I am writing this review on behalf of my father, who is currently staying at this facility. He is 94 years old and has chosen to stay here instead of returning to his home. He uses a wheelchair and often tells me, "Who is going to take care of me like they do here?" The staff provides excellent care by helping him with meals, bathing, haircuts, nail care, and anything else he needs. They truly go above and beyond to make sure he is comfortable and well cared for. One of the things my father enjoys most is the activity room, where he spends most of his day. They offer a variety of engaging activities, including arts and crafts, bingo, exercise classes, movies, and even a candlelight dinner once a month. They also celebrate everyone's birthday! Knowing my father is safe and receiving such compassionate care gives me great peace of mind. I am grateful to all of the staff for everything they do. Thank you for your kindness and dedication!

The staff here is very kind and does their best to take care of their patients. My grandma has been here for a couple of years and everyone I have interacted with has been pleasant and thoughtful towards her. The nurses are definitely busy, but doing their best. The 4 stars are primarily for Wilma, who works in the evening. She was amazing, anything we needed, always in a good mood. Jason (or Jayson) was also helpful. Other good staff members, especially at the front desk when you enter (again, I mostly visited my relative in the evenings). What needs improvement is the physical therapy (and, to some degree speech therapy)...not enough of it (also understaffed), slow start with my relative, also slow start getting her out of bed and into the wheelchair daily. We ended up pulling out my relative (before the 100 days) because not enough physical therapy, not enough improvement. Her speech was also almost impossible to understand. A lot of that is her condition, but some of that is late start and insufficient speech therapy, in my opinion (I'm assuming that it's good speech therapy, since I didn't watch it). It was 3 to a room for my relative, not 2 to a room...except a few times when the middle bed was empty. I was surprised that the outside patio was not bigger, although it is nice. I was even more surprised that multiple times during day and night, it turns into a smoking patio for half an hour at a time. Kinda ridiculous to be encouraging smoking for people this old and kinda ridiculous to effectively exclude us from using the patio during those times...one time we had to leave the patio because it was time for the smokers to come outside. They should have a smaller separate space of their own, if that.

Recently had a family member stay at Guardian after a fall and hospital stay. They referred her their for rehab and additional IV antibiotics. The level of care and attention they gave her while there was tremendous, as well as everyone being so helpful to us for figuring out next steps. If you have anyone that is sent there or need a rehab facility, don't hesitate about Guardian. People like Abigail, Miki, Kathlene, and Min, just to name a few are top talents in their respective fields, I know I'm missing a lot of names and I do apologize as I couldn't remember all the names. But this place is absolutely the best with the best help available.

The rehabilitation hospital is welcoming and homely environment. Staff cared for my mum treat her with respect and patience. They are all super kind ,friendly and supportive staff. Mum felt safe and happy. The facilities are amazing. They always encourage mum to join the activities. First day today for my dad at that facility. Excellent first impression. The staff is super friendly and accommodating. They are all over my dad constantly. His every need is met. The director Alfred is fantastic. Very personable and attentive. He made us feel welcomed from the first moment we arrived. I hope it will be that way throughout my dad’s stay there. Abigail, the physical therapist, is amazing. She understands the physical rehabilitation is everything for the patient like my father. She is encouraging, positive, and very professional. Frankly I am impressed big time. Huge thank you to everyone who try to get my dad back on his feet.

My mom has been here for the 2nd time. We have been her over a month. The Administrator, Alfred greets me every day and asks how my mom is even though he knows how she is. The rehab is why we came here. It is exceptionally amazing. My mom is a difficult patient and they amazingly get her to do rehab 5 days with no issues. The rehab people are courteous even if my mom treats them poorly. They have a smile on their face and I feel bad when my mom sometimes yells at them. Now the nurses/CNA have a hear of gold. They are attentive, caring and compassionate. When I come they always have an answer to my question. The facility goes beyond the call of duty. Visiting hours are over at 8. They have never asked us to leave if we were her after. If I had to drop something off at 8:30. No issues. I would totally recommend this facility to anyone looking for a five star rehab/ nursing home. You will be very happy I promise. They cater to everyone’s needs.

Everything is fine except for the food which is atrocious. They serve you vomit food and the cooks come from prison kitchens. It terrible. Otherwise the staff is very friendly, the place very clean, and most medic nurses are very nice. They had a dinner special last night that was catered. The food was excellent. The only problem is the cooks.

