We have had two short term stints at Avantarra Lake Zurich. Staff both rehab and admin have been amazing both times. Mom has been cared for like family there. They keep track of clothes better than any other facility. The activities are great and the residents are kept busy and safe.

I have had the pleasure of visiting Avantara Lake Zurich almost every day for the past eight months. My mom was first admitted in March for short term rehab for a broken femur/hip. Her therapy and care was outstanding. She was then able to be transferred to the second floor for long term care. Fortunately, through the dedication and guidance of Kyle, the amazing admissions director, my dad was able to join my mom and receive long term care as well. This would not have happened without Kyle's expertise. Thank you, Kyle! My family and I are extremely pleased with the care that my parents have received. Everyone is professional, hard-working, caring and show true concern for all the residents. Since I have spent a lot of time at Avantara LZ, I have witnessed firsthand the dedication of the staff. Patti, an outstanding nurse on the first floor, took care of my mom during her rehab and still asks me how she is doing whenever I see her. Nate, Guest Services director, has assisted us since day 1. He is very visible throughout the facility and always checks in by asking, "What can I do for you today?" The nursing and support staff on the second floor also deserve recognition for their hard work and caring dispositions. A special shout-out to Mel and Anna! You guys rock! Social services has been an excellent guide for me. They have answered many questions and helped in so many ways. A special shout-out to Mary! You are the best! And, finally to all the support staff, which includes the office managers, customer service assistants, activities director/assistants and housekeeping, your friendliness and hard work does not go unnoticed. A special shout-out to Patrick, Franz, Robin, Sylvia, Donna, Jen....thanks for everything that you do!!! I have learned many lessons from the care and concern my parents have been shown as residents of Avantara Lake Zurich. I love this facility. First floor rehab is very clean. Staff is great, particularly Nikki. She helped us put my father in this location painlessly and easy transition. Our Dad came in there not being able to walk. Now, he always gets in trouble for walking unassisted, because his gate is 75 percent better. They care and monitor him closely. We cannot thank Nikki enough for facilitating our father’s intake. Billing problems and questions were always answered and explained thoroughly. Navigating medicare or health care systems is rocket science to me specifically. Nikki knows the ins and outs.

Our family is very grateful to the Avantara Lake Zurich staff, whom have made our grandmother's recovery feasible. Beginning with Christina Sidhu from admissions, who provided us with a seamless transition into the facility. Our grandmother's room is thoughtfully designed & well-maintained. The doctor's, nurses, & therapists are attentive to our grandmother's needs & provide a very caring service. They take their time to explain any changes to our grandmother's care so that we have the reassurance that she will continue to be well. The food is flavorful and varying in options, so our grandmother is always pleased with her meal. We are very grateful to have our grandmother in such great care. My mother has been coming to Avantara as a respite stay for the last eight months they work really well with our hospice company to give me the rest that I need as a full-time caregiver. The third floor staff is amazing with the care they provide. Peter and Steven who are just fantastic, Daisy and Myriam her CNAs. Nurse Patti was his nurse and she took great care of him and my grandmother, always making sure she was doing okay as well or needed anything. All the staff members were remarkably friendly and kind. My mother has been a resident for 4 years. She is currently stable. We can rest assured that nurses Steven, Peter,JennyLou, Raj and Gladys are taking care of her medically. Her CNA'S, Myrium, Ibrahim, Daisy,Lupe. and Alex treat her with loving care and respect. Jennifer offers various activities although Mom does not always wish to participate. Manager Jayne checks with us to make sure Mom's needs are met. The cleaning staff is always busy tending to the floor and residents rooms. Thank you all for taking good care of our mother.

My medical condition was compounded by the fact that one side of my body was impaired due to a old spinal cord injury. The staff and the care team were attentive and professional. I came to Avantara for rehab my tendons tore in both knees and my Range of motion was limited. The team has been great and treated me with respect. When it was time for my discharge I was not ready to go but my insurance cut me. Nikki and Christina helped me with my Medicaid and therapy was able to pick my up again under my part B. Claire, Elizabeth Hector and Amy have been amazing.I would recommend to anyone who needs rehab. P. S. I forgot to mention Epi and Briana - what a wonderful professional caregivers they are!

