I spent 21 days here recovering from surgery for a double quad separation from my knees. The nursing and Physical therapy staff were exceptionl! They got me on my feet and walking again, when some days I didn't see it happening. If I ever need rehab again I wouldn't hesitate to go here! THANK YOU ALL! Bill

Wonderful. I have had five years post recovery and have not fallen yet. Physical Therapy team did me very well. BLT: No lifting bending or twisting. It works. Thank you thank you thank you. Three flights of stairs. Daily. I live. And the friends I had to abandon there.

Time to your room is short, the staff is great, the food is good although my finicky partner had me go to subway a couple of times. If you have a complaint they are in your room in no time and they fix what bothers you. My partner needed a lot of physical therapy and Mike got him up and going in the time we had there. Linda, one of the rn's, was delightful. And I can't say enough about the cna's. They conduct a weekly care team meeting where all the caregivers meet with us and discuss progress of rehab and determine the discharge date. About a 20minute drive from western Beaverton in forest grove, I highly recommend this place. My experience there has been pleasant.

I'm glad to be able to fill out this form. I have been at this facility 2 — 4 rehab. And I have to tell you, it is amazing. The staff here at Forrest Grove, acute has brought me back twice. I cannot be happier. The staff is incredible. Whether it's an RNA therapist, a CNA or kitchen help or housekeeping, they all work like the team together, they are absolutely wonderful to the patients. I was treated like a king, and I was so happy to make the progress I made. I’m currently here and it’s been pretty awesome! (I’m writing this of my own free will, the only influence is appreciation) I have some mental issues so that’s probably part of it but sometimes I feel like a queen here! Literally everyone is friendly or atleast all the staff, which is what’s really important. Therapy here is fun but focused which honestly makes it easier to do and look forward to. I could go on and on n on! Seriously! Everyone talks about healthcare food being bland but here it’s delicious. Consistently my favorite parts of the day are when food comes! I think last month we had chili so good I started singing Rihanna’s only girl in the world cause it tasted so good, I’m a Taurus so food, especially good food = happy bull. They are also really cool with visitors, my mom has practically visited every night and no one has been weird or chastised either of us. I really appreciate that because I’m a total momma’s kid. Anyway great place a sincere thank you to all the staff! I don’t see myself forgetting their kindness for the rest of my life. No lie.

I'm a close relative of someone staying here. As a daily visitor I appreciate the inviting nature of the facility. And though I visit often, it's always unannounced and so I have a sense of how the staff performs "when no one is looking". It is with the utmost gratitude I can say that Forest Grove cares for residents in an exceptionally professional manner, while also delivering tenderly personal moments which seems to make everyone feel truly cared for. I don't want to imagine the difficulties of caring for my loved one alone, and I'm glad to have a partner in Forest Grove Post Acute helping our family through this journey.

Reception, PT, RNs, and CNAs 5/5 stars. I want to give the facility 5/5... but the poor communication makes impossible. Kitchen 3/5 stars, because the breakfast/lunch person can't cook proteins... the are overcooked and dry, sometimes so dry you almost choke. Apparently the only way you can get a runny yolk is if the egg whites are raw, or the entire egg is over hard. The liquid scrambled eggs are even dry. There are some inherent flaws in the menu planning, such as, not checking supplies to see if you have enough stock to serve that meal..chili is on the every day menu. I was got it watered down. ™ COMMUNICATION 1/5 stars. #1 tenet of patient care, LISTEN TO YOUR PATIENT! Be professional. I once asked my CNAs to do something during a vurnable moment, no response. They were busy talking to each other. After, yelling STOP! I said, I ask if you would..... they said they heard me and were taking care of it, thank you, but how was I supposed to know that. If you are talking, you are not listening or communicating with your patient. I know, I used to be very bad about that. "They" schedule meetings for you without asking. I mean, really... I'm not important, my time isn't important. I just sit here waiting on them right, im stuck, limited in my activitiesand abilities. I couldn't possibly have family or guests maybe visiting from 2 hrs away, or important phone appointments. IM NOT INVISIBLE, MY TIME, MY INPUT MATTERS. YOU WORK FOR ME, MY TIME IS TO MANAGED WITH YOUR HELP, MY DAY IS NOT DECIDED BY YOU! Something as simple as asking the CNA to get me X med or whatever ends up convoluted. I get the RN coming in with a ?? What did you want?? Wasting the nurses time as now there are 2 required. With the best of intentions, I'm 100% sure, some CNAs make assumptions about what you want or need, instead of just asking and it creates confusion and a lot of frustration for the the patient. I'm not referring to the skill of anticipating what a patient needs..it's a honed skill based on getting to know your patients and experience. I won't give an example because it would identify the CNA, and I'm sure it with the best of intentions. If you don't know for sure, or doing it to make your job easier, then don't assume. I think it is a reasonable to expect employees that come into contact with patients to know enough English to communicate as related to thiet job. Thus isn't intolerance or passive racism. I ask a non English speaking staff member to come back later and they never do. If I needed help or something they no nothing except one word which they adnausem over and over again, and it doesn't have any context. At best it's frustrating from a patient POV, and if your here, your emotional plate probably already full. At worst, you might not feel safe because they couldn't help in an emergency. I mean does housekeeping know CPR? Lots of room for improvement.

