I have a family member who physically disabled and has lost her sight. Basically, she has to rely 100% on the staff at Bella Terra for everything. I want to call out one person who has been generous, caring, and gone above and beyond supporting my cousin. Her name is Meena Punjabi and she works in the activities department. In addition to the activities, she has taken extra special care of my cousin, making sure that she is eating and eating well, gets something to drink when she is thirsty, and helps her with everything else that she needs. Meena often spends her own personal time helping my cousin, recognizing that she has special needs and provides that special touch to make her feel comfortable and cared for. It’s difficult to be a resident in a facility where there are others who also have needs and we are very blessed that Meena is there and takes the time to make sure my cousin is comfortable & content. We both appreciate everything that she does and I wanted to make sure that the people who run Bella Terra are aware of what an extraordinary person Meena is.

I had emergency surgery on April 15th which resulted in my having a colostomy bag. We had to fight with my insurance to get here. Since I have been here, I’ve grown stronger and am more confident. The entire staff is WONDERFUL. They are friendly and kind and usually quick to respond when I press the call button. I want to commend one aide TIONNA especially. She has been especially helpful and I appreciate that very much.

I was admitted to the facility from the hospital following a stroke; I had trouble walking and speaking. After just 12 days, I felt great. Ms. Agnieszka is wonderful”she looked after me as if I were her own child. I am incredibly happy that I was able to make such a full recovery in such a short time.

My mother fractured her arm and spent about a month in rehab at Bella Terra. I can’t say enough about the wonderful care they gave my mother. The staff was incredibly caring, attentive, and knowledgeable. The therapists and nurses made her feel comfortable and took very good care of her. The staff at Bella Terra has been so kind and hardworking with my family member. Christian, one of the administrators, has especially been available to me and so helpful from the very beginning when paperwork needed to be prepared, then the move-in process and phone/text explanations of different phases of care. The social worker Kelsey is patient and understanding, being there for us with coverage, appointments, and communication with other medical providers and facilities. The nurses and rehab professionals all take time to listen and work with our needs. Bella Terra of Morton Grove really is a wonderful place where I can stop and visit any time, communicate with the team about any questions, and reach conveniently with lots of parking and easy access from either Waukegan or Caldwell. The building is always clean, smells nice and is decorated with care and thoughtfulness. For our post-surgery long rehab needs, this was an excellent choice and I highly recommend them.

Bella Terra staff was amazing during the care of my Godmother who was there from June 2025 until November 26, 2025.CNAS Nikki, Edward and Catalina was amazing to her,Brenda, Simone, Aubrey, John and Charismar was amazing nurses who passionately cared for her. When I left town Christian our assistant administrator was very supportive with visiting her and checking in on my daughter. And I can’t forget about Dan and how wonderful he was to my daughter when my godmother passed away. Bella Terra staff went above and beyond and my family was very grateful for everyone. My brother was a rehab patient at Bella Terra - Morton Grove and received excellent care from kind and professional staff. We would like to particularly thank Brittany, CNA and Leenmy, RN from the medical team and Faye, Director of Wound Care Services. My brother appreciated the therapy services and meals customized to his dietary needs. The on-site kidney dialysis treatments were convenient and provided with individual attention. Kelsey, social worker and discharge coordinator, communicated in a clear and timely manner and was responsive to all our questions. My brother and I highly recommend Bella Terra - Morton Grove.

Bella Terra Morton Grove is a very good nursing home. The staff truly care for residents, even the most difficult ones. My Mother has been living in the memory care ward for several months now. She can often be a challenge behaviorally but the staff take the time to work with her and make sure her needs are met. Professionally, I have been in many nursing homes and this is consistently one of the cleanest with highly attentive staff. Mom says she likes the food, and the meals I've seen look good. Communication is clear, consistent, and timely. I would highly recommend this facility.