It's found in the countless small moments that families rarely see...the gentle reassurance when someone is confused, the extra time spent listening, the familiar smile that helps brighten a difficult day, and the genuine respect shown to every resident regardless of where they are in their journey. My mother has been a resident for quite some time, and we've had the privilege of getting to know many of the incredible people who care for her. Her regular nurses: Peter, Steven, Jenny Lou, and Raj, they have consistently demonstrated professionalism, kindness, and genuine compassion. Her regular CNAs: Daisy, Ibrahim, and Myriam, they have treated her with extraordinary patience and dignity, always going the extra mile to make her feel comfortable and cared for. We are also incredibly grateful to Alex, Lupe, and Nurse Gladys, who have all played important roles in caring for Mom and supporting our family. A very special thank you goes to Jane, the Memory Care Manager on the 3rd Floor. Jane has always been available to answer questions, address concerns, and communicate openly with our family. She leads with compassion, professionalism, and accountability, and it's clear that her leadership helps foster the caring culture we've experienced throughout the unit. My mom has dementia and has been on the third floor of Avantara LZ for several years. It is a very difficult time of life. The staff have been so supportive. I came to avantara for rehab after a open heart surgery. The team was super great since I got here. I was in a lot of pain with my legs and the one person who helped me a lot was a CNA this big guy came out of nowhere and helped me with his strength. His name name is chibinga. And great CNA is Dan who I found out lives close by me. If you need help I would recommend. Day by day my illness and strength is coming back thank you to the team here. I am so grateful for everyone and even my family that came to visit me and lifted my spirits. When I was in the dinning room I saw all the staff showing all the patients kindness. I am leaving this facility with greatness in my heart because of all the amazing staff here at avantara. The nursing staff is extremely patient with my father on the third floor at AVLZ. My dad has had Jenny as a nurse since he arrived in 2023 and she knows exactly how to handle him and is a great asset to the rest of the staff. Communication has been good also from staff when needed. There had a been a time when it seemed like there was a lot of changing of staff for a while which made it difficult at times for my dad in his altered state but now it seem like they have had more consistency which has helped him tremendously. Thanks for all you do!

I feel like when I came here I was greeted nicely, you can feel like the staff care. They got me settled in not even 5 mins of my arrival the staff was in my room getting the items I would need. When the staff came in they were gentle and helpful. The best part is when they would check in on me but I was still able to sleep. I feel like communication could have been better regarding my discharging with my care plan. The staff like Patti, Claire, Sergio, chibinga and Hilda were great. There are others but over all I would recommend this facility to anyone.

The physical and occupational therapy was excellent. The physician that over saw me here at the building knew my physicians in the community that I work with which is very helpful. The nursing team all treated me with respect no matter the time of day. The food was delicious, even though I had dietary restriction. I wanted to highlight one of the nurses, Juan, for doing an amazing job with my mother. My father had a stroke and I expressed to Juan that I was worried how she might react when she heard, as my parents have been married for 50 years. Juan dropped everything he was doing to accompany me and offer support while I told my mom, and he made sure she felt safe and cared for, and she later told me that he checked on her multiple extra times throughout his shift. Words can’t express how appreciative I am of Juan and the rest of the nursing & support staff - Patti, Mary Rose, Tashonda, the nursing director Christina, the physical therapy staff, front desk staff (especially Eddie!), Mary the social worker, and more. I can’t thank you all enough for providing such excellent, compassionate quality of care during such a difficult time.

My mother has been in long‘term care here since 2022, and I have a lot of mixed feelings about Avantara Lake Zurich. In America we live in a late‘stage capitalist hellscape, and the broken long-term care system is just one example of this. The care of our most vulnerable elders has been handed over to underpaid, overworked immigrant staff while the owners of these facilities accumulate wealth, dividends, and real estate portfolios that most families could never dream of. They profit comfortably at the top while residents live two or three to a cramped room and eat food that would crush anyone’s will to live. The moral contrast is staggering. The system is fundamentally broken, not because of the workers, but because of the perverse incentives that let a handful of wealthy owners siphon money upward while the people actually doing the labor struggle under impossible conditions. It’s a machine designed for profit, not dignity, and the residents and staff are the ones paying the price. And yet, the staff themselves have been compassionate, patient, and kind. For the last 25 years I’ve lived overseas, and while my mom has been at Avantara they’ve done everything they can to communicate with me, even when it’s inconvenient or difficult. Many of the staff are caring for our loved ones while their own aging parents are thousands of miles away in Manila or Lagos. They deserve so much more than what this system gives them. To the entire 3rd‘floor team: thank you. You are the ones changing my mother’s diapers, giving her showers, comforting her when she is confused or afraid, and doing the intimate, difficult work that the wealthy owners will never even witness. Your humanity is the only reason this place functions at all. If we lived in a society that valued elders instead of warehousing them, if families weren’t squeezed dry by consumerism and overwork, if care work were respected and properly compensated, maybe things would look different. But here we are, pushing our oldest family members into institutions while the people at the top enjoy multi-million dollar homes and the people doing the actual care barely make enough to support their own families. To the staff: you deserve better. Thank you for everything you do. My frustration is with the system that exploits you, not with you. You are the ones holding this entire broken structure together with compassion, effort, and love.

