Prairie Creek Healthcare is attached to West Bend Health and Rehab. It is an amazing facility with individual apartments for seniors who need a little assistance in their daily routine. The staff is very friendly and always willing to help assist in anyway they can. Great Facility and great people, Really makes this facility feel like home!
