Some of the nurse aides and med aides were not allowing the residents to have choices. If the residents didn't like the food offered for the meal, usually the residents were told that was the only food available. If residents who needed assistance wanted to go to the restroom , they were told ok, give me a few minutes, then as I witnessed the residents would be waiting for a long time and then have an accident in their pants. State inspection was there during one of my days I was there. I reported to the state officials things that I saw not being done … and things like the aides turning their batteries in their beepers backwards so they didn't have to have their beepers buzz with the residents. I was treated very rudely by an aide named Ashley. Dark haired girl who caused a lot of friction among the workers. I watched her as she was setting up the meds for residents. Ashley was busy talking with other aides and laughing, not paying close attention to the med lists and what meds she was giving the residents. I asked Ashley what one of the pills in my container was? Ashley said,once she takes it out of the bubble ’ wrap she doesn't know what that medicine or pill ’’ is that she is giving the residents. I told her to look at the pill, check the cart for the residents name on the bubble ’ wrap and trace back the name of the medicine. Ashley was also reporting that the resident refused their meds but actually putting the meds in her pocket +stealing them). Some residents have pain pills and some have mind altering meds family members could see from the residents actions that the meds hadn't been given.

My mom fell and fractured her femur on June 29, 2025. She had surgery to repair her leg on June 30th and ultimately suffered complications from infection that kept her hospitalized for 177 days. She was transferred to The Neighborhood for rehabilitation on December 24th, after being completely bedridden for the biggest part of her time in the hospital. The idea of placing my mom in a Skilled Care Facility scared me to death. I had done my research and even visited before my mom was transferred on Christmas Eve. From the minute she arrived to the Flat Branch wing, we were impressed by the warm, homey atmosphere of the unit. It was clean and all decorated for Christmas. We were amazed at just how friendly all of the staff was. We never passed a staff member in the hall, whether it be administrators, nurses, housekeepers or bus drivers without them speaking to us. My mom seemed to settle right in, getting to know other residents and participating in the activities that they provided. Listening to her talk about enjoying meal times with the ladies made me smile. Physical and Occupational Therapy began working with her right away. They were the most supportive and encouraging group to be around. Over the course of her 95 day stay, Rebecca and the team took her from being bedridden to walking over 300 feet. She was able to work on activities of daily living that she had been unable to perform for months. Mom’s Care Team met with us to keep me updated on their plans and her progress. The Nurse Practitioner kept close tabs on mom and was always willing to talk to me if I had questions or concerns. They provided transportation for mom the all of her appointments and the drivers were so kind and compassionate. We were able to see the unit transformed with decorations from Christmas to Valentine’s Day to St Patrick’s Day and finally Easter while we were there. And might I add that there was a party to go along with all of those holidays. Susan takes such pride in keeping it festive for the residents. The Neighborhood changed my preconceived ideas of what Skilled Care looked like and I am forever grateful for all that they did for my mom.

My dad was transferred here from the hospital a couple of weeks ago. Facility is clean and up to date. Staff were friendly and caring. My dad stayed here for 2 weeks and it was also nice having him 20 minutes closer to me. He was saying bye to the staff when I was taking him home, but excited to get back home. Thanks for taking good care of my dad. It means a lot!!

I was there for testing. Short staffed only one aide on floor and a med tech. From what I could see they was busting there butts to get their jobs done. Very clean positive environment besides short staffed. I say keep it up team

I am Sandra Meyer, beloved the Neighborhood for my husband. Susan the med tech is awesome. She genuinely cared about my husband, and me. All the workers were great asset to the caring of the elderly. At first meet and walk through several years ago we were impressed with the place. And the nurses that are tasked of taking care of my step-dad are really nice and I have met a couple not so much. But they understaffed so I'm told and if so they need to hire some more. As for the cooks on his floor they are ok but my step-dad is picky and they have been told many times of what he does and doesn't eat but at times they give him food he doesn't eat. As for the laundry he for most part doesn't get his close back and he gets someone else close and can be to small or to big and he has his name on his close and still can't get them back. I even talked to management on his floor about that and still nothing has been done about it.

My mom was admitted in the neighborhood senior living center three days ago and even the transportation person (jazz) was sweet,kind , informative , friendly and helpful and upon arriving I’ve since then have learned that all employees from the kitchen staff through out the facility share the same goal of caring for the patients and making their families feel confident that their loved ones are in good hands thank you neighborhood staff

I have had a most enjoyable experience here at the Neighborhood. The staff was always very accommodating, beyond friendly and extremely welcoming. I feel like family to all of them!!!! All the way from the housekeeping to Dietary, nurses, CEOs and our volunteers and drivers all loving and welcoming!!!! Thank you very much!!!! I highly recommend this place to everyone!!!!

I would leave 0 stars if I could. My mom was there less than 24 hrs. and the care was very neglectful as far as we are concerned! I can not in any way recommend this facility.