The facility is nice and clean. The staff is pretty helpful when I'm there visiting my patient. My patient complains that sometimes the staff response is a bit delayed and the fact that there are 3 patients in the room and the noise level is sometimes uncomfortable but expected. My mom has been admitted at Guardian Rehabilitation Hospital after a stroke. The staff there are incredibly compassionate and attentive to my mom’s needs. My 88 year old father had a great experience with Guardian Rehabilitation Hospital. He was admitted there after a procedure at Cedars Sinai, and ended up staying there for 3 months. He received excellent care overall, but I must also be honest about the work that it took on the part of family (primarily myself) to get to the point of him receiving excellent care. When my dad was first admitted, it was a challenge for him as there was definitely a learning curve for the nurses, staff and caretakers as they learned his specific needs. He was not in a position to advocate for himself, so I went everyday as his advocate and spoke with the staff about his needs. After about a week or two of me advocating, the staff was fully up to date regarding my dad’s particular needs and he received excellent care from then on from everyone there. I can’t stress enough how critical the advocating is as part of the process. Every patient is different, and going into a rehabilitation facility, the staff doesn’t magically know a person’s specific needs outside of just the nursing care and PT. I strongly encourage that if your loved one isn’t in a position to advocate for themselves, then the family should take on the responsibility to advocate for them until the staff understands their needs. Otherwise, I can’t confidently say that the experience will be a good one. But if you put in the time and effort with the staff, they are wonderful. I wish I could remember some names, but truly so many staff stood out in going above and beyond to make sure my dad was taken care of. Further, the facility was wonderful regarding communicating with family members. Alfred, the administrator, could not have been more helpful and caring in seeing that my dad was comfortable and that the family was up to date with his progress. My opinion is that the negative reviews you may end up reading about Guardian are a result of the advocating issue I brought up above. If you or your loved one are there for only a few days, you can expect hiccups until they learn your needs and those days may be very difficult. Truthfully, that’s how I felt the first few days. But hang in there and give them a chance to learn your needs. This is truly a FANTASTIC facility with qualified, loving and compassionate staff and I wouldn’t hesitate to refer Guardian Rehab to any friend or loved one.

I appreciated staffs who help my parents, especially Mr Hsu ,Mr Hong and feeder Irene. They're very considerate , kind. Feeder, Irene has a lot of patient. My mom suffers a middle stage of dementia. My mom prefer Korean food. Guidance provide the delicious Korean food if you request. They clean the room properly. Our parents take a shower regularly. I am grateful to the staffs who cares patients like their own.

I am so grateful to our doctor who’ve recommended this facility. My dad has been to 2 skilled nursing facilities & none of w/c actually were. He got horrible experiences from those facilities. Before my father got discharged from the hospital, we were given a list of Skilled nursing facilities close to Cedars. I immediately saw their 5 star rating & have told the case manager that this is where my father would go. They really have caring & loving staff. When they started to allow visitors, my dad had bragged to me of how they kept his fingernails really clean. His beard was always shaved,, he gets a good haircut & he doesn't stink. He was groomed really well. My dad was at hospital for some respiratory issues that got him debilitated. They got the best rehab & nursing care. They have managed to get him back to his baseline & we were able to take him home safely. I would recommend this facility to anyone needing rehab. My dad only speaks Spanish & they have a lot of Spanish speaking staff.

I am absolutely shocked at any bad reviews for this place!! No facility is completely without faults, but if you have experienced what is out there... you will see,there is no comparison... it is truly head and shoulders above others. I just left Guardian after a four month stay. I was in a horrible car accident and after a month in two different hospitals, seven surgeries, and a brief stay at a different Rehabilitation center; I had the great fortune to get into this facility. When I came to Guardian, I literally could not move any part of my body (I had over 20 fractured and broken bones). I got amazing care while I was there, super therapy, and I have just walked out of the facility to the amazement of all of my doctors!! The staff is incredible! From housekeeping to the physical therapist, nurses, CMAs, and even the activities and business departments...You will not find a more caring, compassionate team! They truly care about the patient's comfort, health, and dignity. They became my family for four months and I love and miss them all. If you find yourself in need of a center for you or a loved one... I can say, hands down, from personal experience... you will not find a more caring or clean place.

As of January, of 2015, it's number three on the Medicare web sight behind the two CountryVillas. The interior has been improved over the years, but you have to stay on the staff. Their proximity to Cedars-Sinai Medical Center is their strong suit.
