Our experience has been awesome! Our loved one is new to Legend Oaks West but has been warmly welcomed from the very beginning. Megan from Admissions has been instrumental in making this transition as smooth as possible. She walked us through the process, was extremely patient answering all of our questions, and was very informative and reassuring along the way. The entire staff has been friendly, courteous, and welcoming. You can genuinely feel the compassion and sincerity in the care and assistance they provide. The facility is clean, and the calendar of activities are diverse and plentiful, which we really appreciate. We are so excited and grateful that our loved one found such a welcoming new home. We truly appreciate everyone who helped make this transition a positive experience for our family!

it's like most other businesses....some good staff; some not so good. It would be nice if the staff would speak better english. It also would be nice if some members were out and out fired. Those definitely are not people friendly. And some are not timely in their response when you call for help. YOU ARE NOT THE ONLY ONE IN HERE was one response. This nurse was definitely one to be fired. So was the one who would refused aid a resident to go to the bathroom. I witnessed her refusal. also the night staff refused to answer the call for help from my roommate. But God Almighty here came at least 5 to finally respond, but not until i wheelchaired myself to his bed side to sit with him. I was lectured not to go to his room, my response was WELL WHEN SOMEONE NEEDS HELP IT'S NOT MY NATURE TO IGNORE IF I CAN HELP. I offered no medical treatment or advice, just my company so he wouldn't feel no one cared.

Everyone has been really nice and helpful. My mom has been here a few times. They are attentive and kind. The nurses and therapists are always so helpful and friendly. I feel comfortable with my mom here. They never treat her like she is a bother.

Great people work her from Consuelo who was my mom attendant she had so much compassion for her to Stephanie her wound nurse was very attentive to her arm that just had surgery to Carmen her nurse were great and Kimberly the therapist that worked with my mom The only thing was the food definitely needs a upgrade but the place is clean and no smells

My friend was transferred there yesterday from Baptist hospital. It was late afternoon. I'm praying for positive results for good rehab results. I just toured that location two weeks ago for my mom, but she is in a pending status with medicaid so she's not there yet. I like that it is rated 5 . If I find that it isn't. I'll post my findings on both of their behaves later. I truly believe in giving credit when credit is due and constructive criticism as a means to improvement. Stay tuned.

My mother had a short stay at this facility in March after suffering from a stroke. The whole PT group that provided her therapy gets 5+ stars from her. But she also says, Ana Rosales, Amanda Perez, Christina Perez and Gracie went above and beyond to make her stay here much more comfortable. Thank you guys ™ from our whole entire family

My mother (Paula) was at this rehab facility for 3 weeks. The receptionist welcomed me at the door. She directed me to where I needed to go. The facility was very clean and fresh smelling. The staff were very helpful and answered my questions. I had a very good visit.

My experience at Legend Oaks Healthcare and Rehabilitation was extremely great!!! From the moment you walk in they make you feel welcomed and at home!!! The employees are very nice, friendly, and very courteous!!! The employee who was up at the front desk even went out of her way to open the door for me and for another family member that was visiting the facility with me that day!!! The facility is very nice, elegant, and extremely clean!!! If there's a place that i would have to use as a Healthcare Rehabilitation facility this place would be the one!!! I want also acknowledge every staff member that I came in contact with. From the receptionist’s who create a welcome environment, the many staff members that I encountered in the hallway with smiles, food service workers and health professionals all left a great impression on me and I’m so very grateful for each one of them. The facility is very clean and the patients are noticeably cared for. My expectations were exceeded and I will definitely tell other families looking for placement about this experience. Ms. Crystal Rodriguez, Social Worker went above and beyond in her professionalism and clear communication in assisting me with the transition to long term care.

I am impressed with the level of care my father is receiving through Legend Oaks on Bertetti in San Antonio. The atmosphere seems to be very inviting plus my father likes the food at this rehabilitation center. The nurses have been responsive and actually call you back when you leave a message (as long as your phone call doesn't get dropped). My MIL has been at one rehab center in San Antonio but she didn't like the food, so I'm glad my father is not only eating nourishing food but it is palatable

After visiting several other Nursing Homes, Legend Oaks West is exemplary! The layout of rooms are extraordinary, two residents in each room are separated by a wall as opposed to a curtain, lots more private personal space! Beginning with the front desk staff, all residents are greeted by their last names! The environment is extremely clean without clutter. Nursing, nutrition, housekeeping, activities, MDS, PT-OT- Speech staff, and spiritual staff go above and beyond on caring for all residents!

I am not one to usually leave reviews especially for places like these but I decided to due to the fact that nobody had checked on my father nor laid him down and he was clearly uncomfortable. I am just confused on why nobody got off their ass and while I am a bit pissed off, the manager Mina apologized and helped lay him down after SEVERAL people told him they would "in a bit." I am just happy some of yall are willing to set the example! Appreciate you Mina you are a beauty inside and out, a rarity these days!

The facility was very clean and the staff was extremely helpful, professional, friendly and seemed to be genuinely caring about their patients. My FIL was transferred from the hospital after an aneurysm. He received therapy and other health services. His room was cool and comfortable. It was a little dark, but opening the shades solved that issue. The channel selection was good and the TV was good too. There was no door between the two occupants so the noise level was a little high. That's the reason why I gave 4 stars. Overall it seems to be a very nice place. We were greeted by a very kind and helpful lady upon entry. The therapy staff was kind and considerate. The vending machines had a good selection of snacks, ice cream and drinks. My FIL was happy with the food and menu. He said " The food is good, I like it" He is content and has no complaints, so neither do we!