I had a Friend who went here for Therapy after getting into an accident and the THERAPY team was a dream, they motivated him to do his best, even when he didn’t always feel like it and todays he’s walking again when we didn’t know if he would. The CNAs were always attentive and polite even when he was not. The nurses helped with his wound care and kept his pain under control. The care managers made sure he got to his appointments even tho dealing with insurance was a headache but it really helped take the pressure off his wife so she could focus on his healing. The administrator is so kind and you can really tell she cares about her team. The doctors and his team made a difference in my friend’s care. Best Part is when I came to visit, it was clean and didn’t smell like a Nursing home, if you know what I mean!

The staff, from Yesenia in front office to the CNA caregivers (Eva, Ariceli), cleaning staff..Marina, to kitchen staff..Roe...(just to name a few) are the most caring, kind, reliable staff anywhere. I had to come to have more physical therapy and care than the hospital had time for after my knee surgery. I was very well taken care of. The CNA's were absolutely wonderful. They took such good care of me. They came when I needed help with in a reasonable amount of time. I got good physical therapy everyday. I gave the staff 5 star's because I feel they deserve that. This facility gets a 5star from me. As an agency nurse I’ve been through tons of facilities and this one takes first place year to year. Great staff, communication. Management is approachable. Team work roams in the whole building. Patient care and advocacy is top notch. The house providers are exactly what your family member needs, they are invested in the patients care and well being, always answer the phone when you call them and are available 24/7. The facility is really clean and has the best housekeeping services I’ve ever been around. The therapy team will get you home before you even know it. Now let me tell you about the admissions manager, thorough is an understatement when it comes to Amanda. As a nurse you always when you’re to get an admit, everything about your admit before even report from the hospital.

My experience here at Forest Grove post acute has been absolutely incredible! This facility was highly recommended by the staff at the hospital I was at I almost didn't choose it because it's a farther distance from my house. But I'm SO glad I listened to my social worker because it was the best decision! My physical therapists Mike, Sarah and Monica are so nice, funny and so good at what they do! I was hardly able to sit up in bed and now I'm walking the halls with assistance! They are incredibly good at what they do! The social workers are so helpful and nice!! They have answered so many of my questions and always take time to listen if I need anything! Amanda is always following through with anything we have talked about, or anything I needed, she is right there to help with a friendly smile and easy to talk to! The director of the facility Kayla is so nice and helpful! You can tell she really cares a lot about the people here wether they are employees or residents! My interactions are mostly with the CNA'S. I've been to another facility as well as two different hospitals recently and the CNAs here stand out from the rest! They are so caring, fun, attentive and professional! I always feel very comfortable in the vulnerable moments, and I feel very cared for daily! It really makes a big difference in your day to be surrounded by people like that, especially when you are bed bound! So many times when they leave your room after helping you with your need, they say "If their is anything else you need just use your call light". 🙂 The CNAs in particular that really stand out to me in terms of going above and beyond are my morning CNAs Mikayla and Bianca! They are proactive in my care, bringing me things before I ask, knowing what I'm going to ask for before I even use my call light! There are so many other great CNAs, I could go on and on! My experience here has honestly made such a big difference in how I see my future!! They take good care of you while you are here and also make sure you are ready for your future!!

