My family and I are pleased with the staff at this facility. There has not been a day since we placed my mom that I have regretted the decision we made to trust the staff with our loved one.

My grandmother lives here. She is staying very active doing many different activities. The staff occasionally sends photos of her doing crafts, art projects, bingo, or even singing karaoke. We love seeing her smiling face. Our family is very happy that grandma has made friends. She is no longer isolated in her home. We know she is safe and staying active. We are grateful that we found such wonderful memory care.

It was a very hard decision to place my mom in a memory care community. It was extremely difficult to make this decision with the fears we had of Covid-19. We just no longer could give mom the care she needed. Mom has now been a resident of Grace Point Place for a few short weeks. She is adjusting well. She is very active and participating in activities. I can call anytime- 24 hours a day to get an update on my mother. Grace Point Place offers a support group where my family and I have been able to meet others that are going through the same adjustment we are. I am thankful that mom has found her new home at Grace Point Place.

A week ago dm 21tashidelek wrote a post that is inaccurate and should be disregarded and removed. The relative she referenced is my husband and we have been together for 36 beautiful yrs. dm 21tashidelek is his 2nd cousin and we have not seen her for over a decade, because she is no longer welcomed by the family, etc. She has no medical information regarding the seriousness of my husbands condition. A restraining order also had to be placed against her by her own family. As wife and POA, I have instructed Grace Point Place (GPP)who may communicate with my husband and her name is not on the list for many reasons, including the protection of my husband. She has been informed of this and constantly harasses GPP and therefore, is not surprised that they do not return her calls as she falsely stated. BTW, my husbands birthday was on the 9th not the 10th as she inaccurately stated. Grace Point Place is an amazing community on so many levels and I highly recommend it for your loved ones.

The staff were welcoming while my husband and I toured the community with Meredith Morris. She is definitely a benefit to your organization. The entire set up is perfect for residents with dementia. The apartments, activity room, dining room, and the secure court yards all are just right for loved ones with memory loss.Its also nice that you have a beauty shop. The therapy services on the premises is a plus since unfortunately general decline continues and the services are right there when ordered. During my visit I didnt see any resident lost and or wandering. Kudos to all the staff. It was very home like ~ very impressive! I definitely would recomnend this community and know my loved ones would be in caring hands.

Grace Point Place has a magnificent staff .Especially the Life Enhancement coordinator or activities director , Lisa goes out of her way for each individual patient and their families. it is not just a job to Lisa it is a vocation she truly cares.
