Best care staff of the four Bay Area post acute facilities we have long term experience with. Professional and attentive, empathetic and friendly. Details count: meals and housekeeping are done to a high standard. If I operated a post acute facility I would do it their way, if I had to choose one again I would pick Vineyard. I used to work for mental health so I have been in many facilities across the state. anyone that has been to one knows "the smell" this one does not. we have all heard horror stories, and while they may be possible here, that was not my experience. I have spent the last month here recovering from a wound and being bed bound for nearly 2 weeks. First experience was meeting Mark (the Mr. fix it) my bed was in need of modification as I am not a small guy. I would meet him again when I sat on my bed wrong and unplugged the airbag inside the mattress. The activities people are those that are perfect for those jobs. the just almost annoyingly happy people. they make events fun and inclusive. how dare people be happy around here, this is a care home. lol. The RN Aileen was just top notch. a nurse that stands out is Ester, there is another Ester in therapy that is pretty awesome too. Mark in PT (not to be confused with Mr. Fix it) was a lot of fun and a great story teller. sadly I am terrible with names, so I cant remember some other staff names, but many of them welcomed me upon my arrival. my family were very fortunate to be able to care for my grandmother up to the end, but if we hadn't been able to, I would have trusted this place with her. those that know me know that's a big deal. I am due to be released home in a few days, and will edit if experience changes. but for now, there is now one facility I trust for my family, and its here.

This review is long overdue. My brother (Alfonso) was at Vineyard Post Acute for approx. 3 months. If it wasn't for Tasha Stone taking interest in my brother that was at Specialty Hospital he would not be here today. Right away at VPA he started to receive the services and care that he needed to recover. PT, OT and speech started working with him right away and got him up and walking. I want to give a shout out to All of the office admin staff, Nurse's (Lourdes and many more), CNA's, PT (Caleb, Sebastian, Mark and many more), OT (Ester) and speech (Skylar). The nurses and CNA's always communicated with the family and were very professional and attentive to my brother’s needs, they would go beyond to create a safe place where he could heal. The facility was clean, maintained and very welcoming. Thank you for ALL you do!! Highly recommended!

I’ve been to a lot of SNFs, and Vineyard Post Acute genuinely stands out. From the moment you walk in, you can tell there’s a difference”it’s clean, organized, and yes it actually smells good. That alone says a lot. Beyond that, the facility runs with strong teamwork and structure. Every department plays its part, communication is solid, and there’s a clear sense of accountability. People show up, handle their responsibilities, and support each other when needed. It’s not easy to find a skilled nursing facility that feels both professional and well-maintained, but Vineyard Post Acute does a great job creating that environment! Highly recommended!

I am a 54 yr old amputee who has been here a couple of weeks and have about a month left to go, but I'll say this. In my 2 weeks here i have experienced excellent food, a great program of both things to do and therapy. And would be a liar if i said the overall staff that work here are anything but unbelievably pleasant, attentive, listen to and genuinely care about me personally I feel like. My great grandmother was at Vineyard a while back, and I wanted to take the time to say that they were amazing! The building was always clean and the staff always attentive and accommodating. The therapy team was also always on top of getting her up and moving and keeping her as strong as she could be, which was not an easy task. I was trying to contact a woman that had left Fort Bragg Hospital I called four of the facilities in Petaluma and got no response to speak of. When I called at Vineyard a woman named Tasha spent a great deal of time on the phone with me. She finally found me the place that I would probably be able to find her. I was so grateful to her and the amount of time she spent with me. She was very patient and polite at all times. This information was a particularly needed service because my friend has no relatives in the area. I am a friend from the senior center and we were having a very difficult titme locating her. Yay for Tasha. Five gold stars in her crown!

