Greatly understaffed on all shifts. Much of the food was inedible. Employees seemed to care for the patients, but they were overworked and spread thin because there were never enough staff to cover the need.

I had my 93 year old brother at Avamere for about 3 weeks. His cognition is excellent but he lives alone and had 2 falls that injured muscle and soft tissue. The Staff seemed fairly good. He had a private room and the meals seemed to be good. However, when he needed something and pressed the nurse call button, there was seldom a punctual response. On one occasion, it took 40 minutes for someone to come to his room and the room was less than 20 feet from the nurse's station. If he had been having a heart attack or stroke, his chances of staying alive would have been slim. Staff also tends to gather in the halls and laugh and talk loudly; not what people who are in pain want to hear.

I was admitted here for rehab the end of March after I had half of my right leg amputated. I came here alone, scared and depressed. The staff here were so welcoming and kind. Everyone from the administration to the nurses and CNA's gave me the best of care for the 21 days I was here. The Occupational and the Physical therapists were the best. I was so impressed with the care I received that when I had a total hip replacement in August I asked to be sent back here. Thank you guys for all you have done!!

My first time in a Skilled Nursing Facility at Avamere of Beaverton. After surgery, I spent 4 days there to recover after my skiing accident and I was pleasantly surprised at how organized this place is and how upbeat a lot of people are! They have their ducks in a row. Mia the director always greets you with a warm hello, LadyByrd came by to chat to make me feel welcomed, Barbara - getting everything that everyone needs at the speed of light, Melony and Robert - making sure everyone has their meds and offering to bring me coffee :b , Este - who gave me a shower after not having one for over a month! Rica who is always going above and beyond with a great attitude to extend a helping hand. Spanish mama who gave me extra protein sausages when I needed it =). PT, Brice for his kindness and wealth of knowledge and Dr. Julien for making it happen to discharge me =). And to all the CNA and nurses that helped me 32A, you have been wonderful and the cooks that have cooked all the delicious meals.Thank you all!

My Mom stayed there for 24 days. She's doing a lot better now and that's due to ARB. Let me get this out of the way. Yes, the outside looks ok, Yes it's hard to find parking out front during the day so park in the back, Yes some of the units are a bit rough/needs updating and Yes it's the staff on the inside that makes you overlook the insignificant things mentioned above. Some input from someone that didn't know much about skilled nursing as I had a biased -- all skilled nursing facilities have fewer nurses to patients by design. It's presumed your loved ones is discharged by a doctor to go here. However, what most people overlook is the ratio of PT/OT to patients -- which is MUCH higher and this is why you send your loved ones here. To set the best expectation for your loved one -- someone from your family/friend NEEDS to be their advocate at ANY skilled nursing facility. If possible they need be present as much as possible. This will make an impact to their recovery. I was fortunate to visit everyday and got to see how a skilled nursing facility operates and see their strengths and weaknesses. I tried to tried to help on the weaknesses if possible. Here's my Mom's experience. She was discharged from the hospital where she stayed over 30+ days. We did an early evening admission when the day crew was done for the day (i,.e., try to do a morning admission if possible). There is fewer staff and I had some communication challenges with the admission nurse. I was a bit worried for my Mom. However, all of my reservations were addressed as I came back the next morning to find out that PT had already stopped by and had my Mom out of bed walking. The day crew (where most of the core work is done) is on top of it. Where ARB stands out is the hospital is short staffed for PT/OT that only worked with her once every 3-4 days. So it was great that she got PT the next morning! Again I was fortunate to visit everyday. In-person communication is how you'll get the best updates and trying to get updates via the phone is challenging for everyone (i.e., loved ones and I see how busy the nurses are here). Here's why this was key to my Mom's discharge. From the get go I worked with the nursing care manager (Jon Starlight) to build a plan of what we needed from ARB for my Mom. His ability to communicate (i.e., prompt/pro-active) is what made the difference in keeping my Mom there. I will never forget Jon hand delivered my Mom's medicine because the nurse was busy! He does the things that get overlooked by the patients and probably management. They are not perfect. There's new staff each shift and you must understand how challenging it can be asking each person to know exactly what your loved one needs. Again, this is not the hospital that's where you can help. Did they have long call button wait times -- yes but that's where you come in and if urgent track down the nurse or ask someone for help. They take the patent's vitals at a minimum every four hours and when you get back from a doctor appointment. No one will turn you down if you have a question or need help. Everyday the plan was executed and the nurses/PT/OT/helpers were great to work with. They really want to see your loved ones get better. This is my way of giving back... Did you work with the care manager to contribute to the discharge plan? Did your loved one get a roommate? It might be good if they prefer the social interaction. Or just ask for a private room. It's not bad if ARB does something different vs. the hospital. Just ask them if you have questions. Don't presume they don't know what they are doing. Does your love one dislike the food? It's actually not that bad and better than the hospital. They have alternative options on the menu AND ask them if you can bring in food for them. Nutrition IMO is the catalyst in getting them better. You'll be surprised how much quicker someone will get better if you can do PT/OT with them in addition to their PT/OT times. Keep in mind, PT/OT doesn't happen on the weekend.

