I was very pleased with the care and Rehabilitation that my brother received after his fall, breaking his hip and elbow. He exceeded all expectations with his recovery in rehab with Ben and Gretchen. My brother has dementia and they, along with the entire staff, quickly gained his trust even though he still had brain fog from his surgery. A bonus to us was the kindness and compassion that everyone we encountered gave the family. We had previous experiences with rehab facilities several years ago as my parents needed it and this place made the others pale in comparison. WARNING: when you check your loved one in they tell you that they will give you our 48 hour notice before termination of services but they mean ONLY 48 hours of notice and no communication a minute before.

I came to work here about two months ago. I absolutely love this place. The residents are happy and well cared for. The employees that work here are friendly and truly care about the people that reside here. This is evidenced by the way they speak them and the care they show at the bedside. As a nurse I appreciate the communication that goes on continuously in an effort to make sure that staff get information needed to do their job and do it well. In the short time I’ve been here I have witnessed and been a part of communicating with residents and family members to ensure that they know and understand any new order. Granted, I work here but I would gladly recommend Creekside Transitional Care and Rehabilitation to anybody looking for a place for their loved one. While here you can expect to receive top notched Physical, Occupational , Speech Therapy, and be treated like a family member by the staff that work here.

I have had nothing but an excellent experience here at Creekside. No waiting for a response to my call light. Everybody willing to make my stay in rehab pleasant. When I was in rehab in another state, I wondered if the care, or lack of care, would actually kill me. Not so here. I feel very safe and cared for and the food is even appetizing. Once in awhile I have asked to have it heated up but staff willingly did so. Very safe place for rehab! I felt I couldn’t have made a better choice.

When my grandma was staying here. The care that she received was beyond amazing. The team here really goes out of their way to take care of their residence and make sure they are all happy. They do an amazing job at also keeping family members informed of everything. Would recommend for all people looking for a care facility

I was in Creekside for many weeks following a horrible illness. From the first moment I arrived until the day I left, I was treated so well by everyone. I healed up and looked forward to each day because I knew very professional, caring, doctors, RN nurses, LPN nurses, CNA Aides, Occupational Therapists, Physical Therapists, Speech Therapists, Dietary, House Keeping, Transportation, Social Services and Office Staff were there to make sure myself and other patients were well tended to. I heard how kindly the patients were spoken to by the the entire staff. They spoke to me the same way. Thank you Dr. Walker, Dr. Ward, Jaime. Juan, Selma, Jay, Lisa, Brin, Josh, Shawna, Rosalinda, Keith, Eileen, Cathy, Brent, Wendy, Maddie, Michael, Gretchen, Riley and Cam. I hope I remembered everyones names. If I did not, please know you are as special as everyone I mentioned. I would recommend this place to any friend or loved one.

during the short time my wife was at this place , they lost her purse, lost her clothes didn't answer her buzzer for help, she was forced to stay in dirty diapers sometimes up to 3 plus hours which left her backside raw and open sores, they hardly ever answer the phone and never call you back, the outside of building is in need of a lot of work, due to lack of maintance .they have resident people answering the door. this place is a joke !and should be closed down !

Had to remove brother in law via ambulance 911. Tried to tell staff ambulance was coming, they would never answer the phone ambulance got there, staff never did pick up. Care in the home was sub par. Brother in law lost over 20 lbs in 2 weeks. final straw was when he fell out of wheel chair after telling staff he was slipping. so happy to have him out of that place.

So far so good. Dad is happy here and says everyone has been kind to him. Although outdated, facilities are roomy and clean. Some of the therapists are trained in the LSVT Big & Loud program which is great since my dad has Parkinson's.

The staff was friendly, up beat and cleaned with no funny smells. The Activity staff had activities that were fun. They provided live entertainment. The bad is the food. And the transportation dept. Over all this is a center with growing pains due to supply and demand. I would recommend this center for short term stays.

I was here for 3 weeks after back surgery. The staff on my floor were really good to me. They always spent time to explain things and kept me comfortable. The Activity room kept us busy with card games and Bingo. Yes, the food was cold and not all that great. But you can have food brought in.

They treat you with professionalism and respect. My mother had need for residential assistance through her rehabilitation process and these folks did a great job. My father had been there previously as he was working through challenges before he passed away and they did a great job there as well. I recommend it - it's clean, the staff are professional, and they do their best to make you feel at home.
