This is the second place my ex-husband has been in the past 2 mos. Have to say it is 100 times better than the previous facility which I will not name. Rooms are very big, even though there are two to a room, it is not a tight fit at all. Staff is wonderful the nurses and PT. Met with the case worker and PT today to go over his plan and they were insightful, pleasant and very competent in their jobs. I am very happy with this facility. The maintenance staff is also amazing, always cleaning when I go in.

Myself and 5 of my cousins came to visit my grandmother and the nurses were very accommodating of our visit. The facility was clean and the residents seemed as happy as their circumstances would allow.

I came for a visit to a friend. I love the morale of the staff. It doesn't smell like a nursing home and the residents seem to be welltaken care of. The A/C on the floors could be better.

As the daughter of a resident who has experienced many rehab and nursing facilities in the area, I can confidently say this is by far the best one. My mother has been a resident here for over two years, and the care she receives is warm, friendly and doing the best they can with the resources they have. The staff is compassionate and attentive. Every employee deserves recognition and a raise for their hard work and dedication. Shoutout to Simone, Paul, Sandra, Tanya, Melissa, Pauline, Steph etc!!

I came to visit with a friend her mom has been in civita for rehab during my visit I had a tour with a woman directly across the room from where my friend mom was so very nice her name was Sally Vanderclock what a pleasure she made the visit. very pleasant and the place is very clean.

My mom was here for a few days. First day everything was perfect. She had her own room. She was in good spirits. Unfortunately what she experienced in the evening when I wasn't able to be there was horrendous! She was forced to shower when she didn't feel well. My mom came in to rehabilitate after an (Aortic Dysection Surgery) they had an aid use a hoyer lift on my mom when she came in walking (makes no sense) PT came in while she was being handled. And immediately told them they can't do that due to her surgery. And put warnings everywhere in her room. The day shift is good. PT is excellent. If you are considering sending anyone you love there. Please be there as much as possible. Especially in the evening!! My mom did have anxiety after her surgery. 80/90% of people who have her type of surgery don't make it. So she is well aware. Which of course caused her to be anxious. But the mom that I left at the rehab wasn't the same mom I picked up. This facility made her recovery so much worse!!!!

I’m an entertainer who visited on Monday afternoon. The staff kept the residents engaged before, during, and after my performance. Everyone had a great time listening and singing along to the music.

I was there for almost a whole month I would say that most of the people there are very nice and accommodating...the aides and nurses took good care of me..the Physical and Occupational therapist were apot on for getting back to walking with my walker...I can also say that the activities director was very organized and pleasant always had fun activities for every one

This place has been a huge help to my great aunt who's been seeking treatment here. I do not have to worry even 1% about her since I know the kind, caring, and always loving staff is constantly giving her the absolute best care possible. They consistantly go above and beyond! Taylor, Cody, Lisa, and so so many others at this facility have been immensely professional and they are all so detail oriented it's amazing! When it comes to my great aunt, it is like they know her already as long as I have! Honestly, I have nothing else to say except a total 10/10 experience all around. Thank you Civita!

I went to visit my dearest sister/friend she’s been in this place for a while and after passing through a bad experience in other places she just love Civita @ West River. I’m very happy to see the way she’s doing and how is being treated by the staff. The nurse so kind and attentive. The only thing that it could be mended was the fact that she was waiting a while longer to be changed and but back to bed. But for the rest and most importantly she feels good and well taken care. Thank you for your time and compassion for her and all the the clients people that is there expecting the care compassion and respect needed for all their clients needs. Thanks! Also with visitors respectful and attentive thank you, God continue to bless you, guide you and gives you the strength and wisdom to continue to do a great job every day.

My husband’s mom was sent to West River after hospital stay for physical therapy before coming home. PT people are awesome and have helped her tremendously. We did have a concern that was handled quickly and effectively. No blame game. They simply reached out to us to let us know they were working on our issue and it was rectified quickly. The place is clean and welcoming. The staff is very pleasant and I love to see Everyone (even office staff) engage with patients and make them feel important and cared for. It’s a very nice place!

Civita @ West River is a great place! My husband’s sister was there for the last eight months of her life. They cared for her with kindness and dignity and we don’t have a single negative thing to say about the staff or the care she received. Our family felt so good about her care that when my husbands sister-in-law needed short term rehabilitation that was the place of choice that her family chose for her. After only a few weeks there she is healthy enough and strong enough to be going home soon. She has told us that the care has been exemplary (and she should know because she’s a retired nurse herself) and we could see that for ourselves when we visit. The nursing staff is kind and caring, not just to the patient but to the families also.

