Really loved how the staff took such genuine loving care of my momma. Nurse Danny and PT and OT staff were amazing!!! The only negative is that they were not timely with showers and it’s hard to reach someone after hours. We can deal with those small negatives the care and love is what is most important to folks who are in the process of healing physically emotionally and spiritually thank you!

Boulder Canyon Health and Rehabilitation provided such meaningful care for my dad during a very difficult time for our family. While the building itself is older, it has a comfortable, homey feel and is very clean. That made a big difference to us right away. My dad was 82, bed-bound, and in the end stages of Parkinson’s. The team treated him with so much respect and dignity, and the level of care he received truly meant everything to us. No one wants to be in the position of placing a loved one in a nursing home, and it can be overwhelming and emotional. We were grateful they accepted him and gave him a place where he felt comfortable during his final months. The staff made a lasting impact on all of us, especially on my dad’s last day. Nurse Daniel was an absolute gift to our family. He brought such a calm, kind presence into the room, and his voice and words lifted everyone’s spirits in a moment we will never forget. He is truly a special person. Nurse Debbie was also wonderful and took great care of my dad throughout his time there. For the four months my dad was at Boulder Canyon, he was cared for with compassion and kindness, and that means more than I can put into words. We are very thankful. *Shared room: My dad had a shared room, which we were nervous about, and had 2 different roommates while there. There is a 3/4 wall with a curtain that separates the rooms, so he never even saw either of his roommates. It was a very large room with a window that could be opened to get fresh air, which my dad appreciated. *Medicaid: he was on Medicaid, and we were glad they accepted him. *Visiting Hours: Another bonus is that they do not have dedicated visiting hours, so we could visit at any time. *My dad liked the meals, no problems

My grandma was placed in care elsewhere when her dementia was diagnosed. That place was so rundown, a/c was broken, the care was awful. Since she started here at Boulder Canyon every time I visit she has clean new sheets, she gained weight and they serve her meals that are to her care. She has her own room with bed and you can tell it’s a clean facility. Not to mention her nurses Taik I believe is how you spell her name in memory care, they make me feel better about how they take care of her.

My mother stayed here from rehab after her surgery, long term care and memory care unit. We really like the care that she received in rahab and memory unit, the staffs are very kind, and helpful. Especially I really appreciated the cares that she received at memory unit, the staffs are very very patient, since my mom doesn't understand the English, they tried to learn Mandarin in order to communicate with my mom, their kindness will be remembered for ever. But I really hope it can do the diet grading better. Since my mom had swallow issue, they started her with puree, upgrade to minced and moist, then regular, obviously my mom had issue with regular, and she lost weight( plus she is very skinny to start with), so I asked to downgrade, the answer that I got the next level is back to minced and moist, but I did find out that there were still two levels( soft bites and easy to chew) in between. My question is why they upgrade from minced and moist to regular(three levels up) instead of one level up at first place, and when I asked to downgrade, and they refused to do so. I had scheduled conference with social worker on Monday August 11, I was thinking to address this again during that time, since I failed to reach the Speech Therapist multiple times. But it was too late, and my mom passed away the day before that. I understand that upgrade the food level needs evaluation, but downgrade should be easy, but it is not the case in this facility. I really hope this change has been done before my mom passed away.

I have to say I can never thank them enough for helping my wife. She had a major stroke on the 4th of July 2023. When I had to choose a nursing rehabilitation center, I was greeted by Chrystal. She took 3 hours out her day just to answer my questions and listen to me and my concerns. Her compassion for her patients is second to none. Boulder Canyon has every therapist on site. 2 speech therapists on staff. That’s not the normal for other nursing homes. It is now June 5th 2024 and my wife is sitting right next to me at home. This is her 4th home visit with me. It hasn’t even been a year. They are miracle workers. My wife is working so hard to get home and so am I and amazingly; Boulder Canyon and Chrystal are working harder to help both my wife and I. I CA a never thank them enough. If I could I would rate them with 1,000,000 stars. They have helped me as well because it’s not easy on me to see the one you love in such a bad situation. They also helped me financially, with all the paperwork it was easy which gave me more time to be with my wife. Thank you Chrystal and everyone who helped me through this. Thank you Boulder Canyon Rehabilitation Center. Recently I had a heart attack and I was admitted to Boulder Canyon Rehabilitation Center. Louis is my daytime nurse and he is awesome. Always has time for anything I need. He is very patient and very helpful and knowledgeable. Amazingly friendly. Thank goodness Louis is here. He is amazing.

