I am Anthony Scott Brown with On The Wings of Love, Inc. 501 C3 Non-Profit Organization and I used to do a full church service at this home before Covid-19. I have been doing this going on three years now. I have now been invied back to do a Bible Study on the first and third Sunday of each month. The staff has always be courteous and very helpful. Mr. Timmons the new Administrator has been a real blessing to work with. The home is always clean and the Residents are always care for very well. This Residence has a very caring and compassionate staff. I am currently doing an in depth Bible study on the First and Third Sunday of every month. Full historical understanding from Genisis to Revelation !

If I could leave a 0 I would! I recently had to endure 5 days at this hell hole I received little to no care there I was left unattended for hours wearing a top mainly naked I have a non weight injury I had a bell to ring to get any help my meds weren't given to me at times 2 to 4 hours late! Was told to have a bowel movement in my diaper well first off I had no diaper I forced myself out of the bed so I could use bedside commode I could go on and on but please if you love your family member or friend do not put them in there they will deteriorate mentally very fast and not recieve the dignity they deserve I'm so happy my family loved me enough to get me out of there!

Very informative meeting with social worker and case manager. They were very professional. Ms. Thomas was the best person I met. She was to the point, understanding. When your loved ones are in a denial situation, it takes a certain personality to ease the pain. She did it for me. Thanks

I brought my mom to Fouraker Hills to stay for rehab and we were welcome with smiles and open arms.Each staff member we encounter was very friendly and eager to assist us with anything we needed and answered any questions we had without any hesitation. My mom was very nervous about coming there and adamant that she did not want to go but upon arrival with the warmth and love the staff showed made her feel a sigh of relief. When I saw the nervousness she had in her face before going there turn into a smile and to look at me and let me know that she would be all right there made me have relief. I can say that the saying that the first impression is the best impression was truly the case at Fouraker Hills.

It’s an older building but the staff seem very nice. My daughter fall out of bed her first night but they are going to use fall pads, didn’t know she moved so much. It’s nice & clean, she will be getting the therapy she needs here in hopes of a full recovery and God will let her come home to her kids. We are so blessed to have a place like this in our time of need Thank you

Maria seem good. She can't tell me whether or not she got a shower, so I gave her one,which it's OK. Ijust have a problem with her clothes, she have some clothes missing and can't tell me where there at, need to have a personal belongings list to know what her children brought her. Mds and the business manager is awesome x10. I can't beginning to tell u how nice they are, how much they make you feel like family. I am so happy to have met them and know they will be part of my aunt care. Social worker tall and light skin she does not help at all and has a bad attitude and the therapy director is along with her. They both pretend like they care about your love ones until they fall like my aunt did and they they try to get them kicked out. Then play dube like they don't know anything about it. However the police knows I filed a police report just so they both can be under the radar so people will know how they are if they do someone else family wrong or if I had to file a sue on mistreatment and neglect. The only reason I gave this company a 5 star because those to interdisciplinary departments are awesome and they made up for the mistreatment. I truly appreciate them womdeeful, kind, professional lady's.

If I could give 3.5 stars I think that would be most appropriate. Fouraker Hills seems to have the basic needs met which is perfectly fine. They have decent sized rooms, availability and the staff seem attentive and professional. They do the best they can with what they have. My loved one is receiving PT and OT and seems to be doing quite well. They like most of their meals with a few exceptions. If I had criticism, it would be directed mostly at the smell of a few of the wings/units/halls. The majority of my visits included running smack into a strong smell of urine and feces. I understand that people are going to have accidents in places like this and my loved one is included in that conversation, but it’s really nothing that a little antiseptic, bleach and air freshener cannot cure. My most recent visit was better in this regard. One of the front desk attendants, while quite kind and accommodating to me, I overheard answer a phone and responded with, Well, I don’t know your father’s name This is fairly unprofessional and not very thoughtful. If someone calls asking to speak to their father, a better way to respond might be something such as, Thank you for calling! What is your father’s first and last name? Overall the facility does it’s job. Some people may find it to be dumpy but it’s essentially a Medicaid facility, so it’s not going to be a Waldorf-Astoria or a Hilton.

Smells like different kinds of poop diapers from every room. 2 people had their room lights on and nurses never came in the 40 mins I was there. They serve raw food. They put my grandmother in the hospital two times.

I was visiting family there and was having a difficult time, Out of ALL the people in the meeting, this one young lady really was the one to resolve the issue. I think her name is Alisha Thomas the social worker. No matter how much they cut her off when she spoke, disregarded her opinion, she still was the one who stuck and stayed until the issue was resolved. Alisha is a star.

I went to visit a dear friend cora Rollins rm 512 her room was clean her bathroom was clean. And they were giving her oral care at the time of my arrival . As I'm not a legal family member, they couldn't give me much information regarding her medical needs. But her nurse went above and beyond her job, to help me get her up into her wheelchair so we could take her for a walk outside. My only concern was that she kept saying she was hungry. But they have her on a feeding tube . Overall, I see people caring for ms. Cora who truly cares for her as a person. Not just a patient. Praise GOD. Fouraker Hills gave me hope when I had lost all hope. Miss Val the Admissions administrator is awesome, in my opinion she genuinely cares about your family member as well as the family. She has gone above and beyond to help me getting my dad placed. The interaction I have seen with the medical staff and my father, especially the occupational therapist Really touched my heart. We’re just getting into this, but so far I Can only say positive things about Fouraker he especially miss Val.

Jack Martin told me that he got a bologna sandwich and potato salad for Christmas dinner. I thought he was joking. That is truly disgusting to serve that for Christmas. Would you want, or eat, that?