5 stars to Avantara for the care I've received during my stay here! There's been countless people involved in my care and want to thank all of them. There's some people I had seen more often than not that I'd love to give special mention to, but everyone here was amazing. Meals were made fresh and the same-day laundry service blew me away, with a team personally picking it up and dropping it off. I was appreciative of the care from Nathan in Guest Services, checking in routinely with how I'm doing. Erick, the director of maintenance came personally and was so thorough and quickly responded to assist with my temperamental window shades. I celebrated a birthday here and the culinary director personally dropped off a banana split for me to enjoy. Even Eddie up front was so polite, kind and on top of any requests i had. So many occurences I could shout out, it is easier to list the names of everyone who was involved in my care (I'm sorry of if I leave anyone out!). They are: Suresh, Alex, Anna, Margaret, Olivia, Donna, Mary, Fonda and Tim in Therapy services helped get stronger again and helped teach me about safety measures post surgery. Edwin in wound care, as well as Albert, Alao, Julia, Brian, Jacob, AJ, Anna, Juan, Raj, Chibinga, MariRose, Pinky, Patti, Pamela, Tashonda, Mercedes, Beth, Evonjay, Melissa, Hilda, Jay, David, Lucritia, Sharon, Dr. Chuang, Jackie, Tee, Sergio, Natalia, Latrice, Trisha are just some of the CNAs and Nurses and others involved in my care and treatment that's helped get me better and more knowledgable about my care. Donna in activities and Frida and Jessica in social services also were fantastic. Big thank you to Alvin and Asalea in culinary that were such a big help (and made it a good birthday!). My mother was admitted to Avantara Rehabilitation in Lake Zurich, Illinois.. The staff was amazing. Robbie at the front desk was a friendly face every morning & pro at his job directing all visitors with his positive attitude. Amy & Juan and Albert, her nurses , were always available, super attentive and ready to answer any and all questions . Super efficient in being Moms daily cheerleaders as well as keeping my family thinking positive . We also experienced a wealth of information from Dr Pinky Patel regarding my mom’s health Anna K was on top of keeping the people here safe and healthy. Josie, Sergio and all the other staff on hand were amazing. They are the heart of a facility like this as they brought cheer in doing their jobs daily regardless if it was showers moving wheel chairs or anything asked of them. Jessica & Freida were also very helpful in the discharge process by making arrangements w health professionals to come to the house. The Physical Therapy department was always so encouraging and applauding mom for her efforts to get better. Susheese made a point to visit mom often to check up on her . Thank you

My wife Cindy has been a resident of Avantara Lake Zurich for over two years and is do well. The staff there are very attentive to her needs. The Nursing staff is always ready to check on her when I have a question about her health. I especially like Dorothy and Leto and Raj and Peter. The CNA’s also take excellent care of her keeping her clean and feed . We especially like Alex , Lupe, Myriam, and Daisy they care for her like I would. They are very loving. My mother has been a resident for about 18 months thus far. I couldn't be happier with the treatment she gotten there. Nursing facilities are difficult places, we all know that, but the staff, without exception, has been so kind and patient with her. They answer my questions, and keep me updated on a regular basis. Although there were many ups and downs with her care and communication with staff over the years, there were some exceptional staff members who went over and beyond. My mother has been at Avantara for about a year and the difference from that summer to this summer is a complete 180 and I could not be happier. Not only has her health improved, but her cognition and demeanor have improved in a very positive direction. She was previously at another facility who's service was extremely poor, but the move to Avantara was exactly the right one! They actually helped her heal from the previous facility and their neglect. On the 3rd floor, the staff has taken such amazing care of her and their attention to detail is really amazing. They are very communicative with her, myself and our family and take care to ensure my mom is always happy. Special shout outs go to Epi (my mom always appreciates her cold Diet Pepsi), Myriam, Luz, Steven, Raj, and Earl! Honestly, I would shout out the whole floor as there are many who I have not had the pleasure with interacting as much but the team works well together. My mother has been in some form of assisted living/SNF for 12 years and Avantara has outshined them all. In fact, many years ago after a joint replacement, my mother had a short stay at the facility, however, it was under another name and management. She did NOT have a positive experience there, but I am glad I gave them another shot seeing as they were under new management. No regrets here! They have also made some very nice upgrades to the building as well and hoping that some more come to 2nd and 3rd floors soon! Jane and her staff in memory care do really care about the residents and have a very well run floor. Thank you so much for the care you show the residents every day!

