This place started out really good and has steadily went downhill. Always short handed. The good is awful My dog eats better. They skip patients baths and hair washing for bedridden people. Personal items come up missing. When they know the State Inspectors are coming they hustle. Their kitchen and the cooks and dieting leave a lot to be desired . It really socks for these elders

I only got as far as the front door as I had to speak to the lady in admissions for a respite. Nothing more than that it smells okay when you come in but I'm very interested in how it smells as you go further on back into the building but entering the front door and speaking to her was a nice experience, but I would like to see how things are in the recreational area

My loved one is currently receiving skilled nursing care and physical therapy from Vivo Healthcare in Orange Park. Her stay there is making a marked difference in her well-being and overall health, which is plain to see. The staff have been beyond helpful and have explained every detail of her care to my family and we feel great about that. It is has been most gratifying to see her improve every day. She's also relaxed and feels comfortable there, which is a great relief to me personally and to our family at large. They have a wonderful and thoughtful calendar of daily activities, such as Bingo and dog therapy visits. Thank you Vivo Orange Park for making such a difference!

From the nurses to the therapists and support staff, everyone is treating her with kindness, patience, and genuine compassion. During a very stressful and emotional time for our family, your staff has made us feel reassured that she was in good hands. The level of attention, professionalism, and respect shown to her means more to us than words can express. It was clear that her comfort, safety, and recovery is truly a priority. We are deeply grateful for the dedication and hard work your team puts in every day. Thank you for taking such wonderful care of my auntie and for supporting our family throughout her stay. Rude uncaring staff and young possibly not trained well enough CNAs. Food disgusting. Not all CNAsare uncaring. They have difficult jobs and people here are in a lot of pain. Sometimes the reason it takes them to get to rooms i believe is that they are short handed and there are a lot of people to help.

NEVER GO TO THIS FACILITY. The nurses don’t deliver your medication until 10:30 pm and by then you are in so much pain. The food all 3 meals were served ice cold. I saw a doctor one time and asked repeatedly for help getting the medication from the doctor to bring it to me earlier and NEVER go a response. In conclusion I told my health care advocate about but I signed myself out and swore I would never go back and I pray that no one else goes there!!

I was there for a week after surgery. Vivo had no problems communicating with my doctors, treating my wounds or giving me my medicine on time. Staff were usually available pretty quickly. The food was standard institutional fare, i.e. it was nice that I still had my appetite without needing the appetite stimulators some of the older patients needed to stay healthy. I took off one star because I was recovering from ankle surgery but couldn't use the parallel bars to strengthen my walking because they were damaged.

My husband was sent here for therapy and they kept him 2 weeks and sent him home when he can barely walk have been fighting 2 days to get his pain meds and still do not have it was told by social work 30 fay supply but that isn't happening as well I am appalled by the professionism of this facility....how can you treat a person this way have spoke to jose about these meds 2 times

My Dad was at Vivo the last few months of his life as he needed more care than his assisted living facility could provide. From the moment we arrived Vivo OP, he was wonderfully cared for. Every day as I would come to visit him after work, the nurses and CNA’s were kind, approachable and always willing to help. I’ve worked in long term care for over ten years and have had the opportunity to visit many different facilities and observe staff at the centers, Vivo OP has a great staff that cares! They care about the about the residents, families and their own teammates and it is evident as you see them work together. When I had a request, the Executive Director Trinity Blocker was quick to respond and assist in getting it resolved as quickly as possible. My Dad passed away in April 2024 at Vivo peacefully with Hospice. Thank you Vivo OP for taking such wonderful care of my Dad!! Your staff was so kind during his care and after he passed to my entire family!!

Recently toured this facility, greeted right away by the friendliest front desk associate. Facility was clean and smelled good. All the staff greeted us in hallway on tour. Rooms clean and tidy. Residents all seemed very happy. The Liason addressed all questions prior to us touring building and was extremely helpful and knowledgeable in relation to my dads dad’s needs and medical care post hospital I was very impressed by her fast responses and appreciate the support with transitioning my loved one from hospital to skilled nursing. She made it very smooth for me and took the stress off the family in so many ways, beyond appreciative of the outcome! Highly recommend this facility ..

My mom was only here for a week before being sent to the hospital, but in that time, everyone here was so nice and welcoming! They treated my Mom so good. Any concern I had was answered in a timely manner. Since my Mom was transferred out to the hospital, I have received several calls checking on how she is doing! Don't judge this book by its cover, it may be an older building, but it is clean and does not smell like some rehab/nursing homes do. And the employees actually care about the patients!!