Avamere Rehab of Beaverton, OR My 93 year old mother was sent to Avamere Rehab of Beaverton, OR from Legacy Good Samaritan hospital following surgery from falling and breaking her hip. It was the closest "desirable" rehab facility to their home in Scappoose, OR. Forcing my 93 year old father to drive over 40 minutes each way to visit his wife. If this was the most desirable rehab facility I'd hate to see what the others are like. While the care appeared to be okay, it was like pulling teeth to get any detailed information from any of the staff. We had to basically figure out everything on our own. I repeatedly told them that my mother had dementia and that she would forget anything they told her in 30 seconds. Also, that my father is hard of hearing and doesn't always catch everything. So please write everything down and put in her room or email important details to us. Of course that never happened. We were never given the "welcome" packet as they said we should have and promised to provide multiple times but we never received it. Simple questions were: What are meal times? Can we bring her food? What clothing should we bring for her? How is laundry handled? What are the hours for her physical therapy? What are the doctors names and contact info? What are visiting hours? This basic info should have been readily available without having to beg for it. After repeated requests and angry emails we finally received some info, but progress reports were never sent although we asked for them to be provided twice a week. It took way too long for the nurses to respond to call lights. She was supposed to have physical therapy one day at 11:55 and there was a big sign on her door to make sure she had her medication 30 minutes prior to PT. My father and I were there and no meds still by 11:40. I had to track down the meds person and see that she received her pain meds immediately. If we had not been there not sure she would have even gotten them. When I pressed them on getting the doctors contact info in an email they said they were not sure they could provide that. WHAT?! That's absurd! Ultimately, I found it with a simple internet search and I was able to talk to the doctor directly. I have never heard of a medical facility not providing an attending physicians contact info to a patient's direct family members who were authorized to receive that info under HIPAA laws. Also, the parking there is abysmal, not enough paved parking requiring parking on uneven gravel/dirt surfaces. And, the sidewalks into the facility are broken and uneven. Both of these result in a serious trip hazard for visitors, most of which are elderly. I see a lawsuit coming if someone falls like my father almost did. The only good thing my father had to say about it was he tasted some of her meals than thought the food was good. I don't expect perfection, but when you ask a simple question you should be able to get an answer without having to beat someone over the head for it. COME ON AVAMERE you can do a better job than this since this is a "for profit" facility. Make some simple changes.

Avamere Beaverton took excellent care of my husband. When he got there he could barely sit up in bed and within 2 weeks he was up and walking. The food was amazing. The staff was very caring and attentive. I was so thankful to everyone there for how safe they made me feel leaving my husband in their care.

I couldn't be more proud of the work our CNA's and Nurses do every day to make sure our patients are safe and well cared for. Many patients or family members are quick to judge on the time of transition from hospital to our SNF, but I hope every person that enters our doors knows that the people working every day are dedicated to doing the right thing day in and day out. If your goal is to make sure your loved one receives the right care at the right time, then I would entrust any of my own loved ones in the warmth, compassion, and camaraderie I witness every day. We are here to make sure your loved one gets to where they need to be and we are ready to work with you to make that happen. Avamere Beaverton has one of the lowest turnover rates in this avenue of medical care. I truly believe that is due to our culture of diversity and respect. We embrace nuance, change, and offer many perspectives and walks of life to accommodate your loved ones with the priority of safety and quality. If you are not OK with different accents, appearances and backgrounds; then our location may not be for you. We have fantastic rehabilitation success rates. I do not recognize the names of most if not all negative reviews. Most positive experiences don't leave reviews and that's fine. We're here to be genuine and get the work done to make sure your loved one gets the most out of their care. THANK YOU to our CNAs , Nurses , Med techs, housekeepers, therapists, dietary staff, social workers, physicians and office staff. This work is impossible without you. Thank you for entrusting us to care for your loved one. This is what we do!