I was here for 8 months. In pain, confused, and couldn't walk. The team of caregivers went through my screaming, yelling, and crying. From being bed ridden for months to semi walking out of there. My PT and OT team Sarah, Monica and Mike pushed me and challenged me everyday until I could walk. Special shootout to my caregivers Mighty Mouse, Maria, Savanah, Trista, Jasmine and the twins plus all the others. Nurses Nicole, Sarah and Lizzy. I was at Prestige for 2 weeks. I was apprehensive at first, but quickly put at ease. I was disappointed to have a roommate, but she proved to be a sweet lady and we got along great. The staff was unfailingly responsive, professional and very respectful. It can be hard to hang on to one's dignity when someone else has to help you clean up, but everyone was so caring. Once in a while there was a bit of a wait, but overall they were there when I needed them. The food seemed to suit my palate. I literally never had a meal I didn't like. They offer many options on hand if the entr©e isn't what you want. Portions are more than adequate, and kitchen staff was great about fulfilling special requests. For instance, I prefer whole milk to 2%, and I got whole milk when I asked for it. The place is clean and well-maintained. PT and OT staff worked with me consistently, and i met my goals and got jome in a timely manner. And when I did have a down or sad moment, the staff was very sweet and caring. I miss some of them! I say if you are placed here, you have hit a home run. Great staff, good food, decent roommate. I'd give it 6 stars if I could!

I give this facility 3-1/2 to 4 stars. I was sent here followinng a 2 week hospital stay. My physicians and other caregivers felt that I needed more intensive nedical care and rehab/therapy than the assisted living facility where I'd been living could provide.i will however make some suggestions based on my observations and experiences here. Overall. The staff is stellar. Caring and responsive. But There are a few inconsistencies and some areas that could use improvement. The old adage that the left hand sometimes doesn't know what the right hand is doing applies. I feel the care provided could be.and should provide more coordinated care. There are times when it takes an extensive amount of time to respond to a call light. Perhaps more caregivers or better coordination and timing during peak times like shift changes and meals . And improved communication and scheduling all around,

Poor follow through. Administrator not willing to help rehab patient with proper equipment/chair, stating it’s our policy. I came in after a head on accident with a semi, multiple fractures and limited movements and in order to be out of the bed for more than an hour I need a comfortable reclining chair. She refused stating I had a w/C that reclined. I can only be in W/C for an hour until painful . Part of rehab is to make sure you can feel as normal as possible and the administration was unwilling to help with this. Don’t come here to rehabilitate if you have a complicated case. It also took her 7 days to see me and she promised she’d be back end of day to let me know. She did not and I had to call facility from me room to reach her. I

As an 88 year old retired physician, I recently spent two weeks in the care of the professional staff at Marquis Rehabilitation and Care Center Forest Grove, recovering from a fractured pelvis. I could not be more pleased and grateful for the compassionate care I received day and night from the hard working, short staffed professionals. The nursing staff were caring and competent, as were the IT and OT staff, all of whom were focussed upon getting me well again. Special thanks to senior RN Mary, who brings a wealth of wisdom and kindly good nursing to her position, and the several delightful young CNA's who made my stay that much more comfortable and rewarding. My grandmother is 96 years old. She has experienced multiple rehabs over the years. This is hands down the very best! None of them come even close to the care she is getting here! I have never heard her rave about the caregivers, the food, the respect she is given, and the overall feeling of the place. She would love to move here if she could.

My husband had back surgery and came here for rehabilitation. An absolute wonderful experience. I was fearful I couldn’t be able to be with him because of the pandemic, but the nurses and staff went above and beyond to take care of my husband and keep him happy, comfortable and moving forward with his recovery. The day I picked him up, they made sure that I understood all his medications and made it 100% comfortable for me to take over, to continue his recovery at home. My husband had nothing negative to say, this rehab is excellent!! I highly recommend!!

We had our family member at Forest Grove Rehab and we cannot say enough good things about the facility. When my dad went there he could hardly walk and when he left he was walking and able to go home independently. The staff there really cared and were so kind. The Nursing care and Rehab was excellent If our family needs any kind of rehab in the future we will only go to Forest Grove Rehab.

I can't say enough good things about Forest Grove Rehab.My father was a patient there and they took such good care of him. I own and run an Adult Foster Home in Forest Grove and I have had several of my residents stay at their facility the quality of care is excellent it is very clean and the staff is very friendly and on the ball when one of my residents need Rehab and I am asked where they should go I say without hesitation Forest Grove Rehab!! - Confirm with community manager that living and services required for you or your loved one's unique needs and budget are met.

Assisted Living Center uses the term "assisted living" or "personal care" to inform users that a community provides assistance with some Activities of Daily Living (ADL's). It may not meet your state's legal definition required for that level of care.