I can not express enough gratitude to everyone @ Bella Terra. Truly helped my mother from day one. She was admitted from the hospital in very poor condition, and stayed for 3 weeks. Private room, best caring staff, best admin to help during the stay, and after with helping get things set up after she was discharged. My dad who is 94 had a silent stroke a while ago. After releasing him from the hospital, he needed a physical therapy to gain back his strength, balance and independency so he was transferred to Bella Terra in Morton Grove. I was extremely worried and unhappy for my dad to be any place far from home for the first time ever especially at this age were he needs to be home loved and cared for. And due to some disturbing stories and reviews I heard and read I was really really worried and anxious. BUT, I have to say that Bella Terra not just proved me wrong also had me to be so thankful and grateful for their amazing care, kindness and professionalism they treated my dad and other residents with. When we arrived we were greeted by the case manager ( Kisha) that she was so kind and understanding. Then a manager ( Mariam Jedo ) stopped by to say hello and offer any help possible if needed especially my dad who speaks only Assyrian if he needs anything when we are not around. She gave me her cell number to get in touch when my dad doesn't answer his phone. I called her many times when I was worried about my dad who is not answering or he is feeling down she would go to his room talk to him, take him out for a walk or to the big room and call me from so we not just can talk but also to face time. and every time I thanked her she would reply in a very nice, sweet and kind tone " no need to thank me. Its my job" Mariam, I cannot thank you and your staff ( Shiella, Coleen, Maureen, Aby, Kim, Alan and all the nurses )( please forgive me if I forgot any names.) enough for all your care, kindness and all you do for the residence including my dad and their families. You guys are simply amazing and you take your job to the next level by all the extra steps that you take which is wayyyy beyond your job. I am forever grateful to you all. God Bless your hearts and bless you always. BTW, you guys do deserve a big appreciation raise for you truly make a difference.

My grandmother recently was a resident at Bella Terra and it was 10/10 experience. She received undivided attention and the staff listened to her needs. The dietary staff was attentive to her restrictions. The therapy staff was diligent in assisting on getting her back to walking. The nursing staff was friendly and easy to work with. The receptionist was kind and welcoming. Thank you to the staff for taking great care of her!

Bella Terra Morton Grove for my Mom was a great place for a short respite stay while my wife and I went out of town. Staff (nurses) were very attentive and caring. Employees Keisha, Glenda and Daphne were very efficient and caring when it came to my Mom being admitted and as well as being discharged. Bella Terra Morton Grove for my dad was a great place for his rehab. Physical Therapy and Staff (nurses) we're very attentive and caring. My dad needed physical therapy for 2 months which they worked very hard in making him stronger. Employee - (Malekia B.), The Adimmision Director was very efficient and caring when it came to my dad when being admitted and as well as leaving the facility after his rehab. Malekia B.is a true gem to your facility. She was the most caring and courteous employee. My dad would definitely go back to the facility.

I just recently had surgery and had to be placed somewhere temporarily My First Choice was Bella Terra nursing home and I came here for the rehab. While doing my stay here it was 100% successful. The staff are very attentive to your needs from the nurses to the CNAs and even the administrators. The staff answered the call light in a very timely manner which was very important to me and the facility is very clean. The food presentation was awesome and they will accommodate your needs according to your taste buds within reason. The physical therapist and occupational therapist helped me regain my strength and mobility in my arms and legs, if there were any problems I had my needs were met accordingly and all questions were answered to a complete understanding. My room was kept clean on a daily basis by housekeeping and I truly have to stay my experience here has been wonderful and if I ever need it to be admitted into another facility for any reason Bella Terra would be my first and last choice. I can't forget to mention that the in-house dialysis was also very convenient, and I was always taking to dialysis on time and in the midst of that being assist with getting dressed or anything else that I needed. I would like to thank everybody from admissions all the way down to the CNAs you guys are great

My brother has been at Bella Terra Morton Grove since April 2020, right after lock down. I wasn't able to visit him except through the window. This was very hard for both of us. The staff has cared for his special needs in a loving and very gentle way. I know this is an extremely challenging time for nursing home residents, their families and the staff who care for them every day. Bella Terra staff have provided the best care my brother's needs. I speak with staff and the doctor when there is a concern, and it is taken care of immediately. The staff updates me and different staff check in. Today Mattie (dietician) called to update me on my brother's weight as I was concerned about it. Amy Springer is always available to answer any questions, address any concerns, as well as schedule our "visits". I've seen Amy calm my brother down when he is anxious. Amy S. has also been able to shed some light about my brother's situation. She has lots of experience and more importantly cares for each resident. It's still difficult, but, I feel better knowing he's in good hands. The CNAs also provide very gentle and thorough care for him. I know it's hard. Rassin walks him, cleans him and helps with his eating. Rassin also interacts with my brother, helping him remember things. That's very special. There are others and I'm sorry I don't know all the names (one is a PT who is very good with him). They're very special, good care workers. I thank you all. We should thank them all. Horrible place , communication sucks. My father is in there for rehab . My father fell two days ago and he had to call me at my cell so that I can call Bella terra to let them know he was on the floor for thirty minutes still after he had buzzed the nurses and on one came to assist him until I call the place. Spoke with director and she asked me if this is my first nursing /rehab home experience , wow so she’s trying to tell me all nursing homes are like this. Never again bella terra .