Needs that we would never have been able or even have known to provide are covered! Crystal makes sure that the women on this wing are personally cared for and feel valued! She treats these women as if each one of them is her mother and will show each one that they are important and involved! She and the nurse regularly keep us up to date and include us in any decisions to be made. Fantastic care!

Boulder Canyon living is wonderful! My beloved grandmother has been in the dementia wing for a few years. Every aspect of her well being is taken care of. She gets personal attention for everything from diet to therapy to activities to medical care-not just physical health, but emotional, too. The Life Engagement Coordinator is Crystal Caprio and she makes sure of it! There are games, dolls, movies, events and, OMG her holiday celebrations-amazing! She is a bright light keeps life going in that wing-and she loves these ladies with her heart! She also teams up with their nurse and dietitians, therapists and calls with updates! We can call her, of course! I want to let people know that Nana’s life at Boulder Canyon has taken away our worries and given our family the peace of mind that we need!

After making a difficult decision to move our loved one to the all female long term memory care side of this facility. I have been so impressed with the staff. From the moment I arrived (un-announced) I was greeted with a warm welcome at the front desk. and when I asked to see our loved ones case manager I was taken right to her office where we talked for about an hour! I was impressed with the helpfulness of every person I met along the way. I was encouraged to see all I chose to see. Her room was lovely and she has friends. Best of all our loved one has never looked more settled and happy. Her hair was styled nice and she was clean. I managed to give her a hug and she gave me a few beautiful smiles before I left.

My mom has been at Boulder Canyon for 9 months now. She took a terrible fall and broke her hip, fractured her pelvic bone and her collar bone, and had a traumatic brain injury. She was in the skilled nursing area for a little over a month and was very well cared for. The team was responsive, caring and eager to help my mom to walk again. Sadly, my mom was not as game for the hard work as they were. It was likely her TBI doing a number on her. She ended up in the Memory Care wing at Boulder Canyon and the love and concern for her health and well-being has been immeasurable. My mom and my family have been supported, held up, listened to, loved and cared for throughout this situation. I cannot say enough good things about this facility and the staff that has cared for us - and I say "us" because it's been a tough transition for all of us! There are too many names to shout but Chrystal, Gwen, Bernadette, Ina, Tika, Puspa, Nadia, all the CNAs and helpers in the memory care and skilled nursing wings, as well as all the PT, OT, and Speech Path staff, you have my endless gratitude for going above and beyond to help my mom and soothe my aching heart! Thank you for all you do!

excuse me people over the last month, i have wrote serval revies on this place, also i must say i am truely sorry, to have to say that most were druing when i was trying to get stable on meds. the staff and this place has delt with a lot of my ups and my downs and has got me back on track. trust me when i say its not as bad as i have stated in the past.in fact to tell you the truth, now that im, staybel on meds. i can say that this place has went above and beyond the call of there job just to put up with me. on friday dec. 1st i leave back for texas with the feeling that im better than when i came in. so come see for your self please. see what they are doing to make the place even better. the food is great the staff are great. take some time to really see how much they help people and judge for yourself,and not listen to someone that was angry please just take time to judge for your self. thank you for your time sincerly james m. bell