The staff there is wonderful. There’s a beautiful, singing nurse that makes my moms day. Her nurse is very easy to talk to, especially how confusing the meds schedule can be for family. They are patient and I know this is a very difficult job. We only had one nurse that didn’t have much of a personality. From Taylor in the office to the nursing staff to physical therapy. It’s all been a very good second time around for my poor mom in rehab. I like the fact that they are concerned with what the family wants and needs are so we could coordinate

Having been in other places, this is a good senior living facility. The staff is very kind, attentive, and respectful to the residents. My mother is well cared for. I would recommend more thorough cleaning, especially of the carpets; I believe that carpets shouldn't be present in this type of facility.

When I came here I was a Hoya life from my bed to my wheelchair now I can put my self in the wheelchair and wash and dress my self and I am walking again with a brace and a hemi walker I am seeing the light at the end of the tunnel be going home soon gsembrat

The people there were great but my ex-husband who I am best friends and did not have good care at all. He got worse there coming from Yale, and when I walked in the room one day he looked like death. They did not even know enough to call us and say that they think that he should go to the hospital as it took me to tell them to do that. Had not eaten in one week or drank. Had a UTI infection, which they were giving him cranberry juice, thought he had pneumonia which he did not. He had been sick for 4 1/2 months he almost died and they knew this and did not do what they were supposed to do.

My husband was at Civita for a month following surgery after having a Whipple surgery. All in all the care was okay; however, there is always room for improvement. Some of the staff would assure my husband that they would do certain tasks i.e. get his mattress replaced, which didn’t happen until the day before discharge. The good thing is that the next person would have a good mattress. My husband was greeted daily by the owner and Anne, which made him feel very comfortable. The nurses and other staff, most times, were attentive to his needs. Thank you for caring for my husband!

This facility is clean, compassionate and surprisingly more organized and proactive than our current healthcare staffing shortage would typically allow for. Originally I was concerned to learn that this is under new management in October and that when I went through prior reviews from 5 months before that some of the poor reviews left still seemed to still be relevant. I must say that within a day of me leaving a message with the facility, my call and concerns were all addressed immediately by the social worker and funneled through the appropriate departments and staff. Within a week complete corrective measures were put into place to accommodate the unique needs of my loved one. I would highly recommend this facility. Just be proactive as anyone should when your loved one requires care outside of home, communicate and give a little patience. They really are a special place.

My father has been at West River 3 times and I have been satisfied with the care at this facility. From the moment you walk into the facility you are greeted and are treated like family. The staff is always available to answerer and address any concerns I have. I recommend this facility to all my family and friends.

The reviews on this sight seem to me to be untrue. My Mother was in 5 facilities before coming there. As far as the food goes it’s the best I have seen and my Mother enjoys the food. Fresh salads and healthy meals served in a lovely dining room or your room. If my mother is out they save her meal to make sure she has eaten. She has had falls but for this age group they are going to happen. I was informed immediately and if it involved her head at all in the slightest way she was taken to the hospital. I find the aides to be attentive and mostly friendly. It does seem that they are spread thin and work hard. It’s a clean place no smell and you would be hard pressed to find a place this nice that takes Medicare.

I have to say that I am very satisfied with the care my mother received at this facility. There were many caring, attentive medical professionals. My mom was on the first floor, and had two nurses in particular, Tatiana and Sherry. Attentive, kind, on top of her care....always in communication with me. The O/T (Susan) and P/T staff (Jay and David) were exceptional. The front desk receptionist, one in particular (an African-American woman), one was VERY pleasant. Always a cheerful greeting and best wishes for a pleasant evening on departure. Of course, it is a nursing home. The food may not be as wonderful as if it were a five-star restaurant, but the food was not horrible, either. There are lots of activities for patients to participate in. The facility is really nice and seems to be very clean. Some of the tvs need to be updated...they are old and outdated. But, there IS cable. There are issues that are common to where you are....lots of sick and elderly patients. Many cognitively impaired. My mom was here for over two weeks. At no time did I ever see anyone mistreated or neglected. The only doctor I ever encountered may have been a good doctor, but he had no bedside manner, at all. Not necessarily a complaint, just an observation. I am a social worker, so I may see things through my professional lens. But, I would definitely send my mom here again, if the need every arose. No one wants to be in a nursing home, I would imagine, but this may be as good as it gets.

One nurse and one aide were fabulous. Rest of the staff was sort of ok. PT was geared for geriatrics patients, not rehab. Staff focused roadblocks to my success, not overcoming my obstacles. Staff was overheard complaining about job when in hallway many times. One patient suffering from dimensia was often yelled at by aide to keep quiet. Communal shower room had PVC shower chairs covered with mold. Toilet in my room did not work and overflowed a lot When maintenance finally showed to look at toilet, area was not cleaned up from sewage overflow.
