I’d give the staff a 5! They took good care of my mom for the many weeks she was at Carlton. And they need more help in each wing! They work hard! PT personnel and management was awesome. The finance lady was nice and helpful. The receptionist greeted us every day with a smile. The rec room was fun and had lots of activities to get people out of their rooms. But something happened with management (we saw the suits walking around) after a couple weeks and at the end, standard products were not available.

I want to thank all the Doctors, Nurses, the Physical Therapy Team, the several CNA'S, the Kitchen Staff, the Janitorial Staff, Speech Therapy, Activities Team, the Phychiatrists Team, the Transportation Team, the Business Office Team, the Administration Team, the Patients l met in Long Care and Short Term Care as l was in the Orthopedics Unit and especially a big shout out to the Rehabilitation Center Director (FRIEND YOU KNOW EXACTLY WHOM I AM TALKING ABOUT & YOUR "ALISON" GIRL ON YOUR GREAT TALENTED NUMBER 1 PHYSICAL THERAPY TEAM IN THE WHOLE UNITED STATES OF AMERICA AS I WAS TOLD BY SEVERAL LONG TERM RESIDENTS WHO LIVE AR THE CARLTON SHORES HEALTH AND REHABILITATION CENTER FOR SEVERAL YEARS)!!! I received the best Healthcare l have ever been given in a Rehabilitation Center in the Central Florida Area and l have been in several Health and Rehabilitation Centers in the Central Florida Area due to many accidents playing mixed league sports in the Central Florida Area, so l have experienced Healthcare in other Health and Rehabilitation Centers in the Central Florida Area so l do have experience and knowledge of different Health and Rehabilitation Centers in the Central Florida Area & l want to give all the Staffing Team at Carlton Shores Health and Rehabilitation Center a big thank you very much for the 2 months of Healing, Recovery and Healthcarel received from the Carlton Shores Health and Rehabilitation Center in Daytona Beach, Florida and l totally give the Center the highest rating possible!!! I also was set up with home care Physical Therapy, Occupational Therapy and Nursing Care to be continued at my home not far from Carlton Shores Health and Rehabilitation Center as well as all the special medical equipment and local transportation l need and l still need to use to this day as l am writing this review!!! My father was at Carlton Shores for73 days, recovering from skin grafts on his legs. DIANE COLON at the front desk was a life saver. She was friendly, kind, and offered assistance as to who I should contact regarding a variety of things. DIANE always has a smile on her face! Dad's wound nurse TYSI was knowledgable in severe wound care, she was always gentle and caring when addressing his wounds. TYSI is an angel for people with wounds. The day-shift nurse Auoynda was over the top in paying attention, offering kindness, and words of reassurance during our stay. She has a huge, caring heart for the elderly. Dad said his night nurse was attentive but I never met her. Nurse Michelle was helpful when things got rough, she was my go to person when things were not getting done. She would have to straighten things up and I appreciate what she did for dad during his stay. Other than that the care was not so good. There were many, many times that he had to sit in a soiled diaper for 1-2 hours because the CNAs were "too busy" to attend to changing him. Yet I would find them behind the nurses station taking on their cell phones instead of assisting the residents. UNACCEPTABLE Dad also had bed sores and lying in soiled diapers did not help his situation. There many times that I would walk throughout the Orthopedic Unit looking for someone to come to my dad's room to help him and could not find anyone. Again UNACCAPTABLE. It appeared there was a new crew of CNAs towards the end of dad's stay because we saw them in his room regularly and they were more attentive. The food service was hit or miss, more miss than hits! Somedays he would get a cold sandwich for dinner, or an entire tray of cold food (mashed potatoes with cold gravy that we would try to remove so he could eat the potatoes), half cooked meat, cold eggs, mixed peas and corn that was always cold, and rice that never seemed to be cooked. Other days, especially Friday's, the food was very good. Fish seemed to be their go-to on Fridays and it was always cooked well and delivered warm. The administrative staff did not seem to care when I spoke with them about the lack of care my father was receiving at the beginning, the first month at least. I wonder if they would change their ways if one of their family members had to stay there for rehab or recuperation.

My husband had a very good experience for the week he was there. Luz Mesa, one of the aides, was especially kind, helpful and professional. He was in the Orthapedic wing, room #305. Michelle was also very attentive and professional and caring about the pain he was in. I would recommend this health and rehab center to anyone. Thank you for your care as it made my daily visits more pleasurable as well.

Lori at Corlton Shores is wonderful to work with. She and her team are always kind, professional, and truly care about their residents and patients. They work seamlessly with Gentiva Hospice to make sure patients receive excellent care and support. Communication is always easy, and the teamwork between everyone involved really stands out. It’s such a comfort knowing patients are in caring and capable hands. Highly recommend!

