DON'T send someone here if you love them! The staff was very nice, if a bit impersonal, but when we came to pick up my grandma, who was staying in the memory care unit, she was wearing someone else's pajamas and all of her pants were missing. She had a massive bruise on her face and a cut from falling out of bed, even though we were assured she would be watched, and the bed had rails on it that were apparently lowered. She was far more confused than usual, her teeth were out, and she was unusually aggressive. I don't know how they treated her, as I wasn't there, but it's extremely worrying.

Clinic declines to accept payment by check. Accepts only cash and credit card payments. Since I can't pay by check, why do I have to pay a transaction fee if I pay by a credit card? The transaction fee is not my business since you decline to accept payment by check. And there is more. A physician, Vishal, DM, lacks sufficient compassion and understanding for the patient. I felt I was the subject of lectures in every possible way. My experience with another visit, this time with a physician, Michelle, PN, was completely different. She was knowledgeable, attentive, compassionate, and objective, and I wasn't a subject; I was a patient.

Stone Oak is a great facility. I walk about on different occasions and see the residents playing bingo or making crafts. This sense of community is very good for the mind and is wholesome. I also want to give kudos to the staff from the receptionist who actually answers the phones , to t Admitting Dept. Rose to the unsung heroes...the CNAS (Brenda, Pete and Ryan)and the woundcare nurse Rita Ann and all nurses. All busy yet, find time to relay messages and go beyond regular nursing home care. Patients and their families are a priority at Stone Oak Care Center I have noticed. My brother is here and was transferred to Stone Oak from Corpus Christi. Greatest decision ever.

Compared to other memory care facilities that I visited, I can give this location high props for the freedom they give their patients. My father is a healthy, walking person who has dementia. I go regularly and find patients walking around or wheeling around freely. They are up and down the halls, in the gathering space watching TV, sitting with others and with the ability to get snacks and water as needed. They are living their unusual life and seem content. The nurses are very kind and understanding of our loved ones personalities. In comparison, I visited two places recently. The locations looked nice and are rated higher in stars but they felt uncomfortable because they were completely silent. I found it odd to see patients with dementia in their beds, in their rooms at 7pm. One home had two giant TV's and they were both off. The nurse said the patients get triggered by TV? That would happen at Stone Oak and doesn't. Sadly the place had nice cozy couches and no one sitting on them at 7pm. Probably because they had no TV. No one walking like at Stone Oak. No wheel chaired cruisers. No digging through drawers, chatting, or eating snacks. These places were not alive, they were sad and not like a life I would want to lead. I would rate Stone Oak as successful in this area. The reason for the 3 1/2 is because the floors and rooms are not cleaned often enough in the memory care area. Other areas are spic and span. They smell lightly like urine all the time (I could forgive it, if it were not often) and the floors are sticky in some places and remain so for days and days (not mopped regularly and it shows). Then the laundry system needs work. They continue to lose my dad's cloths, no matter how many I bring, how I label, they are lost and disappear after one use. If another patient had them on, I could excuse (they all have dementia and well that is what they do) but nope, they just disappear forever. My dad's labeled walker disappeared. Labeled nike pants, gone over night. Socks gone and no where to be found. I don't bring anymore.. If the last two elements can be fixed, I would rate this location a five and hope to in the future. I hope they strive to rise.

Hi Rosie, this is a message of gratitude for taking care of my brother while he was at Stone Oak Care Center when he needed rehab after a short period in the hospital. He arrived in the evening and the staff in hall 600, especially Howard were very welcoming and helped him get settled in. I visited my brother everyday and I could see that the staff was working hard in assisting all the residents. There were a few times that I had questions or concerns and I was fortunate to have met Rosie, the admissions director. Working with Allison the social worker and Dora the director of nursing was very helpful too being that all of the administration worked together so well in order to provide the best care for my brother. I feel Rosie went above and beyond her call of duty and she treated my brother with respect and dignity and she said she treated all of her veterans as family. I know she would check on him, as all of the other residents to make sure they were fine and check if they needed something. Going to a rehab or nursing facility is a difficult thing to do but Stone Oak Care Center tries hard to make each resident feel comfortable during their stay there. Thank you!

From the moment I contacted the facility we were treated like family. Rose’s genuine love for the place and people was evident the moment I first spoke to her. Every conversation felt like a hug. She’s so amazing I call her Rockstar Rose - because she IS a rockstar! I also have to mention Tom “The Bomb”. The fantastic maintenance man that helped us by mounting my father’s tv on the wall the day before he moved in. Tom radiates kindness and went above and beyond to help. Today was my father’s first day. And it couldn’t have gone better thanks to all of the amazing staff - Caitlin, Anthony, Rockstar Rose 2.0, Denise, and Sally. So thankful for Stone Oak Care Center. I just transferred my dad here on the 26th and seen big improvements in his demeanor due to the wonderful treatment he is receiving here. I transferred him and now he is receiving the care he deserves attention, good food, scheduled medications, and they’re keeping him and his room clean. The facility seems nice until you get to Memory care. The administrator will tell you that there are activities and, that they can go outside whenever they want ( weather permitting). I have never seen any activities or available items for residents to use. There seems to be no staff when we visit or only 1 staff member. They lock the door so patients can’t go outside, The patients just wonder up and down hallway. The nurses/ staffs just shrug their shoulders when you ask about anything. My mother’s clothes, toys, hygiene , shoes, glasses, teeth are missing, Staff states that they can’t do anything about it. No calendar of activities is ever shown to family. The hallway always smells of urine, I have asked the staff something about my mother and they don’t know who she is. I never see staff interacting with patients who are wondering, I do see staff on their phones. I have called administrator, lead nurse and they have not returned my calls. For safety, the front doors are locked, we wait a long time for someone to answer, I give it a 2 because the other units appear to be nicer.

Our mother needed some rehab for a couple weeks after she got out of the hospital. So my brother and I went to check out Stone Oak Care Center. We meet Rose Haley (the care navigator). She greeted us kindly and showed us around. Rose helped us get our mom approved late Friday and she stayed late after work getting all the paperwork done to submit to the insurance company. When we actually came and brought our mom there, Rose was right there to greet her and bring her a wheel chair to take her to her room. The first night for our mom wasn't to good because she said the bed was to hard. So Rose stepped up and went to the store after she got off work and bought our mom a Gel topper and then brought it back that night and put it on her bed. I can't brag enough about Rose! She is a care giver and a servant. She truly loves her job. Thank you Rose for being there for our mom. If it wasn't for Rose we probably wouldn't of chosen this place for our mom. Karen M.
