Everyone here is so nice. I feel my mom is getting great care and attention. Today is her 3rd day. PT/OT DirectorChris is amazing! Therapist Tanya was so sweet, engaging and encouraging. Great start. Minus one star- in evening I hear call buttons going off and a slower response from staff. And of course place is dingy (but clean).

Terrible place, uncaring staff, THEIVES stealing clothes & wallets from patients, meds often late or wrong ones given, do not provide locking drawers, or "LOSE" the keys, barely edible to Un-edible food served cold, TV's going 24/7 making a good sleep impossible, leaking toilets, beds without rails to prevent patients from falling out are not "AVAILABLE" even when "BEGGED FOR" by injured patients because of general INCOMPETENCE or uncaring, dishonest staff, which appear friendly but keep patients in state of MISERY. BULLYING by other patients is frequent, along with screaming & wispered THREATS, but no one cares, especially the OVERPAID & SOULESS head Social Worker. Your LOVED ONES would be better off living on the streets in a cardboard box.

Our journey began when our dad was placed in a horrible nursing home directly from the VA Loma Linda Hospital with no warning. One Flew Over the Cookoo's nest should explain it! After some research I got hold of Carmen in admissions at Heritage Park in Upland, she was absolutely fantastic and worked hard to get him transferred out of Riverside and closer to us. We have been very please with everyone at Heritage and the around the clock care is fantastic. Each time we have been there we are greeted with smiles from everyone, the place smells clean (and is) communication with the RN's, LVN's and CNA's is a true blessing. We felt lucky to have our loved one so close and well taken care of. To top it off our greatest non medical help is with the social workers office. Sandra is not only knowledgeable as to what we needed but took the time to help us with the transfer our dad out and to an assisted living facility. In addition Mike, the administrator, listen any time we had a need and was on top of it! I could go on about the physical and occupational therapy, the kitchen staff, the custodial and the front desk because they hold there own too. Longest review I've ever written but trust me when I say it is well deserved!!

My close friend has been in this place for about a year. She came in with a serious Wound on her leg. They ONLY JUSTNOW realized her BONE IS INFECTED !!! Because a Dr who daw her finally decided it would be a good idea to Xray or MRI/whatever it. That is a LONG time to be someplace with JUST Nurses changing Bandages and being- CLUELESS... AN INFECTED BONE !!!!!!! WTF Kind of Place IS this ??? Bad I would say, inept, lazy, any of those words fit this place on the 1st Floor.

Heritage Park in Upland is the WORST and most unprofessional facility I have encountered. My mother was admitted there after surgery at San Antonio Hospital for Rehabilitation and that never happened. I questioned physician orders, the plan of care, discharge planning, medications and no one had answers. They had me speak to the charge nurse and she was just as clueless and unprofessional. They discharged my mom with abdominal bandage and no after care instructions, when we lifted them we realized the staples had never been removed. I called to inquire only to be told call back in the morning, we don't have the information. The facility itself looks run down and the food is terrible. Please DO NOT SEND your loved one here unless there is no other option. I only gave a 1 star because zero is not an option. Heartbreaking to see the poor conditions our elderly must endure for medical care.

My mom had been at Heritage Park since August 2020 when we cant even visit her in her room because of COVID. The staff was accommodating so we could talk to each other and putting her by the exit door with a cell phone that she could use. She always looks well groomed. The nurse calls me with health updates and to my surprise, a house dentist called me with a dental checkup report.Currently, I am now able to visit mom in the TV room and touch her. This is the first time Im able to go inside the facility. It is clean and doesnt smell like the other nursing home facilities that Ive been in the past. The nurses, receptionist and the admissions staff (Jeremiah) are friendly and very helpful. I know they give mom the best care that she needs. I would highly recommend this facility who wanted to put their loved ones here cause I know they will get an excellent care from the staff.

this facility is a standout operation. The staff is kind, . I would like to give age P shout shout to Jeremiah (admissions). He was incredibly HELP FUL during a difficult situation. From the beginning to the discharge, he was absolutely so helpful All and all Heritage Park Nursing Center helped make this not so difficult to endure. Good luck with your journey.

The facility is OK and the care provided is adequate. But the staff, while pleasant could use an infusion of inspiration to truly care for the patients there. They frequently are delayed in delivering medication (you will find 6 of them standing around talking) and there is a brush off culture there. When you speak with them about your concerns or the needs of your loved one, they are all too quick to pacify you with some response and then they move on with the issue unresolved. They are highly disorganized. My impression is the staff, just isnt anchored in to what they are doing and the very important job they are doing. They operate on the surface, with no true intention of creating a best quality of life possible for these poor people who have lost their ability to function in normal life. It seems to be a problem in leadership. I feel it could greatly improve with a deeper ingrained philosophy, training and proper supervision.

My father-in-law was admitted to Heritage Park (HP) on March 10, 2020. He was discharged from San Antonio Hospital in Upland after undergoing a partial hip replacement after falling and breaking his upper femur. 3 days later visitors were banned due to Covid-19. Two weeks after he was admitted, my sister-in-law met with his care team, including a doctor. He had not even started physical therapy! His care team was non-existent. He was not given any medication for pain until we inquired about this. They then started him on Oxycontin, and he was drugged up for the next 30 days! He was incoherent, could not carry on a conversation, and in fact would fall asleep in the middle of a phone call. The staff is a joke, and is constantly redirecting to someone else who is never available to answer questions. They discharged him when Medicare ran out, not letting us know that he was eligible for a medical transport to another facility. He could not stand on his own, could not transfer to a toilet, could not do anything for himself, and yet they threw him out, with half a tank of oxygen for his COPD. Do not use this place if you have other options. Terrible treatment of patients. By the way, now that he is off the Oxycontin, he is regaining his wits and can carry on a conversation.

9 months and counting. My mother has been at this place for nine months now. I continually write reviews because I want others to know that this skilled nursing facility is one that sets the pace and standards that others should notice. Micayla the Activity director and her staff makes the patients feel that they are an extended family member. From the dining area with table class to scheduled activities during the day and lets add there once a Month outings . It is nice to know that your love one is not going to be bored. The staff at Heritage have a great way of including staff and patient interaction. I definitely recommend this place. Sharon

I WAS AT HERITAGE REHAB CENTER FOR ABOUT A MONTH. THE ENTIRE STAFF ARE OUTSTANDING AND THEIR REHABILITATION PT/OT ARE SECOND TO NONE. THEY ARE THE BEST IN CALIFORNIA. THEY TRUELY ARE A FIVE STAR FACILITY. ??????????

I highly recommend this rehab facility. It is the nicest one around. Jeremiah Johnson is kind and helpful. My husband was a patient here. The facility is clean and inviting. The staff is patient and caring. An added plus PT and OT are on the premises! There is a pretty court yard and common areas where family and friends can visit . ??????????

My girlfriends grandma is there and the nurses are lazy and dont do there job at the 100% they should.be.doing for theses seniors . my girlfriends sister came down from a diffrent state .and shes a RN and has been working for senior citizens and she was shocked at how lazy they are .we saw one nurse eating her lunch off of the medicine cart they roll aroumd to give there clients .as she was passing out the pills !!! My girlfriends grandma had poop in her toliet bin for 2 days . my girlfriend had to ask for gloves and clean it out herself !!Overall its a great place but some of the nurses are really lazy ...Response from the owner3 years ago

just met the rudest nurse on their evening shift on the 2nd floor.shes not so happy with her job.i thought it was just me but when i saw how she treated her CNAs,yelling at them at the nurses station in front of everyone,its just her personality.its so sad.the rest of the facility and staff are so nice and caring.its a lovely place.