My mom needed to rehab after surgery. She was scared and unhappy until the therapist Vanessa made the whole process easy and professional. We love you Vanessa! Mom wouldn’t have stayed if it weren’t for you!

My 80 year old dad has been in there for 3 days. He just needs a bit of rehab to be able to breath again. He is supposed to be getting 3 meals a day, yet he got forgotten about completely. No dinner. The next day he was given some sloppy Joe stuff that he can't eat which is y he filled out the menu chart (they ignored) 2night he was supposed to have dinner and they brought 2 hours late with no chicken at all. Cold pasta that was hard to chew and corn bread with no butter. There was supposed to be chicken. He is getting absolutely no protien from food, yet they want him strong?! I questioned his CNA and she said new kitchen help and CNA has no control over kitchen because they yell at her! What the heck is going on in this place? MY DAD HAS NOT HAD A HOT MEAL YET, AND ITS WHATS LEFTOVER BECAUSE THEY KEEP FORGETTING TO SAVE FOOD FOR THE LAST GUY ON THE LIST!!!!DO NOT SEND YOUR LOVED ONES HERE

I have been here a little over two weeks and I cannot say enough about the staff here. Professional, kind and compassionate. The physical therapy team work miracles!. My only suggestion would be to have a front desk person 24 hours, deliveries for food sometimes were complicated due to after eight pm, there’s nobody at the desk..

Overall, it was pretty good, but we did have an incident. 36 hours hadn't even passed after my grandad was checked in, and my grandad had fallen, and no nurse was aware about it. Luckily, we bought him some food and saw him, but we pressed the red button to call the nurses, and no one showed up. We had to go get them.

I was at Carlton Shores for 78 days. My experience was great in some areas but not all areas. First I was meet by all the upper management these staff members were so friendly and knowledgeable about who I was before I came I thought this was awesome. First and foremost, I want to say they have the MOST AMAZING THERAPIST DEPARTMENT !!! Staff are awesome. They work with you until you Are ready to go home they are compassionate, patient, kind, helpful ,,loving they are willing to go the last mile to get you to your goals. There is nothing I could say bad about this department if I ever need a nursing home rehab department again I will definitely go back to Carlton Shore. Because the therapy department is the bomb,And I appreciate everything that they did for me to get me to my goal. I really hated to leave because these people became like my family and friends not just another patient, So if you need rehab, this is the best place to go. Nurses were somewhat knowledgeable about there job but not about patient care, and some were well experienced. They used a lot of Agency nurses and CNA’S some had worked there before so they knew the routine others did not know the routine so some things were missed on the CNA part and the nurses part. Sometimes your room would get cleaned and sometimes it would not. I have pictures but I choose not to share. I did notice that the CNA would use there phones while in the patient’s room I’m not giving attention to the patient, And the nurse sometimes would not come and give you your medicine when you ask for it, Sometimes it would be in an hour after you rang the bell they were Told, but they never would come. The regular nurses & cna were also great on meeting your needs and some nurses too . Everyone from upper management to maintenance they were always willing to help you even if it wasn’t there job great people just need some guidance on a few things. Now the food on the other hand. The most disgusting food I’ve ever tried to eat. It was always cold on the plate. Breakfast was terrible. You got the same thing just about every day scrambled eggs, one piece of toast. But no milk you had to ask for it. Lunch was even worse. The food has no flavor And when you try to get the alternate, you’re we’re told that they don’t have any And I said why you posted it on the menu that you have it the dietary manager refused to talk to me So I had to get my food from outside or my friends would bring it in cause every time Or even smell it I would throw up for this happened many days. I was constantly throwing up. Activities was a great time. They tried their best to keep the residence happy and busy. And of course, bingo and they were willing to go shopping for you on the days that they had set for shopping day. Thank you very much.So if you take your family there just know to bring them food too . Thank you one and all even some things were not always good I still would give this a A+ . Plus. Thank you all .

I was in for a week , after gall stone surgery , i found the rehab quite helpful....the food was quite good .....the only problem I had was I had to wait until the day before I was released to have my stitches removed.....I had requested it a few days before.....

I know my mom is taken very good care of until the times her main caretaker and nurse are on. The other times she wait a long period for a person when light is on. Mom is not crazy about food but I am told that will change after COVID-19. It seems you are adding telephone operators later in the evening bc when cardiac is covering long term they are angry. Sometimes I like to check on her at hs but not too often. We have clothes that never come back from laundry the most current was this week she got 2 new items and her name was in them and they have not returned from laundry and she was upset when I saw her today. I am pleased with the care that my family member is receiving, the staff is very attentive to our needs and answers any concerns we may have. The therapy team has been great and much improvement has been seen in the 2 weeks we have been admitted.