I have a special needs adult sister who broke her hip. Bella Terra has taken her in as a rehab resident and I am very happy with how they have given my sister the extra attention her mental condition needs. As my sister settled in, I had a few questions and the admin director, Malekia, was quick to attend to the requests I had for my sister. She helped my sister feel at ease and made her laugh.. humor is everything right now 🙂 ! Even during this awful COVID time when we cannot visit inside the facility, the staff have made every effort to accommodate my sister, keep her safe, and do their best to keep her spirits up.

I am a 38 year old patient who attended Bella Terra nursing home because of a broken leg one of the staff members their name Malika made it very clear that she would try to keep me there because she can't lose any patience so they sabotage people in order to get the medical insurance her attitude was very nasty and she was very disrespectful the only staff member there that talk to you like she have some sense was Gabby. She was very nice. Everything was all about insurance with these people I was running a temperature of a 102 they did not send me out to the hospital because they were afraid to lose me as a patient and they still wanted me to do physical therapy with a fever. I ran a fever for 4 days straightis they refused to send me to the hospital this is how they do the elderly there they also feed you slop. I filled out my menu Daily and never got the things that I asked for. I had to have my family members to start bringing me food. Also my biggest turn-off about this nursing home is that they make people who are positive for the coronavirus do dialysis with patients who are negative for the coronavirus giving them the risk of catching the infection a lot faster. They did not allow us to go outside or do outside activities so I miss my daughter's graduation they lied about my records and about not seeing that I had already had the coronavirus so they put me on quarantine for 2 weeks again when I became ill with a fever .I had just had the infection in March but if I had caught it I would have only caught it from inside the facility because I had not been anywhere. They treat us like prisoners in there. there are ants in the rooms and different types of bugs that I did not recognize and they don't clean our rooms daily you have to hunt down an associate to get your room cleaned. Some of the staff there act like children and need to grow up. This is not a facility I would send my love one to. there was an elderly lady I seen sitting in the hallway daily and she had on the same clothes for the whole month and a half that I was there. It's just sad

Bella Terra is the bomb.com even through these difficult challenging times. They communicate effectively with patients families and keep them updated on a weekly basis and also provide resources where we can find out Important information and updates I can honestly say I trust my love one under their care even though I am not able to to them,Also not to mention their wonderful admissions Director Malika Bolden she is funny,sweet,compassionate, and in my eyes a VERY hard worker! She has just came back off leave and it feels like she never left I am beyond happy to have her back! I give Bella Terra 5 stars #healthcareheros

My mom has been here multiple times over the last.... 7 years I want to say. The first time was rehab for a knee replacement in 2009. Second time when her prosthetic hip was displaced in 2014. Both for 2-3 weeks each time. And this past year she was in there twice a bit more long-term. Almost two months twice. It's been a rough road in 2019 but was thankful for the peace of mind I had knowing they were helping her.

We were in a room where my mom felt disoriented and not feeling at home. Malekia Bolden, admission director, listened to my concerns and personally got her changed To another unit within 15 minutes. In September it was officially a year since my 34 yr old husband was released from Bella Terra. He stayed for a little over a month because he was bedridden a few months prior and had muscle loss. The improvements he had was just astonishing the physical therapists are one of the BEST ones he’s ever had and he’s been to 4 other places! So I know a good one when I see one, Mike, Seeten, and their whole team is just amazing! They truly do care for the patient. Every other day my husband would surprise me with something new that he could do. Amy was our favorite nurse she was so kind and friendly. I knew he was in good hands with all the nurses he got. The administration, doctor, and specialists were very understanding and willing to help in any way possible. Sending him here was one of the BEST decisions I ever made. Our son was able to visit his father which was important for us to be together as a family. Even if it was for a few hours. I will forever be grateful to everyone in Bella Terra for giving my husband the strength & hope he needed to keep trying. The goal was to come home and they’re the reason he was able to do so a lot sooner than we had expected!