Facilty is clean. There are meal options. Staff seems friendly. I have heard the quality of physical therapy is good. A friend is staying there and I have visited many times. I was involved in their discharge planning which was thoughtful and appropriate.

I have loved one getting rehab, poor quality of care short staffed. Staff that are there have no experience, not being trained correctly on their 1st day and the RN has no clue why she has a abrasion on her forehead and bruising besides her chin area. I spoke with 2 different RN in the area and they had no clue on her treatment plan. I would not recommend The Neighborhoods to anyone.

Most of the nursing staff is caring but there are severe issues with the level of care. I was supposed to have a Wound Vac on an open wound. It took 6 days for staff to get the wound vac on. Then, the dr. ordered it changed 3x wk. I went from Wednesday to Monday without a dressing change. The food was mediocre at best. Inedible at worst. The PT and OT were very caring and did the best they could given my physical limitations. I can not recommend this place to anyone.

Stay out of this place if you want to heal. My loved ones meds were missed for 5 days. All they did was make excuses. Terrible sanitary issues. My loved one spent a week and a half here. The only reason they got the second star was because of the CNAs who tried to be friendly and helpful. I would love to see the results of a health inspection here. The administrator Tina has zero customer service, I was told by staff she does want she wants to, very agressive. I came to get a tour and I was told the Clinical Liaison Jessica was not available so I just waited and left. The front desk girl had no clue and didn’t get us help. This facility was recommended from family friends that they had exceptional stay 2 years ago. (Lastly, it seems several employees have left favorable reviews. I am curious to know if they had a loved one receiving care there.)

After having experienced an inferior level of care for my father at another establishment, it is easy to see why the neighborhoods is so highly rated. All the staff take care of their patients as if they were their own family. The facility is clean, the food is excellent, and above all the care for patients is clearly visible.

Everyone at Mill Creek Village has been so helpful and patient as my parents got settled. Moving into assisted living has been a real change for them and the entire team has been supportive as they adjusted. I am really grateful for such a caring and responsive staff.

Unfortunately, I don't have good feelings about this location. The aides sit around a table in a great room basically ignoring the residents, there are no mentally stimulating activities, the residents have to sit in the same positions for hours on end in a large room with a TV blaring...it's a sad situation. Also, the food is miserable. I don't even think they have a dedicated cook...whoever has time throws together food, then cooks it to mush. For the cost, it should be a resort!

They are very clean and the residents always have clean clothes on but very short of help at nights and weekends. Also had to ask 3 times for a new mattress for my Dad because his had a hole in the center. I wish they wouldn’t alway my father to refuse his bath as well because he goes long periods time not getting a bath but it is on him for refusing. Meals seem to be on the most part good.

We have had a great experience here. From the moment I walked in to ask questions about transferring my mom to them, helping with insurance questions, and the enormous help with my mom’s transfer. My mother is improving quickly, and is very happy with the staff, her therapy and her care level! We are thankful!

I cannot say enough good things about The Neighborhoods by Tiger Place! Our experience with placing our mother in this facility has been wonderful! Entrusting the care of a vulnerable elderly loved one to a long-term care facility requires trust and is a stressful time for families. From the moment my mother arrived, Sihreta and the staff worked hard to make her (and us) feel comfortable and at ease. A few examples of this were allowing us to bring in items from her home and make it her room, asking for her preferences of foods, waking times, etc. and allowing family pets to visit. The staff has been professional, caring and compassionate which has assisted mom to acclimate very quickly to her new environment. This type of personal attention has made me and my family rest much easier each time we leave. Every person we have encountered at this facility, no matter the time of day or evening has taken the time for us and my mother and went the extra mile in assisting us. I am a health care professional myself and I have been impressed with the care my mother is receiving at The Neighborhoods by Tiger Place.

Both of my parents moved into the Neighborhoods this week. The staff has been very helpful and attentive to them both! Sihreta has been very quick to respond to any questions I have. The nurses work hard and I appreciate their attitudes which goes along way especially these days! I would definitely recommend The Neighborhoods!

My father has been in this facility for a while. Their team is caring, kind, and really work very hard to make patients comfortable. They have a strong therapy department and the physical therapists are very motivating. The cooking staff work hard to accommodate special dietary needs. The social work staff are very helpful as well. The facility is always very clean and is well decorated. I have worked in Healthcare for 20 years and this is by far the very best SNF/ICF I have seen. Nursing staff work very hard to resolve any concerns even when they are do short staffed due to COVID.

My husband has been at The Neighborhoods for two months. What a wonderful place. I’ve been to about 15 nursing homes in my lifetime visiting relatives or friends, but The Neighborhoods by far, is the nicest, most professional and patient centered, that I’ve ever visited. The staff, make sure the residents needs are-not only met but exceeded. It’s a facility is less than 10 years old so the accommodations are superior to others in this region that show their age in the furnishings and finishes. Shereta, their social worker has gone above and beyond eliminating obstacles to anything needed on my husbands behalf. She has helped is work through the turbulent days waters of transitioning. So thankful for her! From the start, Amy their director was responsive, and engaged. She took care of concerns or worries and made the difficult move seamless.They do everything to make the residents feel respected, secure and that the staff cares about them. Central MO is so blessed to have such a great option for loved ones that need extra care and help. So thankful for them. They treat residents as if they are family. That’s so important!