I just spent a month at this facility after having major surgery on my leg. I felt truly cared for. Yes, the building is old, but they're working on it, one small piece at a time. I'd love to see that happen more quickly, but what's more important is who's inside that old building. There is a team of physical, occupational, and speech/language therapists that are phenomenal. They work well together, and they truly care about their clients as individuals. They are knowledgeable, encouraging, and highly skilled, and they take their jobs seriously. When clients were unwilling to work, they adjusted the activities to make it feel more doable. I got PT and OT 5 days a week, and I joined an optional group therapy session daily. It was fun, and there were many of us who rarely missed it. There is a staff of nurses and CNAs who took great care of me as well. They used my name consistently, and they did that for others too. They often knew what we needed before we needed it. They were gentle and kind and treated me like a friend, not just a patient. My experience wasn't perfect, but I was very impressed at their willingness to take feedback seriously and make changes when needed (although not much was needed). At the end of the month, I felt ready and confident to go home and complete my healing, thanks to this topnotch team.

Look past the old building itself and recognize the wonderful staff inside! My mom went here for a few days for rehab and the transition, and was so quick and smooth. Chrystal, the memory care coordinator, was beyond kind and responsive to our transition needs. Tika, Cara and Gwen on staff really stood out as employees who love their jobs. I especially liked hearing that. Some of them have been there for years! I can’t speak long-term because she was only there a few days, but we felt so fortunate to have such great staff.

My mom moved into Boulder Canyon's memory care unit a year ago. The Memory Care coordinator, Chrystal, is highly trained in dementia care and one of the most wonderful people I've ever met. As soon as I met with her, I was completely convinced that this would be the perfect place for my mom. Boulder Canyon is an absolute RAY OF SUNSHINE compared to ALL of the other places that my mom lived. Staffing is very professional and compassionate and are very respectful of residents. Staffing ratios to residents are great. Their amazing staff includes on-site PT, OT and ST. The business office is onsite and very nice to work with. There is an on-site salon. Residents are kept busy with activities and entertainment. Meals are very tasty and can be modified as needed for the residents. The staff is highly skilled, motivated, caring and ALWAYS has a very positive can-do attitude and a beautiful team spirit. The staff continue to be very aware of my mom's changing needs and make adjustments as needed. They are excellent about communicating with me. My mom is safe, happy and well cared for. The management and staff are primarily what makes Boulder Canyon such a phenomenal place and a step above the rest. Highly recommend!!!

5 star plus experience I arrived scared, incontinent, unable to walk due to a fractured hip, my arm braced after shoulder replacement surgery. I was immediately embraced by this care team. A phenomenal family of occupational/physical/ speech therapists, nurses, patient care staff, dietary, kitchen staff, case management team, activities team, building maintenance, house keeping every single person played a role in my healing. Every single day. My healing came because we cried, laughed and worked hard at what needed to be done to see me walk out the doors with a cane, my arm on the mend I’ll continue therapy at home. My incontinence passed as I healed The staff cared for my personal needs, my heart needs and brought my body back I am so thankful and indebted to Boulder Canyons values, their care, the expertise I needed when I couldn’t care for myself As a retired nurse. I know and appreciate all they have done for my life. For the dignity they maintained throughout my stay I thank each and every person in this facility from the bottom of my heart

Selecting a care facility for a loved one is a challenging task! When our elderly mom need rehabilitation following a fall, we were faced with finding a good fit’ for her “ a combination of skilled nursing and therapy. We were fortunate to find such at Boulder Canyon Health and Rehabilitation. Though the building may appear a little tired around the edges’, the skilled staff was energetic, innovative, and committed. Patient amenities, activities and supports were plentiful and just what mom needed! From the moment we were greeted by the pleasant front desk staff, we felt a sense of relief that we had selected the right setting. The communication between the therapeutic, medical and support teams was evident in the coordination of the services mom received and staff interactions with family were informative and helpful. Thanks to the efforts of many (Ashlyn, Olivia, Kelsey, Beau, to name a few!) mom was able to move to an assisted living apartment and is becoming more independent every day! While we hope the need for rehabilitation is not in our future(!), we would not hesitate to work with the team at Boulder Canyon again “ Thanks so much!