July of 2018 I had right knee replacement surgery! August of 2018 I completed my PT/OT at Bella Terra Morton Grove. All the staff involved with my care we’re very nice & professional!The facility was beautiful & clean. Upon arrival I was greeted by several staff members (DON, nurse & CNA) of the unit.My room was nice & clean with an attached bathroom! Several staff from different departments stopped by my room to introduce themselves.(Social Worker,Activity Director, dietitian & PT/OT- reviewed my PT/OT schedule! The PT/OT department were awesome! Professional, and nice! All the nurses and CNA’s who assisted with my care were so nice,and professional! The physical therapy room was huge,clean, organize with lots of equipment to complete your PT and OT sessions. The dietary department offers a wide range of nutritional food to choose from the menu & the food is delicious! I am scheduled for left knee replacement surgery & would definitely like to be accepted to come back to the Bella Terra for PT/OT. I would definitely recommend this facility for anyone who require physical therapy for my knee and hip replacement. thank you

I have had to use Bella Terra several times for both of my parents. My mother's recent stay was excellent. She was well cared for and the wound care nurse Adriana went above and beyond in care of two wounds. The staff was friendly and helpful from the reception desk through PT/OT and nursing staff. I would highly recommend.

I would like to thank the staff at Bella Terra for the caring and kindness given to my mom during her stay at the nursing home. My mom was having a hard time transitioning. She was often difficult and contrary. The staff went out of their way to be kind and courteous even in difficult circumstances. Everyone on staff did the most they could to help. I am grateful to the director of Nursing. He came and spoke with me often. He was attentive to my needs and did his best. The social worker, Amy Springer, kept a close eye on my mom and let us know when it was time to transition to Hospice. I would recommend this facility to anyone, but especially those with strong willed parents. Thank you all!! The Family of Sylvia Dick

My parents had been taking care of themselves for sometime now. They had fallen ill and were in the hospital for a short stay. They were going to need rehabilitation. When searching for a rehabilitation and possibly long term nursing facility what I discovered after visiting several facilities was disheartening. Residents were unattended, facility was not clean, terrible odors, and just unacceptable conditions. But then I found Bella Terra, by far the best facility I visited. It was not only clean but that odor of urine would be almost a standard problem, there was nothing but a fresh, non-antiseptic smell. Amy, one of the Social Service Directors was always there to help me with any and all questions or issues I may have had. Even on her busiest days, she and Amy always took time out to help me - no matter what the issue /question was. I cannot say enough about the Physical Therapists at Bella Terra. The work they did with my parents gave them the opportunity to move onto assisted living. There were days my dad would say "wow they really worked me, don't know if I can do it tomorrow". But the therapists were persistent and caring with them and the hard work paid off. It is pretty hard to impress my father, but they did it. The level of care from the Nursing and CNA staff was remarkable and after just a few weeks I knew I choose the best facility for my parents! I was very satisfied and knew that they could not have had better, kinder care. I am very grateful to the staff at Bella Terra and highly recommend them!

I called Bella Terra regarding some questions about the care and needs of my mother. I talked to Krystal and was pleased with her attentiveness and knowledge of the facility and it's amenities. She went above and beyond my expectations to make sure that my mothers needs and my questions were all answered. I have never experienced that type of care before. I will say that Krystal and Bella terra have gone out of their way to please my mothers needs as well as mine in this hard time. I couldn't ask for more. Bella terra is the place I would send anyone in need of exceptional care.

My mother has been at Bella Terra for several months in rehabilitation. I am very pleased with the care she receives from the nurses, CNAs, her physician, respiratory therapist, and wound specialist. I see my mother every day and if there are any questions or requests, everyone responds promptly and in a kind, friendly manner. When she came, she had a very extensive bed sore and she is regularly moved, her wound is getting better. My mother also has a trach tube and the nurses regularly take care of it. The facility is very clean. The medical staff is keeping track of vital signs several times a day and of her test results. When she had an infection, she was immediately hospitalized at Lutheran General. I am much happier with Bella Terra than with several other facilities I had experience with.