While this has been an excruciatingly difficult time for my family, I have nothing to say but good things about the Neighborhood. This facility has been wonderful for my dad rehabilitating and recuperating from a stroke. The staff has been helpful, kind, and very understanding. My dad raves about the nurses and has even enjoyed the food. Physical therapy here is top notch. The administration has also been so very receptive to any questions and has addressed every concern. As an added bonus, the view in my dads room is a wooded forest and very pretty. And I personally want to thank Sihreta and Colleen for being so very patient, helpful, and understanding with my family during this very stressful time.

This facility has had a easy process since the transition of my admission from the hospital with Sihreta Clincal Liaison. The food has been good. Great therapists Steve, and Carren. The nursing staff has taken good care of me. If I needed rehab again I would come back here again.

I cannot imagine any skilled nursing community receiving 5 stars due mainly to the stress the staff is under partly due to the number of patients needing a higher level of care than normal due to the isolation caused by COVID. While I have had times where I have felt the facility was short staffed (hence the 3 star rating), the administration has been extremely open to hearing my concerns and working tirelessly to address my mom's needs. I have sincerely appreciated the patience and responsiveness of Sihreta and Colleen in the front office, and my mom raves about the PT and Speech Therapy team. I am working more closely with Suzanne on the nursing side now, and I appreciate her willingness to seek appropriate accommodations when feasible. These situations are never easy for any family, but having a team that is willing to talk through difficult situations is very helpful.

After removal of a 3.2 inch brain tumor, and several setbacks at the hospital, my mother came to The Neighborhoods by Tigerplace, the weekend before Christmas. Fortunately, Unfortunately, we got the only room left at the time and the "Neighborhood' where she was placed, was not equipped to deal with a brain trauma patient (in my opinion). They seemed to be lacking understanding of the patient's life after a brain issue. We had a few mishaps there and ended back at the hospital til Mid-January, due to some infections and pneumonia. I spoke with Carrie, the Nursing Home Administrator, about some issues we had and she was very quick to assist, and address the situation. I then spoke to another employee in charge of admissions, about moving my mother to a different neighborhood. THANKFULLY, my mother was moved to the "Rehab" area section. The staff has been remarkable!!! I cannot say enough good things about the Charge Nurse in her area now and the Nurses and CNA's that work with my mom. And the Therapists, Physical, Speech and Occupational..... Kristen, Christina, Karyn and Jason you are Top of the Line. The staff has been so patient, caring and understanding with my many and constant questions. My mother came to the facility unable to use her extremities. Here we are after 30ish days of therapy (since her return from the hospital) and my mother is using some of her extremities now. Yes, it is a slow process but patience is the key when dealing with a brain trauma patient. And to see the therapists and staff get as excited about new progress makes my heart happy!! We have made some great friends here after getting off to such a rocky start. The food, according to my mom is substandard. However, my mother is an amazing cook, so it is going to be. It is probably a lot healthier than my mother's cooking. The facility is VERY CLEAN. The building is in a great location. The only issue with the facility is the PARKING IS HORRENDOUS!!!! I speak to the nurse everyday I am there about her concerns / my concerns if there are any and she ALWAYS updates me with a Smile. Rhonda and Jackie, You ROCK!! No one wants to put their loved one in a facility and this was a very hard choice for our family, but one I am now happy with. Yes, like all facilities, they are short staffed....especially on the weekends. WE too have struggled with call light issues, but it has gotten somewhat better. Weekend staffing could still use some work. Communication is key here. If you have concerns, please address them with the nurse or Nursing Home Administrator. It cannot be fixed if they do not know about it. And another bonus, they allow you to bring in some pets.

My father recently went to The Neighborhoods for rehab and to gain strength after a Parkinson’s diagnosis and a wound on his foot that wouldn’t heal. We have been very impressed with the care he has received. It’s difficult for a patient and family to put your loved one in the care of others. I feel like every concern or question I have expressed has been listened to and worked through. All of the staff is great about respecting my father and asking how HE would like things done. That’s so important for his dignity in a time like this. Our family is extremely pleased.

My Aunt is currently in this facility after being in a fatal car wreck which required numerous surgeries to put her back together. This facility is continuously short on help which compromises the care the residents receive. She is continuously left on her bedpan for long periods of time. Sometimes she has to situate her bedpan herself because noone comes to help her. The food is awful and some of it isn't fit to eat. There are a few good nurses there but for the most part the nurses are snotty, rude and lack any interest in doing their job and taking care of the patients. I wouldn't recommend this facility to anyone and I will be glad when my Aunt is able to leave that place.
