The staff is very helpful, attentive and supportive of residence needs and very easy to communicate with. Their admissions coordinator is very effective and will guide you through the admissions process whether it’s private pay or Medicaid pending. The director of nursing is wonderful and has many years of experience in long-term care which is very reassuring. The social worker has been so helpful when it comes to discharge planning or care plan meetings that we have had. The memory care unit staff are amazing. We all love, Miss Patty. I would definitely recommend this building whether it’s for long-term care or skilled nursing rehab rehabilitation. It’s conveniently located in the medical center, which makes it easily accessible

My mom has been a resident in this facility for a year and a half after being in an unacceptable facility. Some things to remember when evaluating a facility: Are you concerning yourself with facility visual aesthetics or care? Smells happen. Are they resolved? Is your loved one clean and well groomed? Are there visible signs of mistreatment? Is the nursing and physician staff cooperative? Does the staff turnover produce improved results? Do you visit often? If your loved one is in Skilled nursing care, you have exhausted other options and life will be changing for your loved one and your family. No facility will be perfect, nor will they do things exactly as you would do them. What you are looking for is dignity for your loved one, quality care, ease of dealing with staff and problem resolution. I have been impressed by the level of attention and quality care my mother has received at this facility. They understand it's not just the resident who is affected by the situation but the family, as well. Patty is a god-send! Hopee is a god-send! The janitor... the food servers... the business staff, Maria... they all work hard to make this place respectful and positive for a difficult situation. I can't offer enough thanks to the wonderful staff who has helped our family transition through this difficult time. I could not do the jobs they do and appreciate each and every one of them and their can-do demeanors. "Thank you for the hand holds, patience and understanding. Thank you for walking with us through this valley, staying with us through the challenges and uncertainty and allowing us the grace to make peace with the finality of the reality." It's been difficult, but you ALL have made the difference. Words don't do justice, but I thank you, nonetheless.

I visited my aunt at Wurzbach Nursing and Rehab this past weekend and had mixed emotions. On one hand, I want to give a huge shout out to the nursing department for really upping their game with resident care. The gentleman handling the hydration system is doing an excellent job; my aunt used to get frequent UTI;s but this new focus has clearly made a massive difference for her health. However, it was still heartbreaking to see her just sitting in front of the nursing station with no engagement or activity. My mother confirmed that while weekdays are a bit better, the weekends have almost nothing planned. I kindly ask admin to please consider adding more engagement activities during the weekend, so residents have something to look forward to besides the church services. The medical care is improving; we just want to see the social engagement meet the same standard.

Have been visiting my family member and administration is doing a great job in the dementia unit from the manager Diana to the nurse (Paty and Rosie) an the wonderful cnas that show their empathy with the pts .. Luis , Maria, Raul, Nanette. Stephanie to name a few. Saw part of the activities and the gentleman with the patients was awesome.. vicente is his name. I asked because I had never seen any one interact with patients like that before.

I attended the February 27 birthday celebration and was extremely disappointed in the event lacked energy excitement and meaningful engagement despite of having a DJ residents looked bored and there was no effort from staff to encourage participation. The Activity Director remained in her office on the computer during the party, and administration showed no involvement. This is unacceptable especially when my family had been told that resident engagement had recently improved. The absence of the CNA Vicente-who usually brings life and joy to these events- was noticeable. This celebration felt rushed, poorly planned and emotionally flat. These residents deserved better.For some, this may be their last birthday, and the lack of care and effort was heartbreaking. I strongly urge the facility to improve event planning and staff engagement.

It is truly heartwarming to witness the remarkable transformation at Wurzbach Nursing Home, where a newfound sense of stability and genuine compassion has replaced the use of outside agencies. Seeing dedicated professionals like Nurse Denise, Santiago Alma and to name a few pour their hearts into their work makes all the difference in the world. I was particularly moved by the change in the Dementia unit; watching Vincente Razo engage and entertain the residents with such kindness was a moment that truly melted my heart. This beautiful environment is a testament to the hard work of unit managers Diana, Emily and CeCi, and I am so grateful to the administration for listening to our families and creating a place that feels like home.

I traveled from Eagle Pass to visit my grandmother at Wurzbach Nursing and Rehab, where she is currently in the Dementia unit. To my surprise, they were celebrating Valentine's Day ” and in the eight years my grandmother has been there, 1 have never seen anything like the celebration they hosted this year. The decorations, the beautiful setup, and even a live Banda playing made the entire event unforgettable. Watching my grandmother sing along to her favorite songs absolutely melted my heart. I haven't seen her that happy in years. The staff involvement was truly unbelievable. Administration went above and beyond ” from the Social Worker (I believe her name is Hope) to the ADON Diana, and even the Therapy Department. The teamwork and joy in the room were something I had never witnessed there before. When I asked my mother how long things had been like this, she told me that ever since some staff changes were made and CNA Vicente was assigned to entertain and engage the residents, the atmosphere has changed completely! He is doing an excellent job. Thank you, Wurzbach Nursing and Rehab, for listening to families' concerns and providing more meaningful activities for our loved ones. These moments matter ” and for some, this might be the last holiday celebration they get to enjoy. I am truly grateful for the memories created that day.