I have nothing but good things to say about Avantara in Lake Zurich. My 83 year old mom went in after a fall and a broken rib. My dad and I did a tour of the place with Nathan. He was amazing and very helpful with any questions we had. He really made us feel comfortable that this is where mom should be. From the Dr's, to the nurses, right down to the aides, everyone was fantastic. Very caring, helpful, and treated my mom like she was their own. Also, a special shout out Juan, Jossy, Maura, Gurneer, Lizzy and of course the physical therapy team. I can't say enough. My mom came home yesterday and was able to climb our 15 steps at home quite easily. It's good to have her home like nothing ever happened. Obviously, I highly recommend Avantara Lake Zurich for your loved one.

My mom has been a patient at Avantara for the past 3 1/2 weeks. She is getting such great care and the staff is truly fabulous. She talks so highly about everyone. I live in the neighborhood, so I am lucky to be able to visit my mom on an almost daily basis and I am able to witness all of this myself!! Just to name a few, Nathan, Patty, Carol, Jessica, etc... they have been so great to my mom. I highly recommend Avantara for anyone that needs a rehab facility. Also they are in the process of remodeling & it is so clean and inviting to visit. I'm very thankful for Avantara!

All of the people here have been so helpful and kind, I'm so thankful that I got to experience their care. Gurneer and Mara were both so sweet and helpful, and Juan was so fantastic and awesome. Chibanga was so kind and an amazingly patient man. Nathan has been so sweet and positive too! Jessica was a wonderful help, essentually being an anchor for my family and I, and even helped with planning for after my stay. Gladys helped me acquire some medical necessities and was a godsend. My grandfather was recently admitted to Avantara Lake Zurich, and I cannot express enough how impressed we've been with his care. The staff has been truly incredible, going above and beyond to ensure his comfort and well-being. From the moment he arrived, everyone has been so attentive, compassionate, and professional. The memory care team at Advantara Lake Zurich has demonstrated exceptional professionalism and compassion in the care of my mother. Jane, Dottie, Lupe, and Alex have been her caregivers since the outset, and their dedication is truly commendable. Their genuine commitment to their work is evident, as they attend to each resident with the same compassion that one would show a family member. Although I have specifically acknowledged my mother's care team, I would like to recognize the numerous staff members whose names I am not familiar with, yet who also provide exemplary care. Additionally, my mother has benefited from the Physical Therapy services offered at the facility, which have proven to be quite advantageous. To all the nurses, therapy team, etc. for keeping me well informed daily on his progress. It is like night and day seeing my father-in-law get better and gain strength so quickly. It is also heartfelt to receive text messages that he is actually HAPPY there and making friends. The staff at Avantara Lake Zurich provides stellar memory care and communication for my disabled brother. As a family member and POA who lives in another state, I appreciate the staff coordinating treatment so he can live near his friends. The quarterly care team updates are also appreciated. My brother is only 61 and fairly mobile, yet the caregivers have adapted to his specific needs. There is some turnover in providers, but I can always get someone on the phone.

My mom has been in and out of Avantara for the past few years and everytime she has left feeling better and recovered. They honestly remember details from her previous stays there and even her sister who sadly passed away there. All the staff at Avantara do their best to care and attend to all their patients and coming from someone who visited twice a day for weeks. My mom raves about how nice all the staff were to her there and gave her big hugs on her discharge day. Big big thanks to nurses Anna, Patti, Christine, Jessica and Nate in social services, and Tricia and Katya in therapy who are outstanding staff members!