The nurses and canna’s are the best of anywhere I’ve been and I have been in four rehabs in last four years. The food is the worst I’ve seen,I don’t think they even have a cook but the head dietician should have to eat three meals a day in the dining room for one week, no specials for himher just exactly what they offer residents. I have lost 14 pounds in two weeks it is so bad. Not a great way to loose weight no strength to want to do exercises.They do offer an alternative menu, but you have to turn in your request without even knowing if you like what they are serving. Let me say even one complete hall of residents request a grilled cheese for diner,you know there is something wrong. You have to turn in your alternative request one hour before e you are served. I suggested the put the menu on the daily events sheet that activities pass out every morning forthose patients that are bed bound and can’t go to nurse station to look and see what is on menu. They said no because they would expect it everyday, well yeah that is the reason for sharing the menu with everyone.the Rehab department is tops in my book,they are great at what they do.they have wonderful helpful suggestion for patients.IFyou listen to them and do what they ask you to, you will get better.very good department

Anytime an Administrator or Director saw me in the hallway they would ask me how I was doing and tell me to keep up the good work. Always respectful and always encouraging. They are always moving throughout the facility seeing that things are working as they should. The physical and occupational therapist were consistent in getting my body to work the way it was supposed to, even when I didn't want to. Once again giving me encouragement like an AA meeting "One day at a time". The nursing staff and CNA'S treated me as if I were family, always checking in on me. Even the housekeeping staff were consistent in keeping my room as if I were at home. When it was time for me to come home my case manager took me step by step in my discharge process so that I would be able to get any help I needed. From the bubbly receptionist when you first come through the door to ever watchful Kitchen Manager in cafeteria, I had more respect here than I sometimes get at home. I truly thank those who were involved with setting me on my road to recovery. If at anytime you find yourself in need of retraining your body after a medical emergency I would suggest you see what Vivo Healthcare Orange Park can do for you and/or your family.

First let me start by saying, if you are reading this, then there is a chance you are looking for help in caring for a loved one and my heart goes out to you. It is a difficult decision, and whatever you decide, know you are making the right choice. My mother who has dementia is now in the care of Vivo Healthcare in Orange Park. I am so pleased and extremely at ease with this decision. Please know that I am not in any way affiliated with them other than my mother being there long term. Every visit to my mother, I see how truly happy she has become. The staff are extremely caring and loving. The first step was meeting with Sha Ron Hartfield and LeSandra Jones (Admissions Director, and Business Office Manager). This meeting gave me reassurance I was moving in the right direction. They are both knowledgeable, understanding, and empathetic in the caregivers dilemma. On the very first day of my mothers admittance I became much more at ease. This is when I met the Unit Manager, Letesia Hankerson, and the Director of Nursing, Roseanna Singletary. Both exemplified professionalism and an extreme passion for fulfilling their mission in the healthcare field. If you are reading this and wondering how would I know their exact names and titles, it is because I collected their business cards knowing I was going to do this write up. Other important key names I want to mention: Lauren, Crystal, Brandon, and Tai for being part of such a wonderful team. Together these and the other staff members not mentioned have a created a home for my mother and the other patients where I know they are truly happy and cared for. I know you're facing a difficult time. Vivo in Orange Park has given me peace of mind. Thank You all to Vivo Healthcare in Orange Park from my family. We appreciate all that you do.

The best place for anyone to heal. Vary clean and the staff is most caring! Food for the most part is good, may be cold by the time everyone is served. 5 star would be great if not for my night nurse, she doesn't care for understanding or trying to help... should not be working in health care at all! My loved one is at this facility and I have had one problem after another. One main problem is their phone system. You call the front desk, they transfer the call then it hangs up. Some staff do not have their name on the VM to leave a message. Further, when you try to call after 6 or 7 pm they seldom answers the phone. Today I have been calling trying to reach the Soc Wkr and I can’t get thru to her for an urgent matter. The entire staff needs to be more professional.

At first my dad was sent here without my knowledge from OPM. I quickly turned to the internet in search of reviews on this place to see who had my father. At first glance the reviews are terrible and are not welcoming. Most of the negative reviews are from the previous company that owned the building a few years ago. After multiple calls and being the persistent person I can be, I found myself a little more at ease with the situation. I would like to commend Tia who is one of the ladies that works at the reception center and Stacy who is a qualified nurse that actually is the one dealing with the patients. There are many employees at this facility that are wonderful, but if it was not for these two I would have yanked my father out of there after the first week. I would just like to personally thank both of you ladies for your hospitality and professionalism throughout the process, and your efforts are definitely appreciated and noticed. Thank you both again for being you and actually taking time to comfort others.