I had a great experience with this nursing home. The staff is caring, professional, and truly attentive to the residents’ needs. The facility is well maintained, clean, and welcoming. I especially want to recognize the maintenance director, Felipe. He is absolutely amazing”very professional, reliable, and always on time. Any maintenance issue is handled quickly and efficiently, and you can really tell he takes pride in his work. Vineyard Post Acute is exceptional! They do an outstanding job in their housekeeping department, which plays a vital role in keeping the facility clean and preventing the spread of bacteria. The rooms and building are always clean, and the facility remains free of unpleasant odors . Forever grateful to the team at Vineyard. When my grandmother was transferred there we didn't think she'd be able to go home again but the therapy team was absolutely amazing! In just a few weeks she was up and walking , we were able to take her home where she truly wanted to be. Again later my 82 year old father in law broke his him and I knew exactly were he needed to be. It was a hard recovery but the therapy team , CNA's and nurses were incredible. So much gratitude for these hardworking people who truly care about their patients. Thank you ™

My nephew has been here for about 3 years. I/ we feel that he receives good quality Care here.The staff and CNA are always nice.They always Greet us with a smile, always pleasant ask us who we are seeing. When my Nephew needs help or assistance.They come to his bed site and help him with his needs, or give him his meds. When he comes down ill they assist medical care for him. They let us know in regards to his health if something changes like If they cannot help him with his medical care they send him over to the hospital were he could get his quality medical attention taking care of. They give quarterly phone meetings to his family in regards to his care in his daily/ weekly, in his daily health, in his medical update at the facility. What's good for him and what's not going well for him.My nephew has great roommate that gets along together.

My husband has been at Vineyard Post Acute since December 2023. His experience has been great. He is treated SO well by all the great staff there from top to bottom, and he has had the BEST care by the Doctors & Nurses. He tells me the meals are always incredible! The administrative staff there has been great, and in particular Gabby has been BEYOND helpful to us. The physical therapy there is the best, and the staff in that department is off the charts! (pun intended) So far it has been a very positive experience for my husband Tom.

Came to VPA on a gurney and was here for 4-5 weeks and left walking. People are exceptional and i would like to mention Nurse Sara, nurse lourdes, Cna sol, cna keyleen, cna rudy, cna jimmy. All of these people are extremely polite and helpful and always went the extra mile for me. Special show out to Pt department because they were always their needed them and used stationary bike. Shahil admissions director and gabby from social services whenever i had a problem and they always helped out and i appreciate them very much. I was very apprehensive when i came here and didnt know what i was getting into and every single person above went above and beyond and did amazing to push through. Closing would i recommend this place 100%. No where is perfect but for acute care facility this is the best and better than petaluma valley hospital.

My mom was just discharged from Vineyard after 6 weeks of post acute care. We were very pleased with the care she received and the cleanliness of the facility. The staff were outstanding and very attentive to her needs. The front desk staff were very nice as well. We dealt with OT, PT, Nurses, CNA's and never once had a bad experience. Everyone was kind. We had family there everyday so we saw everything and have nothing but good things to say. I was only involved with the post acute care so that is what my review is for.

At 37 years old I had a major heart attack and have been in recovery for over a month. The first facility I was sent to after being released from the hospital was Vinyard Post Acute. It has been a fabulous experience, the staff is exceptional, working with nurses, PT, and OT has been nothing but a pleasure. The staff is very knowledgeable and willing to help. Being the youngest patient by many years I have felt like a VIP with my needs quickly being taken care of. My wound care nurse is a Godsend dressing and redressing my wounds which has helped to speed the healing time. The food is probably the best "hospital" food I've had so far in this crazy trip that has had me in 8 different facilities. The support staff is as amazing the nursing and therepy staff continually checking on me and updating me with what's going on. I would recommend this facility to anyone who is in need of rehabilitation therepy or going into a nursing home situation. I will be sad to leave all of the great people I've met here.

I have had the best experience ever! If you have to be in a skilled nursing facility, this is the place to be. The staff is exceptional, the rooms spacious, and the food is decent. There is a beautiful patio in back and plenty of room to sit back and heal. The physical therapy is grueling but without the wonderful therapists who definitely know their job I wouldn't be getting around independently as I do. All I can say is thank goodness for this facility.