The Carlton shores has an open airy atmosphere with funny and amazing staff in all areas I love visiting my mother and getting to sit with residence and staff at the big table like family Guess what everyone is welcome Have seen overworked disgruntled employees at other facilities with no hope in sight Even though the Carlton Shores staff does more than I could have imagined and Graciously, they are consistently hilarious and a joy to be with every visit every shift In Both wings of the facility this holds true regarding the level of care employees are performing Thank you for my family finally being able to sleep at night instead of worry You guys are the best The cream of the crop We are your biggest fans More news later

My mother is there, the place is very clean,the staff is constantly changing, I'm a little worried about all of the people who work there, and all of the turn arounds. But my mother is doing so much better than when she went in. For some reason she is shaking again because of her medication wasn't given to her for two days. I wonder how these people would like it to be there parent that they weren't getting the proper care. Parkinson is a horrible disease. I love my mother and care about her health. Please don't take this wrong. I applaud your efforts

My Dad has had a hard road since surgery. He was released to Carlton Shores just yesterday. Tonight was my first visit with him at this new part of his journey. He can visit with his buddy, Spike (Mom's rat-terrier). That means so much to him. We also got to meet his therapy aid, Millie! She's very friendly and helpful. I think Dad will do really well under the care he receives there.

Very nice place clean, great food and very attentive staff. Their rehab department is great. It was obvious that every therapist was concerned about getting me to my goals and my safety. They went out of their way to make sure that I had everything I needed to be successful. I am glad that I chose them and plan to return for out patient rehab.

Absolutely FANTASTIC facility... Recently my elderly mother fell and broke her pelvis, requiring extended stay and physical therapy. This is an EXTREMELY well managed facility with a 1st class staff including knowledgeable on-site physical therapists. Highly recommended if your loved ones require 24 hour care and/or physical therapy rehabilitation!

My stay at Carlton Shores was quite wonderful. The CNAs the nurses my nurse practitioner and my therapists were the best ever. They were all very attentive, caring, supportive and encouraging. I even had some fun playing bingo a few times. The only negatives I could share with you would be the food. It really needs a lot of work. I understand you'v got to be careful with the salt. However there are many other ways to add flavor. I have been in the Food Service field for the last 30 years and continue to do so. So I know that there are many ways to add flavor without adding salt and sugar. There needs to be more attention to those of us that have diabetes. Many times I got carbohydrates that I did not ask for and that I do not need. Because there was no other alternative and I was hungry, I ate it. Subsequently it raised my sugar many times. So that needs to be looked into and paid more attention to. The other negative would be an experience I had in the activities room. One day after playing bingo a gentleman asked if he could present a talk that he was supposed to be regarding relationship with God and the many different faces of God. My husband and I both being Christians we were interested in hearing him. I am a Biblical scholar having attended Baptist Bible College in Springfield Missouri. So as he kept talking it was very clear to me that he was Jehovah's Witness. Which is fine Everyone's entitled to their own opinion and to believe what they want. However he knew that I was a southern baptist and a Christian and not at all anything like a Jehovah's Witness. Yet he insisted on putting down not only Jesus as the Son of God but then he went on to attack the Trinity of the Father Son and Holy Spirit. And he wasn't kind in his reaction to me when I told him I did believe in the Trinity and why. I then told him that I would no longer listen to the blasphemy and I left. I think perhaps there needs to be more done to look into what people are going to share last minute to make sure there's not going to be something that could possibly offend or make someone very unhappy as I was. The last good thing I want to share is on my next to the last day I desperately needed a haircut. And my niece Chrissy was able to come by and do that for me. Sheila, the activities director was kind enough to allow us to use the beauty salon to do this. I just think that was absolutely wonderful and so kind of her to let us do that. So overall I can't say enough good things about your facility. And I've already recommended it to two people that are looking at facilities to put their loved ones in 1 short-term 1 long-term. Thank you so much for your care for me and as much as I loved it there I hope I don't have to return anytime soon. Thank you so much for everything!

I was a patient at Carlton Shores following total knee replacement. My experience was completely favorable. Food was good, staff was very attentive and the physical therapy staff and equipment was first rate. I have referred numerous friends to Carlton Shores for their recap.

The Physical Therapy staff are awesome. I even went back for my outpatient rehab. The facility is nice & very clean. They are understaffed though making it difficult for the residents who need the extra care. Everyone was always polite & I would recommend Carlton Shores especially if you need rehab.

The long-term care staff works very hard, and I feel they do the best they can do. Unfortunately, they are understaffed, and overworked, making it very difficult to provide the care that the residents need. Some of the staff member's even clock out for lunch and continue to work through their lunch breaks. They do not have enough time to provide the care that is needed for the residents that are 100% incapacitated, which is my mother's case. Most of the days, my mother has to be in a dirty diaper until after 8:00pm, which is when a CNA can clean her before my mother goes to sleep. Please note this is not because the CNA's are not doing their job, it's because they are so understaffed that they do not have the time take care of everything they need to in the course of a day. This is so unfair to the staff, and to the residents.
