NEVER GO TO THIS FACILITY. The nurses don’t deliver your medication until 10:30 pm and by then you are in so much pain. The food all 3 meals were served ice cold. I saw a doctor one time and asked repeatedly for help getting the medication from the doctor to bring it to me earlier and NEVER go a response. In conclusion I told my health care advocate about but I signed myself out and swore I would never go back and I pray that no one else goes there!!

one of the worst care facilities, I wouldn't wish for my enemies. Management, and Cna's are rude, aggressive and I wish one day they will taste what they did to the elderly dependant people.

My mother was a patient at Vivo Orange Park in June. It is an older building, but the staff is wonderful and provided her with excellent care. We especially liked KiKi, our CNA. She was so kind and loving to Mom. I would definitely recommend the Rehab here.

My husband is at vivo until January, vivo is the place that we want for some one we love, gratitud a lot of gratitud for all staff, all departments ,we work together for quality Life of my husband nothing is Perfect but I can see every week the hard job,the respect,the empaty,the compassion,that exist at vivo to take care seniors, personally in muy heart I want to said thanks a lot, God bless you, please keep good job, improve when it needed,and still working together for quality my husband life,I appreciates everything that you do for all senior, I appreciate what you are doing for My husband thanks thanks thanks , Big hug to front desk, directions, amazing Ladys, nursings, therapists, social workers,sighinees, dietitian, chef's,cookers, maintaining, cleaners, all Dr, activities direction, therapy director, it's a long listing,I see all like part of my family,thanks for your amazing Job, amazing care amazing efforts and thanks for help me to help My husband working together big hug and God bless you all!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!

First day place in not clean no one seems happy at all no beds have rails my sister rolled off bed. Old tv has scotch tape holding screws in. Receptionist was on phone dropping F bombs. I'm sitting her shaking my head to how they are open. I'd check out before allowing family members to be admitted.

My Dad was at Vivo the last few months of his life as he needed more care than his assisted living facility could provide. From the moment we arrived Vivo OP, he was wonderfully cared for. Every day as I would come to visit him after work, the nurses and CNA's were kind, approachable and always willing to help. I've worked in long term care for over ten years and have had the opportunity to visit many different facilities and observe staff at the centers, Vivo OP has a great staff that cares! They care about the about the residents, families and their own teammates and it is evident as you see them work together. When I had a request, the Executive Director Trinity Blocker was quick to respond and assist in getting it resolved as quickly as possible. My Dad passed away in April 2024 at Vivo peacefully with Hospice. Thank you Vivo OP for taking such wonderful care of my Dad!! Your staff was so kind during his care and after he passed to my entire family!!

My father admitted to the center after a terrible fall. Everyone was so caring and professional. We were very happy with the care and great physical therapy that got my dad back home to us.

Recently toured this facility, greeted right away by the friendliest front desk associate. Facility was clean and smelled good. All the staff greeted us in hallway on tour. Rooms clean and tidy. Residents all seemed very happy. The Liason addressed all questions prior to us touring building and was extremely helpful and knowledgeable in relation to my dads dad's needs and medical care post hospital... I was very impressed by her fast responses and appreciate the support with transitioning my loved one from hospital to skilled nursing.... She made it very smooth for me and took the stress off the family in so many ways, beyond appreciative of the outcome! Highly recommend this facility ..

I am the Admission Liaison for this facility, I am who you see in the hospital BEFORE you go to rehab! Our facility has an amazing team to help you OR a loved one get back to a place where they can safely get home! If you need long term care we are able to accommodate. I believe care starts at the top, our administrator and director of nursing are the epitome of caring and involved. Believe me I have been to other facilities! The therapy team is top notch and the director is personally invested in getting you back up and "running" I ENCOURAGE anyone looking at facilities, TOUR, see for yourself.

My mom was only here for a week before being sent to the hospital, but in that time, everyone here was so nice and welcoming! They treated my Mom so good. Any concern I had was answered in a timely manner. Since my Mom was transferred out to the hospital, I have received several calls checking on how she is doing! Don't judge this book by its cover, it may be an older building, but it is clean and does not smell like some rehab/nursing homes do. And the employees actually care about the patients!!