Our mom is a resident in memory care. As a family member of someone with Dementia it is difficult to navigate the daily challenges this disease brings. You only hope to give your loved one the best quality of life and enjoyable days. The Nurses and CNA's that help care for our Mom daily are exceptional. My brother is coming from another SNF and the name I will not say but I was very disappointed with his care there. My brother has barely been at Wurzbach for almost a week and in this week I have seen such a positive change in him. The Admission was smooth and all the staff from Admissions to Therapy dept., wound care, nursing, CNAs, housekeeping not to forget the BOM,DON,SW and Administrator! Everyone has gotten involved in his care and he has received exceptional care. The facility is excellent in communicating with me, they are all extremely compassionate, dedicated, knowledgeable. I just can't Thank everyone there enough in the short time my brother has been there! I am so Blessed and greatful to be able to say that my family and I are very fortunate to have found a " home like" atmosphere for my brother to finally call HOME!!!*****

If there was a way to rate this a 6 star facility, I would. The staff is led by a wonderful administrator who everyday commits to providing exceptional service. Clinically they are led by an equally amazing DON. The Tx nurse knows exactly what she is doing and how to care for the patients that she has in her care. The needs for the patients are always put first before the ones caring for them. I can only hope that other facilities are led by their exceptional example that this facility displays on a daily basis.

My sister was a resident at this facility for over 3 years. When admitted, she was in a very dark place as a result partly by medication mismanagement at another nursing home. However, the compassion and patience displayed by their staff, especially the professional care and kindness displayed by Patty Tullos, LVN helped my sister recover and successfully transition to her son’s home. This facility bar far hands down was amazing to my loved one. The care, the compassion, and the integrity they showed and displayed was amazing. The nursing staff was very attentive to all our needs and the rehab team was supportive and made sure our loved one returned their prior level of function to be able to return back home to us. Last but not least the administrator was always available and attentive to our needs as well. I recommend this facility to anyone looking to get great customer service and have your loved one taken care of diligently.

Our mother has dementia, and has been at WNR for over 2 years, and I am comforted that she is safe, and receives excellent personal care. Nurse Patti calls to update me on any issues or concerns. The staff helps her when she gets confused and upset, rarely having to call me on these occasions. I attend/dial in to her quarterly resident reviews and the staff always welcomes my questions and concerns. I try to visit my mother weekly, Nurse Patti always updates me when I get there, and things are always in a good way when i show up. I have seen the staff go out of their way to communicate, play, and have fun with my mother. I really don't think I could ask for anything more. Thank you WNR staff.

This place and it's staff are a blessing and I'm not just saying that just to say it, before I was able to get my mother admitted to wurzbach Nursing home I had looked everywhere and I could not find a place that was nice enough,safe enough and professional enough for my moms standards. My mom has always had financial hardship in her life and that's just part of the reason why she couldn't find a ten Star nursing home. I was driving down wurzbach Rd thinking Lord how in the world can I continue taking care of my mom when I my self am in my sixty's and a disabled Veteran. And right then I glanced to my left and I saw the Golden arches! NO ! Not Mac Donald's, wurzbach Nursing and rehabilitation. This is a good place with good people for my family member. That's all.....

This is an amazing place. My wife has been here ten years. Since Tammy the new DON has come I have seen lots of good changes. CNAs are working more than they used to and a lot of the bad attitudes are different. My wife is getting taken care of and being tended to. She evens gets out on the floor and will care for my wife too. She leads by example and I see her with the residents and family members dancing and having fun. I thank everyone there. My grandfather was there for some time in hospice care. I have got to say coming from California, The skilled nursing care here is top-notch and incredible. Patty was my grandpas nurse and she was impeccable from beginning to end, the nurses here, the medication aids, down to the nursing aides Christina and Louis were amazing!!! I highly recommend this place, and even though on the outside this place looks old, and rundown, please do not judge a book by its cover because when you walk inside” 1. It doesnt smell unlike California SNFs 2. Its clean and presentable unlike California SNFs 3. THE STAFF IS INCREDIBLE! They truly care and show compassion towards their patients. We will always be grateful for the care our special needs family member received at Wurzbach Nursing and Rehabilitation. I remember the day when I was desperately trying to find a place for my brother, Jay. He was being discharged from the hospital and could not go back to the group home he had previous resided in. Mr. Gonzalez was the first voice I heard on the other end of the phone. His calm voice assured me there was a place for my brother. Jay was a special needs person living with autism and cerebral palsy (and a whole host of other labels). Jay had limited verbal and mobility abilities. The whole staff including nurses, administration, specialists, and support staff treated him with respect and dignity. They interacted with him and cared for not only his physical needs but his social and emotional needs. Jay was happy there. Before Wurzbach, Jay had been in a group home setting and it often was a battle for us to get his needs met. But at Wurzbach, he received the best care in his last year of life. We appreciate all that they did for him, and for my 95-year-old mother who frequently visited. When she could not, she called almost every day and the nurses were so kind to bring their phones to Jay so he could talk to "Mama". Rebecca from Vitas Hospice would often face-time with my mother so Jay and mother could see each other. I would highly recommend Wurzbach Nursing and Rehabilitation to anyone who needs special care for their loved ones.

I have my brother Herbert in Wurzback Nursing and Rehab on Wurrzbach Rd. Herbert has been there since 2009. He has dementia and other problems. The staff their have always been helpful. The nurses and aids have always taken good care of my brother Herbert. They will call me if there's any change and let me know.

My mom has been a resident at this facility for 2 years. She is well cared for by a very attentive and caring staff. There are daily events such as exercise, bingo, trivia and especially music. For every holiday, birthday, season, the Wurzbach center provides some wonderful events. Before COVID-19, the families were also invited. Since COVID 19 began, the staff has taken every precaution necessary and still made sure we could communicate with the residents: by telephone, FaceTime, and outside the window. My mom is treated like family and given personalized care; they even braid her hair. What matters most is that my mom is happy!
