If I could give this facility zero stars, I would. My wife was admitted to Sandgate following a transfer from Moffitt Cancer Center. Before admission, I specifically confirmed with facility staff that her prescribed oral chemotherapy medication could be administered at Sandgate, provided we brought the medication with us. We were told this would not be a problem multiple times and were instructed to bring the medication, which we did. Upon arrival, the facility was not prepared with the breathing equipment she required. They reluctantly looked for and found the equipment. My wife had not eaten for many hours due to a 3- hour ambulance ride, but I was told it was too late to get her food, so I had to leave and purchase food myself. Her room also had several issues, including no guard rails on the bed, and non-functioning TV and air conditioning. I was told I was not allowed to stay the night with her. At approximately 4:00 a.m., I received a frantic call because staff could not locate equipment needed to assist my wife's breathing and I heard my wife gasping in the background. The equipment was eventually found and the situation was rectified. The next morning, a physical therapist and I arrived at the same time just in time to prevent my wife from falling from the bed. When I asked whether she had received her chemotherapy medication, I was informed that Sandgate could not administer it after all and she must go to another hospital. This directly contradicted the information provided several times to me, Moffitt, and my wife’s case worker before admission and was the primary reason we chose the facility. Efforts to transfer my wife to a hospital were delayed, and there were significant communication and coordination problems throughout the process. I was told by nurses to make sure I got the director’s attention again before 4:30 because she leaves the office regardless of what is happening. They also did not seem surprised by the misinformation I was given. Later that day, after my wife’s vitals had not been taken for 8 hours, a nurse determined that my wife required urgent hospital care, and she was transported immediately to Lawnwood Regional Medical Center. My wife passed away at Lawnwood four days later. While I cannot say whether the outcome would have been different if she had gone to a competent rehab facility, I was deeply concerned by the lack of preparation, misinformation, communication, and consistency regarding her care, as well as the condition of the rooms. Based on my experience, I would keep my loved ones far away from the facility.

My cousin was transferred to Sandgate Gardens after a hospital stay. Since I don't live in Florida, it's difficult to care for a loved one from so far away. From the moment he arrived at Sandgate Gardens, I knew he was in good hands. The nurses and staff have been exceptional. They treat my cousin with kindness, compassion, and respect, and they keep me informed of any changes or concerns. Even though I live out of state, they make sure I am included in every care team meeting, and it's clear that everyone truly knows my cousin as a person, not just a patient. My 98 year old mom was just transferred there from the hospital, where her treatment wasn’t always the best. The nurses and staff who we’ve met at Sandgate were all caring, compassionate, and personable. I’ve known the medical director for years, and she is our entire family’s doctor/NP, including my mom. With her in charge, I know the patients there are receiving the best possible medical care. The building isn’t brand new, but the building isn’t treating the patients.

Great little skilled nursing facility in the heart of Fort Pierce! The front entrance was neat and well kept! The young lady at the front desk Jazmin was super polite. The admission staff Sheena was super welcoming upon entering the facility. Therapy is the best! They have all kinds of equipment and super knowledgeable AND adaptable to everyone’s needs! Most of the residents were doing some active in the dinner area with a performer which was awesome to see. Was pleasantly surprised at how nice this place was upon entering.

My first day there I was greeted like the president by the staff and manager. My stay there was wonderful until i didn’t want to leave. The staff and nurses was awesome and treated me like a king. The manager and supervisors even stopped by my room to check on me to make sure I was comfortable and had everything. All questions were answered and taken care of. We had a health scare with my grandmother. Someone who we were use to seeing driving, taking care herself, great grands, cooking, you name it she was able to do it. She went to Sandgate for rehab. She was there for a little over a month. The staff was great, very attentive and made sure she was taken care of. She was able to go home being back to usual self. I would to recommend to anyone looking to put their family member in a place where the patient matters.

My partner has been a resident of Sandgate Gardens for quite a few months now. The cleanliness of the facility is questionable. There are reports of roaches from visitors and the patient himself. He was placed in a room with another resident that has a history of causing trouble. That resident had been moved multiple times due to causing issues with other patients. I was on the phone with my partner multiple times and could hear the resident making threats towards my partner. Today, thankfully, my partner is being relocated to another facility. When my son picked him up, all of his clothing had disappeared. There were several items that had sentimental value. I am considering getting my lawyer involved. If you have a choice on where you can place your loved one, choose a different facility.
