The posting for this care experience is for the activity department mostly because they demonstrated professionalism's in the care of my elderly mom in the 5 maths she was stuck in City creek. Specifically thanking Stephanie Rafaela Swanson, Celina, Kim and the entire crew in the activity department. They devised a method of enabling me to FaceTime my mother regularly knowing I live ways out of Sacramento. Thank you activity department for your kindness and non judgemental attitude to me and my elderly mom. God will bless and reward you accordingly. Recommends more training for the nursing staff and the Cordinator for Kaiser patients "Raman" or Arman; she never wants to let you know her name or role. The Cordinator definitely needs some more evaluation of her job.

Visiting a very close family member for hospice care. First impression was very positive and good. Place is cleaned due to recent renovation. The most noticeable thing was how the staff engaged with patients and the quality of care I witnessed during my visit. I am very comfortable of leaving my loved one there because staff are very professional and friendly. The nursing staff are great and communication with upper management was easy and professional. Highly recommend this place for health center needs.

What can I say about this place. Everyone was so nice and helpful. My dad suffers from dementia and fell and broke his hip. To say he was difficult to deal with is an understatement. Not only was everyone dealing with his dementia, but he also reverted back to his native language which is Greek. Not a lot of translator that speak Greek so it made things extra hard. But there was one person that stood out among the rest and that was Joelle. She's the activities director and she had her hands full, but took the time to care for my dad. She was so sweet and patient with him. He wouldn't cooperate unless Joellen was with him. She made sure he ate, drank, did his PT. She made sure he has safe and when I couldn't see him for two day made sure dad could FaceTime me. I was able to text her and check up on him any time I needed to. She always had his favorite snacks waiting for him. She truly cared for dad and I'm extremely grateful to have met her. She truly is one of a kind. Thank you Joellen for giving my family pease in a stressful situation.

My sister has been in City Creek Acute, Sacramento California for over two years. I especially want to thank my sister's Activities Assistant Joellen. I couldn't ask for anyone more caring and attentive with my sister. She also helps with any concerns we might have with the facility.

After my spinal surgeries, I was sent to City Creek post acute facility in Sacramento. I received much better care And a previous skilled nursing facility in Modesto. For the most part, the staff was excellent. I definitely questioned everything. I was my own advocate for my care which I appreciate it. I immediately wanted to introduce myself to the administrator. There was any issues that I could potentially have. I would take those concerns directly to him. The meeting with him was brief And somewhat rushed. Most of the nurses and certain CNA's were excellent. Pinky and Jessica thank you for being such Great caregivers. You made my stay really enjoyable. Rajwant I will miss you. You are the best CNA. I would've given City Creek a five star, but one CNA, who I unfortunately was stuck with, named Gracie was very disrespectful and needs a lot of work with her personal Skills. Come to find out she's only 18 years old. Which answered a lot of questions. I had. I recommend that she continue receiving monitored training. She was the worst Individual I encountered there. In my opinion, an 18-year-old-year-old does not have the compassion to treat people who are sick with dignity and with respect respect that they deserve. I would've really preferred giving the facility a five star however This one individual ruined it for the rest who did an exceptional job. Again, thank you so much to Pinky, Jessica and Rajwant. Thank you for helping me and my recovery. I will miss you all. You were the best. Eric

My name is Terry Rueck, my mom's name is Mary Johnson. She's been in your facility for the last 15 to 18 months. We are thankful for all the hard work that the nurses and assistance provide for her. I felt compelled to write this review because I'm so thankful for specially two of your staff, one is Joellen who is I think the activities Director. The other person is Reetish. I'm not sure if that's how you spell his name but I'm sure you know who he is. They are awesome and I really appreciate the love and care that they show towards my mom. They go out of their way to make her feel special and happy. Thank you for the staff that you have. And thank you for the care that your facility shows towards my mom.

I was transferred from south Sacramento Kaiser to your care facility after a major surgery , during the time I was over there, I did not receive the physical therapy as I expected until I had to asked a few times and received only one session. Luckily I was strong enough to walk around the facility with the help from my sister.

This place really surprised me with how wonderful the experience I had. I have been in another nursing facility at another time and it was awful. Needless to say I wasn't expecting happiness at this place. It was such a good experience and the staff was amazing! Everyone from the doctors to the nurses and CNA's were fantastic. Also the physical therapy and occupational therapists were amazing. I only had one care giver, and he was there on my normal care givers off day during the evening shift. He definitely looked miserable and definitely did not enjoy his job so he took it out on his patients. That's the only issue I had. The food is good and I had an amazing roommate who I look forward to having a friendship with. Thanks for the wonderful treatment I received as I was pretty much miserable after surgery on my right arm. I definitely recommend this place for anyone!

Staff was very nice and polite who had hands on with my dad. Place was clean. I only wished the staff would introduce themselves and what they do. Especially if u have never met them and u r new to the environment. Knowing the rules about meal time not being able to address their hygiene during that time frame. I wish they have gone over my dad's body from head to toes when he left, like they did when he arrived. He developed a pressure sore on his heel. Not sure if it started there or when he got home. We noticed it a week after he left city creek . Was not aware that they were using short and long term insulin on him. They kept saying a sliding scale which I did not understand. Only realized there were using 2 types of insulin as they gave us his drugs at checkout. Not happy and they knew it.I think they need to understand that the layman does not always understand all the info they are telling us. The nurses station always seem too busy.

I have cauda equina and my ability to walk is slowly but surely getting worse. But this facility is currently working with me. I have had 3 major spinal surgeries and I suffer with major chronic pain. But their staff has got to be very well trained they have a ton of patience and knowledge. Their PT staff including Jennifer, Lynn, Swan, Maria Lance to name a few are great. And their administration staff is very helpful their a gentleman by the name of Nick that is very friendly he interacts with the patience with sincerity and plays the piano for whomever wants to hear. And last but not least they are on top of their cleanliness. Which for me is very important.
